A useful daily health log is small: choose one goal, record only the few observations that help you review it, and look for patterns over time. Two minutes is a practical habit-design target—not a medical standard or a guarantee of better health. Tracking can support reflection and conversations with a care team; it cannot diagnose a condition or replace prescribed care.
What should you track each day?
Start with the question you want the log to help answer. For example, if your goal is to build a steadier activity routine, record the activity you did and how it felt. If you are noting a recurring symptom, record when it happened and a brief description. A measurement belongs in the log only when it is relevant to that goal or part of a clinician-directed plan.
- One behavior: a brief note about an activity, meal-related habit, sleep routine, or other behavior relevant to your goal.
- One subjective observation: a symptom, energy level, mood, or other personal observation, if useful.
- One measurement, if appropriate: include it only when it helps with your goal or your care team has asked you to monitor it.
There is no need to capture every number a device offers. NICE recommends self-monitoring, including activity trackers and food or physical-activity diaries, to help people review progress toward diet or activity goals. It also says the effectiveness of digital interventions varies and presents them as an adjunct to existing services, not a substitute for them. This is a low-friction personal routine, not a validated medical protocol. If you are following a prescribed monitoring plan, use your care team’s schedule and instructions instead.

- Choose one goal. Make it specific enough that a short note can help you review progress.
- Pick a format. Use paper, an app, or a wearable only if it captures the information you actually need.
- Record a few relevant entries. Note a behavior or observation, plus a measurement only if it serves your goal or care plan.
- Keep timing consistent where it matters. Consistent timing can make entries easier to interpret, but do not impose a schedule on a clinical measurement that differs from your instructions.
- Review periodically. Look for questions or patterns worth discussing, rather than treating a single reading as a diagnosis.
For example, someone working toward a regular walking habit might write “walked after lunch; felt comfortable” rather than logging every available wearable metric. Someone asked to monitor blood pressure should follow the care team’s instructions for the device, timing, and response to readings.
Choose paper, an app, or a wearable
The best format is the one you can use consistently and that suits the information you need. Compare options on practical grounds:
Rank #2
- FOOD & SYMPTOM DIARY: Track what you ate for all three meals and any snacks, & write down any reactions you experienced, or how you felt
- BOWEL MOVEMENT TRACKER: Monitor the type of stool & number of times you go - with printed Stool Consistency Chart on the front page
- WATER INTAKE MONITOR: Helps you stay hydrated, simply check off one water icon every time you drink a glass of water
- MOOD & ENERGY LOG: See how your diet affects your mood & energy levels - rate them on scales from 1 to 10
- EXERCISE/ACTIVITY MONITOR: Record the description, time and duration of up to 3 different workouts per day - so you can see how your activity levels affect your symptoms
| Consideration | Paper log | App | Wearable |
|---|---|---|---|
| Daily effort | Manual entry; simple to keep focused. | May make entry or review convenient; the effort depends on the app. | May capture some measures automatically, but you may still need to add context. |
| Fit for your goal | Flexible for notes, symptoms, or habits. | Depends on what the app records and how you can use those entries. | Useful only if its measures are relevant and appropriate for your goal or care plan. |
| Sharing a useful summary | You can bring or transcribe selected notes. | Check whether it can export or share a summary you need. | Check whether the information can be accessed or shared in a useful form. |
| Privacy and data control | Consider where the paper is stored and who can see it. | Check the operator, collected data, sharing settings, and deletion or export options. | Check the device and associated service’s data and sharing controls. |
| Clinical monitoring | Can record readings taken with the method your care team specified. | Use only in a way that matches the care team’s instructions. | Do not assume a consumer metric is interchangeable with a prescribed measurement. |
This comparison is a practical decision framework, not an official checklist. Wearable technologies may have clinical potential, but reliability varies and integration into clinical decision-making can be challenging, according to the U.S. Government Accountability Office’s August 6, 2026 assessment of wearable technologies.

When a clinician has asked you to monitor a number
Home remote monitoring can involve measures such as blood pressure, heart rate, weight, oxygen level, or blood sugar. The right device and schedule depend on your care plan. Before you begin, ask your care team:
Rank #3
- How do I use the equipment correctly?
- How often and when should I take a measurement?
- What reading should prompt action, and what should I do?
- Who should I contact with a concerning reading or a question?
Do not change medication or treatment based on a self-created tracking routine. Clarify the alert and follow-up plan with your care team. HHS guidance on using remote patient monitoring
Do not trust every wearable health claim
A device displaying a number does not make that number suitable for medical decisions. In a safety communication dated February 21, 2024, the U.S. Food and Drug Administration warned consumers not to use smartwatches or smart rings that claim to measure blood glucose without piercing the skin. The FDA said it had not authorized, cleared, or approved a smartwatch or smart ring intended to measure or estimate glucose on its own. That warning is distinct from an app displaying readings from an authorized glucose-measuring device. FDA safety communication on smartwatch and smart-ring glucose claims

Keep the log helpful, not stressful
More tracking is not always better. NICE notes that digital tools can contribute to anxiety, unhealthy behaviors such as excessive exercise or disordered eating, and negative mental-health effects for some people. If logging becomes distressing or starts driving unhealthy behavior, simplify or stop the routine and discuss your concerns with a professional. Before entering health information into an app or online service, check who operates it, what information it collects, whether sharing is optional, and how you can delete or export your data. Privacy protections depend on the service: the Office of the National Coordinator for Health Information Technology explains that some records offered by providers or health plans are covered by HIPAA rules, while some standalone personal health record services may fall under the FTC Health Breach Notification Rule.
