Virtual reality (VR) therapy is a treatment or assessment that uses computer-generated immersive experiences as part of a therapeutic intervention. A headset may display the environment, but the headset itself is not therapy: the clinical purpose, protocol, and guidance are what make the experience a therapeutic intervention.
What virtual reality therapy means
In VR therapy, a person interacts with a computer-generated environment during treatment or assessment. The technology is a delivery medium for a therapeutic process—not a standalone treatment. Owning or using a VR headset does not, by itself, amount to receiving therapy.
Immersive environments can present controlled virtual experiences that feel real, as described in a 2024 review in Nature Reviews Psychology. The environment may be used for exposure, skills training, assessment, relaxation, or another intervention; the exact approach depends on the clinical goal and protocol.
How VR therapy is used
Exposure therapy
Virtual reality exposure therapy (VRET) is a prominent form of VR therapy. A clinician can use a virtual setting to present situations relevant to a person’s treatment in a controlled way. Research has examined exposure-based interventions for anxiety conditions, phobias, and post-traumatic stress disorder (PTSD), among other applications.

Other therapeutic and assessment uses
Reviews also discuss VR applications involving psychotic symptoms, eating disorders, depression, stress management, and skills training or assessment. The presence of a VR application in a review does not establish that it is effective for every condition or person; outcomes depend on the particular intervention and evidence available for it.
What current evidence can—and cannot—show
A 2025 U.S. Department of Veterans Affairs Evidence Synthesis Program review searched MEDLINE, Embase, CENTRAL, and PsycINFO through December 2024. It identified 84 eligible studies, of which 45 addressed prioritized mental health disorders. Most relevant studies were small, with total sample sizes under 100, and half evaluated VR exposure therapies. Those figures describe studies eligible for that review, not the entire body of VR therapy research.

Evidence is condition- and comparison-specific. For PTSD, the VA review found that VR prolonged exposure may produce less symptom improvement immediately after treatment than non-VR prolonged exposure, and little or no difference at three months; certainty for those symptom outcomes was low. It also found, with moderate certainty, that VR prolonged exposure probably results in less treatment response immediately after treatment. These findings concern a particular intervention and comparator, not every form of VR therapy.
For stimulant use disorder, VR exposure may reduce craving immediately after treatment compared with treatment as usual, but certainty was low. The VA review found evidence very uncertain for several other prioritized disorders. A 2021 review had likewise described the evidence base as strongest at that time for anxiety disorders and PTSD, sparse for generalized anxiety disorder and obsessive-compulsive disorder, and promising rather than settled for psychosis, autism spectrum disorder, and ADHD. That earlier review is background, not a current efficacy verdict.

What equipment is involved
Immersive VR commonly uses a head-mounted display, often called a VR headset. In a 2023 meta-analysis of 17 randomized controlled trials of VR applications for anxiety disorders, 16 used head-mounted displays. This describes study hardware, not validation or endorsement of a particular consumer headset. A consumer device alone does not provide a treatment protocol or clinical care.
How to assess a VR therapy approach
“VR therapy” covers different interventions, so evaluate the specific approach rather than relying on the label. Useful distinctions include:

- Condition and protocol: What diagnosis or treatment goal is being addressed, and what does the intervention involve?
- Delivery: Is the experience immersive or non-immersive?
- Guidance: Is it clinician-guided or self-guided?
- Evidence comparison: Was it compared with non-VR exposure therapy, treatment as usual, or another option?
- Outcomes and follow-up: Are results reported immediately after treatment, at later follow-up, or both? Consider usability and adverse effects separately from clinical outcomes.
Safety and clinical guidance
The VA synthesis judged the evidence about adverse effects very uncertain across the prioritized disorders. That uncertainty does not establish either that VR therapy is harmful or that it is universally safe. The reviewed sources do not provide a complete, current list of contraindications or individual suitability criteria. Discuss a specific VR intervention with a qualified care provider, who can consider its protocol and your circumstances.
