Off-target DNA insertions are possible, but there is no single rate that applies to every genome-editing treatment. “Off-target” describes where an unintended change occurs; “insertion” describes one kind of sequence change. Depending on the editor, guide, delivery method, cell type and test used, an unintended event may be a small insertion or deletion, a large structural variant, a chromosome rearrangement or integration of donor/vector DNA. Some changes could disrupt normal cell function or genome integrity, yet an off-target edit is not automatically harmful. The meaningful question is how a specific product detects, interprets and controls these events in its intended human cells.

What “off-target DNA insertion” actually means

Genome-editing systems are designed to recognize a target DNA sequence. An off-target edit is a change made somewhere else in the genome. An insertion is a sequence added at a site, but researchers use the phrase “off-target insertion” for several biologically different situations, so precision matters.

Off-target location versus insertion type

  • Off-target means the genomic location is unintended.
  • Insertion means extra DNA has been added. The added sequence can be a short repair-generated sequence, a donor template segment or, in some workflows, vector-derived DNA.
  • Insertion/deletion (indel) is the common small change produced when a cut is repaired inaccurately. An indel can occur at the intended site or at an off-target site.

CRISPR papers also use DNA bulge to describe a mismatch configuration in which the guide RNA and genomic DNA are offset by an extra base. A bulge is a recognition pattern, not proof that a new DNA fragment has integrated into a chromosome. Confusing those meanings can make a molecular finding sound more alarming—or more reassuring—than it is.

Different events that should not be lumped together

Event What happens Why it matters
Small off-target indel Repair after an unintended cut adds or removes a few bases. Could alter a coding sequence, splice signal or regulatory element.
Off-target cleavage and repair The nuclease cuts a non-target site and the cell repairs it. The repair outcome can be an indel, substitution or more complex change.
Large structural variant A deletion, duplication, inversion or other rearrangement spans many bases. Small-site assays may miss it; genes or regulatory regions can be disrupted.
Chromosomal translocation DNA ends from different chromosomes are joined. Gene regulation may change, and abnormal cell clones could gain a growth advantage.
Donor or vector integration Intended donor DNA or delivery-vector sequence inserts at an unintended location. Integration can affect nearby genes or genome stability; its frequency must be measured for the specific product rather than assumed.

Can CRISPR edit the wrong part of DNA?

Yes. A guide RNA can tolerate some sequence differences, and a nuclease may cut a site that resembles the intended sequence. The likelihood depends on the guide’s sequence, the nuclease or editor, chromatin context, how much active editor reaches the cell, and how long it remains there. Cells then repair the break through pathways that can produce different outcomes.

How a wrong-site event can arise

  1. Recognition: the guide RNA binds a genomic sequence that is similar—but not identical—to the intended target. Ordinary base mismatches and DNA/RNA bulges can both affect recognition.
  2. Cutting or chemical editing: a nuclease makes a break, or a base/prime editor performs a chemical change. Different editor classes have different bystander and off-target profiles.
  3. Repair: non-homologous end joining can create short indels; template-directed repair can copy donor sequence; other repair processes can join distant DNA ends.
  4. Selection: if a change gives a cell a survival or growth advantage, that clone may become more prominent during culture or after treatment.

Transient delivery can limit the time available for unintended activity, while persistent expression can create more opportunities. Neither approach is automatically risk-free: the relevant result is the measured profile in the final product’s cells and delivery conditions.

What harm could an off-target insertion cause?

An edit’s clinical significance depends on the affected genomic region, the exact sequence change, the cell type and whether edited cells persist or expand. An insertion in a nonfunctional region may have no observable consequence. A change inside a protein-coding gene, tumor-suppressor pathway, enhancer or splice site could alter cell behavior. A structural rearrangement can affect several genes at once.

  • Loss or alteration of gene function: an insertion or deletion can interrupt a coding sequence or change how RNA is spliced.
  • Abnormal gene regulation: an event near an enhancer or promoter can increase or decrease expression of a nearby gene.
  • Genome instability: translocations, chromosome losses or large rearrangements can impair cell function.
  • Clonal expansion: if an edited cell gains a proliferative advantage, its descendants may become a larger fraction of the treated population.

These are potential consequences, not predictions that every off-target event will cause disease. Somatic treatments edit cells in one treated person and are distinct from heritable germline editing, which raises separate reproductive safety and ethical issues.

What the evidence shows—and what it does not show

Published studies demonstrate that unintended sites and large changes can be detected, but each result is tied to its editor, guide, biological material and assay. The figures below are study-specific observations, not a universal human-treatment frequency.

Study or project Finding How to interpret it
U.S. Food and Drug Administration regulatory-science project (ended 2023) 9 off-target sites among more than 2,000 sites assayed in a study using clinically relevant transient Cas9 delivery in primary human cells and established guide RNAs. In the tested conditions, high-fidelity Cas9 greatly reduced off-target activity compared with wild-type Cas9. The result is not a rate for all guides, products, patients or treatments.
CRISPR/Cas9 bulge study (PubMed-indexed, 2014) 15 confirmed off-target sites among 114 putative loci; the confirmed sites contained a single-base bulge and one to three guide-strand mismatches. Off-target searches should consider bulges as well as ordinary mismatches. This is not a clinical event rate.
Whole-genome structural-variant study (PubMed-indexed, 2023) Deletions of 91.2 kilobases and 136 kilobases were found at atypical, nonhomologous off-target sites in two edited cell lines. Large changes can escape an assessment restricted to small local edits. Two cell lines cannot establish a frequency.
Zebrafish study (Nature Communications, 2022) Structural variants of at least 50 base pairs represented 6% of editing outcomes in founder larvae. The embryos used guides selected for in-vitro off-target activity and were followed across generations. This animal-model result must not be presented as a human clinical risk.

Why no single percentage answers the risk question

Off-target measurements are conditional. A guide that is clean in one cell type can behave differently in another because DNA accessibility, repair pathways and editor exposure change. A test that examines only predicted sites can miss an atypical site; a genome-wide test can discover candidates whose biological importance still requires interpretation.

The FDA project describes a sensitivity-versus-specificity trade-off in prediction methods. A permissive method may identify more candidates but include more false positives. A stringent method may produce a shorter list while missing unusual sites. Sequencing a candidate site proves what happened at that site; it does not prove that every possible change elsewhere was excluded.

How scientists test for off-target edits

A credible assessment uses complementary methods rather than one “off-target test.” FDA recommendations address off-target activity, on-target outcomes, chromosomal integrity, sequencing, sample selection and reporting.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Define the product and conditions. Record the editor or nuclease, guide sequence, dose, delivery vehicle, exposure time, donor template and the exact human cell material to be treated.
  2. Predict candidate sites. Use guide-specific computational tools to search for mismatches, bulges and other sequence features. Treat the output as a list for testing, not as evidence that editing occurred.
  3. Test predicted sites with targeted sequencing. Deep sequencing can quantify low-frequency substitutions and indels at selected loci, provided the assay’s detection limit and controls are validated.
  4. Use genome-wide discovery when justified. Unbiased or broader sequencing can identify atypical sites that prediction misses. Candidate findings then need confirmation with an independent method.
  5. Look for large changes. Whole-genome approaches, long-read or specialized structural-variant analyses can detect large deletions, inversions, duplications and rearrangements that short local amplicons may not reveal.
  6. Assess chromosome integrity. Karyotyping, translocation-focused assays or other validated methods can reveal chromosome-scale abnormalities. The appropriate panel depends on the product and cells.
  7. Check donor or vector sequence where relevant. If a workflow uses a donor template or vector, integration analysis must be designed for that material and delivery system; it should not be inferred from a nuclease-only experiment.
  8. Interpret biological impact. Map confirmed changes to genes and regulatory elements, examine whether altered cells persist or expand, and relate the result to the intended dose and clinical use.

Sampling is part of assay quality. Researchers need appropriate untreated and process controls, enough independent samples, representative lots or donors where applicable, and validated bioinformatics pipelines. Reporting only the largest or most obvious edits can conceal low-frequency events or fail to show the assay’s ability to detect them.

Does a negative test mean there is no risk?

No. A negative result means that no change was detected within the tested sample, genomic scope and assay sensitivity. It does not establish that the event is impossible. Risk is reduced when multiple sensitive, orthogonal assays agree in relevant cells and when the product’s manufacturing and clinical monitoring plans address remaining uncertainty.

“Therefore, an adequate assessment to evaluate off-target editing risk and impact on chromosomal integrity of human GE products is crucial to minimize unintended changes in the target cells or tissues.” — U.S. Food and Drug Administration, Safety Assessment of Genome Editing in Human Gene Therapy Products Using Next-Generation Sequencing; Draft Guidance for Industry, April 2026.

The regulatory emphasis is product-specific: establish what the editor can do in the relevant cells, use sequencing and bioinformatics suited to the product, evaluate chromosomal integrity and report methods and findings transparently.

Questions patients and developers should ask

  • Which off-target sites were tested, and how were those sites selected?
  • Did testing include bulges and atypical nonhomologous sites?
  • What is the assay’s validated detection limit, and how many independent samples were analyzed?
  • Were large structural variants, translocations and donor/vector integration assessed where relevant?
  • Were results obtained in the intended human cells using the final delivery and manufacturing process?
  • How were confirmed edits mapped to genes, regulatory regions and potential clonal-growth pathways?
  • What follow-up monitoring is planned if edited cells persist in the body?

Consumer DNA tests cannot answer these questions. They are not designed to measure rare edits in a treated cell population or to evaluate the genome integrity of a gene-therapy product.

Bottom line: treat off-target insertions as a product-specific possibility

Genome editing can change DNA away from its intended target, and the change may be a small insertion, a large deletion, a translocation or another structural alteration. Experimental studies show why assessments must look beyond a short list of predicted sites. They do not establish a universal rate of off-target insertions in people or a fixed probability of cancer or another clinical outcome.

The strongest safety argument comes from a relevant, layered assessment: guide- and editor-specific prediction; sensitive targeted and genome-wide sequencing; tests for large structural and chromosomal changes; integration analysis when donor or vector DNA is used; and biological interpretation in the intended cells. A negative assay narrows uncertainty within its validated scope—it does not make risk disappear.
