Agentic pentesting can discover adaptive attack paths that scripted scanners miss, but it cannot safely or technically test every asset. Production systems that could crash, segmented networks, air-gapped environments and tightly controlled tools often remain unexploited. Treat every untested asset as unknown, then combine agentic attacks with deterministic regression tests and exposure or control validation.

The other major limitation is variability. An agent’s result depends on the model, prompt, tools, memory, retrieved data, policy and number of attempts. A single successful or failed run is therefore evidence about one execution, not a complete security verdict.

Why agentic pentesting leaves blind spots

Untestable assets are not safe assets

Real exploitation can crash a service, corrupt records, trigger failover or create an outage. Safety rules should keep an autonomous agent away from fragile production systems unless a separately approved test window and rollback plan exist. Network reachability creates another gap: an agent on one segment cannot directly test an isolated zone, and it cannot attack an air-gapped system without a controlled bridge.

A Picus-sponsored article citing Omdia research says organizations pentest 32% of their attack surface on average; the article does not state the Omdia research year. That figure is vendor-reported, but it illustrates the coverage problem: an agent can be highly capable and still provide no exploit verdict for most reachable or prohibited assets.

Probabilistic behavior makes one pass inadequate

Agent behavior changes with attack wording, tool availability and conversation history. NIST’s CAISI reported in 2025 that replacing baseline attacks with model-specific red-team attacks raised success against the strongest tested model from 11% to 81%. In another test, repeating five injection tasks 25 times increased average success from 57% to 80%. A first run can therefore understate risk, while a single success can overstate how consistently an attack works.

Use a four-layer coverage model

An agentic test is more than an LLM prompt. Coverage should be assessed separately at four layers, with retrieval treated as its own authorization surface.

1. LLM reasoning and policy adherence

Test whether the model follows system instructions, recognizes malicious intent, refuses prohibited objectives and preserves constraints over a long conversation. Include urgency framing, role-play, conflicting instructions, hidden text and multi-turn crescendo attacks. A refusal in one wording does not establish resistance to paraphrases or context changes.

2. Tool execution and identity

Probe every tool for unauthorized calls, parameter tampering, credential exposure, privilege escalation and approval bypass. Enforce authorization outside the model: an LLM should not be able to turn a natural-language instruction into an unrestricted shell command, database query or cloud action. Bind approvals to the exact tool, target, parameters and expiry time.

3. Infrastructure and reachable attack surface

Map the agent’s network routes, credentials, service accounts, secrets stores, egress paths and isolation boundaries. Test what happens when a tool times out, returns malformed data or reaches a forbidden host. Infrastructure controls must hold even if the model is fully compromised.

4. Inter-agent communication and protocols

For orchestrated systems, test delegation chains, message tampering, identity spoofing, replay, confused-deputy behavior and trust-boundary crossings. Include implementation flaws in MCP or A2A integrations, substituted dependencies and supply-chain changes. A compromised sub-agent should not be able to silently elevate an orchestrator’s authority.

RAG retrieval is a separate authorization surface

Email, documents, websites and retrieved passages can contain text that looks like instructions. Test retrieval poisoning, cross-tenant leakage, stale permissions and citations that cause an agent to act on untrusted content. Verify that the retrieval layer applies document- and user-level authorization before content reaches the model; do not rely on the model to distinguish data from commands.

How to close the gaps in practice

  1. Inventory and classify assets. Record production, test, segmented and air-gapped systems; data sensitivity; outage tolerance; reachable routes; and the actions an agent could perform.
  2. Define an explicit scope. Name allowed hosts, accounts, tools, time windows, rate limits and prohibited actions. Treat an omitted asset as out of scope, not implicitly approved.
  3. Set a graduated autonomy level. Begin with read-only discovery, move to simulated exploitation, and permit state-changing actions only after a fresh human approval. Add automatic timeouts, rate limits and a circuit breaker.
  4. Apply least privilege. Give each agent and sub-agent the smallest identity and network access needed for its task. Separate discovery credentials from exploitation credentials, and prevent access to production secrets by default.
  5. Build a deterministic baseline. Use repeatable checks for configuration, known vulnerabilities, authentication, authorization, exposed services and security controls. These tests provide regression coverage when an adaptive agent cannot run.
  6. Run adaptive tests repeatedly. Generate model-specific attack variants, execute each abuse case many times, and record success, failure, timeout and refusal separately. Keep a regression suite for every previously found weakness.
  7. Validate exposure without disruptive exploitation. For fragile or unreachable assets, verify reachability, vulnerable versions, configuration, compensating controls, segmentation and detection evidence. Mark the result as exposure validated or control validated, not as a confirmed exploit.
  8. Review and preserve evidence. Have a human approve high-impact actions, examine findings and decide residual risk. Store the agent version, model provider, tool policy, retrieval configuration, prompts, expected outcomes, approvals, denials, timeouts, circuit-breaker events and compensating controls.

How to assess assets an agent cannot safely exploit

Use a separate evidence path rather than forcing a binary vulnerable/not-vulnerable label.

Guardrails required before high-impact autonomy

  • Human approval: require an identified reviewer before destructive, external-facing, credential-changing or data-moving actions.
  • Fresh approvals: invalidate approval when the target, parameters, identity, tool or risk level changes.
  • Independent enforcement: place policy checks, allowlists and deny rules at the gateway or tool layer, outside the LLM.
  • Change management: schedule tests, notify owners, define rollback and record the change ticket.
  • Containment: isolate test credentials, restrict egress, cap concurrency and stop on error thresholds.
  • Manipulation resistance: treat retrieved and user-supplied content as untrusted data unless separately authorized.
  • Auditability: make every tool call, approval, denial and policy decision attributable and timestamped.
  • Secret hygiene: version-control prompts and expected denials without storing live customer data, tokens or credentials.

Microsoft’s red-team-agent guidance states, “Human approval is required before agent actions proceed.” It also warns that agent outputs are AI-generated and can contain errors or inaccuracies, so human review is required before acting on findings. Those capabilities are described as preview guidance and may change; organizations should verify the controls in their deployed version.

What standards add

The OWASP Autonomous Penetration Testing Standard (APTS) describes itself as “a governance standard for autonomous penetration testing platforms.” Its current project page lists 173 tier-required requirements across eight domains. The standard structures scope enforcement, safety controls, human oversight, graduated autonomy, auditability, manipulation resistance, supply-chain trust and reporting. It complements established testing methods rather than replacing them.

OWASP AI Exchange also recommends combining agentic red teaming with conventional application security testing. An MCP server, for example, can expose ordinary SSRF, SQL injection or XSS paths that require established web and API testing in addition to agent-behavior tests.

Compare the three testing modes

Approach Best coverage Limitations Best use
Agentic pentesting Adaptive attack paths, chained reasoning and novel combinations Probabilistic results, safety restrictions and variable tool reach Exploratory attack-path discovery within a controlled scope
Deterministic automation Repeatable checks for known vulnerabilities, configuration and policy Limited creativity and weak coverage of novel chains Frequent regression testing and fix verification
Exposure and control validation Unexploited, fragile, segmented or air-gapped assets Cannot prove that a real exploit would succeed Population-wide prioritization and compensating-control assurance

Measure reliability instead of claiming certainty

A useful report distinguishes the technical result from the confidence in that result.

Metric What to record
Asset coverage In-scope assets, reachable assets, prohibited assets and assets assessed only through controls
Reproduction rate Successful runs divided by attempts for each abuse case, with the number of trials
Task-level severity Impact of the specific action, not merely an average benchmark score
Control outcome Blocked, detected, approved, timed out, circuit-broken or escaped
Retest status Whether the same attack and independent variants were rerun after remediation
Residual risk Untested paths, assumptions, compensating controls and the owner accepting the remaining uncertainty

An illustrative coverage lesson

A Picus-sponsored case describes 480 affected nginx-ui instances: 120 were reachable for agentic testing, 80 were confirmed exploitable, and 360 received no direct exploit verdict. The article presents exposure validation as a way to assess the remaining population through control evidence. These figures are illustrative vendor-reported numbers, not an independent benchmark; the important lesson is to keep “not directly tested” separate from “not vulnerable.”

The practical conclusion is a blended program: let agents search deeply where actions are authorized and reversible, use deterministic suites to retest every fix, and use exposure or control validation for everything that cannot be safely exercised. That combination produces a defensible answer without pretending an autonomous agent can see or safely touch every asset.
