The main risks at mine start-up are connected rather than isolated: unsafe ground, unreliable equipment, incorrect ore and recovery assumptions, water or tailings failures, environmental and community harm, and financial pressure can interact during the production ramp. A mine is not “started” safely or profitably because one switch is turned on. Mine development, equipment, power, water systems, tailings storage, the process plant, trained people and emergency arrangements must become a functioning operating system.

The balance changes with the mining method, commodity, geology, climate, infrastructure and jurisdiction. The evidence available does not establish a universal ranking or a probability that a new mine will miss its target. The practical question is whether the specific operation has demonstrated coordinated readiness and controls for its credible failure modes.

Why production start-up is a connected transition

Production ramp-up is a phased build-up, not a single milestone. One project execution plan filed with the Australian Securities Exchange describes production increasing in stages while several workstreams come online: orebody development, procurement and installation of mining equipment, water treatment and surface-water infrastructure, tailings commissioning and process-plant commissioning. That example illustrates a general dependency: a late water system, an unavailable haul truck fleet, an unready tailings facility or an underperforming plant can hold back the whole chain.

Trial production also exposes assumptions that were difficult to test during construction. Commissioning teams must move from dry tests and isolated equipment checks to continuous operation with variable ore, real water balances, maintenance demands and production shifts. A disclosure for a mining project states that successful commissioning and trial production cannot be guaranteed and identifies breakdowns, maintenance problems, stoppages, delays and resulting costs as risks. This is a disclosed project risk, not an industry-wide incident rate.

The operating conditions that change the risk picture

  • Mining method: Open pits face slope and pit-wall hazards, while underground mines add ground-control, ventilation, access and inrush scenarios. The controls and emergency plans are not interchangeable.
  • Orebody and metallurgy: Hardness, grade distribution, clay content, moisture and mineralogy affect blasting, crushing, throughput and recovery.
  • Climate and water setting: Extreme rainfall, snowmelt, drought, groundwater and flood pathways alter storage capacity and emergency response needs.
  • Infrastructure and jurisdiction: Remote power, roads, ports, skilled labor, permits and regulator requirements can determine whether a technical problem becomes a prolonged outage.

Risk map: what can go wrong after opening

Risk area Typical start-up failure What determines severity
People and ground control Exposure to unstable ground, slopes, underground workings or other operational hazards Mine method, geological conditions, hazard controls, supervision and emergency readiness
Commissioning and equipment Plant or mobile equipment fails to perform; stoppages reduce trial output Design margins, installation quality, spares, maintenance capability and linked infrastructure
Geology, grade and recovery Ore dilution or characteristics differ from the model, reducing throughput, recovery or saleable output Sampling quality, geological variability, mining selectivity and process response to actual ore
Water and tailings Inrush, an incorrect water balance, loss of containment or facility instability Design assumptions, monitoring, storage freeboard, construction quality and emergency action
Environment and community Spills or containment failures affect water, land, wildlife, workers or nearby residents Potential pathways, receptors, response time and the effectiveness of containment and notification
Finance and markets Cost increases, funding constraints or market conditions undermine the operating plan Project financing, contingency, commodity prices, contracts and offtake terms

The table is a framework, not a ranking. A deep underground mine in competent rock can have a different dominant exposure from a shallow open pit with a large water-retention system, and neither can be judged from a generic start-up statistic.

Worker safety and ground control

When production begins, more people, vehicles, explosives, conveyors, crushers and active faces operate at the same time. The central physical risk is loss of control over the ground: a pit-wall or highwall movement at a surface mine, falling or squeezing ground underground, a rockfall, or an unexpected geological structure. A project risk disclosure identifies geological uncertainty as capable of affecting both mine design and operational safety and separately identifies ore dilution and pit-wall instability.

Surface and underground hazards differ

In an open pit, wall angles, benches, catch berms, blasting practices, rainfall and groundwater influence whether material moves into a working area. In an underground mine, stress redistribution, unsupported openings, rockbursts, loose ground, ventilation interactions and water inrush can dominate. The same label—“ground control”—therefore describes different hazards and requires mine-specific designs, inspections, exclusion zones, monitoring and trigger-action plans.

Readiness questions for people and ground

  • Have actual mapped structures, water conditions and observed ground behavior been reconciled with the design model?
  • Are geotechnical inspections, instrumentation and trigger thresholds operating on every active work area?
  • Do workers and supervisors understand stop-work authority, exclusion zones, traffic separation and emergency communications?
  • Can the operation evacuate, account for personnel and provide rescue or medical response when a ground event, fire, inundation or equipment incident occurs?

Personal protective equipment is necessary but cannot substitute for engineered support, stable designs, competent supervision and a functioning emergency system.

Commissioning and equipment reliability

Commissioning turns installed assets into a production system. Pumps must deliver the design flow, crushers and mills must tolerate the actual ore, conveyors must interlock correctly, power systems must remain stable and mobile equipment must be available for the planned mining sequence. Early failures can force repeated stoppages while crews diagnose whether the cause is design, installation, software, controls, operator training or maintenance.

How reliability problems affect the ramp

A single failed item can have a larger effect during ramp-up than during steady state because there may be no spare capacity, incomplete redundancy or limited inventory of critical parts. If the process plant is down, mined ore may accumulate or require rehandling; if mining equipment is unavailable, the plant may be starved; if water treatment or pumping is constrained, other activities may have to stop. The disclosed risk of breakdowns, maintenance issues and stoppages therefore translates into both lost output and additional labor, contractor and repair costs.

Commissioning checks that matter

  1. Verify installation: Confirm that equipment, power, controls, guarding, instrumentation and interlocks match approved designs and are documented.
  2. Test by subsystem: Run water, power, communications, crushing, conveying, grinding, flotation or other process circuits under controlled conditions before combining them.
  3. Use representative ore: Confirm throughput, wear, moisture handling and recovery with the range of ore expected during the first mining phases.
  4. Prove maintenance capacity: Check critical spares, work orders, isolation procedures, lubrication, inspection routes and the availability of trained technicians.
  5. Exercise abnormal scenarios: Test trips, loss of power, blocked chutes, pump failure, water ingress, fire response and safe restart procedures.
  6. Gate the ramp: Increase production only when safety, water, tailings and process constraints remain within approved operating limits.

Geology, grade, recovery and missed production targets

A feasibility model describes an expected orebody and process response; operations reveal the variation. Contacts may be misplaced, grades may be more erratic, dilution may be higher, or hardness and mineralogy may reduce plant throughput. Recovery—the proportion of valuable mineral recovered into a saleable product—can also differ from laboratory or pilot results when scale, residence time, water chemistry and maintenance are different.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Why the first targets are especially uncertain

Early mining often opens only a limited part of the orebody, so production data may not yet represent the full deposit. Mine crews are learning drilling, blasting, loading and blending parameters while the plant is learning how to handle changing feed. If lower-grade material is sent to the plant to maintain tonnes, metal production can still fall; if higher-grade zones are mined early, short-term results may not represent the long-term plan.

The appropriate test is not whether a forecast was exactly correct, but whether the operator has a process for reconciling models with blast, face, stockpile and plant data and then updating schedules, geotechnical designs and recovery assumptions. Without the mine’s technical disclosures, it is not possible to make a site-specific claim about which assumption is most likely to fail.

Water, tailings and containment

Water and tailings are operational safety issues as well as environmental issues. Tailings facilities must be managed through design, construction, operation and decommissioning. The New South Wales Resources Regulator says a failure can injure or kill workers or members of the public and contaminate downstream rivers, land, groundwater and wildlife.

What the NSW compliance program shows

The regulator’s 2025 compliance priority report covered 37 operations assessed from October 2024 to July 2025. It focused on inundation or inrush and uncontrolled movement involving tailings dams, water storages or waste tips. Those 37 operations are the scope of that NSW program—not a global mine count and not an incident frequency.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Recommendations included reviewing site hazard plans and emergency response arrangements and maintaining factor-of-safety calculations as facilities develop. These are regulator findings and guidance for that jurisdiction, not a universal legal checklist. Elsewhere, operators must follow the applicable approvals, design standards and regulator directions.

How a water balance can compound a failure

The Government of British Columbia’s 2015 investigation summary of the August 4, 2014 Mount Polley tailings breach describes a structural failure combined with insufficient beaches and excess supernatant water, producing an erosional breach. The summary identifies inadequate water management and incomplete characterization of surplus water among the shortcomings discussed by the investigation. This documented case should not be treated as proof that every new mine has the same conditions; it demonstrates how design, water storage and operating decisions can interact.

Controls to examine throughout the facility life

  • Whether the water balance includes rainfall, snowmelt, groundwater, process water, seepage, evaporation and credible extreme events.
  • Whether freeboard, beaches, drainage and pumping capacity are measured against current—not only planned—conditions.
  • Whether instrumentation, inspections and independent reviews detect deformation, seepage, pore pressure or changing factors of safety.
  • Whether emergency action levels identify who decides, who is notified and how downstream people and assets are protected.
  • Whether design assumptions are rechecked after construction changes, production changes or unusual weather.

Environmental and community effects after start-up

Environmental risk does not end when permits are obtained. Start-up introduces process chemicals, fuel transfers, waste streams, traffic, dust, noise and larger water movements. A release can travel through a drain, river, groundwater pathway or floodplain, affecting people and ecosystems beyond the lease boundary. The relevant receptors and response time depend on the actual site.

A comprehensive study report for the North American Lithium Spodumene Mine Project records two accidental spills after start-up. In its discussion of one water-related event, the report presents the proponent’s explanation involving water accumulation beneath a tailings-facility liner and hydraulic mechanisms while also recording site observations. That is an account documented in the assessment record and attributed to the proponent; it should not be rewritten as an independent causal finding.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Community risk is also an operating risk

Residents, downstream users, Indigenous communities, workers and emergency services may experience the consequences of a spill, traffic incident, dust event or loss of water access. Clear notification routes, accessible monitoring results, grievance handling and rehearsed emergency coordination can reduce confusion when an event occurs. The quality of those arrangements is mine- and jurisdiction-specific.

Cost, funding and market pressure

A mine can be technically capable of producing and still be unable to sustain the planned ramp. Project disclosures identify funding constraints, cost overruns and market-related conditions as risks. Construction changes, commissioning delays, contractor claims, extra water treatment, accelerated maintenance or replacement equipment can consume contingency precisely when revenue is below plan.

Commodity prices, concentrate treatment terms, transport availability and offtake conditions affect cash flow, but their effect cannot be generalized across commodities or projects. A financial review should examine the mine’s funding runway, remaining capital commitments, contingency, debt covenants, hedging or offtake terms and the assumptions behind its production and price forecasts.

How to evaluate a mine’s start-up readiness

Investors, workers, communities and regulators can use the following questions to test whether a project is moving from construction into controlled operations. They are a decision framework, not a substitute for the mine’s approved technical studies or regulator requirements.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Define the operation: Identify commodity, open-pit or underground method, location, climate, processing route and current phase.
  2. Map dependencies: Confirm that mine development, power, roads, equipment, water treatment, tailings storage and process circuits have owners, dates and tested interfaces.
  3. Compare plan with evidence: Reconcile actual grades, dilution, geotechnical observations, water volumes, equipment availability and recovery against the assumptions used for the ramp.
  4. Check independent oversight: Review regulator conditions, qualified-person reports, facility reviews, inspection findings and outstanding corrective actions.
  5. Test abnormal operations: Ask whether emergency response covers ground movement, inrush, fire, power loss, process upset, spill and tailings instability, and whether exercises have produced improvements.
  6. Assess resilience: Examine critical spares, alternate water routes, storage capacity, communications, skilled staffing, financing and the ability to slow or stop production safely.
  7. Set transparent gates: Require measurable safety, environmental, reliability and financial criteria before increasing throughput.

What cannot be concluded from industry-wide headlines

There is no established global percentage for the chance that a mine will fail to reach its planned production, and the cited sources do not provide a comparable cross-mine start-up incident rate. The NSW assessment of 37 operations cannot be converted into either figure. Nor can one tailings breach or one project’s spill history determine the risk at a different mine with different geology, design and controls.

The defensible conclusion is narrower and more useful: production start-up is a period when construction assumptions, operating systems and natural variability meet. Risk is managed by demonstrating readiness across people, ground, equipment, ore, water, tailings, environment and finance, then adjusting the plan when operating evidence differs from the model.
