Call 911 immediately for even one possible stroke symptom. Most strokes begin suddenly. Use B.E. F.A.S.T.—Balance, Eyes, Face, Arms, Speech, Time—to spot the emergency, record when the symptoms began (or when the person was last known well), and use an ambulance rather than driving. Call even if the symptoms improve or disappear, because a transient ischemic attack (TIA) can be a warning of a major stroke.
Recognize the emergency before you try to identify the exact type
A stroke occurs when blood flow to part of the brain is blocked or when a blood vessel breaks and bleeds. The first minutes are not the time to determine which type it is, find a home remedy, or wait for a second symptom. A sudden change in movement, speech, vision, balance, sensation, thinking, or headache pattern is enough to activate emergency services.
The Centers for Disease Control and Prevention (CDC) advises calling 9-1-1 right away when you or someone else has any major stroke symptom. A person may have only one sign—for example, an arm that suddenly drifts down—or several signs at once. Symptoms can be mild, intermittent, or difficult to describe, especially when the person is frightened or unable to speak clearly.

In a CDC-cited survey, only 38% of people recognized all major stroke symptoms and knew to call 9-1-1. Learning a short checklist in advance makes it less likely that uncertainty will delay care.

Use B.E. F.A.S.T. to check for sudden symptoms

B.E. F.A.S.T. is a recognition aid, not a diagnostic test. A normal smile or clear sentence does not rule out a stroke if the person has sudden vision loss, severe imbalance, numbness, confusion, or another concerning change.
Other sudden warning signs
Call emergency services for any of these abrupt changes, even when B.E. F.A.S.T. is not obviously positive:
- Sudden numbness or weakness of the face, arm, or leg, especially on one side.
- Sudden confusion, inability to understand speech, or difficulty finding or forming words.
- Sudden trouble seeing in one or both eyes, including a curtain-like loss of vision.
- Sudden trouble walking, dizziness, loss of coordination, or unexplained falls.
- A sudden, severe headache with no known cause, particularly when it is unlike previous headaches.
These signs can overlap with migraine, low blood sugar, seizures, medication effects, or other illnesses. That uncertainty is a reason for professional assessment, not a reason to wait. Emergency clinicians can distinguish stroke and its mimics using an examination, blood tests, and brain imaging.

- Make the emergency call. Tell the dispatcher that you suspect a stroke and describe the symptoms, not just that the person feels unwell.
- Give the time information. State when the first symptom was noticed. If the person woke with symptoms or was alone, give the last time they were known to be normal.
- Keep the person safe. Have them sit or lie in a safe position, stay with them, and watch their breathing and responsiveness. Follow the dispatcher’s instructions.
- Gather essential information. Prepare the person’s name, age, medical conditions, medications, allergies, and the location where EMS should meet them.
- Let EMS transport the patient. Do not drive to the hospital unless emergency services specifically direct you to do so.
Do not offer food, drink, aspirin, or other medication while waiting unless a dispatcher or clinician instructs you. A stroke can impair swallowing, and aspirin can worsen bleeding in a hemorrhagic stroke. Do not leave the person alone to search for documents or belongings.

Why an ambulance is safer than a private car
CDC guidance explains that emergency medical services can begin care during transport, collect information, notify hospital staff, and help route the patient to a facility equipped for specialized stroke treatment. Paramedics can also monitor breathing, blood pressure, blood sugar, and neurological changes on the way.
Driving yourself removes those capabilities and can create dangerous delays at traffic lights, parking areas, registration desks, or an unexpected decline in the passenger. Tell the dispatcher if symptoms are worsening, the person becomes unresponsive, has a seizure, or has trouble breathing.
Why every minute matters
The American Stroke Association publishes the estimate that 1.9 million brain cells die every minute a stroke goes untreated. This figure illustrates why emergency evaluation should begin with the first possible sign rather than after symptoms become severe.

For many ischemic strokes, the best-known clot-dissolving treatments can be used only within about four and a half hours of symptom onset. Some carefully selected patients may benefit from treatment later, including mechanical removal of a large clot as soon as possible within up to 24 hours. NINDS notes that getting to a hospital quickly saves lives and improves the chance of successful recovery.

These are clinical treatment windows, not waiting periods. Eligibility depends on the examination, the exact timing, brain imaging, the suspected vessel involved, other health factors, and whether the stroke is caused by a clot or bleeding. Only the treating team can decide whether thrombolysis, thrombectomy, surgery, blood-pressure management, or another treatment is appropriate.
How clinicians distinguish the main emergency patterns

A person at home cannot reliably identify the pattern from symptoms alone. The same facial droop or speech problem can occur with different causes, and the immediate response remains emergency evaluation.
If the symptoms stop: treat it as a TIA emergency
The American Stroke Association says TIA symptoms may last only minutes and can continue for up to 24 hours. Because the episode disappears, people often dismiss it as stress, fatigue, or a temporary pinched nerve. That is unsafe: a TIA can signal a high risk of a completed stroke.
Keep the original symptom time, even if the person looks normal when help arrives. Do not cancel the ambulance, postpone assessment until a clinic opens, or let the person drive. Hospital clinicians may need brain and blood-vessel imaging, heart-rhythm monitoring, and tests for conditions such as atrial fibrillation, high blood pressure, diabetes, or carotid artery disease.

Common situations that cause dangerous delay
The person says they feel fine
Sudden symptoms can be frightening, and a person may deny that anything is wrong. Do not argue about the diagnosis. Describe the observable change to the dispatcher and explain that a rapid examination is needed.
You are unsure when it began
Give the last-known-well time: the latest moment when the person was definitely without symptoms. If a family member finds someone symptomatic after sleep, tell EMS when the person was last seen normal before going to bed.
The main symptom is a headache
A severe headache without a known cause can be a stroke warning sign, especially when it starts suddenly or accompanies vomiting, weakness, confusion, vision change, or loss of balance. Do not assume it is a routine migraine unless emergency clinicians have ruled out dangerous causes.
The person is already taking a blood thinner
Tell EMS the exact medicine and, if known, the dose and last time taken. Do not stop or add medication on your own. The information affects imaging and treatment decisions.

Information to have ready for EMS
- Exact symptom descriptions and which side of the body is affected.
- Time each symptom was first noticed and the last-known-well time.
- Current medicines, especially anticoagulants or antiplatelet drugs.
- Known allergies, previous strokes or TIAs, recent surgery, bleeding disorders, diabetes, seizures, and major heart conditions.
- The person’s usual ability to speak, walk, see, and think, so a new change is clear.
- A phone number for a relative or caregiver who can answer medical-history questions.
Do not delay the call while collecting this information. Give what you know first; EMS can obtain the rest during transport.
What not to do
- Do not wait to see whether a second symptom appears.
- Do not drive the person, allow them to drive, or send them by taxi or rideshare.
- Do not give food, drink, aspirin, pain medicine, or someone else’s prescription.
- Do not ask the person to walk around to prove weakness is real.
- Do not assume a brief episode is harmless because speech, vision, or strength returned.
- Do not spend time searching online for a diagnosis before contacting emergency services.
What happens after the emergency call
Paramedics will assess airway and breathing, check vital signs and blood sugar, ask about the timeline, and perform a neurological screening. The receiving hospital may activate a stroke team before arrival. Clinicians then use brain imaging—often an urgent CT scan, with other scans as needed—to determine whether there is bleeding, a blocked vessel, swelling, or another explanation.
Bring a medication list if it is immediately available, but never return home to find it. A photograph of the list or a pharmacy record can be supplied later. The most valuable information is still the symptom timeline and the person’s current condition.

Sudden balance trouble, vision change, facial droop, arm weakness, speech difficulty, one-sided numbness, confusion, walking trouble, or an unexplained severe headache should be treated as a stroke until professionals evaluate it. Remember B.E. F.A.S.T., record the onset or last-known-well time, and activate 911 without waiting for symptoms to worsen or disappear. The emergency response begins when EMS arrives, not when the patient reaches the hospital.
