Survey Form V02 is a free, single-question NPS form for collecting a 0–10 recommendation score and a follow-up reason. It is built with HTML, CSS, and vanilla JavaScript, so Bootstrap is optional. The supplied demo is front-end only: to collect real answers, connect it to a server endpoint, CMS handler, or form service.
What Survey Form V02 does
Colorlib describes V02 as a dark, in-app survey for SaaS dashboards and quarterly relationship surveys. A respondent chooses a score from 0 to 10, then sees a follow-up tailored to the score group: detractor, passive, or promoter. The page also lists an optional email field with inline format validation.
The described interface includes color-coded score segments, a success state that shows the selected score and receives focus, a polite announcement when the follow-up changes, and a two-row score scale on mobile. These are features stated on Colorlib’s template page, not independently verified behavior. Colorlib’s Survey Form V02 page identifies Aigars Silkalns as the author and lists a September 2026 release and page date of September 25, 2026.

Does V02 require Bootstrap?
No. Colorlib describes the snippet as HTML and CSS with a small amount of vanilla JavaScript; it does not include Bootstrap files, jQuery, or plugins. The Bootstrap wording means the form is positioned to work in Bootstrap 4, 5, or 6 projects, not that you must install Bootstrap. You can use it as a standalone front-end snippet or integrate it into a project using one of those Bootstrap versions.

How to collect actual responses
The form’s visible thank-you state does not mean an answer has been saved. Colorlib’s survey-template roundup states that its templates are front-end only and send no data. To collect responses, connect the form to an endpoint, server-side script, CMS form handler, or form service, then validate submitted values on the server. Colorlib’s roundup provides that data-flow qualification; it does not establish a particular backend or service for V02.
- Decide where submissions will be stored and who can access them before collecting scores or email addresses.
- Wire the form submission to that destination; a client-side success message alone is not persistence.
- Validate the score and any other submitted fields on the server. Inline email-format feedback in the interface is not a substitute for server-side validation.
- Test the complete submission path, including errors and successful saves, before using the form with respondents.
License and commercial use
Colorlib lists the free snippet under Creative Commons Attribution 3.0 (CC BY 3.0). Its license page says free snippets may be used personally or commercially when the required credit link is kept, and says the snippet cannot be resold as a template, theme, or snippet pack. This is Colorlib’s summary of its terms; review the license text and files included with the download for the conditions that apply to your use. The paid website-template plans mentioned on the license page are separate products and are not required to use this free snippet.

Choose based on the interaction you need, not on an assumed performance difference: Colorlib does not provide comparative completion or response-rate data for these templates.
