The immune cells that clear an injury can also cause lasting damage. Macrophages, neutrophils, T cells, innate lymphoid cells and type 2 immune cells promote regeneration when their activity is proportionate, coordinated and short-lived. If the same programs remain intense, mistimed or unresolved, they can destroy tissue, deplete muscle and stimulate fibrosis.

There is no single “healing switch.” The outcome depends on when a cell acts, which signals it receives, the organ involved, how long activation lasts and whether the response reaches resolution.

Why timing determines whether inflammation heals or harms

Inflammation is not the opposite of repair. Early inflammation removes microbes, dead cells and damaged matrix while alerting local progenitor and stromal cells. Repair then requires the response to change: inflammatory signals must recede, blood vessels and epithelium must be rebuilt, and extracellular matrix must be reorganized rather than continually deposited.

A 2022 review in the FEBS Journal captured the principle by comparing acute muscle injury with chronic inflammation in Duchenne muscular dystrophy: “the same cells and molecules in different numbers, concentration and timing contribute to very different outcomes.” In an acute injury, those variables can produce orderly regeneration. In a chronic disease, asynchronous or persistent activation can keep tissue in a damaging maintenance phase.

Injury phase Typical immune task Potential benefit Risk when excessive or mistimed
Initiation Detect danger, recruit leukocytes, kill pathogens and remove debris Rapid containment and a clean site for repair Collateral cell death and amplification of inflammation
Maintenance Coordinate leukocytes with epithelial, vascular and progenitor cells Cell proliferation, angiogenesis and replacement of damaged structures Continued tissue injury, impaired healing and loss of muscle or functional cells
Resolution and remodeling End inflammatory recruitment, clear residual material and reshape matrix Restored architecture with limited scar Persistent profibrotic signaling, excessive matrix deposition and fibrosis

The same cell can therefore appear “inflammatory” in one phase and “reparative” in another. Cell origin, local cytokines, danger signals and the surrounding matrix all influence that transition.

Macrophages are flexible coordinators, not fixed M1 or M2 types

Macrophages arrive from circulating monocytes and also reside permanently in tissues. They sense pathogens, dying cells, cytokines, growth factors and changes in the extracellular matrix. Those inputs can produce inflammatory, reparative, pro-resolving or fibrotic behavior, with cells moving among states rather than occupying two permanent categories.

Early macrophage activity clears the way

Immediately after injury, inflammatory macrophages help eliminate microbes and dead material and release signals that recruit and organize other cells. This phase is necessary in many injuries: suppressing it indiscriminately can leave debris, prolong infection or prevent the signals needed to start regeneration.

Later states support rebuilding

As danger signals decline, macrophages can become pro-resolving and reparative. They support new blood-vessel growth, epithelial expansion, progenitor-cell behavior and extracellular-matrix turnover. Macrophage-derived insulin-like growth factor 1 (IGF-1), vascular endothelial growth factor alpha (VEGF-alpha), transforming growth factor beta (TGF-beta) and Wnt proteins can each contribute to these processes in the right context.

These mediators are not inherently beneficial. TGF-beta and other matrix-regulating signals can help organize a temporary scaffold, yet persistent or dysregulated signaling can activate fibroblasts and produce a dense scar. VEGF-alpha can support angiogenesis during repair, while an abnormal or prolonged response can accompany pathological remodeling. The effect depends on dose, duration, tissue and neighboring signals.

Why the M1/M2 shorthand is incomplete

“M1” and “M2” are useful labels for explaining broad inflammatory and repair-associated tendencies, but they are not mutually exclusive cell types. Single-cell studies and tissue analyses show mixtures of states, transitional populations and substantial differences between organs. A macrophage may express features associated with both categories, or change its profile as the matrix and cytokine environment changes.

The practical question is not whether a macrophage is M1 or M2. It is what the cell is doing now, what signals are maintaining that behavior and whether the tissue is moving toward resolution or continued injury.

Neutrophils and T cells can protect healing or obstruct it

Neutrophils provide fast defense

Neutrophils are among the first responders. They destroy invading organisms, ingest damaged material and release mediators that help initiate and eventually end the inflammatory phase. Their speed is protective when a wound is contaminated or infected.

Uncontrolled recruitment, prolonged survival or excessive mediator release can damage viable cells and interfere with later repair. The problem is not neutrophil presence by itself, but failure to limit the response after the immediate threat has been addressed.

T cells set the response’s direction

T cells shape macrophage behavior and coordinate communication among immune and tissue cells. Regulatory T cells help restrain excessive inflammation and can align macrophage activity with repair. Other T-cell responses may be useful for pathogen control yet harmful when they remain activated after the threat has passed. Their effect therefore depends on antigen exposure, timing and the tissue’s cytokine environment.

Type 2 immunity and innate lymphoid cells have a similar double edge

Type 2 cytokines and the cells that produce them can suppress excessive type 1 inflammation and activate repair programs. They are involved in restoring barriers and coordinating stromal responses after injury.

When type 2 activation is chronic or excessive, however, the same pathways can stimulate fibroblasts and drive pathological fibrosis. A 2018 review in Nature Reviews Immunology summarized the danger: “excessive and chronic activation of the type 2 response can lead to allergic disease, reduction in pathogen defence and lethal fibrosis.” Thus, reducing one inflammatory pathway does not automatically produce healthy regeneration; an overactive repair pathway can be damaging too.

Innate lymphoid cells (ILCs) likewise balance defense, restoration and chronic inflammation. They respond rapidly to tissue-derived signals and can influence epithelial and stromal cells without the antigen-specific receptors used by conventional T cells. A 2019 article in the Journal of Clinical Investigation described ILCs as “a double-edged sword, balancing the inflammatory and reparative responses that arise during injury and disease.”

Repair and inflammation are coupled in epithelial tissues

Epithelial repair depends on continuous cross-talk between immune cells and the cells that form a barrier. Immune-derived growth factors and cytokines can stimulate epithelial proliferation, coordinate vascular support and alter matrix turnover. In return, damaged epithelial cells release danger signals that recruit and reprogram leukocytes.

This coupling makes repair vulnerable to misdirection. Programs that normally close a wound can be co-opted in cancer, where they may support tumor growth or invasion. Similar immune-repair circuits contribute to psoriasis, inflammatory bowel disease and fibrotic disorders when activation repeatedly occurs or fails to resolve.

How to judge whether an immune response is regenerative or destructive

No universal threshold predicts the exact moment when an immune cell switches from beneficial to harmful. Reviews do not establish a single pooled statistic or concentration that applies across organs. Instead, evaluate the response along several axes:

  1. Timing: Is the response confined to the early injury window, or is it continuing after debris and pathogens have been controlled?
  2. Intensity and concentration: Are cytokines and growth factors present at levels appropriate for the tissue, or are they amplifying collateral damage?
  3. Cell state and origin: Are recruited and resident macrophages, neutrophils, T cells or ILCs performing different tasks?
  4. Local environment: What danger signals, matrix changes, oxygen conditions and neighboring-cell signals are present?
  5. Organ context: A response that helps skeletal muscle may have different consequences in lung, liver, skin or intestine.
  6. Endpoint: Is the tissue clearing debris, restoring blood supply and epithelium, or accumulating matrix and losing functional cells?

These criteria explain why a therapy that broadly suppresses inflammation can fail: it may remove damaging signals but also block the early clearance and coordination required for regeneration. Effective immune modulation aims to restore proportion, timing and resolution rather than eliminate inflammation wholesale.

What this means for the “macrophage switch”

The macrophage switch is not a single molecular toggle. It is a change in network behavior as danger signals fall, cytokines change, growth factors accumulate and the matrix is remodeled. Macrophages interpret those inputs and alter their support for angiogenesis, epithelial growth, progenitor differentiation or fibrosis.

In practical terms, healthy regeneration requires the inflammatory and reparative programs to occur in the right sequence and then stop. The same immune machinery can clear a wound and build a replacement, or—if activation is too strong, too long or out of sequence—convert repair into chronic injury and scar formation.
