A diabetic foot wound needs prompt professional assessment if redness or skin-tone change is spreading, warmth, swelling, tenderness, drainage, odor, wound enlargement, fever, or a general feeling of illness is increasing. A painless wound can still be infected because diabetic neuropathy may blunt sensation. Do not try to confirm infection with a single symptom, foot thermometer, photograph, or home test. Contact a clinician the same day for a new or worsening wound; seek emergency care immediately for signs of sepsis, rapidly deteriorating deep tissue, pus with necrosis, or threatened circulation.
This article explains warning patterns and urgency levels. It is not a self-diagnosis checklist. Infection is diagnosed clinically by a qualified professional.
How clinicians decide whether a diabetic foot wound is infected
The International Working Group on the Diabetic Foot and the Infectious Diseases Society of America (IWGDF/IDSA) 2023 guideline defines diabetic foot infection through clinical evidence of inflammation. That means a clinician examines the wound and surrounding tissue for local signs, considers systemic illness, and interprets the findings in the context of diabetes, circulation, neuropathy, and the wound’s depth. No single change proves or rules out infection.

At home, you cannot reliably exclude infection because a wound may look deceptively quiet, especially when sensation or blood flow is impaired. The IWGDF/IDSA guideline specifically advises against using foot temperature, however it is measured, to diagnose infection. A warmer area can be concerning, but a normal reading does not make an infected wound safe.

The American Diabetes Association (ADA) advises that every foot ulcer should be examined by a doctor right away, even when it does not hurt. Treat a new ulcer, blister that has opened, cut, burn, or pressure injury as a reason to arrange professional advice rather than waiting to see whether pain develops.
Warning signs around the wound
Kent Community Health NHS Foundation Trust lists a significant increase in one or more of the following changes as a reason to seek advice. The pattern matters more than any isolated sign, and worsening over hours or days is particularly important.

Changes that can occur during uncomplicated healing
Not every visible change means infection. Kent NHS notes that mild redness, slight swelling, and a small amount of light-yellow discharge can occur during normal healing. The important distinction is whether the change is mild and settling or is increasing, spreading, becoming thicker or cloudier, or appearing with fever or worsening illness. Because diabetes can alter healing and sensation, a clinician should still assess a diabetic ulcer promptly rather than relying on appearance alone.
Why an infected wound may not hurt
Diabetic neuropathy can reduce the ability to feel pain, heat, pressure, or injury in the feet. A person may therefore continue walking on a wound that is enlarging or may not notice a new infection until it is advanced. Poor circulation also limits the foot’s ability to fight infection and repair tissue.

Absence of pain is not reassuring. The ADA’s advice applies even to painless ulcers: have a doctor examine them right away. Check your feet visually and, where you can do so safely, compare changes with the previous day, but do not use a lack of discomfort as permission to wait.
Choose the right level of urgency
Same-day professional contact
Contact your local diabetic-foot or podiatry service, general practitioner, or urgent medical advice line the same day when you suspect infection or have a new or worsening wound. In the UK, Kent NHS advises using NHS 111 when its podiatry service is closed and says not to wait until the next working day if symptoms are getting worse. Other countries use different services and phone numbers, so follow your local emergency and urgent-care pathways.
Tell the service that you have diabetes and describe the wound’s location, how quickly it changed, spreading redness or swelling, drainage, odor, fever, and any loss of sensation or circulation problems. If you have previously been given a diabetic-foot contact route, use it rather than waiting for a routine appointment.

Emergency care now
Call emergency services immediately if you may have sepsis or are becoming severely unwell. Kent NHS lists possible sepsis warning symptoms including:

- confusion or slurred speech;
- severe shivering, extreme muscle pain, or unusual coldness;
- little or no urine;
- severe breathlessness;
- mottled, blue, grey, or unusually pale skin; and
- a feeling that you may die.
In the UK, call 999 for these emergency symptoms. Elsewhere, call the correct local emergency number. Do not wait until every symptom appears: rapid deterioration or a strong sense that something is seriously wrong warrants immediate help.
St George’s University Hospitals advises emergency-department assessment for a foot wound with systemic infection signs or rapidly deteriorating deep ulceration or necrosis with purulent discharge. Acute critical limb ischaemia also requires emergency-level evaluation. These situations can threaten tissue and the limb and cannot be safely managed by monitoring at home.
Why some people are admitted to hospital
You should not try to assign yourself an infection grade. The IWGDF/IDSA guideline recommends using a formal infection-severity classification. It recommends considering hospitalisation for all severe infections and for selected moderate infections when important coexisting conditions make outpatient treatment unsafe or less likely to succeed. The decision belongs to the treating team, who can examine depth, circulation, systemic illness, comorbidities, and the need for surgery, intravenous treatment, or close observation.
What to do while arranging care
- Stop treating the wound as routine. Note when you first saw the change and whether redness, swelling, drainage, odor, wound size, or general symptoms are increasing.
- Contact the appropriate service today. Use a diabetic-foot clinic, podiatry service, primary-care clinician, or urgent advice line. State clearly that you have diabetes and a foot wound.
- Escalate if you deteriorate. If you develop emergency symptoms, call the local emergency number rather than waiting for a callback or routine slot.
- Take useful information with you. Bring your medication list, diabetes treatment details, recent glucose information if available, and any previous diabetic-foot records. Do not delay emergency care while collecting paperwork.
- Follow clinician-directed wound care. Dressings and other wound products may be appropriate after assessment, but they cannot determine whether infection is present or how deep it is.
What not to use as a home diagnosis
A foot thermometer
The IWGDF/IDSA guideline advises against diagnosing infection from foot temperature, regardless of the measuring device. Temperature differences can have many explanations, and a reassuring reading cannot exclude infection.

Odor by itself
An unpleasant smell should be reported, especially if it is new or accompanied by drainage or wound enlargement. Smell alone is not proof of infection, just as lack of odor does not prove that a wound is uninfected.
A photograph or video call alone
Images can help a clinician understand what has changed, but they cannot reliably show depth, tissue involvement, circulation, or systemic illness. Do not postpone an in-person assessment because a photograph looks mild.
Leftover or self-selected antibiotics
Do not start antibiotics left from an earlier illness or choose a product based on a description online. The IWGDF/IDSA guideline advises against treating clinically uninfected ulcers with local or systemic antibiotics merely to prevent infection or promote healing. When infection is present, the clinician must choose treatment based on examination, severity, likely organisms, allergies, kidney function, prior treatment, and local practice.

How diabetes changes the risk picture
Neuropathy can hide pain and pressure damage. Circulation problems can reduce oxygen delivery and make it harder for tissue to resist infection and heal. These factors mean that a small-looking wound may deserve urgent review, while a person who feels well may still have important local disease. Tell the clinician if you have known neuropathy, peripheral arterial disease, previous ulcers or amputations, kidney disease, or immune-suppressing treatment; these details can affect urgency and management.

Questions a clinician will need to answer
Professional assessment is intended to determine whether inflammation is present, how far it extends, and whether deeper structures or circulation are involved. The team may need to establish:
- how quickly the wound and surrounding skin have changed;
- whether redness, warmth, swelling, tenderness, discharge, or odor is increasing;
- whether fever or other systemic symptoms are present;
- how deep the ulcer is and whether there is necrotic tissue or pus;
- whether neuropathy has reduced sensation;
- whether blood flow is adequate for healing; and
- whether the infection is mild, moderate, or severe under the IWGDF/IDSA classification.
Those answers determine whether care can be provided through a specialist outpatient pathway or whether hospital treatment and urgent intervention are needed.

The safest rule is to act on a worsening pattern, not to wait for a dramatic photograph, severe pain, or a positive home test. Diabetic foot infection is a clinical diagnosis, and early professional assessment gives the care team the best chance to prevent progression.
