Survey Form V01 is a free, downloadable customer-satisfaction form template, not a survey service: its demo thanks respondents but does not send or save their answers. You can use it standalone or, according to Colorlib, in Bootstrap 4, 5, or 6 projects; connect a backend before using it to collect real responses.
What Survey Form V01 includes
Colorlib’s Survey Form V01 is a post-delivery survey example framed around a coffee roaster. It asks customers to rate satisfaction from 1 to 5 stars and their likelihood to recommend the business on a 0-to-10 scale. There is also an optional comment field with a character counter. After submission, the demo replaces the form with a thank-you message and a discount code.
The page describes the implementation as HTML and CSS with a few lines of vanilla JavaScript, without jQuery, Bootstrap files, or plugins. Colorlib lists the download at 284 KB. Its page says the template can work by itself or in Bootstrap 4, 5, and 6 projects; that is the publisher’s compatibility claim, not an independent compatibility test. Where do the answers go?
In the demo, nowhere. The thank-you state is a front-end interaction; it does not mean responses have been transmitted or stored. Colorlib’s survey-template roundup says the owner must connect an endpoint or form service to receive answers. Before launch, make sure the form submits to a destination you control and that every input has a name attribute. Validate submitted data on the server as well as in the browser. How to make the form collect responses
Choose either a backend you maintain or a hosted form endpoint. A custom backend offers direct control over implementation and data handling, but you must write and maintain the server-side processing. A hosted endpoint processes HTML form submissions without requiring you to write that backend code; it still needs to be configured for your form.
Use a hosted endpoint
- Create an account with the form service you select and generate an endpoint for your form. Basin documents this endpoint-based approach in its HTML form backend guide; Formspree documents a similar flow in its HTML forms guide.
- Set the form’s
actionto your generated endpoint and itsmethodto the method specified by that service. Give every response input a distinct, meaningfulnameso the receiving endpoint can identify the submitted values. - Submit a test response and confirm it appears at the configured destination. Check that the thank-you behavior reflects an actual successful submission, rather than merely hiding the form in the browser.
- Review the service’s privacy, retention, security, regional processing, and plan-limit terms before collecting customer information. The cited endpoint guides establish how submissions can be processed; they do not establish which service is best for those data-handling requirements.
Build your own backend
Configure the form to submit to an endpoint on your site, then implement server-side validation and storage or forwarding there. This route puts the processing under your control, but leaves you responsible for secure handling, error responses, and any applicable privacy obligations.

Check the license and attribution requirement
The template page identifies the license as CC BY 3.0. Colorlib’s roundup says personal and commercial use are allowed when the credit link is retained. Do not treat the download as unrestricted or remove that attribution link; read the license and the template’s current terms before adapting or redistributing it. Colorlib’s roundup states the credit-link condition.

Is it the right fit?
- Good fit: you need a lightweight starting point for a post-delivery satisfaction survey and are prepared to connect submission handling yourself.
- Not a complete solution: you need a ready-to-use hosted survey that automatically stores responses. The template’s demo behavior alone does not provide that.
- Do not infer performance: the 0-to-10 recommendation question is a field in the form, not evidence that the template improves response rates, completion, or conversions.
