Gene editing is not one procedure with one risk profile. The hazards depend on which DNA sequence is targeted, whether cells are edited inside or outside the body, how the editing system is delivered, which tissues are reached, whether the change is somatic or heritable, and how completely researchers can detect unintended outcomes.

The main concerns are unintended DNA changes, mosaicism (only some cells receiving the intended edit), harms from the delivery system or therapy itself, uncertain long-term effects, and ethical and governance problems. Somatic editing affects the treated person; heritable editing can affect descendants and raises additional safety and consent issues.

Why gene-editing risk cannot be reduced to one number

Gene editing includes different tools, targets and clinical settings. A treatment that edits blood cells outside the body is assessed differently from one that delivers an editing system directly into an organ. Editing a non-reproductive cell in one patient is also fundamentally different from editing an embryo or reproductive-cell lineage that could pass a change to future children.

Risk factor Why it matters
Target and editing tool The system may change DNA at unintended sites or create an unwanted alteration where it was supposed to edit.
Cells and tissue reached Effects depend on which cells receive the product and whether those cells can be removed, monitored or replaced.
Delivery route Ex vivo and in vivo procedures expose patients to different product, injection and tissue-distribution risks.
Intended benefit A serious, otherwise untreatable disease may justify a different level of uncertainty than an enhancement or a condition with effective alternatives.
Inheritance Somatic edits are not intended to pass to children; edits affecting the germline or an embryo could be inherited.
Evidence and oversight Confidence depends on the quality of sequencing, biological follow-up, clinical monitoring and applicable regulatory review.

There is no single, established risk percentage that applies to gene editing as a whole. Safety evidence must be tied to a specific product, target, delivery method, disease and study population.

Unintended edits and damage to genome integrity

Off-target editing

Editing systems can alter DNA at locations other than the intended target. The National Institutes of Health explains that CRISPR is not perfect and can sometimes cut in the wrong spot; researchers do not yet know how every such error could affect a patient. See the NIH overview of gene-therapy risks.

Potential consequences depend on the affected sequence and cell type. An unintended change in a growth-control gene, for example, could be more concerning than a change with no apparent biological effect. Detecting an off-target event does not automatically show that it will cause harm, but failing to detect one makes the risk harder to evaluate.

Unwanted on-target changes

Even at the intended site, DNA repair can produce outcomes other than the planned edit. Insertions, deletions, rearrangements or other structural changes may occur. The U.S. Food and Drug Administration’s April 2026 draft guidance says safety assessment should address both potential off-target editing and loss of genome integrity using next-generation sequencing and bioinformatics. That document is explicitly draft guidance and says it contains non-binding recommendations, not a final worldwide requirement.

Genome-integrity testing matters because a test that checks only whether the desired small edit is present can miss larger or unexpected changes nearby. The appropriate testing strategy depends on the editing product and the cells being treated.

Mosaicism: when only some cells are edited

Mosaicism occurs when a person or developing embryo has a mixture of edited and unedited cells. The National Human Genome Research Institute lists mosaicism alongside off-target changes as a safety concern. In a treatment, some cells may receive the intended correction while others retain the disease-associated sequence or acquire a different alteration.

Mosaicism complicates both treatment benefit and risk assessment. A sample taken from one tissue may not represent what happened in another tissue, and the proportion of edited cells can influence the clinical result. In embryos, the problem is especially difficult: cells can divide and populate different tissues, so the eventual distribution and consequences may be unpredictable.

The National Academies’ discussion of heritable editing says current analytical methods are not sufficient in a clinical context to assess on-target events, off-target events and mosaicism adequately, and that DNA repair after an editing-induced break is difficult to predict. These limitations are described in its chapter on heritable human genome editing.

Risks from delivery and the therapy product itself

Editing accuracy is only one part of safety. A gene-editing treatment also includes a delivery vehicle, manufacturing process, dose and administration route. Those components can cause harm even if the intended DNA change is made correctly.

NIH identifies possible gene-therapy risks that include some cancers, allergic reactions and damage to organs or tissues when an injection or other delivery procedure is involved. These are potential adverse effects, not a frequency estimate and not a claim that every editing therapy causes them.

In vivo treatments, which deliver the editing system directly into the body, must reach the right tissue while limiting exposure elsewhere. Ex vivo treatments remove cells, edit and test them in a laboratory, then return them to the patient; this can allow more inspection before reinfusion but does not eliminate manufacturing, conditioning or reinfusion risks. The actual risk profile depends on the product and protocol.

Long-term uncertainty

Some effects may not be visible during a short clinical trial. An altered cell could behave differently months or years later, and a patient’s broader genetic background may influence the result. Long-term follow-up is therefore important for understanding delayed adverse effects, durability of benefit and changes that were not predicted before treatment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Long-term uncertainty is more consequential when an edit can be inherited. A somatic treatment can still create serious individual risks, but a heritable change could affect people who were not participants and could not consent. The World Health Organization (WHO) distinguishes these situations on its human genome-editing page.

Somatic and heritable editing are not the same

Category What is changed Main additional concern
Somatic editing Non-reproductive cells in the treated person Individual treatment risks, unintended edits, delivery harms and uncertain long-term effects. The edit is not intended to be passed to children.
Germline research Reproductive cells or embryos in a research setting Results may be difficult to predict, especially because repair outcomes and mosaicism can vary among cells.
Heritable editing used to establish a pregnancy An embryo or reproductive-cell lineage in a way that could create an individual carrying the edit Potential effects on the individual and descendants, plus unresolved consent, safety and governance issues.

WHO reports that, in July 2019, its Director-General stated that “it would be irresponsible at this time for anyone to proceed with clinical applications of human germline genome editing.” This is a reported WHO policy statement from 2019, not a claim that every country has adopted identical law or that the statement alone determines current legality.

How delivery and context change the risk question

Comparison axis Questions that determine risk
Ex vivo versus in vivo Can cells be examined before they are returned, or must the product be distributed inside the patient? What tissues might be exposed?
Target tissue and route Is the target accessible, and can the delivery route injure nearby organs or reach unintended tissues?
Purpose and disease severity What benefit is expected, how severe is the condition, and are established alternatives available?
Evidence of unintended changes Were on-target structural changes, off-target sites and genome integrity assessed with methods sensitive enough for this product?
Inheritance and reversibility Could the edit reach reproductive cells or descendants, and is there any practical way to stop or correct an adverse effect?
Oversight and follow-up Which regulator reviewed the product, what monitoring is required, and for how long will participants be followed?

Why measuring safety is technically difficult

A credible assessment must look beyond whether the desired edit appears in a laboratory sample. Investigators need to characterize unintended edits at the target site, changes elsewhere in the genome, the proportion of edited cells and the biological consequences in relevant tissues.

Next-generation sequencing and bioinformatics can help identify variants and structural changes, but no method automatically answers every clinical question. Sampling may miss rare events or tissues that were not tested. Embryo editing presents an additional challenge because a small number of sampled cells may not reveal what will occur as development proceeds.

The FDA’s document, Safety Assessment of Genome Editing in Human Gene Therapy Products Using Next-Generation Sequencing, was issued as draft guidance in April 2026. It frames assessment of editing-specific risks alongside risks of the gene-therapy product itself. Because it is nonbinding draft guidance, it should not be presented as a final rule or as a universal standard outside the United States.

Regulatory oversight is jurisdiction-specific

In the United States, the FDA says it has regulatory authority over genetically manipulated cells and their derivatives. A clinical study generally requires an Investigational New Drug application before it can proceed; details depend on the product and study. The FDA’s overview of therapeutic cloning and genome modification is specific to the U.S. regulatory context.

Rules, approval pathways and restrictions differ elsewhere. WHO’s 2021 governance framework and recommendations call for national and transnational oversight, international collaboration, registries, attention to medical travel, action against unsafe or unregistered activity, and public engagement. These are governance recommendations, not proof that all jurisdictions currently apply the same controls.

Ethical and social limitations

Consent across generations

A patient receiving somatic treatment can consent to personal risks, although consent may still be difficult when evidence is limited. Future descendants cannot consent to a heritable edit, and their interests may differ from those of the people making the decision.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Equity and access

Complex gene-editing treatments may be available only to people with specialist facilities, clinical-trial access or substantial financial resources. Unequal access could widen existing health disparities. Equity concerns include who participates in research, whose genetic conditions receive investment and whether communities are meaningfully involved in decisions.

Disability and enhancement

NHGRI identifies disagreement about editing for disability-related traits, enhancement and the social meaning of genetic difference. These are contested questions, not matters with one universally accepted answer. Policies should distinguish treating a serious disease from selecting or enhancing traits, while recognizing that the boundary can be debated.

Governance and public trust

Weak oversight, unregistered procedures or cross-border travel can make it difficult to verify safety and accountability. WHO’s governance work emphasizes registries, international coordination, education and engagement because scientific capability can advance faster than laws and institutions.

Questions to ask about a proposed gene-editing treatment

Patients and families evaluating a trial or approved therapy can use the following checklist with the treating team and an independent genetics professional:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Which cells are edited, and is the procedure ex vivo or in vivo?
  • What delivery vehicle and route are used, and what non-editing harms are associated with them?
  • How were off-target sites and unintended on-target changes tested?
  • Did testing examine structural changes and genome integrity, not only the intended sequence?
  • How was mosaicism measured, and could untested tissues contain different results?
  • What evidence shows that the expected benefit outweighs available alternatives for this condition?
  • What follow-up is required, for how many years, and who pays for monitoring?
  • Could the edit affect reproductive cells or be inherited?
  • Which regulator and ethics committee reviewed the study, and where can its registration and results be checked?

For decisions involving inherited disease, qualified genetic counseling can help explain inheritance, alternatives and uncertainties without assuming that one option is right for every family.

What the evidence supports today

Some somatic gene-editing applications are being developed or used under medical regulation, but that does not make every editing product safe or interchangeable. Safety must be established for the particular target, delivery system, dose, tissue and patient population. Heritable editing remains a distinct category with greater potential consequences and unresolved ethical and governance questions.

For a detailed treatment of the science, ethics and governance issues, see the National Academies’ Human Genome Editing: Science, Ethics, and Governance.
