The drug behind headlines about turning back skin age is topical rapamycin, also called sirolimus. A small exploratory human trial published in 2019 found changes in two skin-aging markers and reported visible improvement in some participants—but it did not prove that the treatment reverses skin age or establish it as an approved anti-aging therapy.
What is the drug, and is it new?
Rapamycin, also known as sirolimus, inhibits the mTOR pathway. The study that appears to underlie the headline examined a topical formulation of the drug on aging skin. It was published in 2019, so the evidence is not a newly established breakthrough or a newly validated treatment.
The paper’s title is “Topical rapamycin reduces markers of senescence and aging in human skin: an exploratory, prospective, randomized trial.” Its findings support investigating possible effects on skin markers and appearance; they do not substantiate the broader claim that rapamycin turns back a person’s skin age.

Researchers at Drexel University College of Medicine enrolled 36 people older than 40 who had signs of photoaging and loss of dermal volume. Participants applied topical rapamycin to the back of one hand and a placebo to the other every 24 to 48 hours. The study was randomized and placebo-controlled, allowing a comparison between treated and placebo-treated hands.

Nineteen participants discontinued or were lost to follow-up, leaving 17 who completed the study. Tissue analysis was available for only a subset of those completers. That attrition and the limited biopsy sample are important when interpreting the results: the number enrolled is not the number who completed the trial or contributed tissue data.
What changed, and what did the results mean?
Markers measured in skin
The researchers reported a reduction in p16INK4A protein, a marker associated with cellular senescence, with P = 0.008. They also reported an increase in collagen VII, a protein important to the skin’s basement membrane, with P = 0.0077. These are results from the study; they are not percentages of rejuvenation or a measured number of years removed from skin age.

Visible appearance
The authors also described clinical improvement in skin appearance for many of the participants who completed the study. That observation is distinct from the tissue-marker results, and the trial does not establish how large, durable, or broadly reproducible any visible benefit would be.
How strong is the evidence?
The trial offers an early human signal, not proof of a reliable treatment. It was exploratory, had substantial dropout, and included tissue analysis from only a subset of completers. The findings are therefore difficult to generalize beyond the participants and conditions studied.

- What it does not establish: The study does not prove reversal of biological age, whole-body rejuvenation, long-term safety, or broad effectiveness.
- What remains unresolved: The evidence described here does not establish independent replication or current regulatory approval and commercial availability for topical anti-aging use.
Is topical rapamycin an approved anti-aging treatment?
The 2019 study does not establish that topical rapamycin is approved or widely available for anti-aging treatment. Its results alone cannot answer the current regulatory or commercial-status question. Treat claims that it is an established skin-rejuvenation medicine as stronger than the human evidence presented here supports.

Topical rapamycin is a plausible subject of skin-aging research, and one small exploratory trial reported promising changes in markers and appearance. But the study is not enough to conclude that the drug turns back skin age or works as a generally applicable anti-aging treatment. Larger, independently replicated evidence would be needed to support those stronger claims.
