Treatments that destroy or shrink fat cells can reduce a localized bulge, but they are medical procedures—not risk-free beauty services. Possible effects range from temporary pain, redness, swelling and bruising to burns, skin-color changes, scars, nerve injury, infection and, with fat freezing, a delayed complication in which the treated fat enlarges instead of disappearing. These procedures do not treat obesity, produce general weight loss or provide the health benefits of losing weight.
The safest way to assess one is to identify the exact device or injection, the body site and its authorized indication, then review your medical history with a qualified healthcare professional. The risk profile differs substantially between cooling, radiofrequency, light, ultrasound, low-level light and injectable products.
What “destroying fat cells” means
Body-contouring technologies aim to alter a small, visible pocket of fat. Depending on the technology, energy may cool fat, heat tissue, disrupt fat cells mechanically or chemically break down fat around injection sites. The body may then clear some of the affected cells, or the treated area may temporarily look smoother because of changes in fluid, connective tissue or skin appearance.
FDA patient guidance describes non-invasive body contouring as a medical procedure with benefits, limits and risks, even when no incision is made. A treatment can improve the contour of one area without changing your overall weight, body-mass index or health risks associated with obesity. Results can be modest, temporary or different from the promised appearance.

There are no reliable, comparable complication percentages for all of these technologies in the official patient material. A claim that one category is “risk-free” or definitively safer than another is not supported by that evidence.
Risks shared by many technologies
- Short-term local effects: discomfort, tenderness, redness, swelling, bruising, itching or temporary numbness.
- Visible or palpable changes: nodules, lumps, uneven contours, skin-color changes or a result that falls short of expectations.
- Thermal injury: heating devices and ultrasound can cause blisters or burns if energy, contact or cooling is inadequate.
- Nerve effects: temporary or lasting numbness, altered sensation or nerve damage can occur, particularly near treated nerves.
- Scarring and infection: more likely when skin is injured, injections are performed unsafely or a burn is not treated promptly.
- Unpredictable appearance: fat loss can be uneven, and a reduction in one area can make neighboring fullness more noticeable.
“Non-invasive” refers mainly to the absence of a surgical incision. It does not remove the need for screening, informed consent, sterile technique where applicable or a plan for managing complications.
How risks differ by treatment type
| Technology | How it is intended to work | Reported risks and important limits |
|---|---|---|
| Cryolipolysis (fat freezing) | Controlled cooling targets a localized bulge so fat cells are gradually removed. | Redness, bruising, swelling, discomfort, discoloration, prolonged numbness, nodules, freeze burns and temporary nerve effects. Paradoxical adipose hyperplasia (PAH) can create a firm enlarging bulge two to five months later. Hernias have been reported. |
| Radiofrequency (RF) | Heating may reduce circumference or temporarily improve the appearance of cellulite. | Discomfort, redness, swelling, blisters, burns, nodules, pigment changes and scars. Active implants, metal under the skin and tattoos containing metal particles in the treatment area are concerns; people with an IUD should ask a clinician about additional risks. The FDA’s October 15, 2025 warning concerns certain RF microneedling uses, not every RF contouring procedure. |
| Laser or intense pulsed light (IPL) | Light energy heats tissue and can affect fat or the appearance of a treated area. | Pain, redness, swelling, blisters, burns and nodules, plus eye injury without proper protection. Tanning, planned sun exposure, a tendency toward dark marks, photosensitivity or photosensitizing medicines can change suitability. |
| Therapeutic ultrasound | Ultrasound energy may disrupt fat cells. | Burns and nerve damage are possible. Complete contact and adequate ultrasound gel help limit hot spots. Open wounds, compromised skin, active implants or metal under the skin are reasons to avoid treatment. |
| Low-level light therapy | Uses low doses that, unlike lasers and IPL, are described as not heating skin or fat. | Eye injury precautions still apply. Reported risks include sensitivity, burns, pigment changes, itching, nodules, pain and electrical-malfunction injury. Pregnancy, some implanted devices, photosensitivity, cancer at the site, certain medicines, active infection and wounds may make it unsuitable. |
| Fat-dissolving injections | Injected substances break down fat around injection sites. | Unapproved online products have been linked to permanent scars, serious infections, skin deformities, cysts and deep painful knots. The FDA-evaluated drug is approved only for use under the chin; approval should not be assumed for other body areas. |
Fat freezing: the distinctive delayed complication
Paradoxical adipose hyperplasia
PAH is the most distinctive serious risk associated with cryolipolysis. Instead of shrinking, the treated fatty tissue becomes a firm, visibly enlarged bulge. FDA reports that reported cases appeared two to five months after treatment. That window is an onset range, not a probability estimate.
PAH does not normally disappear on its own. Correcting it may require surgery, creating the costs, anesthesia risks, scarring and recovery associated with an operation. Ask before treatment how the provider identifies PAH, who will evaluate a delayed enlargement and whether corrective care is available if it occurs.

Other cooling-related effects
Expected short-term reactions include redness, swelling, bruising, discomfort and temporary discoloration or numbness. Less common problems include prolonged numbness, nodules and freeze burns. When the area under the chin is treated, temporary nerve effects around the jaw or throat have been reported.
Who should avoid or postpone cooling
Cold-sensitivity disorders and poor circulation at the treatment site are important contraindications. Treatment should not be placed over a known hernia or an area with structural weakness because hernias have been reported. Safety and effectiveness of over-the-counter or home-use fat-freezing devices have not been established; a home device should not be treated as equivalent to supervised clinical care.
Heat, light and ultrasound: burn and nerve considerations
Radiofrequency
RF devices deliver heat. The same heat that may change circumference or cellulite appearance can produce pain, swelling, blisters, burns, nodules, pigment changes or scars. Tell the clinician about pacemakers and other active implants, metal beneath the skin and tattoos that contain metal particles in the intended area. If you have an intrauterine device, ask specifically whether the device and treatment location create additional risk.

It described reports of burns, scarring, fat loss, disfigurement and nerve damage, including injuries requiring medical intervention or surgical repair. This warning is about specified RF microneedling uses; it should not be generalized to every RF body-contouring device. Ask which exact device and mode will be used.

Laser and IPL
Light-based systems can injure the eyes, so appropriate protective goggles are essential for everyone in the treatment room when indicated by the device instructions. Pain, redness, swelling, blisters, burns and nodules are reported effects. Recent tanning or planned sun exposure can raise the chance of an unwanted skin response. A history of dark marks after an injury, light sensitivity or medicines that increase photosensitivity should be discussed before treatment.
Therapeutic ultrasound
Ultrasound requires consistent contact between the applicator and skin. Gaps or inadequate gel can concentrate energy and increase burn risk. FDA lists open wounds, compromised skin, active implants and metal under the skin as reasons not to have this treatment. Nerve damage is another possible complication, not merely a cosmetic inconvenience.
Low-level light therapy
Low-level light therapy is not the same as a laser or IPL treatment: the described low doses do not heat skin or fat. That difference does not make it risk-free. Eye protection, electrical safety and skin screening still matter. Reported problems include sensitivity, burns, pigment changes, itching, nodules and pain. It may not be recommended during pregnancy, with some implanted devices, in people with photosensitivity or certain medicines, over a site with a cancer history, or where there is an active infection or wound.

Injection “fat dissolvers” need extra scrutiny
Injection lipolysis can cause local inflammation and tissue injury even when performed by a professional. The FDA has warned specifically about products sold online without approval. Reported adverse reactions include permanent scars, serious infections, skin deformities, cysts and deep, painful knots. Unsanitary injection technique can add infection and scarring risks.

Do not infer that an injectable is authorized for every body part simply because it is marketed as a fat dissolver. The drug evaluated by the FDA is approved only for use under the chin. Before an injection, ask for the product’s exact name, regulatory status, authorized body site, storage and handling information, and the clinician’s plan if infection, tissue death or a persistent lump develops.
Medical factors that can change your eligibility
A consultation should cover more than your desired contour. Bring a complete list of diagnoses, medicines, implants and prior procedures. Depending on the technology, relevant issues include:
- Cold sensitivity disorders or poor circulation for cryolipolysis.
- A hernia or weak abdominal wall where a cooling applicator might be placed.
- Active implants, metal beneath the skin or metal-containing tattoos for RF; ask separately about an IUD.
- Open wounds, damaged skin, infection or a cancer history at the site.
- Photosensitivity, medicines that increase light sensitivity, recent tanning or a tendency to develop dark marks after injury for laser, IPL or low-level light.
- Pregnancy and some implanted devices for low-level light therapy.
- Any condition that could impair healing or make a burn, infection or surgery more dangerous.
The provider should explain whether the proposed device is authorized for the exact body site and purpose, not merely whether the device is cleared or legally marketed for a different use.
How to compare options before consenting
Use the same questions for every consultation so that a persuasive sales presentation does not substitute for risk information.

- Identify the technology. Get the manufacturer, model, applicator and treatment mode in writing.
- Confirm the indication. Ask whether the device or drug is FDA authorized for your intended body area and cosmetic goal.
- Review personal contraindications. Discuss circulation, cold sensitivity, implants, metal, skin disease, medicines, pregnancy and prior reactions.
- Separate common from serious effects. Ask which reactions are expected, how long they usually last and which symptoms require urgent review.
- Discuss delayed problems. Ask how PAH, burns, nerve injury, infection, pigment changes or persistent nodules would be recognized and treated.
- Set an outcome boundary. Clarify the expected amount of contour change, whether more than one session is proposed and how long any improvement may last.
- Check follow-up. Confirm who answers questions after treatment, the emergency contact and whether corrective care or referral is available.
- Read the patient material. FDA advises reviewing available patient brochures and understanding benefits, limitations and risks before deciding.
Warning signs after treatment
Contact the treating clinician promptly for worsening pain, spreading redness, increasing warmth, drainage, fever, a blister or burn, new weakness, severe or persistent numbness, a rapidly enlarging lump, marked skin discoloration or an unexpected firm bulge months after fat freezing. Seek urgent medical care for severe pain, signs of a serious infection, breathing or swallowing difficulty, facial weakness or any rapidly progressing injury.
Do not puncture blisters, massage a painful hard lump aggressively or apply unapproved creams while waiting for advice. Keep photographs and note when symptoms began, the device or product used and the treatment settings if you have them; those details help a clinician assess the injury.
What the evidence can—and cannot—tell you
Official FDA materials identify the types of complications and, for PAH, a reported two-to-five-month onset window. They do not provide a single, comparable incidence rate for every technology and complication. Therefore, a clinic’s claim that a treatment has a specific tiny risk should be checked against the exact device, study population, treatment settings and follow-up period rather than accepted as a universal number.

A realistic decision weighs the likely cosmetic benefit against the possibility of no meaningful change, an uneven result, a temporary effect or a complication requiring medical treatment.
Bottom line
Fat-cell-destroying treatments can have real medical consequences. Cooling has the unique delayed risk of PAH and should not be used over hernias or in people with cold-related circulation problems. RF, light and ultrasound can burn skin or injure nerves; low-level light has its own eye, skin and device-related precautions; and unapproved injections can cause permanent scars, infection and deformity. Choose only after verifying the exact product and authorized use, reviewing your individual risk factors with a qualified healthcare professional and agreeing on a clear follow-up plan.
