Epilepsy is a brain disease that creates an ongoing tendency to have seizures. Its causes include structural brain injury or malformation, genetic conditions, infections, metabolic or immune disorders, and— in about half of cases worldwide—no identified cause. A single seizure does not automatically mean epilepsy. Clinicians make the diagnosis from the event history and examination, then use tests such as EEG and MRI to support the diagnosis, classify seizures and look for an underlying cause. Antiseizure medicine is the usual first treatment; surgery, implanted stimulation devices or a supervised ketogenic diet may help selected people whose seizures remain uncontrolled.
The information below explains the distinctions and decisions involved. It is general education, not an individual diagnosis or treatment plan.
What epilepsy is—and what one seizure means
A seizure is a temporary episode of abnormal electrical activity in the brain. Epilepsy is the disease diagnosis: a continuing predisposition to have unprovoked seizures. The terms are related, but they are not interchangeable.
The International League Against Epilepsy (ILAE) practical definition allows a diagnosis when any of these applies:

- At least two unprovoked seizures occurring more than 24 hours apart.
- One unprovoked seizure plus a sufficiently high probability of further seizures—comparable to the recurrence risk after two unprovoked seizures.
- Diagnosis of a recognized epilepsy syndrome.
An event caused by an immediate, correctable problem—such as low blood sugar—may be classified differently from an unprovoked seizure. That is why clinicians investigate what happened before, during and after an event rather than diagnosing epilepsy from the episode alone.
The World Health Organization (WHO) estimates that about 50 million people worldwide have epilepsy (2024). It also states that up to 10% of people worldwide have one seizure during their lifetime; “one seizure does not signify epilepsy.”
What causes epilepsy?
WHO groups causes into six broad categories. More than one factor can be relevant, and testing may not identify a cause even when epilepsy is genuine. WHO reports that the cause remains unknown in about 50% of cases globally.

|---|---|---|
| Structural | Brain damage before or around birth, congenital brain abnormalities, severe head injury, stroke or a brain tumor | An injury or physical change alters networks that can generate seizures. MRI is often used to look for such abnormalities. |
| Genetic | Inherited or presumed genetic conditions and genetic syndromes associated with epilepsy or brain malformations | A genetic contribution does not necessarily mean a parent has epilepsy; the pattern and syndrome determine whether genetic evaluation is useful. |
| Infectious | Meningitis, encephalitis or neurocysticercosis | Infection can injure brain tissue or provoke inflammation that lowers the seizure threshold. |
| Metabolic | Disorders of the body’s chemistry, including low blood sugar as a potential seizure trigger | Blood tests and treatment of the disturbance help distinguish an acute symptomatic event from an enduring epilepsy tendency. |
| Immune | Immune-mediated inflammation affecting the brain | The clinical picture and specialist testing determine whether an immune cause is plausible. |
| Unknown | No cause identified after appropriate assessment | “Unknown” does not mean the seizures are imaginary or untreatable; it describes the current limit of what testing can establish. |
These categories describe causes of epilepsy, not a checklist that every person must match. Some people have a clear stroke or malformation; others have a recognized syndrome; many never receive a specific explanation.

How doctors evaluate a suspected first seizure
Diagnosis starts with reconstructing the event. NICE guideline NG217 recommends a detailed account from the person and, when appropriate, family members or carers. An eyewitness description or a video recorded safely on a phone can clarify movements, awareness, duration and recovery. The clinician also performs a physical examination and asks about previous episodes, medical history and possible triggers.
Checking for conditions that can mimic or provoke a seizure
Not every collapse, shaking episode or period of altered awareness is epilepsy. NICE recommends a 12-lead electrocardiogram (ECG) after a suspected first seizure because certain heart conditions can cause fainting or other events that resemble an epileptic seizure. Clinicians also consider metabolic disturbances, including low blood sugar, and test or correct them when appropriate.
The ECG and metabolic assessment do not “prove” epilepsy. Their purpose is to find a dangerous mimic or an immediate cause that changes urgent management and the way the event is classified.

Which tests are used?
EEG: a supporting test, not a rule-out test
An electroencephalogram (EEG) records electrical activity from the scalp. When a person has a suspected seizure, EEG can support the clinical diagnosis and provide information about seizure type or an epilepsy syndrome.

A routine EEG may be normal even when someone has epilepsy. NICE’s explicit recommendation is: “Do not use EEG to exclude a diagnosis of epilepsy.” A normal recording therefore cannot overrule a convincing history or examination.
If an EEG is requested after a first seizure, NICE recommends performing it as soon as possible, ideally within 72 hours. If the routine study is normal and uncertainty remains, clinicians may consider a sleep-deprived EEG. If doubt persists, ambulatory EEG monitoring for up to 48 hours may capture abnormalities that a short routine test misses.
MRI and CT: looking for structural causes
NICE recommends offering MRI to children, young people and adults who have been diagnosed with epilepsy, with exceptions for specified syndromes such as idiopathic generalised epilepsy and self-limited epilepsy with centrotemporal spikes. MRI looks for structural abnormalities that could explain the seizures and influence treatment decisions.
When MRI is contraindicated, CT may be considered. Imaging findings are interpreted alongside the history, examination and EEG; a scan is not an independent substitute for the clinical diagnosis.

| Assessment | Main purpose | Important limitation |
|---|---|---|
| History, examination and eyewitness account | Establish what happened, identify seizure features and assess alternative explanations | Descriptions can be incomplete, especially when nobody witnessed the event. |
| 12-lead ECG | Detect heart problems that can resemble an epileptic seizure | It evaluates cardiac causes, not brain electrical activity. |
| Metabolic evaluation | Find disturbances such as low blood sugar that can provoke an event | Correcting an acute disturbance does not by itself establish chronic epilepsy. |
| EEG | Support diagnosis and help classify seizure type or syndrome | A normal routine EEG cannot rule epilepsy out. |
| MRI | Identify structural brain abnormalities and possible causes | Imaging must be interpreted in clinical context; some syndromes do not require the same imaging pathway. |
| CT | Alternative structural imaging when MRI cannot be performed | It is used selectively rather than as a universal replacement for MRI. |
How the diagnosis is reached
No single test independently establishes every case. A clinician combines the event description, examination, evidence for or against a mimic, EEG findings when available, and imaging results. The final diagnosis also includes the seizure type and, when possible, the epilepsy syndrome and cause.
This approach explains two common situations:
- One dramatic event with a normal EEG: epilepsy is not automatically excluded. The clinician may seek more history, arrange sleep-deprived or ambulatory EEG, and reassess other causes.
- An abnormal EEG without a compatible event history: an EEG finding must still be interpreted clinically; it is not a diagnosis in isolation.
Because recurrence risk and treatment differ by seizure type, syndrome and cause, a specialist may refine the diagnosis after the initial assessment rather than treating every seizure pattern identically.

Antiseizure medicines
Antiseizure medication is the most common first-line treatment. The choice depends on seizure type or syndrome, suspected cause, other medicines and health conditions, potential adverse effects, and the person’s circumstances. It can take time to find a medicine and dose that control seizures without unacceptable side effects. Some people need a combination of medicines.

WHO estimates that up to 70% of people with epilepsy could become seizure-free with appropriate antiseizure medication. This is a population estimate, not a guarantee for an individual. Response, tolerability and the diagnosis itself determine the next step.

When seizures continue despite medication
Persistent seizures despite appropriate treatment warrant specialist review. Depending on the seizure type, suspected cause and prior medication response, options can include:
| Option | When it may be considered | Key decision factors |
|---|---|---|
| Epilepsy surgery | Selected people whose seizures arise from a region that can be treated safely and whose seizures remain uncontrolled | Seizure focus, expected control, surgical risks, brain function and personal circumstances |
| Vagus nerve stimulation | Selected drug-resistant epilepsy cases | Seizure pattern, prior treatment response, device benefits and procedure-related considerations |
| Responsive nerve stimulation | Selected cases in which a responsive implanted device is clinically appropriate | Seizure focus, suitability for implantation and expected benefit |
| Ketogenic diet | Some people whose seizures are not adequately controlled with medication | Strict adherence, nutritional safety and supervision by healthcare professionals and a dietitian |
These approaches are not interchangeable and are not suitable for everyone. A clinician weighs seizure control against side effects, procedural risks, dietary demands and the person’s goals.
Can medication be stopped?
Reducing or stopping antiseizure medication is a clinician-led decision. It depends on the seizure history, epilepsy syndrome, examination and test results, social circumstances and the risk that seizures will return. WHO notes that discontinuation can be considered after two seizure-free years when relevant clinical, social and personal factors have been reviewed. Two seizure-free years is not an automatic stopping rule, and medication should not be changed without medical guidance.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteHow treatment decisions are individualized
At each review, clinicians use the same core questions:
- What seizure type or syndrome best fits the events?
- Is there a structural, genetic, infectious, metabolic, immune or unknown cause?
- How well has each medicine controlled seizures, and what side effects occurred?
- Would surgery, stimulation or a supervised ketogenic diet offer a reasonable benefit?
- How do work, school, driving, pregnancy plans, other illnesses and personal preferences affect the safest plan?
This framework prevents a normal EEG from falsely ruling epilepsy out, avoids treating an acute metabolic event as lifelong epilepsy without context, and keeps advanced treatments focused on people most likely to benefit.
Quick Recap
Key points to remember
- One seizure is not automatically epilepsy; the ILAE definition also considers recurrence risk and epilepsy syndromes.
- Causes span structural, genetic, infectious, metabolic, immune and unknown categories. WHO says the cause is unknown in about half of cases globally.
- History and examination establish the clinical picture. ECG and metabolic assessment help identify mimics or immediate contributors.
- EEG supports diagnosis and classification, but a normal routine EEG does not rule epilepsy out. NICE suggests sleep-deprived and then ambulatory EEG when uncertainty remains.
- MRI is used to look for structural abnormalities, with CT considered when MRI is contraindicated.
- Medication is common first-line care; surgery, implanted stimulation and a supervised ketogenic diet are options for selected people with ongoing seizures.
