Fraud prevention is not a fraud-team project or a cybersecurity feature. It is an enterprise operating capability that joins cyber controls, threat intelligence, fraud analysis, financial-crime investigation, business operations and customer recourse. A CISO’s job is to make those connections explicit, then ensure that risk scores inform accountable decisions rather than becoming unexplained reasons to approve, decline or delay a customer.

The practical model is to map fraud across its attack-and-loss lifecycle, give the relevant teams a shared operating process, connect each loss scenario to mission impact and risk tolerance, and continuously test scoring, controls, privacy and redress.

What fraud operations means at enterprise level

Fraud operations is the coordinated capability for detecting, investigating, interrupting and learning from attempts to obtain money, access or services through deception or misuse. It includes customer and application fraud, account takeover, unauthorized payments, insider-assisted activity and related financial-crime concerns. The capability spans more than transaction monitoring: identity proofing, authentication, email security, endpoint controls, payment operations, investigations, customer support and recovery all affect the same loss scenarios.

FS-ISAC’s 2025 Leveling Up: A Cyber Fraud Prevention Framework for Financial Services states: “To succeed, an institution requires data, intelligence, and specific capabilities — all of which may be siloed in cybersecurity, fraud, financial crimes/anti-money laundering, and other teams.” That observation is operationally important. A phishing indicator held by a security team may explain a spike in account takeover; a fraud investigator’s mule-account pattern may improve identity or payment controls; a customer-support complaint may reveal that an authentication control is blocking legitimate users.

The framework is designed for financial services. It supplies a shared structure and vocabulary, not a universal regulatory standard. Other sectors can adapt the method, but should not present the FS-ISAC model as a requirement outside its intended context.

What a joined operating capability contains

  • Cybersecurity and threat intelligence: telemetry, indicators, vulnerability information and adversary tradecraft.
  • Fraud and analytics: behavioral patterns, transaction anomalies, case triage and loss analysis.
  • Financial crime and AML: investigations, sanctions-related concerns, suspicious activity processes and lawful information sharing.
  • Business and operations: payment, onboarding, servicing and recovery procedures that can either stop or enable a loss.
  • Customer and legal functions: communication, accessibility, privacy, complaints, regulatory reporting and correction or redress.

Each group retains its specialist controls, but they use common case identifiers, escalation paths, definitions and lessons-learned practices. That prevents a fraud event from being closed in one queue while its enabling weakness remains open elsewhere.

Use a five-phase lifecycle to see where losses develop

FS-ISAC’s framework describes five phases: recon, initial access, positioning, execution and monetization. An investigation can begin at whichever phase first exposes an indicator, then work backward and forward to find related activity. The lifecycle applies to scenarios including application fraud and account takeover and is also discussed in the context of money laundering and sanctions avoidance.

Phase What the actor is doing Questions for control owners
Recon Selecting targets, collecting information, preparing infrastructure and planning. What public, customer, employee or third-party information is being gathered? Which signals show targeting before an account or application is touched?
Initial access Obtaining a foothold against a consumer, institution or related entity. Is the entry point phishing, credential reuse, malware, social engineering, a compromised vendor or an application channel? Which team owns the first containment action?
Positioning Changing or collecting account information so funds or data can be sent to infrastructure controlled by the actor. Are contact details, devices, beneficiaries, recovery factors or permissions changing together? Can customer-service and identity teams see the same sequence?
Execution Converting stolen access or data into money, often through legitimate business procedures that send unauthorized funds. Which payment, withdrawal, credit, refund or transfer process is being used? What step can pause the transaction for a human or stronger verification?
Monetization Receiving stolen funds through a payment method or transfer. What destination, mule, cash-out or conversion pattern appears after execution? How quickly can downstream institutions and investigators receive an indicator?

The value of the lifecycle is not a claim that every event follows five clean steps. It is a way to ask adjacent teams for evidence and controls. If a payment analyst sees monetization, the team can look for the positioning change that enabled it and the initial-access signal that preceded it. Sector sharing is also useful when the same actor attacks multiple targets in close succession.

Build the cross-functional operating model

A CISO can turn the lifecycle into a repeatable operating process without reorganizing every department. Start with a standing group that includes cybersecurity, threat intelligence, fraud, financial-crime or AML, data analytics, relevant business operations and customer-impact representatives.

  1. Choose a loss scenario. Define the event in business terms, such as an account takeover followed by an unauthorized transfer, rather than beginning with a tool alert.
  2. Bring existing indicators. Each team contributes the signals it already has: domains, devices, identities, payment destinations, behavioral patterns, complaints, case notes and control failures.
  3. Place indicators on the lifecycle. Record the phase, timestamp, affected asset or customer journey and confidence level. Permit an indicator to relate to more than one phase when the evidence warrants it.
  4. Ask for adjacent-phase evidence. The team that found an execution signal asks security for initial-access evidence and operations for the procedure that allowed the transaction.
  5. Assign an owner and decision deadline. Every investigative or control action needs a named owner, an expected customer or loss impact and a time by which the decision must be made.
  6. Share the lesson. Update detection logic, playbooks, training, third-party requirements and relevant sector-sharing channels after the case is resolved.

Use a common case record or at least a cross-reference convention. A fraud case number that cannot be connected to a security incident, payment hold or customer complaint makes trend analysis unnecessarily difficult. Access should still follow least privilege and legal requirements; collaboration does not mean giving every team unrestricted access to sensitive investigation data.

Govern risk scores as decision support

There is no universal fraud-score formula or threshold. A score is meaningful only in relation to a specific decision, loss scenario, evidence base and risk tolerance. NIST’s fourth-edition digital identity guidance (SP 800-63A) says relying parties should periodically review provider fraud checks, thresholds, investigations and control effectiveness. It also notes that results vary with data sources, technologies and the applicant population.

Write a decision contract before tuning a model

Document what the score is allowed to influence. A score might route an application to manual review, require stronger authentication, delay a payment for investigation or trigger a customer contact. It should not silently become an irreversible verdict. The decision contract should state:

  • the decision and the accountable business owner;
  • the fraud or loss scenario being addressed;
  • the acceptable risk appetite and tolerance for that scenario;
  • the action at each band or threshold;
  • the evidence and data sources used;
  • the maximum delay or customer friction permitted;
  • who may override the result and what documentation is required.

Evaluate evidence quality, not just model output

Record data freshness, coverage, provenance, missingness and known biases. A score trained or calibrated on one population may perform differently for a new product, region, channel or customer segment. Changes in authentication, payment behavior, attacker tactics or data availability can alter both false-positive and false-negative rates. Periodic review should therefore examine outcomes by relevant population and channel, not just an overall accuracy number.

Set thresholds with operational consequences in view

A low threshold may catch more suspicious activity while sending legitimate customers to review. A high threshold may reduce friction while allowing more losses through. The right choice depends on the impact of the underlying scenario and the organization’s stated tolerance. Keep threshold changes versioned, approved and reversible, with a record of the evidence and expected effect.

Monitor outcomes and drift

Track confirmed fraud, prevented loss, investigation yield, review queues, customer abandonment, complaint rates, time to decision, recovery amounts and post-decision discoveries. Compare these measures with the score band that generated the action. A score that appears stable while the population or attack pattern changes is not necessarily effective.

Provide override, correction and redress paths

Human review is not a substitute for governance. Define when an analyst can override a score, require a reason code, and audit whether overrides cluster by team, channel or customer group. NIST also calls for documented fraud-check practices and redress procedures for applicants who fail checks. Customers need a practical way to report an error, submit relevant information and receive a decision within a stated process.

Assess privacy risk and minimize data

NIST calls for privacy-risk assessment of fraud checks and mitigation technology. Collect only data that has a documented purpose, restrict retention, protect investigative information and assess the consequences of linking identity, device, behavioral and transaction data. Explain the relevant processing to customers where required, and involve privacy and legal teams before introducing a new signal or external data source.

Connect loss prevention to mission impact

Loss prevention becomes an executive discipline when it is tied to what the organization must keep operating, not only to the number of alerts. NIST IR 8286D, Update 1 (2025), puts the principle this way: “The management of enterprise risk requires a comprehensive understanding of mission-essential functions (i.e., what must go right) and the potential risk scenarios that jeopardize those functions (i.e., what might go wrong).”

Apply that principle to fraud by mapping each material scenario to the mission-essential function it threatens. An account-takeover wave may affect payment integrity, customer trust, liquidity, regulatory obligations and service availability at the same time. A smaller event affecting a critical corporate account may have greater mission impact than a larger number of low-value attempts.

NIST SP 800-221 describes integrating ICT risk into enterprise risk management through risk registers and the enterprise risk profile. A fraud scenario that depends on an identity provider, email system, payment platform or outsourced processor should therefore appear in the relevant ICT and enterprise records, with dependencies and treatment decisions visible to leadership.

Regulatory and standards context: apply scope carefully

FTC Safeguards Rule example in the United States

The Federal Trade Commission’s Safeguards Rule business guidance applies to covered financial institutions, not automatically to every company or every fraud operation. It describes a written information-security program appropriate to an institution’s size, complexity, activities and the sensitivity of information. Topics include risk assessment, safeguards, testing and monitoring, staff training, service-provider oversight, change management, incident response and written reporting.

The guidance states that a notification event involving the unencrypted information of at least 500 consumers must be reported as soon as possible and no later than 30 days after discovery. It also states that the Qualified Individual must provide a written report to the board or governing body at least annually. Confirm the current rule, entity coverage, state requirements and other applicable obligations with qualified legal counsel before relying on these examples.

NIST identity and risk publications

SP 800-63A supports recurring review of fraud checks, thresholds, effectiveness, privacy risk, documentation and redress. NIST SP 800-39 frames information-security risk as organization-wide, involving mission, operations, assets, individuals and other organizations. SP 800-30 Rev. 1 structures risk assessment through preparation, conduct and maintenance. These latter publications are foundational and older; check the current NIST catalog for status and successor guidance before labeling them the latest edition.

Cyber-resiliency practices that support fraud work

FBI cyber-resiliency guidance recommends maintaining an asset inventory, using risk-based vulnerability management, setting remediation timelines according to risk and running quarterly cross-functional tabletop exercises. It identifies email as a favored initial-access vector for intrusions and fraud. Treat this as general resilience advice, not as a fraud-scoring standard. Include fraud, financial-crime and business-operation participants in exercises so that a technical containment action is tested alongside payment holds, customer communication and investigation procedures.

Build an executive dashboard that drives decisions

A useful dashboard shows whether controls reduce material risk and whether customers are being treated fairly. Organize measures by scenario and lifecycle phase rather than presenting one enterprise fraud number.

  • Exposure: assets, journeys, accounts, channels and third parties in scope.
  • Detection: time from indicator to triage, coverage of relevant phases and quality of shared indicators.
  • Decision: score-band volumes, manual-review workload, override rates and decision latency.
  • Outcome: confirmed fraud, prevented and recovered loss, residual loss and recurrence.
  • Customer impact: false-positive indicators, abandonment, complaints, accessibility issues and redress outcomes.
  • Control health: open control gaps, testing results, provider performance and overdue remediation.

Set review cadences appropriate to volatility. A threshold that affects a high-volume onboarding flow may need more frequent review than a low-volume corporate process. Every review should record what changed in the population, threat pattern, data, technology, policy or customer journey and what action follows.

Common failure modes and practical fixes

Siloed queues

Symptom: security, fraud and AML each investigate related indicators without a shared case view. Fix: use the lifecycle as a common taxonomy, cross-reference cases and hold recurring joint reviews.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Score worship

Symptom: staff treat a number as self-justifying and cannot explain the resulting action. Fix: require a decision contract, reason codes, threshold approval and auditable overrides.

Optimizing only for prevented loss

Symptom: declines and delays increase while customer harm and legitimate-business loss remain invisible. Fix: report customer outcomes, review burden, recovery and residual risk alongside prevented loss.

Unreviewed data expansion

Symptom: new device, behavioral or third-party data is added without a purpose, retention or privacy assessment. Fix: apply data minimization, documented purpose, access controls and privacy-risk review before deployment.

Ignoring recovery and learning

Symptom: an incident closes after a payment is blocked, but the enabling weakness and related targets are not addressed. Fix: update controls across the lifecycle, notify relevant teams and share indicators and lessons lawfully.

A CISO’s decision checklist

  • Can we name the mission-essential function and loss scenario behind each major fraud control?
  • Which teams own evidence in recon, initial access, positioning, execution and monetization?
  • What does each score change, who is accountable and how can the decision be challenged?
  • When were thresholds, provider checks and control effectiveness last reviewed?
  • How do we detect population or attack-pattern changes that could invalidate performance?
  • What privacy assessment, retention rule and customer explanation apply to each data source?
  • How do applicants and customers obtain correction or redress after a failed check?
  • Are material fraud risks represented in ICT risk registers and the enterprise risk profile?
  • Do tabletop exercises test payment operations, customer support, legal reporting and recovery as well as technical containment?
  • Which legal and regulatory requirements apply to our entity, jurisdictions and service providers?

When these answers are documented and reviewed, fraud operations becomes a measurable enterprise capability: coordinated across teams, connected to mission impact, and accountable to both risk leaders and the customers whose activity the controls are meant to protect.
