The pharmacy locker code in Silent Hill: Townfall is 6539. The CRTV broadcast tells you the order for four poster clues; read their digits in that order. You must enter the pharmacy with the Scalpel first—the combination opens the rusty locker inside, not the pharmacy door.
How to reach the pharmacy locker
- Use Zoe’s Lanyard at the clinic entrance to get inside.
- Find the green box in the Z Ellis room and take the Scalpel.
- Return to the pharmacy and use the Scalpel on its door.
- Go into the pharmacy, then reach the back room and the rusty locker.
The pharmacy entrance and its rear locker are separate obstacles: the Scalpel opens the door, while 6539 opens the locker.
How the pharmacy posters reveal 6539
Inspect the four posters and use the CRTV broadcast to determine their sequence. In broadcast order, take one digit from each poster:
| Order | Poster | What to read | Digit |
|---|---|---|---|
| 1 | “Washing my hands” | Count the fingers shown | 6 |
| 2 | “Prescription drugs and you” | Count the five listed steps | 5 |
| 3 | “Blood pressure” | Read the blood-pressure machine value | 3 |
| 4 | “Taking Antibiotics” | Count the pills left in the packet | 9 |
Enter the digits in that order: 6539. For the hand poster, count the fingers shown; it is not asking you to count both hands.

- Experienced entirely in first person, explore, evade, and survive using a limited set of weapons and tools, including a pocket television used to tune into unstable signals. Evasion is tense; combat is frenetic, while narrative driven puzzles reveal a truth that refuses to stay submerged.
- SILENT HILL: Townfall is a full-length, self-contained psychological horror set against the cold, isolated backdrop of Scotland, 1996
What the locker contains and where the note goes
The locker contains the Prescription Dosage Error note. At the clinic scales, place it in the middle position, with the Broken Stethoscope on the left and Zoe’s Lanyard on the right.
If the code or route is not working
- 6539 does not open the pharmacy door: use the Scalpel from the clinic on the pharmacy door. The combination is for the locker inside.
- You cannot enter the clinic: use Zoe’s Lanyard at the clinic entrance.
- The combination seems correct but progress has not advanced: inspect the CRTV signal and make sure you have collected the linked item or clue. One route guide reports that progress can depend on collecting them.
- Your poster sequence looks different: available walkthroughs do not establish whether the solution changes by difficulty or update. One guide says it has not replayed the puzzle on every difficulty or platform, and another cautions that puzzle steps may change with updates. Follow the clues shown in your own game rather than assuming the reported code is universal.
Walkthrough reports agree on 6539, the CRTV-and-poster clue mechanism, and the note’s role in the clinic-scales objective. The detailed clue breakdown is an independent guide that attributes its answer to GamesRadar and PowerPyx; those original walkthroughs were not available in the cited source set.
