MCP tool calls are structured requests that let an AI client ask an MCP server to perform an app-specific operation. The server publishes tools with names, descriptions, and input schemas; a connected client makes those definitions available to the model; the model can select a tool; the host sends the call to the server; and the server returns output that is added to the model’s context. The server’s implementation, credentials, exposed tools, and host settings—not MCP by itself—determine which app data can be reached.
MCP in one sentence
The Model Context Protocol (MCP) standardizes communication between an AI client and a server that exposes callable tools and readable data. An MCP server publishes tool definitions and runs tool calls. The protocol does not contain a universal database connector or automatically grant access to every application. Instead, it gives the client a consistent way to discover an operation, provide arguments, receive a result, and continue the conversation.
That separation matters. A server might use an application’s API, query a database, read files, or perform a computation. Another server could expose completely different operations. The model sees the interface the server advertises and the results it returns, while the server and its configuration determine what happens behind that interface.
“The Model Context Protocol (MCP) allows servers to expose tools that can be invoked by language models.” This wording comes from the Model Context Protocol specification.

What happens during an MCP tool call?
A call is a sequence involving the server, client or host, model, and the underlying app. In OpenAI’s Responses API flow, the API discovers available tools, the model may choose one, the API calls the remote server, and the server output is returned to the model’s context.
- The server publishes tools. Each tool has a name, descriptive metadata, and an input schema. The metadata tells a client what operation exists and which arguments are expected. A tool could represent a database query, an application API request, or a calculation.
- The client discovers the definitions. The client retrieves the server’s tool list and makes the relevant definitions available to the model. The Agents API also discovers tools from a connected server.
- The model decides whether to call one. Tool selection is model-controlled in MCP’s interaction design. A host can choose how that decision is presented to a user; MCP does not prescribe a single interface or approval screen.
- The host dispatches the request. The call contains the selected tool name and its arguments. The connected server receives that structured request and runs its implementation.
- The server performs the operation. It applies its own code, credentials, permissions, and connection to the target application or data store. The server may return a direct result, an error, or a link to a resource.
- The result is added to context. The host passes the server output back into the model’s context. The model can use it to answer the user, make another tool call, or explain an error.
In the Responses API, an MCP call item represents the arguments, output, error information, and server label associated with the operation. Those fields let the surrounding application distinguish what was requested, which server handled it, and whether execution succeeded.
What an MCP call contains
The exact wire representation depends on the host API, but the conceptual parts are consistent.

| Part | Purpose |
|---|---|
| Server | Publishes the available operations and executes the selected call. |
| Tool name | Identifies the operation the model wants to invoke. |
| Input schema | Defines the arguments and their expected shape so the client and model can form a valid request. |
| Arguments | Values supplied for this particular invocation. |
| Output | Data returned by the server for the model to use. |
| Error information | Details indicating that the server could not complete the operation. |
| Server label | Identifies the connected server associated with the call in the Responses API representation. |
A tool definition is an interface contract, not a promise that a record exists or that an operation will be authorized. The schema can make an argument syntactically valid while the server still rejects it because of credentials, policy, application state, or a downstream failure.
How an MCP server reaches app data
MCP defines the communication boundary; the server supplies the app-specific access. A server implementation can call the application’s public or internal API, issue a database query, read file contents, or perform another operation. It can then transform that result into the output sent to the model.

For example, an organization might run a server that exposes a read operation for a business system. The tool definition could describe a record lookup and require an identifier. When the model supplies that identifier, the server uses credentials configured for that integration, asks the business system for the record, and returns only the fields its implementation chooses to expose. MCP does not decide which fields are selected, whether writes are allowed, or how the application authenticates the request; those decisions belong to the server and its host configuration.
Consequently, the accessible data depends on four practical boundaries:
- Implementation: the code determines which application endpoints, tables, files, or computations are reachable.
- Credentials: the identity used by the server or host determines what the underlying application permits.
- Exposed tools: a server may publish only a subset of its possible operations.
- Host configuration: allowlists, approval settings, transport, and runtime placement can further restrict execution.
The model does not gain a general-purpose view of the app. It receives the definitions and outputs that the integration makes available.
Tools and resources are different MCP primitives
MCP distinguishes callable tools from readable resources. A tool is a function a model can invoke. A resource is data or content that a client can read, such as file contents, database records, or API responses. A tool can also return a resource link rather than embedding all content directly in its result.
| Primitive | Typical interaction | What it represents |
|---|---|---|
| Tool | The model or host invokes it with arguments. | An operation such as querying, updating, transforming, or calculating. |
| Resource | The client reads or retrieves it. | Content or data, including files, records, or API responses. |
This distinction helps explain why an integration may expose data without exposing an action that changes it. A read-only resource does not automatically imply a write-capable tool, and a tool’s ability to return data does not mean every resource on the server is visible.

Connection methods and where the server runs
The connection mode determines how the client reaches the server and where execution takes place. OpenAI documents remote MCP connections over Streamable HTTP or HTTP/SSE in the Responses API. Its Agents API guidance also describes HTTP connections made from OpenAI’s service, HTTP connections made from an execution environment, and local standard-input/standard-output (stdio) connections for a process in that environment.
| Connection approach | Server location or reachability | Where it is described |
|---|---|---|
| Streamable HTTP | Remote MCP server reachable over HTTP. | Responses API remote MCP connections. |
| HTTP/SSE | Remote server using HTTP with Server-Sent Events. | Responses API remote MCP connections. |
| HTTP from OpenAI’s service | The service makes the HTTP connection to the server. | Agents API connection guidance. |
| HTTP from an execution environment | An execution environment makes the HTTP connection. | Agents API connection guidance. |
| Local stdio | A server process runs locally in the execution environment and communicates through standard input and output. | Agents API connection guidance. |
These are deployment choices, not different meanings of a tool call. Select a mode based on where the server must run, which network can reach it, how credentials are held, and what data may leave the execution environment. A host may support only some of these options, so its product documentation controls the available configuration.
Authentication, approvals, and tool allowlists
Access control is part of the integration design rather than a property that MCP supplies automatically. The server needs an identity and credentials suitable for the app operation it performs. Depending on the host, credentials may be held by the remote server, supplied through the host environment, or managed by a connector. The resulting permissions are the permissions of that configured identity.
OpenAI documents two important controls:
- Approval behavior: a tool call can require explicit user approval or be allowed automatically. Approval settings affect whether a model-selected operation is dispatched without an additional confirmation step.
- Discovery restrictions: the Agents API lets developers restrict which tools an agent can discover. Limiting discovery reduces the set of operations the model can select.
OpenAI’s guidance also warns that sensitive data may be sent to a connector or remote server. Review the data path, the server operator, the credentials in use, and the approval policy before exposing an app integration. Defaults and available controls can vary by product, host, and deployment, so do not assume that one MCP client’s behavior applies to another.
An illustrative call from question to answer
Consider a server that publishes a read-only tool for retrieving records from an internal application. The tool definition states what the operation does and requires a record identifier. The following sequence shows the division of responsibilities; the tool name and record are illustrative, not protocol-required names.

- A user asks the AI client for a particular record.
- The client has already retrieved the server’s tool definition and includes it in the model’s available tools.
- The model determines that the record lookup tool is relevant and constructs arguments that conform to the published schema.
- The host checks its policy. If approval is required, it pauses for confirmation; if the call is allowed automatically, it dispatches the request.
- The server receives the tool name and arguments, authenticates to the underlying application, and performs the lookup within its configured permissions.
- The server returns the selected record fields or an error. It could also return a resource link for content the client should read separately.
- The host adds that result to the model’s context. The model then answers using the returned data rather than directly querying the application itself.
If the server account cannot see the record, the call can fail even though the model supplied valid arguments. If the host’s allowlist excludes the tool, the model may never discover it. If the remote endpoint is unreachable, no application query occurs. Each failure belongs to a different layer of the chain.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Choosing an MCP integration design
When evaluating an integration, compare the dimensions that affect actual data access rather than focusing only on the protocol label.
| Decision dimension | Questions to answer |
|---|---|
| Server placement and transport | Does the server need to be remote, reachable by the service, reachable only from an execution environment, or run locally over stdio? |
| Authentication and credentials | Which identity contacts the app, where are its credentials stored, and what permissions does that identity have? |
| Exposed and allowed tools | Which operations does the server publish, and which of those may the agent discover or invoke? |
| Approval policy | Which calls require a user confirmation and which, if any, can run automatically? |
| Data sharing | What arguments and results are sent to a connector or remote server, and could they contain sensitive information? |
A narrowly scoped, read-only tool set can reduce exposure compared with publishing every operation an application supports. Separating servers by environment or data domain can also make credentials and auditing easier to reason about, although the specific isolation features depend on the host and deployment.
Common failure modes and what to check
The tool never appears
Check that the server is reachable, that tool discovery completed, and that the tool is included in the host or agent’s allowed set. A server cannot be selected if it did not publish the tool or if discovery restrictions removed it.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →The call is waiting for confirmation
That behavior usually reflects an approval policy rather than an application error. Inspect whether the host requires explicit approval for this operation and whether the user has completed the approval step.

The server returns an authorization error
Verify which credentials the server is using and whether that identity can perform the requested operation in the underlying app. A valid MCP schema does not override application permissions.
The result is empty or incomplete
Review the server implementation and the fields it chooses to return. The app may contain more data than the server exposes, or the configured account may have a narrower view.
The remote connection fails
Confirm that the selected transport matches the host, that the server is reachable from the location making the connection, and that the server is running. A local stdio server must exist in the execution environment; a remote HTTP server must be reachable by the service or environment assigned to it.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesThere is a data-sharing concern
Identify the arguments and output that cross the connector boundary, then review approval settings, allowlists, credentials, and server ownership. If the data is sensitive, expose only the tools and fields required for the task.
What MCP does—and does not—guarantee
- It does provide: a common way for clients to discover server-published tools, send structured arguments, receive results, and work with resources.
- It does not provide: universal access to an application, automatic authorization, a mandatory user-interface pattern, or a guarantee that every server supports the same operations.
- It leaves to the integration: API and database implementation, credential handling, tool exposure, transport deployment, approval behavior, and data filtering.
That boundary is the key to understanding app-data access. The model can request only what the host makes discoverable, and the server can return only what its implementation and credentials permit.
Bottom line
An MCP tool call is the handoff between a model-driven client and an app-connected server. The client discovers a tool, the model may select it, the host dispatches a structured request, and the server performs the operation and returns context for the model. To determine what data is really accessible, inspect the server’s implementation, credentials, exposed and allowed tools, transport, approval policy, and the information shared with the connector.
