AI agent observability is the practice of recording and inspecting an agent’s complete execution path—not merely checking whether its API returned a 200 response. A useful record shows the request, model calls, retrieved context, tool decisions, retries, errors, timing, and final result, all connected in one trace. Operational metrics then reveal whether those behaviors are common, while logs, user feedback, and domain evaluations explain whether they were correct and safe.
This matters because an agent can finish successfully while choosing the wrong tool, using irrelevant documents, leaking policy-sensitive information, or producing an answer that sounds plausible but fails the user’s task. Production observability lets you answer “Why did the agent do this?”, detect regressions, and turn real failures into repeatable tests.
Why an agent’s final answer is not enough
A conventional service dashboard tells you whether requests arrived, how long they took, and how many failed. Those signals are necessary, but an agent is a decision-making workflow. It may call several models, search a knowledge base, invoke external tools, retry after an error, or hand work to another process before returning one response.

Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Two requests can therefore have the same successful HTTP status and radically different quality. One may retrieve the right policy document and use an approved tool. Another may retrieve an outdated passage, call an inappropriate tool, loop through retries, and still return text. Looking only at the endpoint hides the trajectory that produced the outcome.

Observability reconstructs that trajectory. It connects each operation to a request-level trace, preserves parent-child timing, records outcomes and errors, and combines technical health with task and safety results. The goal is not to save every piece of content forever; it is to collect enough evidence to diagnose behavior under an explicit privacy policy.
The signal hierarchy for production agents
Use the signals in this order when designing an implementation. Traces are the primary diagnostic record. Aggregate metrics show whether a problem is isolated or systemic. Logs and events add detail that should not be placed in metric labels, while user and domain feedback establish whether the behavior was useful and acceptable.
| Priority | Signal | What it answers | Typical examples |
|---|---|---|---|
| 1 | Execution traces | What did this run do, and where did it go wrong or slow down? | Agent span, model calls, retrieval, tool calls, retries, errors, final response |
| 2 | Operational and agent metrics | How often is a behavior occurring? | Latency, timeouts, availability, task completion, tool-failure rate, loops, handoffs |
| 3 | Logs and events | What diagnostic or policy detail explains the metric or trace? | Exceptions, guardrail decisions, rate limits, deployment changes |
| 4 | User and domain feedback | Did the result satisfy the real task and its safety requirements? | Ratings, corrections, acceptance, expert labels, quality and safety evaluations |
1. Build a correlated execution trace
Start with one trace per agent run. Give it a stable trace identifier and preserve parent-child relationships so every model, retrieval, and tool operation can be located in sequence. The trace should include start and end times, duration, status, errors, and retry information.
Use a span hierarchy that mirrors the workflow
OpenTelemetry’s GenAI walkthrough illustrates a top-level invoke_agent span with child spans for model operations such as chat and tool operations such as execute_tool. A retrieval step can be another child span. This structure lets an engineer see whether latency came from orchestration, a model, a search, or an external service.
For each span, record an operation name, parent span, timestamps, status, and error details. Add model identity, provider or deployment, tool identity, and workflow version where those fields are available. In multi-turn systems, retain the relationship between turns without mixing unrelated conversations.

Capture decisions, not just transcripts
A transcript of the final answer is insufficient. Capture which tool the model selected, whether the call was authorized, which retrieval operation ran, and whether the tool result was accepted or rejected. Preserve retry and fallback transitions. If content capture is allowed, store the relevant prompt, completion, tool arguments, and result; if it is not, retain redacted or hashed identifiers and structured summaries that still support diagnosis.
Measure model timing and token use
Record model-operation duration and token counts alongside the span. OpenTelemetry’s conventions include gen_ai.client.operation.duration and gen_ai.client.token.usage, with token types that can distinguish input from output. Correlating these values with workflow and model identity exposes slow deployments, unusually long contexts, and output-heavy requests. Attribute names are still evolving, so verify the convention version used by your instrumentation.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minute2. Monitor infrastructure and agent behavior together
Keep standard service metrics, then add measures that describe decisions and outcomes. A dashboard that reports only uptime can miss a sudden rise in invalid tool calls or an agent that completes fewer tasks while returning HTTP 200 responses.
| Metric group | Track | Useful breakdowns |
|---|---|---|
| Service health | Request volume, latency percentiles, timeouts, error rates, availability | Workflow, endpoint, region, deployment |
| Agent execution | Task-completion rate, run count, retries, loops, escalations and handoffs | Agent version, task type, tenant or environment |
| Tools | Calls per tool, success and failure rates, authorization denials | Tool name, operation, error class |
| Models | Input and output tokens, operation duration, provider or deployment | Model, route, prompt or workflow version |
| Quality and safety | Acceptance, correction, satisfaction, domain checks, policy and safety failures | Task category, evaluator, severity |
Break latency down by model, retrieval, tool, and orchestration spans. A single end-to-end percentile cannot tell you which component needs work. Likewise, report tool-call failure rate and run count by tool rather than one combined tool metric; a rarely used tool and a critical tool should not be interpreted identically.
3. Account for tokens, latency, and cost
Token use is both a performance signal and a resource signal. Record the model or deployment identity and input/output usage for every model operation. Compare token counts with duration and task type to find context bloat, runaway loops, or a prompt change that increases output length.

A dollar-cost metric must be calculated from the provider’s current pricing for the applicable model, region, and billing arrangement. Do not hard-code a universal price into telemetry. Store the usage facts and apply a versioned pricing table so historical reports remain explainable when rates change.

Pick the symptom - the matching free tool is one click away.
4. Define outcome and quality signals for the actual task
“Completed” should mean that the user’s task was fulfilled, not merely that the model generated text. Define an application-specific completion event: a reservation was correctly changed, a support case was resolved, or a document passed the required checks. Record whether the user accepted, corrected, or rejected the result when that information is available.
Use layered evaluation
- Deterministic checks: schema validity, required fields, permission rules, citation presence, or successful execution of a transaction.
- Domain checks: factuality, relevance, completeness, or workflow-specific business rules reviewed by qualified people.
- Safety and policy checks: prohibited content, data-exposure attempts, unsafe actions, and guardrail outcomes.
- Human feedback: ratings, edits, escalations, and explicit explanations of failure.
Aggregate rates show movement, but sampled trace review explains it. A drop in satisfaction could come from retrieval drift, a tool outage, a prompt change, or a policy rule that became too strict. Keep the trace link with each evaluation record so an evaluator can inspect the evidence.
5. Instrument retrieval and tool quality
Retrieval-augmented workflows
Record retrieval duration and enough metadata to identify the corpus, index, and version used. Under your data policy, retain identifiers for the selected documents or chunks. Measure relevance or downstream usefulness when it can be judged; there is no universal relevance metric that fits every application.
Useful diagnostic questions include: Did the search return any results? Did the agent select the intended source? Was the source current for the request? Did the final answer rely on retrieved material or ignore it? These questions require linking retrieval spans to the model decision and final evaluation.

Tool calls
For each call, capture the intended tool, operation, authorization result, duration, execution status, retry count, and a safe result summary. Tool arguments may contain secrets or personal data, so apply redaction before storage. Distinguish model-requested calls from calls made by deterministic orchestration code; the distinction helps identify whether a failure is a planning problem or an implementation problem.
6. Use structured logs and events for context
Logs and events complement traces. Put exceptions, rate-limit responses, guardrail decisions, policy outcomes, and deployment or configuration changes in structured records linked to the trace ID. Do not put high-cardinality prompts or full tool payloads into metric labels; that creates expensive, difficult-to-query time series and increases exposure risk.
Events are especially useful for changes that explain a step-function in behavior: a model deployment, retrieval-index refresh, tool-schema change, feature-flag update, or policy revision. An alert should lead from an aggregate metric to a trace and then to the relevant event.
How to implement observability step by step
- Map the run. Draw the agent’s actual path from request receipt through orchestration, model calls, retrieval, tools, external services, retries, and response. Identify where a trace context can be propagated.
- Create the root span. Start an
invoke_agent-style span for each run. Attach workflow, environment, version, and a privacy classification. - Add child spans. Instrument every model, retrieval, tool, and external-service operation. Preserve parent IDs, timestamps, statuses, and errors.
- Standardize core attributes. Record model and tool identity, operation name, token usage, duration, retry count, and deployment version. Use the current OpenTelemetry GenAI conventions where they apply, and document any application-specific fields.
- Define outcome events. Emit task completion, user acceptance or correction, quality scores, and safety results as correlated records rather than inferring them from HTTP status.
- Choose content capture deliberately. Decide which prompts, completions, retrieved passages, and tool arguments may be stored. Implement redaction, access controls, sampling, and retention before enabling broad capture.
- Build drill-down views. Every latency, failure, or quality chart should link to representative traces. Support filtering by workflow, model, tool, version, and severity.
- Test the instrumentation. Generate known success, timeout, tool-error, retry, policy-block, and retrieval-miss cases. Verify that spans close correctly and that sensitive fields are absent or redacted as designed.
Alert on conditions that have an owner
Create alerts for sustained, actionable changes rather than every unusual trace. Examples include a rise in timeouts or tool failures, latency or token use beyond a workflow-specific objective, a drop in completion or satisfaction, and an increase in safety or policy failures.

Turn production traces into an improvement loop
Observability pays off when it changes the system. Review anomalous and high-impact traces, annotate the failure mode, and select representative cases rather than only the most dramatic example.

- Find a repeatable failure. Group traces by symptoms such as wrong tool, irrelevant retrieval, policy violation, hallucinated answer, timeout, or excessive looping.
- Create an evaluation example. Preserve the input, expected behavior, relevant context, and a clear pass/fail or graded rubric. Remove or transform sensitive data.
- Reproduce before editing. Run the case against the current prompt, tool schema, retrieval configuration, model, and policy so the baseline is recorded.
- Change one cause at a time. Possible fixes include prompt constraints, tool validation, retrieval filters, timeout handling, model routing, or policy logic.
- Rerun the evaluation set. Check the targeted failure and regression metrics such as latency, token use, tool errors, and safety outcomes.
- Deploy with monitoring. Sample online evaluations to detect drift, and compare post-change traces with the baseline.
Automated evaluators are useful for scale, but calibrate them against human or domain-expert judgments for consequential tasks. Vendor-authored production guidance describes this trace-to-dataset-to-evaluation pattern; it is a practical workflow, not a universal standard.
Privacy, security, and retention
Prompts, completions, retrieved documents, and tool arguments can contain personal, confidential, or security-sensitive information. A May 14, 2026 OpenTelemetry walkthrough states that prompt content and tool arguments are not captured by default in its demonstrated setup because of this risk; content capture is opt-in.
Apply the minimum-data principle:
- Capture full content only when it is necessary for a defined diagnostic or evaluation purpose.
- Redact secrets and direct identifiers before export; transform values when exact content is not needed.
- Restrict trace access by role and environment, and audit who can view sensitive fields.
- Use sampling for routine traffic while retaining complete traces for approved test cases and severe incidents.
- Set retention periods based on the application’s risk, contracts, and governing obligations. There is no universal schedule established for every agent.
Standards and platform-selection criteria
OpenTelemetry offers a common vocabulary for traces, metrics, and logs, with GenAI semantic conventions for model and agent operations. Those conventions are still in active development. Check the current version and your framework’s instrumentation before depending on a particular attribute name or assuming that two systems capture identical fields.

When evaluating an observability platform or building an internal stack, compare capabilities on these axes:
| Area | Questions to ask |
|---|---|
| Instrumentation | Does it support your framework and OpenTelemetry context propagation? |
| Trace depth | Can engineers inspect nested model, retrieval, tool, retry, and handoff spans? |
| Search and sampling | Can you find traces by workflow, version, error, user-safe labels, and time window without indexing sensitive content? |
| Evaluation | Can reviewers annotate traces, create datasets, and compare versions? |
| Alerting and integration | Can alerts reach the systems and owners responsible for remediation? |
| Data controls | Are residency, access, redaction, encryption, sampling, and retention configurable? |
| Scale and cost | How does pricing change with trace volume, content capture, retention, and evaluation runs? |
No single product feature set defines agent observability. Select the combination that gives your team a reliable trace, actionable aggregates, defensible quality checks, and controls appropriate to the data.
How mature is adoption?
A 2026 State of Agent Engineering survey published by LangChain reported that 89% of all organizations and 94% of teams with production agents had some observability; 62% of all organizations reported detailed tracing and 72% of production-agent teams reported full tracing. The same survey reported offline evaluation at 52% and online evaluation at 37%. These are vendor-published survey results, not a general industry census, so they should be read with the survey’s methodology and population in mind.
Bottom line
Make the execution trace your primary evidence: correlate the agent run with every model, retrieval, and tool operation, then layer on latency, errors, token use, completion, feedback, quality, and safety metrics. Link logs and events for context, protect content by default, and use representative failures to build evaluations that verify each change. That combination turns “the agent returned an answer” into a defensible explanation of what happened and a repeatable way to improve it.
