UnitedHealth Group was larger by reported revenue in the latest common quarter, but the two companies are not built the same way. UnitedHealth combines UnitedHealthcare insurance with three Optum businesses. CVS Health combines Aetna insurance, a large pharmacy-benefit and health-services operation, and a consumer pharmacy network. For the quarter ended June 30, 2026, UnitedHealth reported $112.0 billion of revenue and $8.0 billion of earnings from operations; CVS reported $106.096 billion of revenue, $4.703 billion of GAAP operating income and $5.157 billion of adjusted operating income. Those figures show scale, not a simple winner: revenue includes different mixes of premiums, pharmacy activity, services and retail sales, while the earnings figures use different bases unless you deliberately align them.
This comparison uses Q2 2026 as the latest common reported period available at October 13, 2026 06:26:41 UTC. UnitedHealth had scheduled its Q3 release for October 13, after that cutoff, so Q3 results are not included.
UnitedHealth Group and CVS Health at a glance
Both companies sit across several parts of the healthcare value chain, yet their centers of gravity differ. UnitedHealth’s structure is insurance plus technology-enabled care, data and pharmacy services. CVS Health’s structure is insurance plus pharmacy-benefit administration, healthcare services and a broad retail pharmacy footprint.

Why the comparison is harder than the headline numbers
A health insurer, a pharmacy-benefit manager, a clinic operator and a retail pharmacy record economic activity differently. An insurer records premiums and medical costs. A pharmacy-benefit operation processes prescriptions and may report activities that involve plan sponsors, pharmacies and manufacturers. A retail pharmacy records sales and customer payments. A care, data or technology unit records service fees. Putting all of those streams beside one another creates a useful scale comparison, but not an apples-to-apples measure of underlying economics.
Different segment architectures
UnitedHealth reports two complementary businesses: UnitedHealthcare, its health-benefits operation, and Optum, which delivers care and health services using technology and data. Its four reportable segments are UnitedHealthcare, Optum Health, Optum Insight and Optum Rx, as described in the 2025 Form 10-K.
CVS Health reports four segments: Health Care Benefits, Health Services, Pharmacy & Consumer Wellness, and Corporate/Other. Health Care Benefits is the Aetna insurance business and related services. Health Services includes pharmacy-benefit management and healthcare services. Pharmacy & Consumer Wellness contains retail and other pharmacy operations plus consumer health products. Corporate/Other captures activities that are not assigned to the operating segments. The definitions and segment reporting are in CVS Health’s 2025 Form 10-K.

How UnitedHealth Group makes money
UnitedHealthcare: insurance and benefits administration
UnitedHealthcare collects premiums or receives payments to arrange and administer medical benefits for employers, government programs and individual members. Its economics depend on enrollment, premium rates, the mix of products and markets, medical utilization, provider prices, quality programs and administrative expenses. A quarter with higher premiums can still produce weaker earnings if medical costs rise faster.
Optum Health: care delivery and clinical services
Optum Health provides care and related services. Its model can include directly delivered care, physician and provider relationships, value-based arrangements and other clinical operations. Revenue and profit are influenced by patient volume, reimbursement terms, care-management performance, medical costs in risk arrangements and the pace of integrating operations. It is not simply another insurance book.
Optum Insight: data, analytics and technology
Optum Insight supplies data, analytics, consulting and technology-enabled services to healthcare organizations. Compared with an insurance segment, this business is more service- and information-oriented. Contracts, software and advisory work can have different timing and margin characteristics from premiums and claims.
Optum Rx: pharmacy-benefit and prescription services
Optum Rx manages pharmacy benefits and prescription-related services. Its role can involve negotiating and administering drug benefits, operating mail and specialty pharmacy capabilities and serving plan sponsors and members. Gross prescription activity should not automatically be treated as equivalent to external revenue because intercompany transactions and reporting conventions matter.

UnitedHealth presents UnitedHealthcare and Optum as complementary businesses. An insurance relationship can create demand for pharmacy, care, analytics and technology services, while Optum capabilities can support benefit administration and care coordination. That integration may improve operating reach, but it also means consolidated results include internal transactions and eliminations that are not visible in a simple segment-by-segment addition.
How CVS Health makes money
Health Care Benefits: Aetna insurance
Health Care Benefits provides health insurance and related services through Aetna. The segment’s financial outcome is driven by enrollment, premiums, government and commercial product mix, medical utilization, provider rates, benefit design and administrative costs. CVS’s medical benefit ratio (MBR) is a key indicator: it measures medical costs relative to applicable premium revenue for the insurance segment, so a lower ratio generally leaves more premium dollars before other expenses, but it is not a group-wide profit margin.
Health Services: PBM and healthcare services
Health Services includes pharmacy-benefit management and healthcare services. A PBM administers prescription benefits for plan sponsors and members, handling functions such as network arrangements, claims processing, formulary administration and pharmacy services. The segment can also include care and clinical services. Its revenue presentation may reflect the role CVS plays in arranging or managing prescription transactions, not just a retail sale at a CVS pharmacy.
Pharmacy & Consumer Wellness: stores and consumer products
Pharmacy & Consumer Wellness operates retail and other pharmacies and sells consumer health products. Store count, prescription volume, front-store sales, reimbursement rates, labor, rent and inventory costs all affect the segment. Retail revenue can be large in dollars while carrying economics that differ from insurance premiums or PBM service fees.

Corporate/Other
Corporate/Other contains items that are not assigned to the operating segments, including corporate costs and other activities identified in the annual filing. It matters when moving from segment results to consolidated GAAP income.
CVS’s reported physical and membership reach
At December 31, 2025, CVS reported approximately 9,000 retail locations, more than 1,000 walk-in and primary-care clinics, about 87 million PBM plan members and service to an estimated more than 37 million people through insurance products and related services. These are company-reported indicators, not independent market-share estimates. A PBM plan member, an insured person, a clinic visitor and a retail location are not interchangeable units.
Q2 2026 financial comparison
Revenue and operating earnings
| Q2 2026 measure | UnitedHealth Group | CVS Health | Reading the difference |
|---|---|---|---|
| Consolidated revenue | $112.0 billion | $106.096 billion | UnitedHealth’s reported total was about $5.9 billion higher for the quarter ended June 30, 2026. |
| GAAP operating income or equivalent reported operating measure | $8.0 billion earnings from operations | $4.703 billion GAAP operating income | These are the closest like-for-like operating figures supplied, although the release labels differ. |
| Adjusted operating income | Not used as the comparison figure here | $5.157 billion | CVS’s adjusted figure includes management-defined adjustments and should not be compared with UnitedHealth’s GAAP figure without a reconciliation. |
| Net margin | 4.9% reported in the Q2 release | Not stated in the cited Q2 material | Do not infer a CVS net margin from operating income. |
The $5.9 billion gap is a one-quarter revenue difference, not a valuation judgment, quality ranking or forecast. Nor does it establish that one company has better margins. UnitedHealth’s consolidated result includes insurance, care, data, technology and pharmacy services. CVS’s total includes insurance, PBM and healthcare services, retail pharmacy and consumer products.
GAAP versus adjusted results
GAAP operating income is prepared under generally accepted accounting principles. An adjusted operating figure removes or reclassifies items according to the company’s stated reconciliation. Adjusted results can help readers understand management’s view of ongoing operations, but the adjustments must be identified before comparing companies. A sound comparison therefore uses UnitedHealth GAAP earnings from operations against CVS GAAP operating income first, then presents CVS’s $5.157 billion adjusted operating income as a separate measure.
Intersegment eliminations and gross activity
Both groups have businesses that transact with one another. An insurer may purchase pharmacy, care or analytics services from an affiliated operation; a PBM or pharmacy business may record activity involving related entities. Consolidated statements eliminate intercompany revenue and expense. In addition, pharmacy reporting can involve gross claims or pass-through amounts that do not represent the same economic value as a service fee or retail margin. For that reason, adding segment revenue or comparing headline totals without reading the segment definitions can overstate the similarity of the businesses.
What CVS’s medical benefit ratio says—and does not say
CVS reported an 87.4% Health Care Benefits MBR in Q2 2026, down from 89.9% in Q2 2025. The company attributed the reduction primarily to improved underlying performance in its Government business and the absence of the prior-year premium deficiency reserve. The change is meaningful for that insurance segment and quarter, but it should not be treated as proof of a permanent trend.
MBR is also not directly interchangeable with UnitedHealth’s 4.9% net margin or $8.0 billion earnings from operations. MBR focuses on medical costs relative to insurance premium revenue. Net margin is a consolidated bottom-line measure after operating and non-operating items. Operating earnings are a dollar measure before the items included below operating income. Each answers a different question.

Scale: which company is “bigger” depends on the denominator
By Q2 2026 consolidated revenue, UnitedHealth was larger: $112.0 billion versus CVS’s $106.096 billion. That is the clearest answer if “bigger” means one quarter’s reported sales.
Other definitions produce different comparisons:
- Insurance scale: compare covered lives, product mix and premium revenue, not total group revenue.
- PBM scale: compare plan members, prescription volume and services, recognizing that a PBM member count is not an insured-life count.
- Care-delivery scale: compare clinics, clinicians, attributed patients or care arrangements, depending on the disclosed measure.
- Retail scale: compare locations, prescriptions and consumer sales; CVS discloses approximately 9,000 retail locations as of Dec. 31, 2025.
- Technology and data scale: examine contract revenue, customers and recurring service characteristics rather than physical locations.
Because each company emphasizes different operating statistics, there is no single “people served” number that settles the question.

1. Align the reporting period
Start with the same quarter or the same fiscal year. Here, both figures are for Q2 2026 ended June 30, 2026. Do not compare UnitedHealth’s Q2 with CVS’s full fiscal 2025 or with a trailing twelve-month figure.
2. Identify the accounting measure
Label each number as revenue, GAAP operating income, adjusted operating income, net income, earnings from operations or margin. If one value is adjusted, show the GAAP counterpart and explain the reconciliation before drawing a conclusion.
3. Map the business mix
Separate insurance, PBM and prescription services, retail pharmacy, care delivery, data and technology. A company with more retail sales may report more gross revenue without having the same margin structure as an insurer or software-like service business.

4. Read the cost indicator for the relevant segment
For insurance, examine medical cost ratios such as CVS’s MBR alongside enrollment, premiums and utilization. For retail and pharmacy services, examine reimbursement, prescription volume, product mix and operating costs. Do not substitute a group net margin for a segment cost ratio.

5. Check internal activity and eliminations
Read the segment footnotes in the annual filings to understand transactions among affiliated businesses and the eliminations made in consolidation. This prevents gross prescription or service activity from being mistaken for equivalent external sales.
6. Separate reported performance from outlook
Quarterly results describe what happened in the stated period. Management guidance describes an expectation and can change. The Q2 releases include outlook discussion, but an outlook is not a substitute for reported Q3 results.
What the 2025 filings add
The audited fiscal 2025 Form 10-K filings are the source of record for annual revenue, earnings, cash flow, balance-sheet items, risk factors and detailed segment definitions. UnitedHealth’s filing is available from the SEC-hosted 2025 Form 10-K; CVS Health’s is available from the SEC-hosted 2025 Form 10-K.
Those filings should be used when making an annual comparison because they provide a complete-year basis and explain accounting policies, eliminations, risks and segment measures. A quarterly release is useful for the latest snapshot, but it does not replace the audited annual disclosures.
Management framing and evidence limits
UnitedHealth Group chief executive officer Stephen Hemsley said in the July 16, 2026 Q2 release: “Our results and outlook reflect the continuing progress in our work to simplify how we operate, improve both affordability and the healthcare experience for patients and care providers, and apply modern technology to create real improvement for people.” This is the company’s framing of its priorities, not independent evidence that each outcome has been achieved.
No comparable CVS chief-executive quotation is used here. The numerical comparison is therefore based on the companies’ reported results and filings rather than a forced side-by-side of executive statements.
Bottom-line interpretation
UnitedHealth Group was the larger company by Q2 2026 revenue, with $112.0 billion compared with CVS Health’s $106.096 billion. UnitedHealth also reported $8.0 billion of earnings from operations and a 4.9% net margin. CVS reported $4.703 billion of GAAP operating income and $5.157 billion on its adjusted measure. Those figures establish relative reported scale for one aligned quarter, but they do not prove superior long-term profitability.
