AI safety evaluations are targeted tests, not certificates that a model is safe. Companies typically measure dangerous capabilities, harmful-use behavior, refusal and alignment behavior, resistance to adversarial prompts, and selected frontier-risk scenarios. The result describes how a particular model performed on a particular test design, under stated conditions and assumptions.

Those tests can reveal important failure modes before release, but they do not by themselves measure every risk in a deployed product. Tool access, private data, user incentives, affected people’s experience, long-term behavior, and harms that were not anticipated by the test designers can all change the outcome.

What an AI safety evaluation actually is

An evaluation is a structured measurement of a model’s capability or behavior against a defined set of tasks, prompts, scenarios, graders, and thresholds. The test might ask whether a model can generate a dangerous procedure, whether it refuses a prohibited request, whether it follows a policy under pressure, or whether it can complete a sequence of actions with access to tools.

That scope matters. A passing score means the model met the test’s success criteria in the tested setting. It does not establish that the model will behave the same way in every language, product, workflow, organization, or future model version. A failed test is evidence of a specific weakness; a passing test is evidence bounded by the test’s coverage and validity.

Google DeepMind describes the evaluation domain broadly as the measurement of AI capabilities, behavior, and impact, and reports using diverse approaches for its Gemini models. OpenAI and Anthropic likewise combine several kinds of internal and external testing rather than relying on one universal safety score.

What companies commonly test

Dangerous capabilities

Capability evaluations ask what a model can do when the task is technically difficult or potentially hazardous. Depending on the threat model, this can include scientific reasoning, code generation, cyber-related problem solving, autonomous planning, persuasion, or assistance with biological research. The purpose is often to identify whether a model has crossed a capability threshold that warrants additional safeguards.

These tests may score task completion, quality of an answer, the amount of human intervention required, or the model’s ability to chain actions over several steps. A capability result is not automatically a prediction of harm: the same capability can be benign in one product and dangerous when combined with tools, access, or a malicious operator.

Harmful-use and policy-violation behavior

Red-teamers and automated test suites present requests that violate a company’s usage policies. They may probe for instructions involving violence, fraud, privacy invasion, malware, extremist activity, or other prohibited conduct. Evaluators record whether the model complies, partially complies, gives a safe completion, or refuses.

Good reports distinguish between a refusal that is technically correct and one that is so vague that it frustrates legitimate users. They also test near-boundary cases, because a system that blocks obvious wording may still respond to euphemisms, translations, role-play, or multi-turn attempts to evade safeguards.

Refusal, alignment, and instruction following

Alignment evaluations examine whether a model follows the developer’s and product’s rules while still completing legitimate requests. Tests can include conflicting instructions, attempts to override a system message, prompt injection, requests for sensitive personal data, and situations where the safest response is to ask a clarifying question.

Evaluators may measure refusal rates, the quality of explanations, consistency across paraphrases, and whether the model leaks restricted information while appearing to refuse. A high refusal rate alone is not a safety success if the model also blocks harmless tasks or offers enough detail to enable the prohibited activity.

Adversarial robustness

Adversarial testing deliberately searches for inputs that defeat normal safeguards. Red teams vary wording, language, formatting, conversation history, attached files, and tool instructions. They may use automated attack generation, human creativity, or both.

Robustness results depend heavily on the attack budget. A short, fixed list of prompts measures something different from an open-ended campaign with many testers and repeated attempts. Reports should state how many attacks were tried, whether the model was reset between attempts, and how success was judged.

Frontier-risk scenarios

Some evaluations focus on risks associated with highly capable or agentic systems, such as autonomous replication, strategic deception, cyber operations, chemical or biological assistance, or the ability to pursue goals over long horizons. These tests are often simulations or controlled environments rather than ordinary consumer use.

Such scenarios can be valuable early-warning instruments, but they involve assumptions about available tools, permissions, time, and the model’s objective. A result should therefore be read as evidence about behavior under that scenario, not as a complete forecast of future real-world conduct.

How the tests are run and why the setting changes the result

Pre-release and controlled testing

Many company evaluations occur before release or in sandboxes designed to be difficult for the model. Access may be limited, tools may be simulated, and evaluators may know the model’s architecture and intended failure modes. These conditions improve repeatability and reduce the chance of real-world damage while a weakness is being investigated.

They also differ from deployment. A public product may add retrieval, browsing, code execution, memory, external APIs, customer data, and automated actions. Each integration changes the system being evaluated. A model that behaves acceptably in a text-only test can create a different risk profile when it can send messages, modify records, or make decisions at scale.

A joint exercise is not necessarily a common benchmark

In 2025, OpenAI and Anthropic reported a joint exercise using each lab’s internal safety and misalignment evaluations on Claude Opus 4, Claude Sonnet 4, GPT-4o, GPT-4.1, o3, and o4-mini. The disclosure explicitly cautioned that the exercise was not intended to provide exact apples-to-apples comparisons because each lab had different access levels and deeper familiarity with its own models.

That caveat is central to interpreting cross-company results. Even when two organizations use the same broad label, they may differ in prompts, hidden context, graders, thresholds, tool permissions, sampling settings, and model versions. A table of percentages can look precise while the underlying measurements are not equivalent.

Metrics and graders

Safety reports may use binary pass/fail judgments, severity-weighted scores, calibration measures, refusal rates, or human ratings. Automated graders are efficient but can miss subtle enabling detail, sarcasm, cultural context, or a response that sounds safe while still revealing the key operational step. Human graders add judgment but introduce disagreement and sampling limits.

The report should identify the evaluator, the rubric, the number of examples, the confidence or uncertainty information, and whether the prompts were held out from model development. Without those details, a headline score is difficult to interpret or reproduce.

What a safety result can legitimately tell you

  • It can show how the tested model behaved on the tested tasks under the stated permissions and threat model.
  • It can identify concrete failure examples that engineers can use to improve filters, training, tooling, or access controls.
  • It can support a release or mitigation decision when the organization has defined thresholds and documented what happens if a threshold is exceeded.
  • It can provide a baseline for regression testing after a model, policy, system prompt, or tool integration changes.

It cannot, by itself, prove that no harmful behavior exists, that a deployment is safe for every population, or that two companies’ scores are directly comparable.

What company evaluations often leave out

Deployment context and downstream use

Lab tests usually observe a model in isolation. Real systems include interfaces, retrieval pipelines, plugins, permissions, monitoring, business incentives, and human operators. Users may copy outputs into medical, financial, educational, employment, or public-sector decisions. A downstream organization may also optimize for speed or revenue in ways that increase exposure to known weaknesses.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The NIST AI Risk Management Framework says measurement should be connected to the deployment context and informed by domain experts and end users. That means evaluating the complete use case, not just the underlying model. It also means documenting who can be affected, what decisions are made, what data flows through the system, and which human controls remain available.

Generalizability and unknown failure modes

A finite test suite cannot cover every language, dialect, domain, prompt sequence, accessibility need, or future update. NIST guidance calls for documenting limitations on generalizability. A model that passes English prompts may behave differently in another language; a model tested on short questions may fail after a long conversation; a benchmark created before a product update may no longer represent the deployed system.

Unknown failure modes are especially difficult because they are absent from the test designer’s assumptions. Robust evaluation therefore needs exploratory testing, monitoring for novel incidents, and a process for adding new cases when users or external researchers discover them.

Affected people’s feedback and recourse

Benchmark participants are not the same as people affected by an AI system. A hiring applicant, patient, borrower, student, or person whose image or text is used for training may experience a harm that a laboratory score does not capture. NIST identifies feedback from end users and impacted communities, problem reporting, and appeal processes as relevant evaluation inputs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Without a usable reporting channel, organizations may learn only about failures that are visible to engineers. Without recourse, a person can be harmed by an automated output without a practical way to correct the record, challenge a decision, or obtain human review.

Long-horizon and deployment-stage harms

A 2025 literature review covering 1,178 safety and reliability papers found that corporate research is increasingly concentrated on pre-deployment alignment and testing, with less attention to some deployment-stage issues. The review identified gaps involving healthcare, finance, misinformation, persuasive or addictive features, hallucinations, and copyright. This is a finding about the reviewed literature, not a census proving that every company neglects every one of these areas.

Long-horizon harms can emerge gradually: users may over-rely on a system, a ranking model may shift access to opportunities, or generated misinformation may be amplified by distribution channels. Short laboratory sessions rarely reveal these feedback loops.

Transparency and comparability

Companies may withhold prompts, model settings, incident counts, failure examples, or thresholds because of security, privacy, or competitive concerns. Limited disclosure makes independent scrutiny harder. It also prevents readers from knowing whether a reported improvement reflects a safer model, a narrower test, a changed grader, or a different sampling method.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OpenAI and Anthropic’s warning about apples-to-apples comparisons is therefore more than a footnote. Comparisons should account for evaluator independence, access levels, test construction, model version, uncertainty, and the decision the result was meant to inform.

How to compare safety claims from different companies

Use the following axes instead of ranking organizations by a single score.

Comparison axis Questions to ask Why it matters
Risk scope Does the program cover misuse, autonomy, cyber, biological risk, privacy, bias, misinformation, and other social harms? A narrow program may be strong in one area while saying nothing about another.
Test setting Was the model text-only, tool-enabled, connected to private data, or placed in a realistic workflow? Permissions and integrations can change the system’s behavior and potential impact.
Evaluator independence Were tests run by the developer, outside experts, affected communities, or a mixture? Independent and community perspectives can expose assumptions internal teams miss.
Coverage of affected people Were end users and impacted groups consulted, and is there a reporting or appeal route? Laboratory success does not establish that people can detect or remedy harm.
Methods and uncertainty Are prompts, rubrics, sample sizes, model versions, thresholds, and limitations disclosed? Readers need enough information to judge validity and reproduce the result.
Repeat testing Are evaluations rerun after model, policy, system-prompt, or tool changes? Safety can regress when any component of the deployed system changes.
Decision linkage Does a result trigger a documented release gate, mitigation, rollback, or monitoring requirement? A measurement matters more when it changes what the organization does.

What a stronger evaluation program adds

Use a lifecycle risk framework

NIST’s AI RMF organizes work around governing, mapping, measuring, and managing risk. In practice, that means assigning accountability, describing the intended use and affected parties, selecting measurements that fit the use case, and applying mitigations before and after deployment.

  1. Govern: define responsibility, escalation paths, documentation requirements, and the conditions under which a system cannot be released.
  2. Map: describe the users, environment, data, decisions, dependencies, foreseeable misuse, and people who may be affected.
  3. Measure: combine capability tests, adversarial testing, domain validation, human review, incident data, and uncertainty estimates.
  4. Manage: reduce exposure with access controls, safer defaults, monitoring, user education, rollback plans, and remediation for people harmed by the system.

Test the whole sociotechnical system

Model-level tests should be supplemented with tool-enabled scenarios, realistic workloads, red teams that do not share the developer’s assumptions, and domain experts who understand the consequences of an error. Testers should vary user roles, languages, data quality, time pressure, and the amount of human supervision.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Include human and community input

NIST’s ARIA pilot illustrates this direction through a measurement-tree approach that combines expert annotators with human testers to assess whether an evaluation is valid for its intended purpose. The key question is not only whether a response is technically correct, but whether the measurement captures the impact that matters in the real setting.

Monitor after release

Post-deployment evaluation should track incidents, near misses, user complaints, distribution shifts, newly discovered attacks, and changes in how people rely on the system. Reports should record which model and configuration produced an event, because a later update may change behavior even when the product name stays the same.

A practical checklist for reading an AI safety report

  • Identify the exact system: note the model version, date, product configuration, tools, retrieval sources, and access permissions.
  • Define the claim: determine whether the report measures capability, refusal, robustness, fairness, reliability, or a specific frontier-risk scenario.
  • Inspect the threat model: ask which attacker, user, data, time horizon, and resources the test assumes.
  • Check the sample and grader: look for prompt counts, languages, human versus automated judging, disagreement rates, and severity definitions.
  • Find the boundaries: read stated limitations, excluded populations, untested languages, unavailable tools, and conditions that were not simulated.
  • Look for action: verify whether failures led to a mitigation, release delay, access restriction, monitoring change, or documented acceptance decision.
  • Seek post-release evidence: examine incident reporting, update testing, user feedback, and appeal or correction mechanisms.

Can a model pass a safety test and still be dangerous?

Yes. Passing means that the tested behavior stayed within the test’s acceptance criteria. A model may still fail on an untested language, a multi-turn jailbreak, a novel tool chain, a different user population, or a product workflow that creates incentives the laboratory did not reproduce.

Risk can also arise from combinations. A model with modest standalone capability may become more consequential when connected to a database, granted permission to send messages, or deployed to thousands of users. Conversely, a model that performs poorly in a simulated high-risk scenario may be harmless if the real product removes the relevant tools and imposes effective human review. Evaluation must therefore be interpreted alongside system design and operational controls.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What we can and cannot quantify today

There is no broadly accepted single statistic showing what percentage of real-world risk company evaluations miss. The omissions are structural and context-dependent: a test can be excellent for one decision and irrelevant to another. The defensible conclusion is not that every evaluation is inadequate, but that no finite company benchmark should be treated as a complete measurement of a deployed sociotechnical system.

The most credible safety claim is correspondingly specific: it names the model and configuration, describes the tested risks and conditions, reports uncertainty and limitations, explains who evaluated it, and shows how the findings changed release or monitoring decisions. Broader assurances require evidence from deployment, independent testing, affected people, and continuing oversight.
