Immunologically “cold” tumors lack one or more conditions needed for an effective antitumor immune response: visible tumor antigens, activated immune priming, entry of immune cells, or sustained immune-cell function. Approaches under study try to remove those barriers by causing immunogenic tumor-cell death, stimulating innate sensing, recruiting immune effectors, opening physical routes into the tumor, reducing suppressive cells and signals, or combining these measures with checkpoint blockade.
No single treatment reliably converts every cold tumor into a “hot” one. The relevant question is which barrier dominates in a particular cancer, what combination and delivery route address it, and whether clinical evidence—not just a plausible mechanism—supports the plan.
What “cold” means in a tumor
“Cold” is a biological description, not a single diagnosis or a standardized clinical stage. A cold tumor may contain few infiltrating T cells, present little recognizable antigen, fail to activate dendritic cells, exclude immune cells behind abnormal blood vessels or dense stroma, or contain suppressive cells and molecules that disable the cells that do enter. Several of these problems can coexist in the same tumor, and the dominant barrier can differ between patients with the same cancer type.
By contrast, an immunologically “hot” tumor generally has evidence of antigen release and presentation, inflammatory signaling, and immune-cell infiltration. Turning cold to hot is shorthand for improving those features; it is not, by itself, a validated endpoint or a guarantee of tumor shrinkage, durability, or safety.

A map of the barriers and the strategies aimed at them
| Barrier or goal | Approaches described in current reviews and cancer research | What the evidence supports |
|---|---|---|
| More tumor-cell death, antigen release, or visibility | Chemotherapy, radiotherapy, oncolytic viruses, and other immunogenic-cell-death approaches | These can provide an immune-conditioning backbone, but they do not establish complete conversion in every cold tumor. |
| Better innate sensing and antigen priming | Intratumoral Toll-like receptor (TLR) agonists, STING agonists, other innate agonists, and cancer-treatment vaccines | Mechanistically promising and under study; combinations, biomarkers, safety, and efficacy remain important research questions. |
| Recruitment and activation of immune effectors | Vaccines, adoptive cell transfer, chemokine or trafficking approaches, and strategies that improve antigen presentation | The intended immune process can be described, but a mechanism is not proof of clinical response. |
| Less physical exclusion or suppression | Vascular normalization, extracellular-matrix and stroma interventions, suppressor-cell targeting, and related microenvironment therapies | These aim at barriers that restrict access or function; benefit depends on tumor context and combination design. |
| Checkpoint-blockade combinations | Radiation, chemotherapy, intratumoral therapies, vaccines, or targeted treatments paired with checkpoint inhibitors | Rational combinations are context-dependent. Randomized radiation combinations have not shown a uniform advantage over either treatment alone. |
1. Increase tumor visibility with cell death and antigen release
Chemotherapy as immune conditioning
Some cytotoxic drugs can kill tumor cells in ways that release antigens and danger signals. Those signals may help dendritic cells capture tumor material and present it to T cells. Chemotherapy can also reduce selected suppressive immune populations, depending on the drug, dose, schedule, and tumor setting. It is therefore sometimes used as an immune-conditioning backbone rather than only as direct tumor control.
The effect is not automatic. A regimen can reduce immune cells, damage normal tissue, or create lymphopenia that limits the response a later immunotherapy needs. The relevant evidence is tied to a specific cancer, drug combination, timing, and dose; “chemotherapy makes cold tumors hot” is too broad a claim.
Radiotherapy: a possible partner, not a universal conversion tool
Radiation can increase tumor-cell immunogenicity, release antigens, and alter the tumor microenvironment. It can also promote inflammatory signals that help immune recognition. At the same time, radiation may damage immune cells, induce immunosuppressive pathways, or produce effects confined to the treated site.
The National Cancer Institute’s Immuno-Radiotherapy Interest Group describes these opposing possibilities and notes that numerous randomized trials combining radiation with immunotherapy have not shown a clear therapeutic advantage over either treatment alone. Radiation is therefore best discussed as a context-dependent immune-conditioning partner. Tumor type, irradiated volume, dose and fractionation, checkpoint drug, treatment sequence, and whether disease is localized or metastatic all matter.

Immunogenic-cell-death approaches beyond standard cytotoxic therapy
Researchers are testing treatments designed to make dying tumor cells especially noticeable to the immune system. The desired sequence is tumor-cell death, release of antigens and danger signals, uptake by antigen-presenting cells, and expansion of tumor-reactive T cells. Demonstrating each step in a laboratory model does not establish that patients will experience a meaningful or durable response.
2. Activate innate sensing and antigen presentation
Intratumoral TLR agonists
Toll-like receptor agonists are intended to stimulate innate immune sensors in or around the tumor. Local activation can induce inflammatory cytokines, mature dendritic cells, and improve presentation of tumor antigens to T cells. Injecting the drug into a tumor may concentrate exposure at the site while limiting systemic exposure, although reaching every tumor deposit is a practical challenge.
STING agonists and related innate agonists
STING signaling can promote type I interferon production and help initiate antitumor T-cell priming. STING agonists and other innate stimulators are being evaluated alone and with checkpoint blockade or other therapies. Reviews of intratumoral therapy identify optimization of the molecule, delivery route, dosing schedule, biomarkers, safety, and efficacy as unresolved issues. A strong interferon signal in a model does not prove that a particular agent benefits patients.
Oncolytic viruses
Oncolytic viruses are engineered or selected to infect and kill tumor cells while releasing tumor antigens and inflammatory signals. The infection can also recruit immune cells to a site that previously had little T-cell activity. In a 2018 Cancer Currents account, the National Cancer Institute described studies in which oncolytic-virus treatment increased T-cell infiltration in tumors that had low T-cell levels before treatment.

That article used talimogene laherparepvec (T-VEC) and pembrolizumab in melanoma trials as a historical illustration of the combination concept; it should not be read as a statement of the combination’s current trial status or as evidence that it works for every patient. Antoni Ribas, M.D., Ph.D., described the injection’s intended role this way: “The injection [of T-VEC] is like lighting a match—it’s the spark that starts a fire.” The analogy captures the goal of initiating immunity, not a guaranteed clinical outcome.
Cancer-treatment vaccines
Cancer-treatment vaccines present tumor-associated or tumor-specific antigens, or help the patient generate an immune response against them. Some are personalized to mutations found in an individual tumor; others use shared antigens. Their aim is to improve the quality and breadth of T-cell priming, especially when spontaneous priming is weak. The National Cancer Institute distinguishes treatment vaccines from preventive vaccines and also classifies oncolytic viruses as a separate therapeutic approach. Vaccine effectiveness depends on antigen selection, delivery, the patient’s antigen-presentation machinery, and whether suppressive conditions are removed at the same time.
3. Help immune cells reach the tumor
Correct abnormal tumor blood vessels
Tumor vessels can be disorganized, leaky, poorly perfused, and hostile to lymphocyte entry. Vascular-normalization strategies seek to improve perfusion and endothelial signaling so that circulating T cells can cross into the tumor. The objective is not simply to create more vessels; excessive or abnormal angiogenesis can worsen exclusion. Anti-angiogenic treatment may therefore be paired with immunotherapy in carefully selected settings, with dosing intended to improve access rather than cause intolerable vascular toxicity.

Address extracellular matrix and stromal barriers
A dense extracellular matrix can raise tissue pressure and create a physical wall around tumor nests. Cancer-associated fibroblasts and other stromal cells may also secrete factors that repel or disable immune cells. Approaches that remodel stroma or matrix are designed to open pathways for infiltration. Because the matrix also supports normal tissue structure, indiscriminate disruption can cause toxicity or facilitate invasion; the target, timing, and extent of remodeling matter.

Change chemokine and trafficking signals
Chemokines guide immune cells through the bloodstream and tissue. Cold tumors may lack signals that attract effector T cells or may produce competing signals that draw suppressive cells instead. Trafficking approaches try to increase recruitment of tumor-reactive cells, alter chemokine gradients, or improve adhesion and endothelial crossing. A rise in cell counts is not enough: the recruited cells must recognize the tumor, remain functional, and avoid rapid suppression.
4. Remove suppressive cells and signals
Cold microenvironments can contain regulatory T cells, myeloid-derived suppressor cells, tumor-associated macrophages, and other populations that inhibit antigen presentation or T-cell activity. They may also contain suppressive cytokines, metabolic constraints, hypoxia, and checkpoint ligands. Therapies aimed at these components include selective depletion or reprogramming of suppressive cells, blocking inhibitory pathways, and changing tumor metabolism or hypoxia.
Checkpoint inhibitors release specific brakes on T cells, but they work best when tumor-reactive cells are present and can reach the tumor. Giving a checkpoint drug to a profoundly excluded tumor may leave the central access or priming problem untouched. Conversely, broad suppression of regulatory pathways can produce immune-related toxicity. The therapeutic window and the identity of the dominant suppressive mechanism must be established for each setting.
5. Use adoptive cell transfer when the tumor will not prime immunity
Adoptive cell transfer supplies immune cells rather than relying entirely on the patient’s tumor to generate them. Options under investigation include tumor-infiltrating lymphocytes, genetically engineered T-cell receptors, and chimeric antigen receptor (CAR) T cells. The cells must recognize an antigen that is present on the tumor, survive and expand after infusion, and enter a microenvironment that may still be suppressive.

Preparative chemotherapy, cytokine support, antigen heterogeneity, and the risk of on-target effects in normal tissues all influence the balance of benefit and harm. Adoptive transfer can bypass part of the priming problem, but it does not automatically solve physical exclusion, antigen loss, or an inhibitory microenvironment.
6. Combine immune conditioning with checkpoint blockade
Checkpoint blockade is often the partner used to preserve or reinvigorate T-cell activity after another treatment has released antigen or improved access. Common combination concepts pair checkpoint inhibitors with chemotherapy, radiotherapy, oncolytic viruses, innate agonists, vaccines, vascular or stromal interventions, or targeted therapies.
The order and schedule can be as important as the drug list. A therapy that releases antigen may need to precede a treatment that expands or sustains T cells. An intratumoral therapy may be useful for an accessible lesion while systemic checkpoint blockade addresses distant disease. Combination toxicity can also prevent delivery of the intended dose. Reviews of cold-tumor conversion emphasize biomarkers and tumor-microenvironment measurements because a mechanistically attractive combination may fail if it targets the wrong barrier.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteHow to judge whether a proposed strategy is credible
- Define the tumor context. Record the cancer type, stage, metastatic sites, prior treatments, accessible lesions, and relevant molecular or immune features.
- Identify the dominant barrier. Ask whether the main problem is absent antigen, weak priming, poor trafficking, stromal exclusion, suppressive cells, or exhausted effector cells. More than one answer is common.
- Match the intervention to the barrier. Use an antigen-releasing treatment for a visibility problem, an innate agonist or vaccine for priming, vascular or stromal measures for access, and suppressor or checkpoint targeting for functional inhibition.
- Check the evidence level. Separate laboratory findings, animal studies, early dose-finding trials, single-arm response signals, and randomized clinical results. A review can map hypotheses, but current efficacy and safety require primary trial data.
- Specify delivery and timing. Intratumoral, intravenous, radiation-site, and cellular therapies have different reach and risks. State which lesions are treated and how the sequence is expected to work.
- Plan measurements and stopping rules. Imaging, biopsies, immune-cell infiltration, antigen expression, circulating markers, and adverse events should be linked to predefined decisions rather than interpreted after the fact.
What remains uncertain
There is no validated universal recipe that turns all cold tumors hot, and “hotness” itself does not guarantee tumor control. A treatment may increase T-cell infiltration without producing tumor shrinkage, or may work in one tumor type and fail in another. Biomarkers can be imperfect, biopsy samples can miss regional variation, and a combination that is safe in a trial population may be unsuitable for someone with different organ function, autoimmune disease, infection risk, or prior therapy.
Reviews of cold-tumor strategies consistently call for better combinations, predictive biomarkers, safer delivery, and stronger clinical efficacy data. The 2024 National Cancer Institute Immunotherapy with Radiotherapy Workshop reflects continuing translational work rather than a settled standard regimen.
Questions to take to an oncology team
- Which immune barrier is most evident in this tumor: antigen visibility, priming, access, suppression, or T-cell function?
- Is the proposed treatment approved for this cancer and setting, supported by randomized evidence, or available only in a clinical trial?
- Why is this delivery route and treatment sequence expected to address the identified barrier?
- What biomarker or early assessment would show that the intended immune change occurred?
- Which toxicities are specific to the combination, and what alternatives exist if the tumor does not respond?
For an individual patient, the appropriate strategy depends on the cancer and clinical context and must be decided with a qualified oncology team. The most defensible approach is to connect a defined biological barrier to a specific intervention, then weigh patient-level risks against the strength of clinical evidence.
