Microrobots could eventually help doctors diagnose disease or deliver treatment in places conventional instruments cannot easily reach. Their use is not risk-free or routinely established, however. The main concerns are biological compatibility, reliable movement and control, imaging and tracking, material stability, and what happens to the device after the procedure.
Risk is specific to the robot, payload, body location, exposure time, and clinical procedure. Current medical-microrobotics reviews describe an emerging field with substantial translation barriers—not a single technology with one predictable human complication rate.
Why microrobots remain an emerging medical technology
A medical microrobot is a very small device designed to move, carry a payload, sense its surroundings, or perform a localized task inside the body. Researchers are investigating systems that might reach difficult anatomy, release drugs near a target, assist diagnosis, or carry out other minimally invasive procedures. The scale is attractive because a device could work close to diseased tissue while limiting exposure elsewhere.

That promise does not demonstrate clinical readiness. Veronica Iacovacci, Eric Diller, Daniel Ahmed, and Arianna Menciassi describe the field in the 2024 Annual Review of Biomedical Engineering article Medical Microrobots as an effort to create miniature robots controllable inside the human body. A separate review, Roadmap for Clinical Translation of Mobile Microrobotics, first published in Advanced Materials on February 21, 2024, identifies safety, microfabrication, imaging, and other unresolved engineering issues. Reviews of wireless soft robotic medical devices and of microrobot deployment and clearance reach a similar conclusion: a laboratory demonstration is not equivalent to a proven human treatment.

There is no universal adverse-event percentage for “microrobots.” Different platforms use different materials, propulsion methods, imaging systems, payloads, and body environments. The available reviews are not a clinical adverse-event registry and do not establish a general harm rate, a complete risk-benefit estimate, or approval status for a particular device.
Biological compatibility: what the body may do to the device
Biocompatibility means more than a material being described as medical-grade. The complete system—including the robot body, coatings, joints or moving parts, magnetic or acoustic components, degradation products, and therapeutic payload—must be evaluated in the tissue and fluids where it will operate.
Material and coating interactions
A surface that is acceptable in one location or for a brief exposure may behave differently in blood, mucus, urine, joint fluid, or an inflamed tissue. Proteins can adsorb to a surface, cells can attach to it, and the surrounding chemistry can alter friction or propulsion. A coating intended to reduce unwanted interaction can itself wear, swell, detach, or change the robot’s movement. These are design questions that require evidence for the specific construction; the reviews do not establish a common rate of immune reactions or tissue injury across platforms.
Inflammation, immune response, and local injury
The body may recognize a device or its degradation products as foreign. Researchers therefore need to examine inflammation, immune activation, clotting, toxicity, and effects on nearby tissue. Mechanical contact creates another concern: a device could scrape, compress, puncture, or obstruct a delicate structure if its size, shape, stiffness, or route is unsuitable. Such outcomes are plausible safety endpoints to test, not documented universal results of using microrobots in patients.
Payload-specific risk
A robot carrying a drug, diagnostic agent, energy source, or biological material adds risks beyond the robot itself. The payload must remain contained during transit, be released at the intended location, and avoid an excessive dose or unintended exposure. A design that is safe when empty may not be safe after loading, and a payload can change buoyancy, magnetic response, dimensions, or the force required to move the device.

Navigation and actuation can fail in living anatomy
Microrobots cannot be treated like remotely driven miniature cars. Their propulsion and steering depend on the chosen actuation method—such as magnetic fields, acoustic energy, light, chemical reactions, or another mechanism—and on the properties of the surrounding environment.
Biological barriers and moving surroundings
Laboratory fluids and channels are usually controlled. Inside the body, flow, tissue motion, peristalsis, breathing, heartbeats, branching anatomy, valves, mucus, and changing viscosity can push a device away from its intended path. Narrow passages may be blocked by debris or biological material. A robot that moves well in a dish may lose speed, orientation, or traction in a living environment.
Loss of reach or control
If actuation is too weak, the device may not reach its target. If it is too strong or poorly focused, it may move beyond the target or exert unwanted force on tissue. A control system may also have limited ability to reverse, stop, or recover the robot. The clinical consequence could be an incomplete treatment, delivery to the wrong site, or a need for an additional procedure. These are engineering failure modes that must be characterized for each design; the cited reviews do not report one general patient outcome rate.
External equipment and operator dependence
Many systems require field generators, transducers, cameras, catheters, software, and trained operators. Positioning the patient and equipment correctly can be part of the safety case. Failures in calibration, power, communication, software, or synchronization can affect movement even when the robot itself works as designed. A clinical protocol must specify safe shutdown, reversal, and recovery procedures rather than assuming that removing power automatically solves every problem.
Imaging and tracking determine whether the robot can be used safely
A clinician needs to know where the device is, whether it is moving, and whether it has reached the intended target. This makes imaging and tracking central risk controls, not optional conveniences. The 2024 clinical-translation reviews identify imaging as a major barrier.

Limited visibility
Microscale devices can be difficult to distinguish from surrounding tissue. Some imaging methods have limited depth, resolution, frame rate, or field of view. Others may expose the patient to energy or require contrast agents. A marker that is visible in one imaging mode may not remain visible after a coating changes or a payload is released.
Uncertainty between observations
Even when a system produces intermittent position updates, the robot may move between frames. Blood flow or tissue motion can make the last confirmed location different from the current one. If tracking is lost, clinicians need a predefined response: stop actuation, switch imaging mode, attempt retrieval, or use another intervention. The sources do not provide a general rate of misplacement or injury caused by tracking failures, so those risks should be presented as system-specific questions.
Material stability and the device’s fate after treatment
A microrobot must be safe not only while performing its task but also after the task ends. The 2026 National Science Review review Deployment and clearance of microrobots for localized therapy: past, present and future frames degradation, retrieval, and clearance as context-dependent safety issues.
Stable, degradable, retrievable, or clearable?
Every design needs an explicit end-of-use plan. A stable robot may need to be recovered mechanically or guided out through a natural route. A degradable robot must break down into products that are safe at the expected dose and exposure time. A device intended to clear through the body must actually be small and chemically suitable enough for that route. No single strategy is universally optimal.
What can go wrong after deployment?
- A material may corrode, swell, fracture, or lose structural integrity before the procedure is complete.
- A coating or component may detach and become a separate foreign body.
- Degradation may release particles, metals, polymers, or payload faster than intended.
- A robot may lodge in a narrow passage or become difficult to retrieve after its propulsion changes.
- Residual material may interfere with later imaging, procedures, or medical devices.
These possibilities do not establish that a particular microrobot will cause harm. They identify the tests and contingency plans a responsible development program must address.

Clinical workflow creates system-level risks
The robot is only one part of a medical system. Deployment, navigation, imaging, actuation, communication, retrieval, sterilization, manufacturing, and documentation all affect safety. The review Clinical translation of wireless soft robotic medical devices, published in Nature Reviews Bioengineering on March 11, 2024, emphasizes the broader translation challenge for wireless devices.
Procedure and deployment
Introducing a device through a catheter, needle, endoscope, or another route can cause risk before the robot starts moving. The route must accommodate the device’s dimensions and any delivery tool. A clinician also needs to know whether the robot can be released, repositioned, and removed without damaging tissue.
Sterility, manufacturing, and consistency
Microscale fabrication can make quality control difficult. Small variations in dimensions, magnetic properties, coating thickness, or payload loading may change performance. Manufacturing must produce consistent devices, and sterilization must not alter their shape, materials, or control response. A promising prototype is not automatically a reproducible medical product.
Human factors and emergency response
Operators need clear indications, contraindications, training, alarms, and stop rules. The protocol should define what happens if imaging is lost, the robot stops moving, the patient’s anatomy changes, or the external actuation system fails. Emergency teams must know what equipment is present and whether the device can be located or removed.

| Risk axis | Questions to ask | Why it matters |
|---|---|---|
| Biological compatibility | What are the body, coatings, payload, and degradation products? Which tissues contact them, and for how long? | Compatibility depends on the complete device and exposure, not the label “microrobot.” |
| Actuation and navigation | How is movement produced? Can the operator stop, reverse, and steer it in the target anatomy? | Flow, tissue motion, barriers, and branching paths can change laboratory performance. |
| Imaging and tracking | Which modality locates the robot, at what depth and resolution, and what happens if tracking is interrupted? | Safe operation requires knowledge of location and behavior during use. |
| Material stability | Does the device remain intact for the required time? Can components corrode, swell, fracture, or detach? | Unexpected changes can alter propulsion, dosing, or retrieval. |
| Post-treatment fate | Is it retrieved, degraded, or cleared? What evidence supports that route and its by-products? | Residual material can create risks after the intended treatment. |
| Workflow integration | What delivery tools, generators, imaging equipment, software, staffing, and emergency procedures are required? | Failures often occur at interfaces between the robot and the clinical system. |
What evidence should exist before human use?
A credible safety case should connect the exact device to the exact procedure. Useful evidence includes material and coating characterization, payload-release testing, sterility and manufacturing controls, simulated-body testing, animal or other preclinical studies where appropriate, tracking performance, failure and recovery tests, and a defined plan for retrieval, degradation, or clearance.
Testing should use conditions that resemble the intended anatomy: realistic flow, temperature, viscosity, tissue motion, imaging depth, and exposure duration. Developers should report not only successful runs but also loss of control, tracking interruptions, incomplete deployment, material damage, and recovery attempts. Evidence from a different robot, body site, or payload cannot automatically be transferred to a new system.
Clinical experts and microrobotics researchers must work together. The authors of Roadmap for Clinical Translation of Mobile Microrobotics state that clinical translation is possible only through close collaboration to address technical challenges in microfabrication, safety, and imaging. That collaboration also helps define a useful endpoint: a device that can be operated, monitored, and recovered within ordinary clinical practice, not merely one that moves under ideal laboratory conditions.

Questions a patient or clinician should ask about a proposed system
- Is this device being used in an approved clinical procedure, a regulated trial, or laboratory research?
- What materials, coatings, and payloads enter the body, and what tissue contacts them?
- How is the robot powered or steered, and what are the stop and recovery procedures?
- How will its location be confirmed throughout the procedure?
- What is the planned fate after treatment: removal, degradation, or natural clearance?
- What happens if the robot cannot reach the target, stops, breaks, or cannot be seen?
- What evidence applies to this exact design, body site, dose, and duration rather than to microrobots generally?
- Which risks come from the robot itself, and which come from the delivery route or external equipment?
What current reviews establish—and what they do not
The reviews establish a consistent set of translational concerns: safety and biocompatibility, actuation and navigation, imaging and tracking, material stability, deployment, retrieval or clearance, fabrication, and integration into clinical workflows. They support careful risk assessment for each platform.
They do not establish a universal human harm rate, a general probability of immune reaction, a guaranteed retrieval method, or a blanket conclusion that microrobots are safe or unsafe. Patient risk can only be estimated from evidence for the particular robot, procedure, anatomy, payload, and clinical setting. Until that evidence exists, microrobots should be described as a promising but developing technology rather than a routine treatment category.
