Weight-bearing exercise loads your bones while you are upright and working against gravity; resistance exercise makes your muscles push or pull against a resistance. Walking, stair climbing and dancing are weight-bearing. Bands, free weights, machines and body-weight drills are resistance. A single movement can be both—for example, a squat loads the skeleton through the legs while the muscles work against resistance.
Neither category is universally “better.” Guidance from the Bone Health & Osteoporosis Foundation (BHOF), the U.S. National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS), and the Royal Osteoporosis Society supports a combination chosen for your health, fracture history, ability and goals, with balance practice when appropriate.
What weight-bearing and resistance exercise actually mean
Weight-bearing: bones carry load in an upright position
Weight-bearing activity puts some of your body weight through your bones while you are upright and moving against gravity. Impact can increase the loading stimulus, but not every weight-bearing activity is high impact. Walking is weight-bearing even though one foot generally remains on the ground; jogging and jumping add larger, faster forces.
The BHOF describes weight-bearing exercise as activity done on your feet so your bones and muscles work against gravity. Its examples include walking, stairs, dancing, jogging and racket sports (BHOF guidance). The Royal Osteoporosis Society separately discusses impact exercise, because impact level matters when deciding what is suitable for someone with fragile bones (Royal Osteoporosis Society impact guidance).

Resistance: muscles work against a chosen force
Resistance exercise—also called muscle-strengthening or strength training—asks a muscle to contract against an opposing force. The resistance may come from your body weight, elastic bands, free weights, cable systems or a weight machine. The exercise is defined by the muscular effort, not by whether you are standing.
A seated machine press can be resistance training without much body-weight loading through the spine or legs. Conversely, a standing body-weight squat is resistance training and weight-bearing at the same time. NIAMS lists body-weight movements, bands, weights and machines as resistance options in its bone-health guidance (NIAMS, “Exercise for Your Bone Health”).
How the two categories differ
| Feature | Weight-bearing exercise | Resistance exercise |
|---|---|---|
| Primary stimulus | Upright bones and joints support body weight and work against gravity; impact may add force. | Muscles generate force against body weight, bands, weights or machines. |
| Typical examples | Walking, stairs, dancing, jogging and racket sports. | Band exercises, free-weight lifts, machine exercises and body-weight drills. |
| Can categories overlap? | Yes. A standing squat or stair climb can load bones and challenge muscles against resistance. | Yes. A loaded exercise performed standing can provide both resistance and weight-bearing stimulus. |
| Water or equipment support | Swimming is not an impact exercise because water supports body weight. Cycling is also not an impact exercise because the bike supports you. | Pool or cycling work can strengthen muscles and improve general fitness, but they do not replace a bone-loading activity. |
| Main safety question | Is the impact and upright loading appropriate for your fracture risk, balance and joints? | Can you control the resistance and movement without pain, unsafe spinal positions or excessive strain? |
The distinction describes the loading stimulus, not a ranking. Many useful activities sit in both columns, while swimming and cycling can be excellent cardiovascular choices without providing the same impact stimulus as walking or jogging.

Lower-impact weight-bearing choices
- Walking: A practical starting point that puts body weight through the legs without the repeated higher impact of running.
- Stair climbing: Adds repeated lifting of body weight and can be done on stairs or a step machine, provided balance and joint tolerance are adequate.
- Dancing: Can range from gentle stepping to faster, more impact-heavy patterns. Choose the style and pace you can control.
Higher-impact choices
- Jogging: Produces more impact than ordinary walking and therefore requires more attention to joints, balance, footwear, conditioning and fracture history.
- Racket sports: Combine upright loading with quick changes of direction. They may also challenge balance and coordination.
- Other impact patterns: The Royal Osteoporosis Society’s impact guidance explains that impact exercise is one way to give bones a stronger mechanical stimulus, but the appropriate level depends on the individual (Royal Osteoporosis Society).
“Higher impact” is not a prescription. A person with osteoporosis, a previous spinal fracture, several fractures or significant balance problems may need a lower-impact option or professional modification before attempting jogging, jumping or rapid directional changes.
Examples of resistance exercise
Body-weight resistance
Chair stands, controlled squats, wall push-ups and other body-weight movements use your own mass as resistance. When performed standing, they may also be weight-bearing. A supported variation can reduce the balance demand while preserving the muscle-strengthening purpose.

Bands and tubing
Elastic bands provide resistance that changes through the movement. They are portable and can be anchored or held for pulling, pressing and leg exercises. Select a resistance that allows controlled technique; a band is not automatically gentle if it pulls you into an unsafe position.
Free weights
Dumbbells, kettlebells and other free weights let you add resistance to a movement. They can be used standing, seated or supported. The same weight can be appropriate for one movement and excessive for another, so progression should be individualized.
Machines
Resistance machines guide the path of movement and may provide support for people who find free-weight balance difficult. They still require correct setup and a load that can be moved without pain or loss of control.
NIAMS presents all four—body weight, bands, free weights and machines—as ways to perform strengthening work (NIAMS). Equipment is optional: a well-chosen body-weight routine can be resistance training.
Why bone-health programs usually include both
Bones respond to mechanical loading, while muscles need progressively challenging work to maintain strength. Weight-bearing activity supplies upright or impact loading; resistance exercise lets you target specific muscles and increase the force in a controlled way. The practical result is complementary, not competitive.

NIAMS says a combination of weight-bearing, resistance and balance training is best for building and maintaining healthy bones and helping prevent falls and fractures (NIAMS). Balance work matters because a fall can cause a fracture even when a person’s bone density has not suddenly changed. Balance exercises may include supported single-leg practice or other drills selected for your ability; anyone unsteady should use appropriate support and professional instruction.
The 2024 clinical guideline from the National Osteoporosis Guideline Group (NOGG) reports that combined weight-bearing and resistance-strengthening programs are effective at reducing bone loss at the femoral neck and lumbar spine in postmenopausal women. The guideline’s cited conclusion does not provide a single effect size in the available summary, so it should not be converted into a promised percentage reduction (NOGG 2024 guideline).
How much activity is suggested?
Different organizations frame frequency in different ways. These are population-level guidance points, not an individualized treatment plan or a guarantee of a particular bone-density result.
| Organization and context | Weight-bearing or aerobic guidance | Strength guidance |
|---|---|---|
| BHOF exercise-frequency page (undated, accessed October 13, 2026) | 30 minutes of weight-bearing exercise on most days. | Muscle-strengthening exercise two to three days each week. |
| NIAMS general adult activity guidance (page publication date not stated in the search result) | At least 150 minutes of moderate-intensity activity or 75 minutes of vigorous activity weekly. | Muscle strengthening at least twice weekly. |
BHOF’s frequency guidance is available at “How much exercise do you need?”. NIAMS places its general targets in its bone-health overview (NIAMS). Do not assume that meeting a general minutes target makes jogging, jumping or a particular lifting load safe for you.
A practical way to combine the categories
- Start with your medical context. Note any osteoporosis or low-bone-density diagnosis, spinal or other fracture, persistent pain, balance limitation, joint problem and current treatment. Ask a healthcare professional or qualified exercise professional with osteoporosis experience which movements and impact levels fit your situation.
- Choose a tolerable weight-bearing base. Walking is often easier to scale than jogging or jumping. Increase duration or pace only while you can maintain control and your joints tolerate the activity.
- Add resistance work on separate or planned days. Use body weight, bands, free weights or machines. Include movements for the major muscle groups, but select supported versions when balance or fracture risk makes unsupported movements unsuitable.
- Include balance practice when appropriate. Use a stable support and a clear area. A professional can select drills if you have fallen, feel unsteady or have a neurological or joint condition.
- Progress one variable at a time. A change in impact, duration, resistance or complexity can alter the load. Make gradual changes that preserve technique rather than chasing a heavier weight or harder impact.
- Stop and reassess warning signs. New or worsening pain, loss of control, unusual back pain or symptoms after a known fracture warrant medical advice rather than simply reducing rest time or pushing through.
This framework deliberately avoids a universal set-and-repetition prescription. The right dose depends on age, training history, bone status, medication, fracture pattern, balance and other health conditions.

Choosing between activities when your goal is bone health
| Your priority | Questions to ask about an activity | Examples from the guidance |
|---|---|---|
| Upright bone loading | Am I on my feet, and does the activity work against gravity? | Walking, stairs, dancing. |
| More impact | Can I safely tolerate faster landings or repeated impact, given fractures, joints and balance? | Jogging, selected racket sports. |
| Muscle strength | What is the resistance, and can I control the full movement? | Bands, free weights, machines, body-weight exercises. |
| Fall prevention | Does the plan challenge balance safely, with support if needed? | Balance training combined with weight-bearing and resistance work. |
| Cardiovascular fitness with little impact | Would support from water or a bike help me exercise, while I add bone-loading work separately if appropriate? | Swimming or cycling for general fitness; they are not impact exercises. |
| Accessibility | Can I perform the movement regularly, with safe space, stable equipment and a progression I can manage? | Walking routes, chair-supported body-weight work, bands or machines. |
Safety if you have osteoporosis or a fracture history
Get individual advice before changing impact or load
People with osteoporosis, low bone density, spinal fractures, multiple fractures or other physical limitations should ask a healthcare professional or qualified exercise professional about suitable movements and progression. The safest choice can depend on where a fracture occurred, whether it has healed, current pain, balance, medications and other diagnoses.
Be especially careful with spinal flexion
BHOF warns that bending forward from the waist can raise spinal-fracture risk in people with osteoporosis. That warning matters for exercises and everyday tasks that repeatedly round or load the spine. Ask a professional to demonstrate safer alternatives and how to hinge, lift or modify a movement for your body (BHOF frequently asked questions).
Do not confuse “low impact” with “risk free”
Walking may be lower impact than jogging, but an uneven surface, poor balance or a sudden increase in distance can still create problems. A resistance band can snap or pull you off balance; a machine can be unsafe if adjusted incorrectly. Clear the exercise area, use stable support where appropriate and learn unfamiliar equipment from a qualified person.
Exercise complements, rather than replaces, treatment
Exercise is important for strength, function, balance and bone loading, but it does not replace osteoporosis medicine when medication is indicated. Discuss your full fracture-prevention plan with the clinician managing your bone health. The Royal Osteoporosis Society addresses the question of whether exercise can substitute for osteoporosis medicine in its exercise information (Royal Osteoporosis Society).

“Anything that makes me breathe harder is weight-bearing.”
Not necessarily. Swimming and cycling can raise cardiovascular fitness while water or a bicycle supports much of your body weight. They can remain valuable activities, but they do not provide the same impact loading as walking, stairs or jogging.

“Resistance means lifting heavy weights.”
Resistance can be body weight, a band, a machine or a light free weight. The defining feature is that muscles work against a force. A controlled body-weight movement may be a suitable starting point.
“More impact is always better for bones.”
Impact is only one part of the decision. Fracture history, spinal safety, balance, joints and technique determine whether a particular impact level is appropriate. The Royal Osteoporosis Society’s impact guidance is intended to help people choose safely, not to prescribe the same exercise to everyone.
“If an exercise is resistance training, it must also load my bones.”
A seated or lying machine exercise can strengthen muscles without putting the same upright load through the skeleton. Conversely, many standing drills provide both stimuli. Check the position and force involved rather than relying on the label.

How to judge whether a plan is well designed
- It identifies at least one suitable weight-bearing activity and a separate or overlapping resistance component.
- It includes balance practice when a professional considers it appropriate.
- It accounts for osteoporosis diagnosis, bone density, prior fractures, pain, medications, balance and other conditions.
- It uses controlled technique and a progression that does not increase impact and resistance aggressively at the same time.
- It includes a way to modify or stop an exercise if pain, instability or unusual symptoms appear.
- It treats swimming and cycling as useful fitness options rather than silently counting them as impact exercise.
Bottom line for planning bone-strengthening exercise
Weight-bearing exercise describes bones supporting your body against gravity, often while standing or moving with impact. Resistance exercise describes muscles working against body weight, bands, weights or machines. Because a movement can be both, the most useful plan is usually not an either-or choice: combine appropriate weight-bearing and resistance work, add balance training when suitable, and have a clinician or osteoporosis-aware exercise professional tailor impact, spinal positions and progression to your fracture risk.
