Spawn: The Eternal follows Al Simmons, a murdered government assassin who returns from Hell as Spawn after making a bargain with the demon Malebolgia. He accepts the deal to see his former wife, Wanda, but the price is service as a Hellspawn—an instrument for Hell’s coming Armageddon. The game turns that origin into a quest through three realms and different points in time, as Spawn searches for answers and a way to escape Malebolgia’s control.
This guide separates the game’s manual-based story from the wider Spawn mythology, then maps the characters and relationships a new player needs to understand the stakes.
Who is Spawn?
Spawn is the resurrected form of Al Simmons. Before becoming a supernatural fighter, Simmons was a highly trained government assassin. He eventually questioned the purpose of the missions he was ordered to carry out. An associate responded by having him burned to death.
Simmons did not return to ordinary human life. In Hell, he negotiated with Malebolgia, the ruler of Hell’s Eighth Sphere. Simmons wanted one thing: the chance to see Wanda, the woman he loved and had lost. Malebolgia offered resurrection, but the bargain made Simmons a Hellspawn—a soldier expected to serve in Hell’s army.

That arrangement creates Spawn’s central conflict. He has the powers of Hell, yet he does not want to remain Hell’s property. He is useful to the infernal hierarchy precisely because he is powerful, but his growing determination to make his own choices turns him against the beings that brought him back.
What happened to Al Simmons?
His death and the bargain
Simmons’s death is the dividing line between the human character and the supernatural protagonist. He enters Hell as a dead man seeking a personal reunion, not as someone volunteering for a military campaign. Malebolgia turns that private desire into a contract: Simmons can return, but he must come back as one of the Hellspawn.
The game manual expresses the identity break directly. Its premise asks whether the resurrected figure is still Al Simmons and answers that he is now “one of the Hellspawn.” The transformation is therefore more than a costume change. Simmons has a new status, new obligations imposed by Hell and a body altered by resurrection.

The cost of resurrection
Spawn returns scarred, with missing memories and a living costume wrapped around him. Those details show that resurrection has consequences. He is not restored to the exact life he had before death, and the supernatural abilities that make him formidable are tied to the same infernal system he distrusts.

The living costume and powers are part of the Hellspawn transformation. The scars and memory loss communicate what the bargain took from him: a stable sense of identity, an ordinary human future and control over the conditions of his return.
Why did Spawn make a deal with Malebolgia?
Wanda is the emotional reason; Malebolgia is the power collecting the debt. Simmons accepts the bargain because he wants to see his beloved former wife again. Malebolgia uses that longing to secure another soldier for Hell.
This is why the origin is not simply a story about a hero receiving powers. Spawn’s abilities arrive with an obligation he never truly embraces. Hell expects him to contribute to its coming Armageddon, while Spawn increasingly resists the role assigned to him. His objective becomes reclaiming agency without surrendering the supernatural strength that the bargain gave him.

The game’s starting situation
Spawn: The Eternal uses the origin premise as the foundation for a game-specific adventure. Spawn has returned from death, Malebolgia’s forces regard him as an asset, and the player must navigate a world shaped by the consequences of the pact.

The manual-based story describes travel through three realms at different points in time. The exact significance of each location is part of the mystery the player uncovers rather than a simple tour of Spawn’s biography. Creatures sent by Malebolgia attack along the way, making the journey both an investigation and a fight for survival.
What Spawn is trying to accomplish
The immediate objective is to move through hostile realms, discover secrets that may lead back to Hell and learn how Malebolgia’s plan affects Spawn’s future. The game is not merely asking whether Spawn can defeat individual monsters. It asks whether he can use Hell’s powers while refusing to remain Hell’s weapon.
- Survive the journey: Spawn must pass through dangerous realms populated by creatures connected to Malebolgia.
- Uncover the hidden information: The environments and encounters reveal clues that point toward Hell and the source of Spawn’s predicament.
- Confront the power behind the bargain: The story ultimately allows Spawn to attack Malebolgia in search of freedom.
This progression gives the game a clear dramatic shape: resurrection creates the debt, exploration reveals how the debt is enforced and confrontation offers a chance to reject the arrangement.

The manual’s account is a specific game plot, not a complete summary of every Spawn comic storyline. It establishes the three-realm journey, the attacks by Malebolgia’s creatures, the search for secrets and the possibility of confronting Malebolgia. It does not make every later comic development part of the game’s continuity.

Important wider-mythology figures
The Spawn universe extends beyond the characters needed to follow the basic game premise. These figures belong to the broader mythology and should not automatically be treated as events or roles in Spawn: The Eternal.
Angela
Angela is a warrior angel. In the wider conflict between Heaven and Hell, she can become an ally to Spawn. Her presence shows that the supernatural power struggle is not limited to Malebolgia and his servants.
Mammon
Mammon is an agent of Hell. He belongs to the larger infernal network surrounding Spawn rather than replacing Malebolgia as the ruler who makes the original bargain.
Redeemer
Redeemer is Heaven’s counterpart to a Spawn. The character represents the opposing side of the Heaven-versus-Hell conflict and helps explain why Spawn’s existence matters to more than one faction.

How the characters fit together
The personal triangle around Wanda
Wanda is the bridge between Al’s human past and his supernatural present. Simmons’s desire to see her motivates the bargain, while Terry is now her husband and Cyan is part of their family. Spawn therefore faces a painful contradiction: he has returned because of Wanda, but returning does not restore the life he remembers.
The infernal chain of control
Malebolgia stands at the top of the specific bargain that creates Spawn. Violator’s assignment to keep tabs on him demonstrates how that bargain is enforced. Spawn’s powers may help him survive, but the same powers mark him as a participant in Hell’s military plans.
Human allies in a supernatural conflict
Sam Burke and Twitch Williams are detectives and friends of Spawn. Their role is important for orientation: they represent human investigation and personal loyalty in a story otherwise dominated by demons, angels and supernatural armies. Cogliostro occupies a different position as an older guide who helps Spawn understand the wider mythology.
Game story versus the wider comics mythology
New players can avoid confusion by sorting information into four layers: the shared origin, the game’s immediate plot, the surrounding relationships and later or broader franchise developments.

Image Comics’ Spawn #118 explicitly connects Spawn’s present identity with Al Simmons and the original pact with Malebolgia. That linkage supports the basic origin, while the game manual determines which events belong specifically to Spawn: The Eternal.
What new players should remember before starting
- Spawn is Al Simmons after death. The name refers to a transformed identity, not a separate person who merely resembles Simmons.
- Wanda explains the bargain. Simmons accepts resurrection because he wants to see her again.
- Malebolgia owns the contract’s terms. He resurrects Simmons and expects a Hellspawn soldier in return.
- The costume and powers have a cost. They are part of the Hellspawn transformation, alongside scars and missing memories.
- Hell wants a military asset, not an independent hero. Spawn’s resistance to that assignment drives the story.
- The game’s practical goal is discovery and escape. Move through the realms, learn what connects them to Hell and seek a way out of Malebolgia’s control.
- Keep continuity labels in mind. The three-realm journey belongs to the game; the larger cast and Heaven-versus-Hell developments belong to the wider Spawn mythology unless the game specifically establishes them.
Why the story matters
Spawn’s premise combines a straightforward supernatural power fantasy with a question about ownership. Simmons receives extraordinary abilities, but they arrive through a contract made under emotional pressure. Every advantage reminds him of the authority that expects obedience.
That tension gives Spawn: The Eternal its narrative purpose. The monsters and realms are obstacles, but the deeper problem is whether a resurrected person can define himself after another power has rewritten his body, assigned his allegiance and tied his return to a debt. Spawn’s search for Malebolgia is therefore also a search for the right to decide what “Spawn” means.
