Yes. A person in their 20s, 30s or 40s can have a stroke. High blood pressure, diabetes, cholesterol problems, smoking, obesity, inactivity, alcohol misuse and heart disease remain important causes, but clinicians also investigate conditions that are relatively more prominent in younger patients. These include cervical artery dissection, clotting disorders, cerebral venous thrombosis, migraine (especially with aura), pregnancy or the postpartum period, estrogen exposure, autoimmune or kidney disease, cancer, sickle-cell disease, family history and a patent foramen ovale (PFO).

Stroke symptoms at any age are an emergency. Sudden facial drooping, arm weakness, speech trouble, vision or balance problems, confusion, numbness or a severe headache require an immediate call to emergency services; do not wait to see whether symptoms improve.

Young age does not protect against stroke

The Centers for Disease Control and Prevention (CDC) states that anyone can have a stroke at any age. About one in seven strokes occur in adolescents and young adults ages 15 to 49, according to CDC information published in 2024. Rising rates of obesity, high blood pressure and diabetes among younger people are contributing to that burden.

“Young adult” is not a single medical category, and the exact work-up depends on the person’s symptoms, imaging and history. A 22-year-old with a neck injury and a 42-year-old with longstanding hypertension may have entirely different mechanisms. Doctors therefore assess ordinary vascular risks and, when appropriate, broaden the search to conditions enriched in younger adults.

How a stroke occurs

Most evaluations first establish whether the event was ischemic (a blocked blood vessel) or hemorrhagic (bleeding). Ischemic strokes can result from a clot forming in a diseased artery, an embolus traveling from the heart or another artery, or a clot in the brain’s venous drainage system. Hemorrhages have different causes, but high blood pressure and vascular abnormalities are among the issues clinicians consider. The distinction matters because tests and treatments differ.

Comparison What it means for a younger patient
Arterial versus venous An arterial blockage deprives brain tissue of blood; cerebral venous thrombosis (CVT) blocks venous drainage and is especially relevant in younger women of childbearing potential.
Common versus younger-adult-enriched Hypertension and diabetes are common at every age. Dissection, pregnancy-related factors, selected thrombophilias and PFO-related questions receive more attention when a patient is young or lacks the usual risks.
Modifiable versus nonmodifiable Blood pressure, smoking, glucose, cholesterol, weight, activity and alcohol exposure can often be changed. Age, family history, some inherited diseases and a prior dissection cannot.

Common, modifiable risk factors doctors still check

Younger patients sometimes assume a stroke must have an exotic explanation. In practice, traditional vascular risks remain frequent and actionable. A clinician may ask about past measurements, medications, adherence and exposures rather than relying on a single normal reading.

  • High blood pressure: Persistent hypertension damages artery walls and is a major target for prevention. Doctors review home or ambulatory readings when office values may not reflect usual pressure.
  • Diabetes or elevated glucose: High glucose accelerates vascular injury. Testing may identify previously unrecognized diabetes or prediabetes.
  • High cholesterol and other atherosclerotic markers: Lipid results, family history of early cardiovascular disease and physical findings can point to inherited or acquired risk.
  • Smoking and nicotine exposure: Cigarettes increase clotting and vascular injury. Clinicians also ask about vaping or other nicotine products because the relevant exposure is not limited to traditional cigarettes.
  • Obesity, diet, inactivity and poor sleep: These factors often cluster with hypertension, diabetes and abnormal lipids. Sleep patterns and possible sleep-disordered breathing may be discussed as part of overall risk assessment.
  • Alcohol misuse: Heavy or binge drinking can worsen blood pressure, trigger rhythm problems and contribute to hemorrhagic as well as ischemic risk.
  • Heart disease and atrial fibrillation: An abnormal rhythm or structural heart problem can send an embolus to the brain. A normal pulse during one examination does not exclude intermittent atrial fibrillation.

The 2024 American Heart Association/American Stroke Association (AHA/ASA) primary-prevention guidance emphasizes regular primary care and treatment of blood pressure, cholesterol, glucose and obesity, along with physical activity, smoking cessation and a healthy dietary pattern.

Conditions that receive extra attention in younger adults

Cervical artery dissection

A tear in the wall of an extracranial carotid or vertebral artery can create a clot that travels to the brain. AHA/ASA describes dissection as a mechanism relevant mostly to younger individuals. It may follow trauma, sudden neck movement or a medical procedure, but it can also occur spontaneously. Neck pain, headache or unusual neurologic symptoms may precede the stroke, so the history of recent injury or neck strain matters even when the event seemed minor.

Inherited or acquired clotting tendencies

Hypercoagulable states make blood more likely to clot. Doctors do not order every thrombophilia test for every patient: testing is selected when the stroke is unexplained or when the history raises suspicion. Clues include a personal or family history of unprovoked deep-vein thrombosis or pulmonary embolism, repeated pregnancy loss, clotting at an unusual site or systemic features suggesting an autoimmune or inflammatory disorder. Results must be interpreted in context because an abnormal laboratory finding does not automatically prove it caused the stroke, and treatment depends on the overall clinical situation.

Cerebral venous thrombosis

CVT is a clot in the veins or venous sinuses that drain the brain. The AHA reports that CVT accounts for about 0.5% to 3% of all strokes globally and has its highest rates in younger women of childbearing potential. Headache is common, but seizures, visual symptoms, confusion or focal weakness can also occur. Pregnancy, the postpartum period, estrogen exposure, dehydration, infection, inflammatory disease and inherited clotting tendencies may all influence the evaluation.

Migraine, particularly migraine with aura

Migraine is not proof that a stroke occurred or that it caused one. Nevertheless, clinicians record migraine type, aura symptoms, frequency and changes from the person’s usual pattern. In an AHA summary of a Colorado analysis, nontraditional factors were associated with 43% of strokes in women younger than 35 and 31% in men younger than 35; migraine was associated with nearly 35% of strokes in women and 20% in men in that analysis. Those are study-specific associations, not an individual prediction. A sudden, new or unusually severe neurologic episode should be treated as possible stroke rather than assumed to be migraine.

Pregnancy, the postpartum period and estrogen exposure

Pregnancy and the weeks after delivery change blood pressure, coagulation and circulation. Hypertensive disorders of pregnancy, including preeclampsia, are important clues in a young woman with stroke. Doctors also ask about recent delivery, miscarriage, fertility treatment and complications. Estrogen-containing birth-control pills and other estrogen exposures are considered alongside smoking, migraine with aura, hypertension and personal or family clotting history; the combination of factors matters more than any single item.

Patent foramen ovale and other heart findings

A PFO is a passage between the heart’s upper chambers that persists in some adults. It can allow a venous clot to cross into arterial circulation, but its presence alone does not establish causation because PFOs are relatively common. In a younger person with an otherwise unexplained embolic-appearing stroke, clinicians may use echocardiography and additional testing to decide whether the PFO is likely relevant and whether another source of clot is more plausible.

Autoimmune, kidney, cancer and blood disorders

Systemic lupus and other autoimmune diseases can inflame vessels or increase clotting. Kidney disease can alter vascular risk and coagulation. Cancer may produce a prothrombotic state. Sickle-cell disease can impair blood flow and is a recognized stroke risk. Family history of early stroke or unexplained clotting may prompt evaluation for inherited disorders. These conditions are not diagnosed from age alone; symptoms, examination and targeted tests determine whether they belong in the work-up.

What doctors do after a stroke in a young adult

The evaluation is usually staged so urgent treatment is not delayed while less common causes are investigated.

  1. Confirm the event and type: Emergency brain imaging, typically computed tomography or magnetic resonance imaging when available, distinguishes bleeding from an ischemic injury and can reveal the location and pattern.
  2. Map the blood vessels: CT or MR angiography can look for a blocked artery, aneurysm, vascular malformation or cervical dissection. When venous disease is suspected, venous imaging is added.
  3. Reconstruct the timeline and exposures: Doctors ask when symptoms began, whether there was neck trauma, infection, severe headache, pregnancy or recent delivery, medication use, estrogen exposure, smoking, alcohol or recreational-drug exposure, and whether similar episodes occurred before.
  4. Check ordinary vascular risks: Blood-pressure measurements, glucose or diabetes testing, a lipid profile, weight and smoking history identify treatable contributors. Kidney function and blood counts can add important context.
  5. Search for a cardiac source: An electrocardiogram and heart-rhythm monitoring look for atrial fibrillation or another arrhythmia. Echocardiography evaluates heart structure, valve disease, clot and, when appropriate, a PFO.
  6. Order targeted blood tests: Depending on the presentation, clinicians may investigate autoimmune disease, selected thrombophilias, sickle-cell disease, infection or cancer. Broad panels without a clinical reason can produce misleading results, so specialists interpret these tests with the timing of the stroke and any medications in mind.
  7. Plan follow-up: If the initial studies do not identify a mechanism, the team reviews results, monitors for intermittent rhythm disorders and reassesses new information rather than closing the case after one normal test.

What “cryptogenic stroke” means

“Cryptogenic” means no cause was identified after the initial evaluation; it does not mean that no cause exists. The label describes the state of knowledge, not a unique disease. The AHA’s 2025 SECRETO summary of unexplained strokes in people under 50 highlighted nontraditional factors such as chronic kidney disease, cancer, venous clots and migraine with aura, with patterns differing according to PFO status. A cryptogenic diagnosis therefore still warrants a structured review of arteries, the heart, blood clotting, medications and systemic disease.

Prevention after the evaluation

Prevention is individualized according to the mechanism found. A clinician may prescribe blood-pressure, cholesterol, diabetes or rhythm treatment, antithrombotic therapy, or another intervention when indicated; the correct choice depends on stroke type and test results. Do not start aspirin, anticoagulants or supplements on your own.

  • Keep scheduled primary-care and neurology visits, even when you feel fully recovered.
  • Measure and treat blood pressure; ask how often it should be checked at home.
  • Address cholesterol, glucose and weight with a plan you can sustain.
  • Stop smoking and discuss nicotine treatment rather than switching products without support.
  • Build regular physical activity gradually and follow rehabilitation guidance if deficits remain.
  • Use a Mediterranean-style eating pattern, which the 2024 AHA/ASA prevention guideline recommends for adults without prior cardiovascular disease.
  • Review contraception, pregnancy planning and migraine treatment with the relevant clinician, especially if aura, smoking, hypertension or prior clotting is present.
  • Limit alcohol and avoid illicit stimulants, which can sharply affect blood pressure and vessels.

Recognize symptoms and act immediately

Use FAST: Face drooping, Arm weakness, Speech difficulty, Time to call emergency services. Also seek emergency care for sudden vision loss, numbness or weakness, severe headache, confusion, trouble walking, dizziness or loss of balance. Note the last time the person was known to be well, keep medications available for responders and do not drive the person yourself unless emergency services are unavailable. Symptoms that resolve can represent a transient ischemic attack and still require urgent assessment.
