AI manuscript review is a bounded assistant, not a scientific validator. It can improve language, organize a paper, summarize literature, check explicit journal requirements, and flag possible inconsistencies. It cannot independently prove that experiments happened as described, data and statistics are valid, references are genuine and supportive, text is original, or conclusions follow from results. Those judgments remain the responsibility of qualified authors, reviewers, and editors.
What “AI manuscript review” actually covers
The phrase can describe very different activities. A spelling-and-style pass, a checklist against a journal’s author instructions, a literature summary, and an attempted scientific critique may all be marketed as manuscript review, but they do not have the same reliability.
Most systems process the text, tables, and sometimes uploaded references. They identify patterns that resemble problems in their training data or match rules supplied by the user. A system with retrieval access may also search a collection of articles, but access to a source does not guarantee that the model interpreted it correctly. The output is therefore best treated as triage: a list of edits and questions for a human to investigate.
Use the result to decide where to spend expert attention. Do not treat a green score, a clean report, or a confident explanation as evidence that the underlying study is sound.

What AI can help detect or improve
Language, clarity, and readability
Language models are generally strongest at surface-level communication. They can identify grammatical errors, ambiguous pronouns, inconsistent terminology, overly long sentences, passive constructions, and abrupt transitions. They can suggest plainer wording, restructure a paragraph, or point out where an abstract does not clearly state the objective, methods, results, and conclusion.
These edits still need author review. A stylistic rewrite can change a technical qualifier, turn a correlation into an apparently causal statement, or remove a limitation that matters. Keep the original wording available and compare every substantive change.
Organization and explicit completeness checks
When the criteria are explicit, an AI tool can check whether a manuscript appears to contain required sections, keywords, declarations, figure legends, data-availability statements, or reporting-checklist items. It can compare headings with a journal’s instructions that you provide and flag missing elements or inconsistent numbering.

This is a presence check, not proof of adequacy. A methods section can exist while omitting the information needed to reproduce the work; a conflict-of-interest statement can be present but incomplete. The author must inspect the content against the journal’s current instructions.
Summaries, reference organization, and possible gaps
AI can group papers by topic, produce a preliminary outline of a literature review, extract recurring concepts, and suggest areas that appear under-discussed. It can also normalize reference formatting or identify duplicate entries.
Rank #2
Every source and every extracted claim must be opened and checked by a person. Models can omit a qualifying result, merge findings from different studies, mistake a preprint for a peer-reviewed article, or invent a citation that looks plausible. A suggested “research gap” is a prompt for searching and reading, not an established gap.
Internal consistency and red-flag triage
A tool may notice that the sample size in the abstract differs from the results, a table total does not match the text, a control group disappears from one analysis, units change between sections, or a citation is listed but never mentioned. It can flag an unusual value, a missing control, or a method that appears inconsistent with the stated outcome.
Such flags are leads. The system does not know whether a discrepancy reflects a typographical error, a prespecified exclusion, a subgroup analysis, or a legitimate change in units. Resolve each item against the protocol, data, code, laboratory records, and source papers.
Explanations of statistical output
An AI assistant can translate a regression table into plain language, explain what a confidence interval means, or suggest tests that might be relevant. This can help a researcher formulate questions for a statistician.
It cannot establish that the assumptions of a test hold, that the model was prespecified, that missing data were handled appropriately, that multiple comparisons were controlled, or that a p-value supports the claimed conclusion. Recalculate results and inspect diagnostics independently; never paste an AI interpretation into a paper without checking it against the analysis.

What AI cannot safely certify
That the data and study are truthful
Text analysis cannot verify that participants were recruited as described, instruments were calibrated, images are authentic, experiments were repeated, or a dataset was not altered. A fluent methods section is not evidence that the work occurred. Verification requires records, raw data, code, laboratory documentation, ethics approvals, or other appropriate evidence.

Sampling quality, causal claims, and reproducibility
A model may recognize words associated with randomization or adjustment, but it cannot determine whether the sampling frame created selection bias, whether a control was adequate, whether confounding was addressed, or whether an identification strategy supports a causal claim. It also cannot reproduce the study simply by reading the prose. Subject-matter and statistical experts must evaluate design, implementation, and robustness.
Statistical validity
No general-purpose reviewer can safely certify assumptions, power, model specification, effect-size interpretation, or the difference between exploratory and confirmatory analysis from text alone. AI may miss a subtle dependence between observations or endorse an analysis that sounds conventional but is inappropriate for the design. Treat any proposed test or interpretation as advisory until independently rerun and documented.
Authentic and supportive citations
AI-generated references can be incorrect, hallucinated, or fabricated. A citation may exist but fail to support the sentence attached to it; a quotation may be inaccurate or taken out of context; a retracted article may be presented as current. Search the publisher or index record, open the original article, check the exact wording and page, and confirm that the cited result supports the claim actually made.
Absence of plagiarism or copyright problems
Similarity tools and language models can identify matching phrases, but they cannot make a final legal or editorial judgment about plagiarism, fair use, permissions, translation, or reuse of figures. ICMJE places responsibility for attribution, permissions, and plagiarism prevention on humans. Compare suspected overlap with the original source and follow the journal’s process.

Whether text was written by a human or whether policy was violated
AI-generated-text detectors estimate whether wording resembles model output. Springer Nature warns that the accuracy and usefulness of such tools are “far from definitive,” and that no tool can confirm whether AI use was safe and policy compliant. A detector score is not proof of authorship, misconduct, or a breach of a journal rule. False positives can affect non-native English writers, heavily edited prose, and formulaic scientific language.
Confidentiality of an unpublished manuscript
Uploading a submission to a public or non-retaining service can expose confidential text, reviewer comments, participant information, or unpublished results. ICMJE advises that editors, reviewers, and publishers should not upload submitted manuscripts into AI systems where confidentiality cannot be assured without the authors’ explicit permission. A tool’s retention, training, human-access, security, and deletion terms matter as much as its apparent accuracy.
The final editorial or peer-review decision
Elsevier states that AI cannot replace a reviewer’s critical thinking or independent evaluation because it may generate incorrect, incomplete, or biased conclusions. A model can help a reviewer organize notes, but it cannot weigh novelty, significance, ethics, or disciplinary context on the reviewer’s behalf.
Capability versus certification
| Task | Useful AI contribution | What a human must still do |
|---|---|---|
| Copy editing | Suggest grammar, terminology, clarity, and structure improvements | Confirm that edits preserve technical meaning and qualifications |
| Journal compliance | Check headings, declarations, word limits, figure labels, and supplied checklist items | Read the current author instructions and verify substantive completeness |
| Literature review | Cluster papers, outline themes, and suggest search terms or possible gaps | Retrieve every source, assess quality and date, and verify each claim |
| Reference checking | Flag missing entries, duplicates, or unusual citation patterns | Confirm that each reference exists, is accurately quoted, and supports the statement |
| Methods review | Raise questions about missing controls, definitions, or inconsistent descriptions | Inspect the protocol, records, data, code, and domain-specific assumptions |
| Statistics | Explain output or list analyses to discuss with an expert | Check design, assumptions, model specification, multiplicity, and rerun results |
| Originality and AI-use detection | Surface possible phrase matches or machine-like patterns | Investigate context, permissions, authorship, and the applicable policy |
| Peer review | Help format notes or identify questions | Make the independent evaluation and recommendation |
A safe workflow for authors
- Read the journal’s current AI policy first. Check whether assistance with language, analysis, images, or references is allowed; whether disclosure is required; and whether submitted text may be processed by an external service.
- Remove unnecessary sensitive material. Do not upload names, identifying clinical details, confidential reviewer comments, proprietary data, or the full manuscript when a short excerpt will answer the editing question. Use a system whose retention, training, access, and deletion terms are acceptable for the work and permitted by the journal.
- Define a narrow task. Ask for grammar corrections, a checklist comparison, or questions about an unclear paragraph rather than an unbounded verdict such as “Is this paper valid?” State the discipline, study design, definitions, and the limits of the requested review.
- Keep an audit trail. Record the tool name and version, date, purpose, material provided, important prompts, and changes accepted. This makes disclosure and correction possible if an output proves wrong.
- Separate flags from findings. Put every AI suggestion into a review list marked “verify.” Do not label a citation fabricated, a result biased, or a method invalid until a qualified person checks the underlying evidence.
- Verify sources at the original article. For each reference or quotation, confirm bibliographic details, wording, context, retraction status, and support for the exact claim. Replace or remove any item that cannot be verified.
- Recheck analyses independently. Reopen the dataset and code, rerun reported models, inspect assumptions and diagnostics, and reconcile every number in the abstract, text, tables, and figures. Ask a statistician or domain expert when the design is outside your expertise.
- Review every accepted wording change. Ensure that qualifiers such as “associated with,” “may,” confidence intervals, subgroup limits, and uncertainty have not been weakened or removed.
- Disclose substantive assistance. Follow the journal’s required location and wording. ICMJE says humans remain responsible for accuracy; COPE states that AI tools cannot be listed as authors. Credit the people who performed the intellectual work and retain accountability for the manuscript.
Guidance for reviewers and editors
Protect the submission
Before using any assistant, determine whether the service stores prompts, trains on them, permits human review, transfers data across borders, or retains files after deletion. If confidentiality cannot be assured, do not upload the manuscript. Ask the editor or publisher for an approved environment instead.

Formatting notes, creating a checklist, or improving the clarity of a reviewer’s own text may be compatible with policy. Delegating the scientific judgment, recommendation, or decision to a language model is not. Read the entire manuscript yourself, disclose permitted use, and preserve the reasoning behind the recommendation.
Best Value
Handle red flags fairly
An AI-identified citation mismatch, image anomaly, or statistical concern is a reason to investigate through the journal’s established process. It is not a finding of misconduct. Seek the original files, clarification from the authors, or specialist review, and document how the issue was resolved.
Common failure modes and how to recover
- Confident but nonexistent references: Search each item in a trusted scholarly index or the journal site; delete anything that cannot be found.
- A real paper attached to the wrong claim: Read the cited sections and rewrite the sentence to match the evidence, including its limitations.
- Overstated causal language: Compare the claim with the design and analysis; replace causal verbs when the study supports association only.
- Missed numerical inconsistency: Recalculate from the raw data or analysis code and reconcile all presentations of the result.
- False plagiarism or AI-detector alarm: Inspect the alleged overlap and context; do not accuse an author from a score alone.
- Confidential text exposed: Stop further uploads, document what was shared, consult the institution and journal, and follow applicable privacy or incident procedures.
- Meaning changed during editing: Restore the original technical statement, then request a constrained rewrite that preserves variables, units, uncertainty, and qualifiers.
How to judge an AI review workflow
Evaluate the whole process rather than a single score. Six questions reveal whether it is suitable for your manuscript:
- Scope: Is it limited to copy editing and checklists, or does it claim to evaluate scientific validity?
- Evidence access: Can a human inspect the authoritative paper, dataset, protocol, or code behind each flag?
- Error handling: Does the report show uncertainty, quotations, and reasons, or only unsupported labels?
- Confidentiality: What are the retention, training, access, deletion, and jurisdiction terms?
- Transparency: Can you record the tool, version, date, purpose, and changes for disclosure?
- Human control: Is a qualified person required to verify every consequential output before submission or decision?
What the policy landscape requires
ICMJE states that AI can produce authoritative-sounding output that is incorrect, incomplete, or biased, so humans are ultimately responsible for reviewing and ensuring accuracy. It also assigns humans responsibility for attribution, permissions, plagiarism prevention, confidentiality, and disclosure.

Springer Nature uses a risk-based approach and cautions that detector accuracy is not definitive. Nature distinguishes assistive uses from unsafe practices such as fabricating citations, generating core reasoning without disclosure, or delegating peer review to a language model. COPE states that an AI tool cannot be an author and calls for transparent disclosure when AI contributes to writing, images, or data work. Policies change, so consult the journal’s current wording at submission.
What is known about accuracy
There is no single, independently validated accuracy number for “AI manuscript review” across disciplines and systems. Available policy and standards documents describe useful applications and risks rather than a universal benchmark. A 2024 BMJ bibliometric analysis found that publisher guidance commonly highlights language improvement and literature synthesis while warning about inaccuracy, bias, plagiarism, and accountability. Performance depends on the model, prompt, field, language, source access, and the reviewer’s verification process.
Practical decision rule
Use AI when the task has an observable answer and a human can check it quickly: grammar, formatting, section presence, duplicate references, or a list of questions to investigate. Do not use an AI output as the evidence for a claim about truth, originality, authorship, ethics, statistical validity, or policy compliance. If a decision could affect a participant, an author’s reputation, or publication, require qualified human review and retain the supporting evidence.
