Urine tests for lung cancer are being studied, but none is established for routine screening. A 2024 study found that two urine metabolites could help identify nonsmokers at elevated risk who might be candidates for low-dose CT (LDCT); the authors say prospective studies are still needed. The findings do not show that a urine test can diagnose cancer, replace a CT scan, or improve survival.
Can a urine test detect lung cancer early?
Researchers are investigating whether signals in urine can help identify lung cancer or flag people for further evaluation. One line of research measures creatine riboside (CR) and N-acetylneuraminic acid (NANA), two metabolites. Other studies examine different signals, including microRNA carried in extracellular vesicles and biosensor readings. These are distinct approaches, not versions of one interchangeable test.
What the CR and NANA study found
A 2024 study published in Clinical Cancer Research measured urine metabolites with liquid chromatography–mass spectrometry. It compared people with lung cancer with age- and sex-matched controls in an exploratory cohort of 846 participants and an independent validation cohort of 505. The study included analysis of nonsmokers, who do not meet the smoking-history criteria in the cited US screening recommendation.
Recommended Free Tools
For the combined CR and NANA analysis among nonsmokers, the area under the receiver operating characteristic curve (AUC) was 0.94 in the exploratory cohort and 0.80 in the validation cohort. AUC describes how well a model separates cases from controls in the studied data; it is not the chance that a person with a positive result has cancer. These results also do not establish performance in routine population screening or show that testing improves health outcomes. The study authors said the biomarkers could help identify high-risk nonsmokers as candidates for LDCT and that prospective studies are warranted. Read the 2024 study.

- Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
- Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
Other urine-based approaches
Earlier work also explored urinary metabolites. A 2014 study reported CR and NANA as candidate diagnostic and prognostic markers; a 2016 prospective cohort study examined whether metabolites were elevated before diagnosis and reported associations between CR and lung cancer risk, with results varying by population subgroup. Neither establishes a recommended screening assay. 2014 study; 2016 cohort study.
Two separate reports published in 2026 describe other technologies. A case-control study of urinary extracellular-vesicle microRNA reported early-stage detection sensitivity and specificity of 82.0% and 92.5% in a training set, and 88.2% and 87.0% in a test set. A separate biosensor report described a 238-patient cohort and reported sensitivity of 91%, specificity of 86%, and AUC of 0.86. These are results from their respective study cohorts, not established population-screening performance, and cannot be compared as if the studies tested the same assay in the same people. Is a urine screening test available now?
The studies cited here do not establish a urine test for routine lung cancer screening or a consumer test that readers can use to determine whether they have cancer. The CR and NANA work uses laboratory analysis, and the other approaches remain research-stage in the cited reports. A urine biomarker result should not be treated as a diagnosis.
Can a urine test replace a CT scan?
No. The cited evidence does not show that urine testing can replace LDCT, or that screening with a urine test reduces deaths from lung cancer. In the United States, the US Preventive Services Task Force (USPSTF) recommendation says biomarker measurement has not been found beneficial as a screening modality. It recommends annual LDCT for eligible high-risk adults. Read the USPSTF recommendation.

The CR and NANA study’s proposed role is potential risk triage: identifying people who might warrant LDCT, not substituting a urine result for imaging. Whether the biomarkers can do that safely and usefully in practice remains to be established through prospective studies.

The USPSTF recommendation published March 9, 2021 applies in the United States. It recommends annual LDCT for adults aged 50 to 80 who have at least a 20 pack-year smoking history and currently smoke or quit within the past 15 years. A pack-year means smoking an average of one pack per day for one year; clinicians can help calculate a person’s history.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
- Screening should stop once a person has not smoked for 15 years.
- It should also stop if a health problem substantially limits life expectancy or the ability or willingness to undergo curative lung surgery.
Guidelines can change, and eligibility depends on individual circumstances. If you may qualify, discuss LDCT with a clinician rather than waiting for or seeking an unvalidated urine test. People outside these criteria can ask a clinician about their individual risk; the cited recommendation does not establish urine biomarkers as an alternative screening route.
