A temple-mounted wearable is defined by where it sits, not by a single measurement. Depending on its sensors and software, it may detect a reflected-light pulse waveform, skin temperature and heat flow, electrical contact, or motion. It can then display heart rate, blood oxygen, an estimated core temperature, a wear-position signal, or a proprietary relative index. Those outputs are not interchangeable: a pulse waveform is a sensor signal, an estimated temperature is an algorithmic calculation, and a relative hemodynamic index is not an absolute measurement of brain blood flow.

What “temple-mounted” tells you—and what it does not

The temple is a placement site over tissue that includes superficial blood vessels and a relatively stable area of the head. That location can be useful for optical, thermal, capacitive, and motion sensors, but the location alone does not establish what a device measures or how accurate it is.

Start by separating three layers:

  • Sensed input: reflected light, skin temperature, heat flux, electrical contact, acceleration, or another raw signal.
  • Calculated metric: heart rate, oxygen saturation, temperature estimate, contact status, or movement classification derived from the input.
  • Inferred index: a proprietary score or trend that combines several signals and may be normalized to the wearer’s own baseline.

Two products mounted in the same place can therefore perform entirely different jobs. A clinical temperature monitor, an overnight wellness sensor, and a research optical hemodynamic device should not be treated as substitutes.

What a temple wearable may measure

Optical pulse and pulse-derived metrics

Reflectance photoplethysmography (PPG) shines light into tissue and measures changes in the light returned to the sensor. Blood volume changes with each pulse, altering the reflected signal. Algorithms can use that waveform to calculate pulse rate or heart rate and to extract waveform features.

Some systems use multiple wavelengths. A 2026 preprint on the BrainFlow wearable describes green, red, and infrared temple PPG. Its software combines optical perfusion, hemoglobin-related changes, and pulse-waveform morphology into BrainFlow, a dimensionless index normalized to each participant’s baseline. That makes it a relative trend measure, not an absolute cerebral-blood-flow value.

Do not relabel a PPG-derived index as direct brain oxygen, neural activity, or blood flow unless the specific device has demonstrated that exact output against an appropriate reference. BrainFlow has been compared with transcranial Doppler (TCD) and, in a separate summary, depth-resolved near-infrared spectroscopy (NIRS). Those comparisons can show whether the index changes in a similar direction under tested conditions; they do not convert it into a direct, absolute measurement.

Overnight heart rate, oxygen saturation, and inferred patterns

A current product page from Oralable describes a temple sensor intended to provide overnight heart-rate and blood-oxygen monitoring, along with inferred “quiet,” “tonic,” and “phasic” patterns. The page labels the system as a wellness device, says it is not intended to diagnose, and states that clinic kits have not shipped. The page does not provide independent accuracy statistics for those features. Treat the descriptions as the manufacturer’s feature claims, not as clinical validation.

DeRoyal’s intended-use wording is: “The Temple Touch Pro System is intended to measure and monitor core body temperature of all patients of all ages, by applying the sensor unit on the region over the temporal artery.” That is a manufacturer labeling statement. It describes intended use, not proof that every age group, environment, or home-use situation will perform identically.

Contact, fit, and activity

A temple sensor can also determine whether a device is being worn correctly. In a smart-glasses proof of concept, capacitive electrodes were used to detect contact and wear position, while motion sensors helped classify activity. The investigators found that electrode size and physical fit mattered; sensitivity could deteriorate with different hairstyles and head shapes. Temperature sensors in that design reacted too slowly and were too affected by ambient temperature to provide rapid wear-position feedback. Those findings apply to that prototype and task, not automatically to all smart glasses or head-worn devices.

How reliable are the readings?

Reliability is metric-specific and condition-specific. A device may track heart-rate changes reasonably well while producing a poor temperature estimate in a cold room, or show a useful personal trend without being suitable for a clinical diagnosis. The relevant question is always: reliable for which output, compared with what reference, in which population and conditions?

Temple Touch Pro results in 2022

A prospective study of 100 adults under general anesthesia compared Temple Touch Pro readings with distal esophageal temperature. The mean bias was −0.04°C, with 95% limits of agreement from −0.99°C to +0.91°C.

The near-zero average bias means that the device was not systematically high or low on average in that sample. It does not mean every individual reading was close to the reference. The limits of agreement show the expected spread of individual differences, and that spread was wider than the 2017 estimate. Reporting both studies is more informative than reducing the evidence to “accurate within half a degree.”

TCD measures velocity in the middle cerebral artery; BrainFlow is a baseline-normalized relative index. A correlation indicates that values moved together within the tested participants and tasks. It does not establish that the wearable reports an interchangeable velocity, an absolute flow rate, brain oxygen level, disease status, or treatment response. The preprint also identifies uncontrolled between-session influences, including time of day and room temperature. Its small, healthy-adult sample and preprint status limit how broadly the result can be applied.

Correlation, bias, and agreement are different

Performance claims become easier to interpret when the statistics are separated:

Statistic Question it answers What it cannot prove
Correlation Do the wearable and reference tend to rise and fall together? That individual readings are close enough to substitute for the reference.
Bias Is the average wearable reading higher or lower than the reference? How large the errors are for individual people.
Limits of agreement How far individual differences may spread around the average difference? That every result falls inside a clinically acceptable threshold.
Concordance correlation Do values both correlate and lie near the line of equality? Performance outside the studied population, device version, or conditions.

A strong correlation can coexist with clinically important error. For a product comparison, look for the named reference instrument, participant count and health status, tested activity, error or agreement statistics, and the exact algorithm or software version.

Why placement and environment change results

Fit and contact

Optical sensors need stable contact and a usable optical path. A loose temple device can admit ambient light or change the distance and angle between emitter and skin. Hair, head shape, frame pressure, and the precise location over the temple can alter contact. Capacitive wear detection is especially dependent on electrode geometry and skin contact.

  • Hard hat clip- attaches for easy access

Movement

Head movement, walking, cycling, and facial motion can introduce artifacts into optical waveforms. Algorithms may reject noisy segments, smooth them, or report a value with reduced confidence. A reading obtained while sitting still should not automatically be expected during vigorous movement.

Ambient temperature

Thermal estimates depend on heat transfer between skin, sensor, and surroundings. A manufacturer may claim that a system is unaffected by ambient or warming conditions, but that remains a product claim unless independent testing demonstrates it for the intended use. In the smart-glasses prototype, temperature sensors were too slow and ambient-sensitive for quick fit feedback.

Time and session effects

Physiology changes with time of day, posture, activity, hydration, and room conditions. The BrainFlow/TCD preprint notes time of day and room temperature as relevant uncontrolled sources of variation. A personal baseline can make a relative index useful for detecting change, but it also means a value from one session may not be directly comparable with another if conditions differ.

How to judge a temple wearable before relying on it

  1. Identify the raw sensor. Determine whether the device uses PPG, thermal sensing and heat flux, capacitive contact, motion sensing, or a combination.
  2. Name the displayed output precisely. Is it heart rate, oxygen saturation, an estimated temperature, contact status, or a proprietary relative score?
  3. Find the reference method. Stronger validation names a comparator such as ECG or a chest strap for heart rate, esophageal or nasopharyngeal temperature for core temperature, or TCD/NIRS for hemodynamic comparisons.
  4. Check the population and task. Healthy volunteers at rest, athletes in motion, sleeping users, and anesthetized surgical patients are different validation groups.
  5. Read agreement statistics, not only a headline correlation. Look for bias, limits of agreement, absolute error, confidence intervals, and the proportion inside a clinically meaningful range.
  6. Confirm the device and software version. A validation result for one hardware revision or algorithm may not apply to a later consumer release.
  7. Match the claim to the decision. A trend monitor may help you notice change, while a diagnostic or treatment decision requires an appropriately validated medical instrument and professional interpretation.

What each evidence category supports

Device or output Evidence supports Do not infer
Temple PPG heart-rate output Pulse-related changes can be detected optically; accuracy depends on fit, movement, and algorithm. That every temple PPG device is clinically interchangeable with ECG.
Temple Touch Pro temperature estimate Agreement with internal temperature references was measured in surgical-anesthesia studies. Identical accuracy in home, exercise, pediatric, febrile, or extreme-temperature use without supporting data.
BrainFlow index A relative, baseline-normalized signal tracked the direction and time course of TCD velocity in a small healthy-adult preprint study. Absolute cerebral blood flow, brain oxygen, neural activity, disease screening, or treatment guidance.
Smart-glasses contact sensing Capacitive and motion signals can identify wear position and activity in a prototype. That fit detection will work equally well across hairstyles, head shapes, and commercial designs.
Oralable overnight features The manufacturer describes heart-rate, blood-oxygen, and inferred overnight patterns as wellness features. Independent clinical accuracy or a diagnostic sleep-study result.

Practical use and failure checks

If the value jumps or disappears

  • Re-seat the device at the intended temple location and ensure the sensor is flush with skin.
  • Remove hair or accessories that interfere with contact, if the device instructions allow it.
  • Stop briefly and compare a stable, motion-free segment with the moving segment.
  • Check whether the app marks the interval as low quality, incomplete, or inferred.
  • Repeat under similar room temperature, posture, and time-of-day conditions before interpreting a trend.

If temperature does not match a clinical thermometer

Do not average the two readings or silently recalibrate one from a single comparison. Verify placement, allow the sensor to equilibrate, and follow the product’s measurement procedure. If a high or low temperature could affect urgent care, use the clinically appropriate reference method and seek medical advice rather than relying on a consumer estimate.

If a proprietary index is presented as a number

Ask whether the scale is absolute or normalized to a personal baseline, what reference was used, and what decision threshold has been validated. Without those details, interpret the value as a trend within that device and user, not as a universal physiological unit.

Bottom line for readers

A temple-mounted wearable can measure optical pulse signals, estimate temperature from skin and heat-flow measurements, detect contact and motion, or calculate a proprietary trend. The most defensible reliability claims are narrow: Temple Touch Pro studies found specific levels of agreement with internal temperature references during anesthesia; a 2026 small preprint found that BrainFlow tracked TCD velocity changes within healthy adults under selected challenges. Neither result validates every temple wearable or every displayed metric. Readings are most useful when the sensor, algorithm, reference standard, population, and conditions all match the question you are trying to answer.
