SSRIs and SNRIs are both prescription antidepressant classes, but they do not act on exactly the same chemical signals. Selective serotonin reuptake inhibitors (SSRIs) primarily increase serotonin signalling. Serotonin-norepinephrine reuptake inhibitors (SNRIs) increase serotonin and noradrenaline signalling. That difference can affect blood pressure, side-effect patterns and the way each medicine fits with a person’s other prescriptions, over-the-counter products and supplements.

Neither class is universally safer or more effective for every person. The NHS says, “The different types of antidepressant work about as well as each other. But side effects vary between different treatments and people.” The useful comparison is therefore not a blanket winner, but how a specific medicine matches your previous response, health conditions, current medicines and tolerance for particular risks. A prescriber or pharmacist should check the complete list before treatment starts, changes or stops.

What is the difference between an SSRI and an SNRI?

Both classes reduce the reuptake of neurotransmitters, leaving more of those chemical messengers available for signalling between nerve cells. SSRIs target serotonin reuptake. SNRIs target serotonin reuptake and noradrenaline reuptake.

Class Main signalling target Examples named by the NHS Comparison point
SSRI Serotonin Sertraline, citalopram, escitalopram, fluoxetine and paroxetine Common effects often involve nausea, headache, dry mouth, tiredness, sleep and sexual function. The exact profile differs by medicine and person.
SNRI Serotonin and noradrenaline Duloxetine and venlafaxine Many effects overlap with SSRIs. The NHS specifically cautions that SNRIs can raise blood pressure, so blood-pressure history and monitoring may matter.

The class label is only a starting point. Two medicines in the same class can differ in half-life, metabolism, dose range, interactions and the side effects an individual notices. A decision should be made at the medicine-and-person level rather than by assuming every SSRI or every SNRI behaves identically.

Which side effects are common?

Side effects overlap substantially. NHS information for SSRIs includes nausea, headache, dry mouth, tiredness, sleep problems and sexual problems. NHS information for venlafaxine, an SNRI, includes nausea, dizziness or drowsiness, dry mouth, bowel changes, sexual problems, insomnia and weight changes. The lists are examples, not a prediction of what will happen to one person.

Effect or concern How it appears in the guidance What the comparison means
Nausea Listed among SSRI effects and common with venlafaxine. Occurs in both classes; the individual medicine, dose and person influence severity and duration.
Headache Listed for SSRIs. Do not assume an SNRI will prevent it or that every SSRI causes it.
Dry mouth Listed for SSRIs and venlafaxine. Shared effect; persistent or troublesome symptoms warrant a medication review.
Tiredness, dizziness or drowsiness Tiredness is listed for SSRIs; dizziness or drowsiness is listed for venlafaxine. Effects can differ in direction and intensity. Be cautious with activities requiring alertness until you know how your prescribed medicine affects you.
Sleep changes SSRIs may cause sleep problems. Venlafaxine guidance includes insomnia. Sleep effects are not a simple class-wide advantage; timing and the exact drug may matter.
Sexual problems Listed for SSRIs and venlafaxine. Both classes can affect sexual function. Raise this early rather than stopping treatment alone.
Bowel changes Listed in venlafaxine guidance. It is a documented venlafaxine concern, but the presence or absence of a symptom cannot be inferred for every SNRI or SSRI.
Weight changes Listed in venlafaxine guidance. Weight effects vary by medicine and person; monitor a meaningful change and discuss it with the prescriber.

When side effects improve

The NHS notes that many antidepressant side effects ease after a couple of weeks, although some can persist. Do not use that general pattern as a reason to tolerate a severe or alarming reaction. Record when a symptom began, whether it is worsening, and any dose or medicine change, then contact the prescriber or pharmacist for advice.

Why the same side effect can feel different

Risk and intensity depend on the selected medicine, dose, other medicines, age, medical conditions and individual sensitivity. A previous bad reaction to one class is important history, but it does not prove that every medicine in the other class will be suitable or unsuitable. The Specialist Pharmacy Service (SPS) advises considering previous response, past class-related side effects, comorbidities, age and individual needs.

Blood pressure: the notable SNRI distinction

The NHS states that SNRIs such as venlafaxine and duloxetine can raise blood pressure. This is a class-level caution, not a claim that every patient will experience a rise or that every SSRI is free of blood-pressure considerations. A prescriber may want a current blood-pressure reading, a history of hypertension and a plan for follow-up, especially when an SNRI is being considered.

Tell the clinician about diagnosed high blood pressure, cardiovascular problems, home readings that have changed, and medicines that already affect blood pressure. Do not change an antihypertensive or antidepressant dose without professional instructions. If a new or severe symptom follows a medication change, seek medical advice rather than trying to correct it by skipping doses.

How medication interactions differ

Interaction checking is medicine-specific. SPS explains that risk depends on the medicines involved and on the person’s other medicines. The complete list must include prescriptions, pharmacy purchases, pain relievers, cold remedies, recreational substances and supplements. The examples below identify reasons to request a professional check; they are not a complete interaction database.

Combination or situation Why it matters What to do
Another antidepressant or a planned switch Overlapping serotonergic effects can increase adverse effects, including serotonin syndrome. The safest interval and sequence depend on the two exact agents. Give the prescriber every current and recently stopped antidepressant. Do not overlap, taper or cross-taper on your own.
Certain pain medicines Some pain medicines can add serotonergic effects or otherwise change risk. Ask a pharmacist to check the exact product and dose before taking it with an SSRI or SNRI.
Certain antiemetics Some medicines for nausea can contribute to serotonergic combinations. Check the active ingredient, including products supplied outside your usual pharmacy.
Lithium NIMH and SPS identify lithium among medicines that can be relevant to serotonin-toxicity risk when combined with serotonergic antidepressants. Never start, stop or alter lithium or an antidepressant without the treating clinician’s plan.
Triptans NIMH lists triptans as a possible serotonergic combination. Tell the prescriber about migraine medicines by name, not just “a headache tablet.”
St John’s wort NIMH identifies this supplement as a possible contributor to serotonergic interactions. Disclose it and every other supplement. “Natural” does not mean interaction-free.
Anticoagulants with venlafaxine NHS venlafaxine guidance names anticoagulants such as warfarin and apixaban as medicines that may not mix well. Request a medication review. This is not a blanket instruction to stop an anticoagulant.
NSAIDs with venlafaxine NHS guidance names NSAIDs such as aspirin and ibuprofen as potential medicine concerns. Ask whether the specific product is appropriate; do not discontinue prescribed treatment without advice.

Why an interaction checker alone is not enough

A database may flag a pair of ingredients but cannot always determine the right action for your dose, kidney or liver function, age, pregnancy status, treatment history or reason for use. It may also miss an ingredient hidden in a combination cold remedy or supplement. Bring the packaging or a current written list to a pharmacist or prescriber and include medicines used only occasionally.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How to describe an interaction risk clearly

Use the exact product name rather than a broad category. “Ibuprofen 200 mg taken twice this week” is more useful than “an occasional painkiller.” Include when you last took a medicine, whether it is prescribed or bought over the counter, and whether a supplement contains St John’s wort or multiple herbal ingredients. Tell the clinician about medicines prescribed by another service, because an interaction review is only as complete as the list supplied.

Ask whether the concern is serotonin toxicity, blood pressure, bleeding risk or another mechanism. Different mechanisms require different decisions, and a warning does not automatically mean a medicine must be stopped. The pharmacist or prescriber can decide whether an alternative, additional monitoring or a different schedule is appropriate.

Serotonin syndrome: the urgent interaction concern

NIMH describes serotonin syndrome as rare but potentially life-threatening. It can occur when serotonergic medicines or supplements are combined, including during an inadequately planned antidepressant switch. Symptoms listed by NIMH include agitation, muscle twitches, hallucinations, high temperature and unusual blood-pressure changes. NHS venlafaxine guidance also mentions fast heartbeat, sweating, shaking, twitching muscles and confusion or agitation.

This is not a complete symptom checklist and cannot diagnose a reaction. If a serious reaction is suspected after starting, combining or changing medicines, seek urgent medical help and tell clinicians exactly what was taken and when. Keep the medicine containers or an up-to-date list available. Do not take an additional dose, stop essential treatment or add another product while waiting for individualized instructions unless an emergency clinician tells you to.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Comparing the classes in everyday treatment decisions

Patients often ask whether an SSRI or SNRI is “stronger.” The class descriptions do not provide a universal ranking. An SNRI’s additional noradrenaline action may be clinically relevant for some people, but it also creates the NHS blood-pressure caution. An SSRI’s primarily serotonergic action does not eliminate side effects or interactions. The practical question is which exact medicine offers a reasonable balance of expected benefit, tolerability and compatibility with the person’s health context.

Keep a short symptom record during follow-up: the medicine and dose, the date a symptom started, sleep changes, nausea or bowel changes, dizziness, sexual effects, weight changes and any blood-pressure reading requested by the clinician. This record helps distinguish a transient early effect from a persistent problem and gives the prescriber information for a dose adjustment or an alternative. Do not make that adjustment independently.

Shared decision-making also includes priorities that are easy to overlook. Tell the prescriber if sexual side effects, insomnia, daytime drowsiness, dizziness or weight change would be especially difficult for you. A past response matters, but so does the reason a previous medicine was stopped. Clear priorities make it easier to compare two reasonable options without treating one class as automatically safer.

How clinicians decide between an SSRI and an SNRI

There is no reliable “safer class” answer for everyone. A prescriber generally weighs several questions together:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • What has worked before? Previous benefit, lack of benefit and the time a medicine was taken are relevant.
  • Which side effects were unacceptable? A history of persistent sexual problems, sleep disruption, nausea, dizziness or weight change can guide the next choice, but does not guarantee a different drug will avoid the effect.
  • Are there medical conditions to account for? Blood-pressure history is particularly relevant when considering an SNRI. Other comorbidities and age also affect the risk assessment.
  • What else is being taken? Include prescriptions, over-the-counter products, supplements and occasional medicines. Interaction risk belongs to the exact combination, not just the class names.
  • How important is a simple discontinuation plan? Half-life and prior withdrawal experience matter when choosing and later tapering a medicine.

For some people, an SSRI may be a reasonable first option; for others, an SNRI may be selected because of the overall clinical picture or a prior response. That decision requires a clinician who knows the diagnosis, treatment goals and health history.

Before starting: a medication-review checklist

  1. Write the complete list. Record each medicine’s name, strength, timing and purpose. Add non-prescription pain medicines, cold products, migraine medicines, sleep aids, vitamins, herbal products and recreational substances.
  2. Note relevant history. Include high blood pressure, heart or liver problems, previous antidepressant reactions, allergies and any earlier withdrawal symptoms.
  3. Ask for the exact interaction assessment. Request a pharmacist or prescriber review of the proposed medicine against every item on the list, rather than relying on a class-level statement.
  4. Clarify the monitoring plan. Ask what symptoms to watch for, whether blood pressure should be checked, when follow-up is expected and whom to contact if side effects appear.
  5. Confirm the formulation and directions. Check the medicine name, dose, release type and schedule on the dispensing label. Do not assume two products with similar names are interchangeable.
  6. Keep one current list. Update it whenever a medicine starts, stops or changes, and show it at appointments and urgent-care visits.

Stopping, tapering and switching safely

Do not stop an SSRI or SNRI abruptly or self-switch between classes. The NHS recommends discussing discontinuation with a doctor and gradually reducing the dose. SPS identifies paroxetine and venlafaxine as short-half-life examples associated with a greater likelihood of withdrawal effects.

A switch may involve a taper, a pause, a carefully planned overlap or another schedule, depending on the exact medicines. The choice is not determined by “SSRI versus SNRI” alone. Serotonin-toxicity risk, withdrawal risk, the reason for treatment and the person’s previous response all have to be considered.

What to ask at the pharmacy or appointment

  • Which exact medicine is being proposed, and why is it preferred for my circumstances?
  • Could it affect blood pressure or interact with anything on my list?
  • Which early side effects are expected to settle, and which require prompt contact?
  • Does any pain medicine, migraine treatment, antiemetic or supplement I use create a serotonergic combination?
  • If it does not suit me, how will the dose be reduced or the medicine changed?
  • When should we review benefit, side effects and any monitoring results?

Bring the answers into your medication record. A clear list and a documented plan reduce the chance that a new prescriber, pharmacist or urgent-care clinician misses a recent change.

Bottom line for choosing between the classes

SSRIs primarily affect serotonin; SNRIs affect serotonin and noradrenaline. Nausea, dry mouth, sleep changes, tiredness or dizziness and sexual problems can occur with either class, while venlafaxine guidance also highlights bowel and weight changes. SNRIs carry a specific NHS caution about possible blood-pressure increases. Interactions depend on the individual medicine and the full medication and supplement list, with particular review needed for serotonergic combinations and, with venlafaxine, anticoagulants and NSAIDs. Use a prescriber- or pharmacist-guided plan for starting, switching, tapering or stopping, and seek urgent help for a suspected serious reaction.

Frequently Asked Questions

What is the main difference between an SSRI and an SNRI?

SSRIs primarily affect serotonin reuptake. SNRIs affect serotonin and noradrenaline reuptake. The difference does not make one class universally better or safer; the exact medicine, dose, health history and other medicines determine the practical choice.

Do SSRIs or SNRIs have worse side effects?

Neither class is always worse. Nausea, dry mouth, sleep changes, tiredness or dizziness and sexual problems can occur with both. Venlafaxine guidance also lists bowel and weight changes, while the NHS cautions that SNRIs can raise blood pressure. Individual responses vary.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can an SSRI or SNRI raise blood pressure?

The NHS specifically says SNRIs such as venlafaxine and duloxetine can raise blood pressure. Tell your prescriber about hypertension, cardiovascular problems and changing home readings. Do not alter blood-pressure or antidepressant treatment without professional advice.

Can these medicines interact with painkillers?

Some pain medicines can contribute to serotonergic effects. NHS venlafaxine guidance also names NSAIDs such as aspirin and ibuprofen as potential medicine concerns. Ask a pharmacist to check the exact product, dose and frequency rather than assuming every painkiller is interchangeable.

Is St John’s wort safe with an SSRI or SNRI?

NIMH identifies St John’s wort as a possible contributor to serotonergic interactions. Disclose it and every other supplement before starting or changing an antidepressant. Do not add or stop it as a self-directed substitute for prescribed treatment.

Can I switch from an SSRI to an SNRI myself?

No. Switching may require a taper, pause, carefully planned overlap or another schedule. The plan depends on the two exact medicines, withdrawal likelihood and serotonin-toxicity risk. Paroxetine and venlafaxine are short-half-life examples associated with greater withdrawal likelihood.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When should I seek urgent help?

Serotonin syndrome is rare but potentially life-threatening. Concerning symptoms listed by NIMH and the NHS include agitation or confusion, muscle twitching, hallucinations, high temperature, sweating, shaking, fast heartbeat and unusual blood-pressure changes. Seek urgent medical help if a serious reaction is suspected.

What should I bring to a medication review?

Bring an up-to-date list or the containers for prescriptions, over-the-counter products, occasional pain or cold medicines, migraine treatments, supplements and recreational substances. Include strengths, timing, recent changes and previous antidepressant side effects so the exact combination can be assessed.

The Bottom Line

SSRIs and SNRIs share many effects, but SNRIs also affect noradrenaline and may raise blood pressure. Compare exact medicines—not class labels—against your health history and complete medication list. Use a clinician-guided plan for interactions, switching, tapering and stopping.
