Nanoparticles are not a single cancer treatment with a predictable safety profile. A nanoscale carrier can package or bind a drug, alter how long it circulates, change where it distributes, release a payload in response to conditions, or support imaging and other treatment strategies. Those capabilities may improve delivery, but they also make exposure, clearance, immune effects, manufacturing quality and toxicity harder to predict.
The central risk is formulation-specific: carrier material, particle size, surface chemistry, payload, administration route and intended use all influence biological behavior. A promising design goal—such as targeting a tumor—does not by itself prove that a particular product reaches the relevant cancer cells, releases active drug there or avoids harmful exposure elsewhere. The National Cancer Institute (NCI) and peer-reviewed safety reviews emphasize evaluating each formulation on its own evidence.
Why “nanoparticles” cannot be assessed as one treatment class
Nanomedicines may use different materials and carry different payloads, from conventional chemotherapy to biologically active molecules. Even two particles made from a similar material can behave differently if their size, surface coating, shape, drug loading or route of administration differs. The same property that keeps a particle in circulation or helps it interact with a cell can also alter immune recognition, organ distribution or elimination.
For that reason, safety claims must be tied to a named formulation and its intended use. Results from one lipid, polymer, metal or other carrier should not automatically be applied to another. The 2020 review “Safety Considerations of Cancer Nanomedicine—A Key Step toward Translation” identifies material-specific properties, absorption, distribution, metabolism and excretion (ADME), toxicokinetics and non-obvious nanosafety endpoints as central to regulatory decisions (Liu et al., 2020).

Challenge 1: Reaching the right tumor cells and releasing the drug
Targeting is a design objective, not a guarantee
A carrier may be engineered to circulate, bind a marker or accumulate in tumor tissue, but tumors are biologically heterogeneous. Blood flow, tissue structure, cell-surface markers, pressure, immune-cell activity and the tumor microenvironment differ between cancers and between patients. A particle that accumulates in one tumor model may distribute differently in another. Evidence must therefore show more than the presence of particles near a tumor: it should establish whether the intended cancer cells receive the formulation and whether the drug remains pharmacologically active.
Integrity can change between injection and action
A 2025 expert discussion in Nature Communications stresses verifying nanoparticle integrity from administration to the site of action (“Q&A Translational Cancer Nanomedicine,” 2025). A carrier can aggregate, shed a coating, exchange material with blood components or release part of its payload before reaching the tumor. Investigators need to know whether the particle remains intact when that is required, breaks down at the intended location and releases the drug at a useful rate.
Route of administration changes the problem
Interactions can differ substantially after oral, intravenous, intratumoral or intramuscular administration. Oral delivery encounters the gastrointestinal tract and absorption barriers; intravenous delivery exposes the formulation immediately to blood proteins and immune surveillance; direct intratumoral injection creates a different distribution pattern from systemic dosing. A result obtained with one route cannot establish safety or biodistribution for another.

Measuring the carrier alone may not reveal where active pharmacological exposure occurs. The 2025 Q&A recommends examining levels of released drug and encapsulated or conjugated drug in circulation, tumor tissue and off-target organs. A particle can remain in one location while free drug diffuses elsewhere, or it can release too slowly to achieve an effective concentration in cancer cells.

Challenge 2: Unpredictable distribution, clearance and off-target toxicity
Protein-corona formation changes what the body sees
Once in biological fluids, nanoparticles interact with proteins and cells. Adsorbed proteins—often described as a “protein corona”—can alter the particle’s apparent identity, uptake by immune cells, circulation time and tissue distribution. These interactions complicate efforts to predict behavior from measurements made in a buffer or from the original surface coating alone.
Liver and kidneys may handle much of the exposure
Clearance can involve the liver, kidneys and immune-cell systems. Developers must determine where the carrier and its payload go, how long they remain and which breakdown products are formed. Both short-term and long-term exposure matter, particularly when repeated doses are planned. A formulation that is cleared slowly may create cumulative exposure even when each individual dose appears tolerable.
The organ with the most particles is not necessarily the organ most harmed
Biodistribution data are informative but do not by themselves identify dose-limiting toxicity. The 2025 Q&A uses chemotherapy-loaded liposomes as an example: high liver accumulation does not necessarily mean that the liver is the tissue experiencing the most clinically important injury. Toxicity can depend on the active drug released, intracellular processing, local sensitivity and exposure to other organs.

Carrier and payload can contribute separate risks
The payload retains its own pharmacological effects, while the carrier can add biological interactions or change the payload’s exposure. Depending on the formulation, investigators may need to examine injury in clearance organs or other tissues, infusion reactions and complement-activation-related pseudoallergy. These are potential formulation-specific issues to investigate, not effects that occur with every nanoparticle treatment.

Challenge 3: Immune effects may be intended, amplified or unexpected
Nanoparticles are not automatically immunologically inert. As Invitee 2 states in the 2025 Nature Communications Q&A, “Many nanotechnology carriers are not immunologically inert.” A carrier may activate innate immune pathways, alter cytokine release or change how immune cells process the payload. In some cancer strategies, immune stimulation is part of the therapeutic design; in others, it is an unwanted reaction.
Carrier and payload activity can overlap
If a lipid nanoparticle carries an immunostimulatory molecule, the carrier’s own immune interactions could amplify the payload. Excessive activation may raise concerns about systemic inflammation or cytokine-storm risk. The relevant question is not simply whether the payload is immunostimulatory, but whether the combined formulation produces a response that is predictable, controllable and appropriate for the intended route and dose.
Animal models do not perfectly predict human responses
Species differences in complement proteins, immune-cell populations, metabolism and prior immune exposure can limit how well animal findings forecast human reactions. Preclinical results therefore need to be interpreted alongside the intended clinical use, dosing schedule and known limitations of the model. A lack of severe findings in animals does not prove that an immune-sensitive patient population will experience no risk.

Nanomedicine safety testing must cover more than conventional single-dose toxicity. The formulation’s ADME profile and toxicokinetics should be measured, along with exposure after repeat dosing, persistence of material, degradation products and interactions with the payload. Studies should examine relevant organs and tissues over time, not only the first days after administration.

Important endpoints can include clinical chemistry, organ histopathology, blood and immune parameters, complement activation, infusion reactions, reproductive or developmental effects where relevant, and delayed or cumulative toxicity. The precise panel depends on the material, payload, route, patient population and planned treatment duration. “No visible accumulation” or “no acute reaction” is not a complete long-term safety assessment.
Challenge 5: Manufacturing, sterility and characterization
Small physical changes can change biological behavior
Manufacturers must control particle size distribution, surface characteristics, morphology, payload concentration, encapsulation or conjugation, release behavior, impurities and stability. Batch-to-batch variation can alter circulation, uptake or immune activation. Processes that work at laboratory scale may produce a different product when volumes, mixing energy, temperature or storage conditions change.
Sterility and endotoxin contamination are critical failure points
The 2025 expert Q&A reports that approximately 30% of nanomedicine submissions to the NCI Nanotechnology Characterization Laboratory failed the characterization described in that discussion in connection with sterility and bacterial endotoxin contamination. This is a figure about laboratory characterization of submissions, not a rate of clinical failure, patient harm or all nanoparticle medicines.
The NCI Nanotechnology Characterization Laboratory is a collaboration involving NCI, the U.S. Food and Drug Administration and the National Institute of Standards and Technology. It provides preclinical characterization and safety testing intended to identify material and manufacturing problems before clinical development advances.

What evidence is needed before a formulation can be trusted?
| Question | Evidence to examine | Risk if unresolved |
|---|---|---|
| Does the product remain the intended particle? | Particle size, surface chemistry, morphology, payload loading, integrity and release behavior before and after contact with biological fluids | Unexpected distribution, premature release or a different immune response |
| Where does it go? | ADME, toxicokinetics and measurements of carrier, released drug and encapsulated or conjugated drug in blood, tumor and off-target organs | Active drug may reach the wrong tissue or persist longer than expected |
| Does it reach relevant cancer cells? | Evidence that the formulation reaches intended cells and releases pharmacologically active payload at the site of action | Accumulation without effective treatment, or exposure without tumor selectivity |
| What does the immune system do? | Complement, cytokine, immune-cell and infusion-reaction assessments interpreted for the intended route and dose | Hypersensitivity, unwanted inflammation or amplified immune stimulation |
| Can every batch be made safely? | Reproducible manufacturing, stability, sterility, endotoxin control and impurity testing | Variable potency, contamination or inconsistent toxicity |
| What happens after repeated or delayed exposure? | Short- and long-term toxicity studies, persistence and clearance of carrier and payload | Cumulative, delayed or organ-specific injury missed by acute testing |
Clinical translation is progressing, but research promise is not approval
The NCI reports that “Significant progress has been made in implementing nano-based cancer therapies and diagnostics in the clinic, with many more interventions under development.” That statement supports a balanced conclusion: some nano-based interventions have entered clinical use, while a large pipeline remains investigational.
A mechanism, animal result or early-stage trial does not establish that a formulation is approved, available for a particular cancer or proven superior to other treatments. Regulatory status, eligibility, dosing and known adverse effects must be checked for the specific product and indication. Availability can also differ by country and change as trials and labeling are updated.
How developers should investigate a nanoparticle cancer treatment
- Define the formulation precisely. Record the carrier material, size range, surface chemistry, payload, loading method, intended release mechanism, route and dosing schedule. Avoid treating “nanoparticle” as the product name.
- Characterize the material under relevant conditions. Test the product in the formulation used for patients and after exposure to blood or other biological fluids. Confirm integrity, aggregation, payload retention, release and stability.
- Map carrier and drug exposure separately. Measure carrier, free drug and encapsulated or conjugated drug in circulation, tumor tissue and likely off-target organs. Include time points that reveal both early distribution and delayed clearance.
- Assess ADME and toxicokinetics. Determine absorption, distribution, metabolism and excretion, including the roles of liver, kidneys and immune-cell clearance. Model repeat dosing when repeated treatment is planned.
- Test efficacy at the cellular level. Establish whether the active payload reaches the relevant cancer cells, remains potent and is released at a concentration and timing capable of producing the intended effect.
- Run formulation-specific toxicity and immune studies. Examine organ injury, complement activation, cytokines, immune-cell interactions, infusion reactions and delayed effects. Interpret animal findings with species and model limitations in mind.
- Demonstrate manufacturing control. Show reproducible batches, validated sterility and endotoxin controls, acceptable impurities, stability through storage and transport, and consistent release specifications.
- Match conclusions to clinical evidence. Separate laboratory findings, animal data, early human safety observations and evidence of clinical benefit. Confirm the current regulatory label or trial status for the exact indication.
Questions patients and clinicians should ask about a specific product
- What are the carrier material, payload, particle-size range and administration route?
- Is the formulation approved for this cancer and treatment setting, or is it investigational?
- What human data show where the carrier and released drug distribute?
- Which organs clear the formulation, and what monitoring is used for those organs?
- What infusion, hypersensitivity, complement or cytokine-related reactions have been observed?
- What is known about repeated dosing, delayed toxicity and interactions with other treatments?
- How are sterility, bacterial endotoxin, particle consistency and payload release controlled between batches?
- Does evidence show delivery to relevant tumor cells, or only average accumulation in tumor tissue?
Bottom line
The main challenges are biological and practical: a nanoparticle may not reach the intended tumor cells or release drug at the right time; it may distribute to clearance organs and other tissues in ways that are difficult to predict; its carrier and payload may trigger overlapping immune effects; and small manufacturing or contamination problems can change safety. Because these risks depend on the exact formulation, route and cancer indication, the meaningful unit of evaluation is not “nanoparticles” in general but one fully characterized product supported by biodistribution, toxicology, immune, manufacturing and clinical evidence.
