A biological-age test does not reveal one objectively established “true age” for your body. It applies a model to selected biological measurements and reports an estimate—such as an age-like score, a difference from a comparison group, or a pace-of-aging measure. What the result means depends on the test’s measurements, sample, model and validation.

What does a biological-age test actually measure?

“Biological age” is a broad label, not the name of one standardized measurement. Tests may use DNA methylation patterns, routine clinical measurements or a combination of inputs. Each method captures only selected aspects of biology; none, by itself, represents every dimension of aging. The 2025 Annual Review of Public Health and a 2024 review on validating biomarkers of aging describe epigenetic clocks as model-based measures built around particular targets.

DNA methylation clocks use selected patterns, not a whole-body age reading

DNA methylation clocks analyze methylation at selected sites in DNA and apply an algorithm to those measurements. The model is trained for a particular purpose. Some clocks estimate chronological age; others are designed around associations with morbidity, mortality or a pace-related measure. A clock trained to estimate chronological age should not be interpreted as measuring healthspan or aging speed unless its validation supports that use.

Age-like scores and pace measures are not interchangeable

A test may report an age in years, an age difference relative to a reference group, or a measure intended to reflect pace of aging. DunedinPACE, for example, is discussed as a pace-of-aging biomarker; its output is not an age in years. The name “biological age” alone does not tell you which kind of result you received.

How accurate are biological-age tests?

There is no general accuracy percentage that applies to all biological-age tests. Accuracy depends on what the model is intended to estimate, how the sample is analyzed, and the evidence supporting that specific test. The available reviews do not establish one validated consumer-facing accuracy rate that can be generalized across products.

Prediction in a group is not a personal forecast

A clock may show statistical associations with health outcomes across a study population without providing a dependable forecast for an individual. A model can help rank or compare groups yet lack clear individual thresholds, repeatability evidence or proof that acting on its result improves health. The distinction between population-level prediction and use as a personal biomarker is examined in “From Population Science to the Clinic? Limits of Epigenetic Clocks as Personal Biomarkers” (2025).

Measurement validity and health usefulness are different questions

The FDA distinguishes analytical validity—whether a test measures what it claims reliably—from clinical validity—whether the measurement predicts a health state. Neither question by itself establishes that using the result improves a person’s decisions or health. FDA guidance advises consumers not to base medical decisions solely on a test result or use it as a replacement for recommended screening and healthcare visits. See the FDA’s Direct-to-Consumer Tests guidance.

Why can two biological-age tests give different results?

Two results are not necessarily readings from the same universal scale. Services may use different clocks, algorithms, laboratory methods, reference populations and interpretation rules. Sample type matters as well: saliva, cheek cells and blood are different tissues, and agreement across tissues is a subject of ongoing research. A 2025 cross-tissue comparison discusses why agreement sufficient for ranking people in a research sample may not establish close agreement for an individual consumer.

Validity can also depend on whether the model has been evaluated in people like the person taking the test. The 2025 Annual Review considers population and context—including race, sex, lifestyle and environmental influences—as relevant to epigenetic-aging measures. When results conflict, first compare what each test claims to measure, then its sample type, clock and validation; a higher number on one service is not automatically evidence of faster aging than a lower number on another.

Can a biological-age test tell whether you are aging faster?

Only if the specific measure is designed and validated to estimate pace of aging—and even then, the result is a biomarker estimate, not a definitive personal diagnosis. An age-like score is not automatically a pace measure, and a clock’s association with outcomes in a population does not establish an individual’s rate of aging.

Can you lower a biological-age score?

A lower score on a later test does not, by itself, show that an intervention slowed aging or improved health. For a meaningful comparison over time, the measurement should use the same model, assay, sample type, collection protocol and interpretation framework. Even a reproducible biomarker change is not automatically a clinical benefit; it needs to be connected to meaningful outcomes such as disease, disability or mortality risk. A 2026 review considers the responsiveness of epigenetic-aging biomarkers to longevity interventions in humans: “Responsiveness of epigenetic aging biomarkers to longevity interventions in humans.”

What should you check before buying or interpreting a test?

  • Sample and collection: Find out whether the test uses saliva, cheek cells, blood or another specimen, and follow its collection instructions. Direct-to-consumer tests commonly involve collecting a specimen and sending it to a company for analysis, according to FDA guidance.
  • Method: Look for the exact clock, markers or algorithm, rather than relying on a generic “biological age” label.
  • Intended meaning: Check whether the output estimates chronological age, a difference from a reference group, a health outcome or pace of aging.
  • Validation: Look for the population and outcome used to evaluate the method, and whether independent evidence addresses reliability and repeatability in people like you.
  • Limits and interpretation: A useful report explains what its result can and cannot establish and makes clear that it is not a diagnosis or substitute for clinical evaluation.

What FDA oversight means for direct-to-consumer tests

FDA oversight depends on a test’s intended use and risk. The agency says many non-medical, general-wellness or low-risk tests are generally not reviewed before being offered, while moderate- to high-risk tests intended for medical purposes are generally reviewed for claims and validity. That does not establish the regulatory status of any particular biological-age product: check the specific test and its stated intended use in the FDA’s direct-to-consumer test guidance.
