Successfully removing the intended protein does not guarantee that a degrader is safe. Toxicity can arise because the target also performs an essential job in healthy tissue, because the recruited degradation machinery removes other proteins, or because exposure, tissue distribution, persistence and degradation kinetics produce harmful effects. A target-depletion assay confirms that the intended protein fell; it does not prove that degradation was selective or identify the cause of an adverse event.

To explain a toxicity signal, investigators must connect dose and tissue exposure with depletion of the target or other proteins, disruption of a normal biological function and the observed phenotype. That requires compound-specific, time-aware evidence rather than target engagement alone.

Why confirmed target loss does not prove safety

Targeted protein degraders work by inducing or stabilizing an interaction between a protein of interest and cellular destruction machinery. In the ubiquitin–proteasome pathway, an E3 ubiquitin ligase brings the target into productive proximity, attaches ubiquitin chains and directs the protein to the proteasome. A PROTAC uses separate target-binding and ligase-binding portions connected by a linker. A molecular glue is smaller and promotes or stabilizes a new protein–protein interface.

When a laboratory assay shows that the intended target has been depleted, it answers one narrow question: did the amount of that protein decrease under those conditions? It does not answer whether the same compound depleted other proteins, whether the target was removed in an unintended organ, whether depletion lasted too long, or whether the measured target loss explains a clinical symptom.

Three mechanisms can therefore coexist:

  • The intended protein can be removed in a healthy tissue where it has a normal, essential function.
  • The recruited ligase can be redirected toward unintended substrates, sometimes called neosubstrates.
  • Exposure and event-driven pharmacology can make the depth, location or duration of degradation harmful even when the target is the correct one.

The same adverse event may involve more than one mechanism. For example, a compound could deplete its intended target in tumor cells while also depleting a neosubstrate in a normal organ.

The main toxicity mechanisms

Mechanism What happens Evidence that helps distinguish it Why target-depletion data miss it
On-target, off-tissue toxicity The intended protein is depleted in healthy tissue where it has a physiological role. Target depletion in the affected tissue, a plausible normal function, exposure at that site and a matching tissue phenotype. The assay may use tumor cells or another tissue and may not measure normal-organ effects.
Unintended substrate degradation The recruited E3 ligase recognizes and removes proteins other than the intended target. Unbiased proteomics, confirmation of the candidate substrate, tissue expression and a phenotype that follows its loss. A single-target immunoassay does not survey the rest of the proteome.
Context-dependent selectivity Ternary-complex geometry, protein abundance, competition and kinetics change which proteins are degraded. Experiments across relevant tissues, ligase levels, doses and time points. Selectivity in one cell line or at one time point may not hold elsewhere.
Exposure and persistence Drug concentration, distribution or prolonged downstream effects create toxicity despite correct target engagement. Pharmacokinetic/pharmacodynamic profiles, recovery studies and exposure–response relationships. Target loss is a pharmacodynamic observation, not a complete description of exposure or recovery.

On-target toxicity in healthy tissue

The target may have a normal job outside the disease

A protein selected because it supports cancer-cell survival, proliferation or signaling may also be needed by healthy cells. Degrading it in those cells is “on-target” at the molecular level, but the effect is harmful because the drug reaches tissue that was not intended to be depleted. The relevant question is not simply whether the target is present; it is whether the target’s normal function is important in the exposed tissue and whether the amount and duration of depletion exceed that tissue’s tolerance.

Tissue distribution determines where the mechanism operates

Two tissues can express the same target yet experience different effects because drug concentrations, uptake, protein turnover and recovery differ. The recruited E3 ligase matters as well. A tissue with little of the ligase may degrade less target even when the target itself is abundant. Conversely, high ligase expression can make a tissue especially sensitive.

A review of targeted protein degradation describes platelet exposure as an example: lower expression of the von Hippel–Lindau (VHL) ligase in platelets has been associated with reduced platelet toxicity for a BCL-XL-targeting PROTAC. This illustrates a general principle, not a guarantee for every VHL-recruiting compound: ligase distribution can shape the safety window.

How to recognize an on-target, off-tissue signal

An on-target explanation becomes more credible when the intended protein is measurably depleted in the affected tissue, the tissue normally depends on that protein, the phenotype is consistent with loss of its function and the effect tracks with exposure and recovery. A lack of target depletion in the affected tissue weakens, but does not automatically eliminate, the hypothesis; sampling may be incomplete, the relevant cell type may be rare, or the biological effect may persist after the protein begins to return.

Unintended protein degradation and neosubstrates

Recruiting an E3 ligase changes its recognition landscape

Degraders do not merely bind a target and switch on a universal disposal system. They create a new molecular context in which the recruited ligase can recognize proteins that it did not efficiently process before. Those unintended proteins are often called neosubstrates. Their degradation can produce toxicity even when the intended target is removed exactly as designed.

This risk is particularly well documented for compounds that recruit cereblon (CRBN), an E3-ligase component. CRBN-recruiting molecular glues and PROTACs can alter the proteins presented to the ligase. Reported examples of neosubstrates for some CRBN-based degraders include IKZF1 and SALL4. The existence and importance of a neosubstrate are compound-specific: a finding with one chemical structure cannot automatically be assigned to every CRBN degrader.

What the thalidomide example shows

Thalidomide and related immunomodulatory imide drugs provide a mechanistic example of how unintended degradation can cause harm. Research has identified SALL4 as a thalidomide-dependent CRBN substrate and linked SALL4 degradation to teratogenicity. Reviews describe CRBN-mediated neosubstrate degradation, including SALL4, as part of the explanation for thalidomide-related birth defects.

This example supports a specific causal chain for the compounds and phenotypes studied. It does not establish that every adverse event from a CRBN-recruiting degrader is caused by SALL4, nor that a degrader recruiting another ligase will share the same liability. Attribution requires evidence for the actual compound, substrate, tissue and phenotype.

Why degradation selectivity changes with tissue and time

Ternary-complex geometry matters more than affinity alone

Binding affinity between a degrader and its target is not sufficient to predict degradation. Productive ternary-complex formation must place the target and ligase in a geometry that permits ubiquitination. A compound can bind tightly yet form an unproductive complex, while a weaker binder can degrade efficiently if its geometry and kinetics are favorable.

Protein abundance and competition alter the outcome

The amount of target, ligase and potential neosubstrates varies among cell types. Substrates can compete for a limited pool of ligase, and changes in expression can shift which protein is preferentially ubiquitinated. Molecular context, including complex formation and accessibility of the relevant surface, also affects the result. Consequently, a selectivity profile measured in one cell line is not automatically a universal profile across organs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Time and the event-driven nature of degradation

Degraders initiate an event that can outlast the original binding interaction. Once a protein has been ubiquitinated and destroyed, the cell must synthesize new protein before function returns. Repeated dosing or slow resynthesis can therefore create prolonged pharmacology even when plasma drug levels fall quickly. Early and late time points may show different sets of depleted proteins.

The hook effect and exposure extremes

At high concentrations, some bifunctional degraders show a “hook effect”: excess compound occupies target or ligase sites in separate, nonproductive complexes and reduces ternary-complex formation. Dose escalation can therefore produce a non-linear relationship between concentration and degradation. Pharmacokinetic/pharmacodynamic modeling must account for this possibility rather than assuming that more exposure always means proportionally more target loss.

How to investigate a toxicity signal

A practical investigation should proceed from the affected tissue and phenotype, not from an assumption that confirmed target engagement settles the question.

  1. Define the phenotype precisely. Record the organ, cell type, timing, severity and reversibility of the adverse effect. Distinguish a laboratory biomarker from tissue injury or functional impairment.
  2. Measure exposure at the relevant site. Relate dose to plasma and tissue concentrations, active metabolites, dosing interval and duration. Include pharmacokinetic/pharmacodynamic measurements rather than relying on a single end-of-study concentration.
  3. Check intended-target depletion in the affected tissue. Use a validated quantitative assay and sample the cell population that could produce the phenotype. Measure both depth of depletion and recovery after dosing stops.
  4. Survey other proteins without prespecifying the answer. Unbiased proteomics can identify depleted proteins that a single-target assay would miss. Follow promising candidates with targeted assays and confirm that the change depends on the degrader and the recruited ligase.
  5. Test biological plausibility. Ask whether the target or candidate neosubstrate is expressed in the affected tissue and whether its known function fits the phenotype. Compare tissues with different target and ligase abundance.
  6. Use matched perturbation experiments. Where feasible, compare systems with and without the relevant E3 ligase, or with the candidate substrate genetically reduced or protected. Rescue experiments that restore the protein or block its degradation strengthen causal attribution.
  7. Align molecular and clinical timelines. Determine whether protein loss precedes the phenotype, whether both follow exposure, and whether recovery of the protein parallels recovery of function. A mismatch may indicate an indirect pathway or a different toxic mechanism.

The 2024 mechanistic review calls for thorough targeted and unbiased characterization of off-target degradation before clinical investigation. Regulatory development guidance also emphasizes proteomics-based profiling and PK/PD assessment for event-driven pharmacology.

What evidence is strong enough to assign causality?

Evidence level What it establishes What remains uncertain
Intended target is depleted The degrader engages and reduces the measured target under the tested conditions. Whether other proteins are affected or whether target loss causes the toxicity.
Target depletion in the affected tissue The on-target, off-tissue hypothesis becomes plausible. Whether target loss, rather than another depleted protein, drives the phenotype.
Proteomic change in a candidate neosubstrate An unintended degradation event is documented. Whether that protein’s loss is sufficient to produce the observed adverse event.
Ligase dependence and genetic rescue The molecular event and phenotype are linked to the recruited ligase or substrate. How the relationship translates across species, doses and patient populations.
Exposure, timing and recovery alignment The proposed mechanism fits the drug’s PK/PD behavior and the clinical course. Rare effects or interactions not represented in the tested models.

No single experiment assigns every toxicity to one mechanism. The most persuasive explanation combines tissue-aware exposure data, depletion measurements, proteomics, genetic perturbation and a matching functional phenotype.

Why model and species results can mislead

A toxicity model may fail to reveal a liability if it lacks the relevant ligase, target or neosubstrate. Expression can differ between a human organ and the animal or cell line used for testing. Even when a protein is present, its abundance, binding context and resynthesis rate may not match the human situation.

Conversely, a finding in one model should not be generalized without checking whether the same molecular components are present in the tissue of concern. Species comparisons should document target and ligase expression, candidate-substrate conservation and the exposure achieved, rather than treating a negative result as proof of safety.

A framework for comparing degrader candidates or mechanisms

When two compounds or proposed explanations must be compared, evaluate them on the same axes:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Target and normal-tissue function: Is the intended protein present in the affected healthy tissue, and what physiological role does it serve there?
  • Ligase and recruiter biology: Which E3 ligase is recruited, where is it expressed, and are compound-induced neosubstrates known or suspected?
  • Degradation selectivity: Is there evidence for productive ternary-complex formation and selective ubiquitination across relevant proteins and tissues?
  • Exposure and time: What dose, tissue concentration, degradation rate, dosing duration and recovery period accompany the effect?
  • Evidence quality: Is the conclusion based only on target depletion, or does it include proteomics, genetic perturbation, rescue and a matching tissue phenotype?

These are mechanistic comparison axes, not a validated clinical scoring system. They help prevent a common error: treating successful removal of the intended target as proof that every downstream effect is on-target and safe.

The practical answer

Degrader toxicity after successful target removal usually reflects one of three realities: the target is needed in healthy tissue, the degrader removes additional proteins, or the drug’s exposure and persistence make an otherwise intended event harmful. CRBN-associated neosubstrate biology, including the SALL4 example linked to thalidomide teratogenicity, demonstrates how unintended degradation can matter, but it is not a universal explanation for all degraders or all adverse events.

For a particular compound, the cause can be assigned only by matching the phenotype to tissue exposure, intended-target and off-target depletion, ligase biology, timing and functional rescue. Without that compound-specific evidence, “the target was successfully degraded” is an engagement result—not a toxicity diagnosis.
