TMS and ECT are different clinician-delivered treatments, not interchangeable versions of the same procedure. Repetitive transcranial magnetic stimulation (rTMS, usually called TMS) uses magnetic pulses to stimulate a targeted brain area and generally does not require anesthesia or intentionally cause a seizure. Electroconvulsive therapy (ECT) uses electrical stimulation to induce a brief seizure under general anesthesia. The choice depends on factors such as symptom severity and urgency, previous treatment response, medical status, memory concerns, and patient preferences.
How are TMS and ECT different?
| Decision point | TMS (usually rTMS) | ECT |
|---|---|---|
| How stimulation works | A coil placed against the scalp delivers repeated magnetic pulses. These induce weak electrical currents that stimulate neurons and neural circuits in a targeted area associated with mood regulation. | Electrodes deliver a controlled electrical current through the brain, intentionally inducing a brief seizure as part of treatment. |
| Anesthesia and seizure | Generally performed without anesthesia and does not intentionally induce a seizure. Seizure is an uncommon but serious possible adverse effect. | Performed under general anesthesia with a muscle relaxant to reduce movement during the induced seizure. |
| Typical setting | Usually an outpatient clinical or office setting. | Outpatient care or treatment during a hospital stay, depending on the person’s circumstances. |
| Typical course described by NIMH | Often five days per week for four to six weeks. Session duration and accelerated schedules vary by protocol. | Often three sessions per week, commonly six to twelve treatments until symptoms improve. The course and schedule vary. |
| Commonly described treatment context | Moderate-to-severe depression when medication has not worked well enough or has not been tolerated. | Severe or treatment-resistant depression, and some situations where a rapid response is important. It may also be considered for severe mania or catatonia. |
| Common effects | Scalp discomfort or pain, headache, facial muscle twitching, and lightheadedness. | Confusion, disorientation, memory problems, headache, nausea, jaw pain, or muscle aches. |
These are general patterns, not a treatment plan. Device labeling, protocols, a person’s health history, and local clinical practice affect the details. The schedules above are typical descriptions from the National Institute of Mental Health (NIMH), not guarantees about an individual’s course.
What happens during each treatment?
TMS: targeted magnetic pulses
For rTMS, a clinician positions a magnetic coil against the scalp and delivers a series of pulses. The resulting magnetic field induces weak electrical currents in a targeted brain area. The treatment is intended to stimulate neurons and neural circuits involved in mood regulation. Because rTMS does not generally require anesthesia, people are usually awake during the procedure and receive treatment in a clinical or office setting.
A session may take a few minutes or tens of minutes, depending on the protocol. Some accelerated protocols deliver multiple sessions in a day and can shorten the overall course, but availability and schedules depend on the clinic and treatment approach. Ask which protocol is being proposed and how long a visit is expected to take.

ECT: electrical stimulation and a controlled seizure
ECT is a medical procedure in which a controlled electrical current passes through the brain to induce a brief seizure. The person receives general anesthesia and a muscle relaxant, which reduces movement. ECT is delivered as a course of treatments rather than as a single session; care may take place in a hospital or on an outpatient basis.
Electrode placement and pulse width are meaningful details to discuss with the treating doctor. NIMH notes that modern brief- or ultrabrief-pulse approaches may be used. The clinical team can explain which approach is planned and how it relates to the person’s treatment goals and risk considerations.
When might a clinician consider ECT instead of TMS?
Neither treatment is automatically appropriate for everyone with depression. NIMH says brain stimulation is generally considered when psychotherapy and medication have not reduced symptoms sufficiently. A clinician may consider brain stimulation earlier when depression is severe and life-threatening—for example, when someone is at high risk of suicide or has stopped eating or drinking. The choice depends on the person’s needs, preferences, and medical situation and requires assessment by a healthcare professional.

ECT may be considered for severe or treatment-resistant depression, including circumstances in which a rapid improvement is important. Mayo Clinic also identifies severe mania and catatonia as possible contexts for ECT. Whether ECT is suitable depends on an individual medical and psychiatric assessment; the presence of a diagnosis alone does not determine the choice.
ECT involves anesthesia and intentionally induces a seizure, so the clinical team evaluates medical risks as part of planning. That evaluation and discussion of memory effects are important parts of informed decision-making, not reasons to assume the procedure is either suitable or unsuitable without an assessment.

Situations in which TMS may be considered
rTMS is used for moderate-to-severe depression when medicines have proved ineffective or intolerable. Its device-specific indications and age criteria can differ and may change, so they should be checked against current FDA information and the labeling for the particular device being considered. NIMH’s page text reports that rTMS is not approved for youth under 15 and also notes a 2024 clearance for adjunctive depression treatment in adolescents aged 15 and older. Those details do not establish that every device or protocol has the same indication.
For a person weighing TMS and ECT, the practical questions are not only which treatment is available, but also how urgent the need for improvement is, what treatments have already been tried, and which risks or burdens matter most to that person. A clinician can explain how those factors affect the options in the specific case.

Typical TMS schedule
NIMH describes a typical rTMS course as treatment five days a week for four to six weeks. Individual sessions can range from a few minutes to tens of minutes depending on the protocol. Accelerated approaches can provide multiple sessions in a day and shorten the overall course, but not every clinic offers them, and a shorter calendar schedule does not by itself establish which treatment is best for a particular patient.
Typical ECT schedule
NIMH describes ECT as commonly given three times a week until symptoms improve, often within six to twelve treatments. ECT may be delivered as outpatient care or during a hospital stay. The actual schedule and number of treatments depend on the clinical situation and response.
For either treatment, ask the care team how it will assess progress, when it will review the plan, and what happens if improvement is insufficient or side effects become difficult. The typical course is a guide to what is commonly done, not a promise of a particular number of visits or outcome.

What side effects and risks should patients consider?
TMS effects and precautions
Commonly described rTMS effects include scalp discomfort or pain, headache, facial muscle twitching, and lightheadedness. These effects are often mild to moderate, but their impact varies. Uncommon serious risks can include seizure, mania, and hearing loss if appropriate ear protection is not used. TMS is not risk-free simply because it usually does not require anesthesia.

Metal implants or implanted devices may make rTMS inappropriate or require specific screening. Before treatment, tell the clinical team about implants, devices, relevant medical history, and any new symptoms or health changes. Ask what screening is needed for the particular coil and protocol and how hearing protection is handled.
ECT effects and precautions
ECT can cause temporary confusion or disorientation and memory problems, including difficulty recalling events around the treatment period. Headache, nausea, jaw pain, and muscle aches can also occur. Memory effects matter in the decision: they often improve, but their degree and duration vary. Ask the treating clinician what memory changes to watch for and how they will be assessed in the individual case.
General anesthesia and temporary increases in heart rate and blood pressure are also relevant to ECT’s medical risk assessment. The care team can explain what evaluation is needed and how a person’s health affects planning. Do not assume that a general description of common effects predicts an individual’s experience.
Does TMS work as well as ECT?
The available source material does not establish a universal head-to-head efficacy winner for TMS versus ECT, nor does it provide a current, directly comparable response or remission percentage across patient groups. NIMH describes ECT as having therapeutic effects that rTMS does not match, while the sources do not supply a direct comparison that supports declaring one option universally superior for every patient. ECT is also described as a particularly rapid option for severe illness. These points support a qualified discussion of clinical roles, not a promise of outcome or a one-size-fits-all ranking.

One important distinction: magnetic seizure therapy (MST) is not ordinary TMS or rTMS. MST uses magnetic pulses to deliberately induce a seizure under anesthesia and remains under investigation. A 2023 NIMH research highlight described a trial comparing MST with a specific ECT protocol; it is not evidence that TMS and ECT have equal efficacy. The figure that 21 million U.S. adults experienced a depressive episode in 2021, reported in that highlight, is background context and says nothing about either procedure’s effectiveness.
What should I ask a clinician before choosing?
A useful appointment is a chance to connect the general differences to a person’s history, priorities, and treatment plan. Mayo Clinic psychiatrist Dr. Simon Kung advises discussing the risks and benefits and how TMS may fit into the treatment plan. Consider bringing questions such as:
- Why are you considering TMS, ECT, or another treatment in my situation, and what clinical goal would it address?
- How urgent is treatment, and how does that urgency affect the options?
- Which treatments have I tried, how did I respond, and does that history change your recommendation?
- For TMS, which device and protocol are you proposing, what is its current indication, and what screening is needed for implants or devices?
- For ECT, what electrode placement and pulse width are planned, what anesthesia assessment is needed, and how will memory effects be monitored?
- What schedule should I expect, how will we judge whether treatment is helping, and when will we reconsider the plan?
- What side effects or warning signs should I report promptly, and whom should I contact?
- What follow-up care is planned after the acute course, including medication, psychotherapy, maintenance ECT, or other support?
- How do access, travel, time away from work or school, and support after treatment affect the plan?
What happens after a course of treatment?
Improvement does not remove the need for follow-up. NIMH notes that medication, maintenance ECT, or both may be used after ECT. After rTMS, standard depression care such as medication and psychotherapy may continue. The appropriate plan depends on the person’s response and clinical needs; discuss continuation and follow-up before the treatment course ends rather than treating the last session as the end of care.
When to seek urgent help
If someone may be in immediate danger, is at high risk of suicide, or has stopped eating or drinking because of severe symptoms, seek prompt professional or emergency help. Do not wait for a routine TMS or ECT appointment to address an immediate crisis. A qualified clinician can assess safety and determine what care is needed.

Frequently Asked Questions
Is TMS the same as electroconvulsive therapy?
No. TMS, usually repetitive TMS (rTMS) for depression, uses repeated magnetic pulses to stimulate a targeted area and generally does not require anesthesia or intentionally cause a seizure. ECT uses electrical stimulation to intentionally induce a brief seizure under general anesthesia, with a muscle relaxant to reduce movement. They differ in procedure, schedule, typical clinical context, and side-effect profile.
Does TMS cause seizures or memory loss?
TMS does not intentionally induce a seizure, although seizure is an uncommon serious adverse effect. Common effects include temporary scalp discomfort or pain, headache, facial muscle twitching, and lightheadedness. Memory problems are more commonly associated with ECT; ECT-related confusion and memory effects often improve, but their degree and duration vary. Discuss personal risks and any new symptoms with the treating clinician.
When might ECT be considered instead of TMS?
ECT may be considered for severe or treatment-resistant depression and when a rapid improvement is important; Mayo Clinic also identifies severe mania and catatonia as possible contexts. Brain stimulation may be considered earlier when severe depression is life-threatening, such as when someone is at high risk of suicide or has stopped eating or drinking. A clinician weighs urgency, prior treatment, medical status, preferences, and risks rather than using a diagnosis alone to choose.
Recommended Free Tools
How many TMS or ECT sessions are usually needed?
NIMH describes a typical TMS course as five days a week for four to six weeks, with sessions ranging from a few minutes to tens of minutes depending on protocol. Accelerated schedules may deliver several sessions per day. ECT is commonly given three times a week, often for six to twelve treatments until symptoms improve. Actual courses vary, so ask the care team how it will track response and review the plan.
Which treatment works better, TMS or ECT?
The sources cited here do not provide directly comparable response or remission percentages establishing a universal winner. NIMH describes ECT as having therapeutic effects that rTMS does not match and notes ECT’s role when a rapid response may matter, but that does not mean ECT is right for everyone. The best option depends on the clinical situation and should be discussed with a qualified clinician.
Is magnetic seizure therapy another name for TMS?
No. Magnetic seizure therapy (MST) deliberately uses magnetic pulses to induce a seizure under anesthesia and remains under investigation. Ordinary TMS/rTMS generally does not induce a seizure and is a different treatment. A 2023 NIMH highlight described an MST-versus-ECT trial; it should not be treated as a direct comparison of TMS with ECT.
The Bottom Line
TMS and ECT are distinct clinical procedures: rTMS uses targeted magnetic pulses and usually requires no anesthesia, while ECT uses electrical stimulation to induce a brief seizure under anesthesia. Their treatment schedules and risks differ, and neither is a universal fit. A clinician can help weigh urgency, prior treatment response, medical considerations, memory concerns, preferences, and follow-up needs.
