Short answer: Brain research can reveal patterns associated with depression in groups, but no brain scan is currently a reliable, routinely recommended way to diagnose major depressive disorder (MDD) in an individual. Diagnosis remains a clinical process based on symptoms, their impact, personal history, context, and the exclusion of plausible medical or psychiatric alternatives.
Imaging can still be important when a person has signs suggesting a neurological or structural problem. The key is to distinguish a research finding about populations from a clinically validated test that changes care for one patient.
What “diagnosing depression with a brain scan” would require
A diagnostic test must do more than find an association. To be useful in practice, a scan or other brain measurement would need to distinguish depression from other causes of similar symptoms in representative patients, work reliably in a new clinic, be compared with a sound clinical reference standard, and improve decisions or outcomes beyond a careful assessment.
Many neuroscience studies instead ask whether groups differ on average. A study might report a structural, functional, electrical, or chemical pattern among participants diagnosed with depression. That can help researchers understand biology or develop hypotheses. It does not establish that the same pattern identifies depression in one person, reveals its cause, or tells a clinician which treatment will work.

| Question to ask of a brain-test claim | What a clinically useful answer would require |
|---|---|
| Purpose | Is the method describing group differences, predicting an outcome, separating selected research groups, or diagnosing an individual in routine care? |
| Validation | Was it tested independently, in representative patients, against a defensible clinical reference standard? |
| Clinical utility | Does using it improve decisions or patient outcomes compared with a comprehensive assessment? |
| Alternative explanations | Could the result reflect medication, age, another illness, or the reason the scan was ordered rather than depression itself? |
| Scope | Does the evidence concern adults with MDD, or a different population, diagnosis, or experimental setting? |
The available clinical guidance addresses when testing is appropriate; it does not provide performance figures for competing MRI, functional MRI, EEG, blood-biomarker, or machine-learning approaches. It is therefore not responsible to attach an accuracy percentage or claim that one specific scan is ready for diagnosis.
What current clinical guidance says about neuroimaging
The 2023 CANMAT update on clinical guidelines for adult MDD states that evidence does not support routine ECG, EEG, or neuroimaging for people presenting with MDD. These tests should be ordered when clinical indications make them relevant, not as automatic confirmation of depression.
That recommendation is about clinical work-up, not a claim that the brain is unrelated to depression or that imaging has no scientific value. A scan can be the right investigation when the symptoms raise concern about another neurological or structural condition.

Situations in which imaging may be considered
CANMAT gives examples that should prompt clinicians to consider neuroimaging:
- Neurological signs on examination or in the history.
- New or persistent cognitive impairment.
- A sudden change in mood, behaviour, or personality.
- New-onset depression later in life, when a cerebral event or structural abnormality such as a tumour or metastasis needs to be ruled out.
In these situations, the scan is usually looking for an alternative explanation or a coexisting problem. A normal result would not, by itself, prove that depression is present or absent; an abnormal result would require clinical interpretation and appropriate follow-up.

How depression is diagnosed in practice
Depression is diagnosed through a structured clinical assessment rather than a single physical measurement. NICE’s assessment quality statement describes a comprehensive discussion of factors affecting the development, course, and severity of depression, allowing the clinician to confirm the diagnosis and plan care.
The NHS explanation of diagnosis describes the practical elements a general practitioner may cover:
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →- Current symptoms and general health.
- How feelings and symptoms affect work, relationships, sleep, and ordinary activities.
- Previous physical and mental-health history.
- Home circumstances, lifestyle, and recent events or possible triggers.
- Other explanations for the symptoms, including medical conditions or medication effects.
There is no physical test that confirms depression. A clinician may examine you or order urine or blood tests to rule out conditions that can produce similar symptoms. Those tests are for differential diagnosis, not a laboratory confirmation of MDD.
A typical assessment sequence
- Describe the problem. Explain what has changed, when it began, how persistent it is, and which parts of life are affected. Include sleep, appetite, energy, concentration, pleasure, movement, and thoughts of death or self-harm where relevant.
- Review history and context. Discuss previous episodes, treatments, family history, major stressors, substance use, medical illnesses, and medicines.
- Consider competing explanations. The clinician evaluates whether symptoms could be better explained by another mental-health condition, a physical illness, a substance, or a medication.
- Use questionnaires only as aids. A symptom scale can organize information and track change, but it does not replace diagnostic judgement.
- Choose targeted tests. Examination, laboratory tests, or imaging are selected when the history or examination provides a reason to investigate a specific alternative.
Why a positive depression questionnaire is not a diagnosis
Questionnaires are useful for identifying symptoms and measuring change over time. They apply standardized questions and can help a busy service notice people who need a fuller conversation. However, a score above a cutoff is not proof of MDD.
CANMAT advises that a positive screen should be followed by a full diagnostic assessment. Symptoms such as low mood, fatigue, poor concentration, or sleep change can occur in several psychiatric and medical conditions. The clinician must establish the pattern, duration, severity, functional impact, and appropriate diagnosis before treatment decisions are made.

What the 2025 Canadian screening recommendation does—and does not—say
The Canadian Task Force on Preventive Health Care’s 2025 update addresses a different question: whether all adults aged 18 and older should be routinely screened with a cutoff-based instrument when they are not already suspected of having depression.

Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteThe recommendation against routine instrument-based screening applies to people in that general adult population who are not already being assessed for depression. It excludes adults with clinical suspicion, a current diagnosis, a personal history of depression, or another mental-health disorder. It is not advice to ignore symptoms, avoid seeking help, or decline a clinical assessment when concerns exist.
This distinction matters because screening, diagnosis, and monitoring are separate uses. A screening program seeks to find possible cases in people without a clinical suspicion. Diagnosis confirms what is causing symptoms in a person being assessed. Monitoring follows symptoms after a diagnosis or treatment plan.
How to interpret headlines about MRI, fMRI, EEG, or “brain biomarkers”
News reports often compress a complex study into “scientists found the depression scan.” Before accepting that conclusion, check what the investigators actually measured and what population they studied.
Group averages are not individual tests
A statistically different average between two study groups can coexist with substantial overlap between individuals. A pattern that is interesting at the group level may therefore classify a new patient poorly. The study’s sample, exclusions, medication status, age range, and comparison group all affect what the result means.

Prediction is not diagnosis
A model that predicts relapse, treatment response, or a future outcome is answering a different question from “Does this person currently have MDD?” Predictive research can be valuable without being a diagnostic service.
Independent validation matters
A classifier can appear impressive when developed and tested on closely related data. Independent testing in ordinary clinical populations is needed to show that performance survives changes in scanners, sites, demographics, comorbidities, and referral patterns. The sources reviewed here do not establish such performance for a specific imaging classifier.
Clinical usefulness is the final hurdle
Even a reproducible association must show that ordering the test changes management in a beneficial way. If the result does not improve diagnosis, treatment selection, safety, or outcomes beyond a careful assessment, routine testing exposes people to cost, incidental findings, and possible confusion without a demonstrated gain.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. When a brain scan may be the right next step
Ask what clinical question the scan is intended to answer. A clinician may recommend imaging because of neurological findings, cognitive decline that is new or persistent, an abrupt personality or behaviour change, or late-life onset of symptoms. In those cases, the goal is to investigate conditions such as a cerebral event or structural lesion, not to obtain a simple “depression-positive” image.
If imaging is suggested, ask:
- What specific condition are we trying to rule out?
- What symptom or examination finding led to this recommendation?
- What will a normal or abnormal result change?
- Are there safer or more appropriate first tests?
- Who will interpret the result and arrange follow-up?
Do not postpone an assessment for depression while waiting for a scan that is not clinically indicated. A person can have depression with normal imaging, and a scan finding does not replace evaluation of mood symptoms and safety.

What brain research can legitimately contribute
Neuroscience can improve understanding of how depression relates to brain structure, function, and biological processes. It can help generate hypotheses, identify possible subgroups, study mechanisms, and explore whether a treatment affects a measurable biological signal. Those are legitimate scientific goals.
Moving from those findings to a patient-facing diagnostic tool requires evidence that is not supplied by an association alone. Researchers must demonstrate reproducibility, clinically meaningful discrimination, transportability to diverse patients, and a benefit to decisions. Until then, “associated with depression” should not be rewritten as “diagnoses depression.”
Reliable reference points for patients and clinicians
For diagnostic terminology and clinical descriptions, the World Health Organization publishes the ICD-11 Clinical descriptions and diagnostic requirements (CDDR). It is a clinician-facing reference for accurate and reliable identification and diagnosis of mental, behavioural, and neurodevelopmental disorders; it is not a brain test or a self-diagnosis product.
When reading a claim about a scan, look for the study’s population, comparison standard, independent validation, and intended use. When deciding what to do about symptoms, use a qualified clinician’s assessment rather than an online image, score, or marketing claim.
