The short answer: a U.S. “storm overflow” usually means a combined sewer overflow (CSO)—rainwater and wastewater share one pipe, and excess wet-weather flow is released through a relief outfall. A “sewage spill” is broader; in a separate sanitary system, the technical term is usually sanitary sewer overflow (SSO), meaning sewage escaped because of a blockage, break, equipment failure, excessive inflow or another problem. Both can contain untreated sewage and create health and water-quality hazards. The sewer design and discharge mechanism—not simply whether it rained—determine the classification.
The central difference: one shared pipe versus a sewage-system failure
In the United States, the Environmental Protection Agency (EPA) distinguishes these events by the type of sewer that released the flow:
| Question | Combined sewer overflow (CSO), often called a storm overflow | Sanitary sewer overflow (SSO), a type of sewage spill |
|---|---|---|
| System design | A combined sewer carries sanitary wastewater and stormwater runoff in the same pipe. | A sanitary sewer primarily carries wastewater. Stormwater is normally collected by a separate drainage system, although groundwater, rainwater or other unwanted flows can enter through defects or improper connections. |
| Typical trigger | Rainfall or snowmelt adds enough runoff to exceed the combined system’s pipes, storage or treatment capacity. | Sewage escapes because of a blockage, broken pipe, inflow and infiltration, pump or power failure, design or maintenance problem, vandalism or another operational failure. Wet weather can contribute, but it is not required. |
| Discharge | Combined stormwater and untreated or partly treated wastewater may leave through a permitted relief outfall. | Untreated or partly treated sewage leaves the sanitary collection system. |
| Possible destinations | Nearby streams, rivers, lakes, estuaries or other receiving waters. | Waterways, streets, yards, parks, basements or other buildings, depending on where the failure occurs. |
| What the label tells you | The event is associated with a combined system and wet-weather capacity limits; it does not mean the discharge is safe or fully treated. | Sewage escaped a sanitary sewer; the label does not identify the cause or the current exposure risk without the incident details. |
EPA describes the combined-sewer distinction in its Combined Sewer Overflow Basics and its CSO program overview. EPA’s sanitary-sewer terminology appears in its SSO overview. These are U.S. terms and regulatory categories; utilities in other countries may use “storm overflow,” “sewer overflow” or “sewage spill” differently.

What is a combined sewer overflow?
How a combined system works in dry weather
A combined sewer was engineered to carry household and commercial wastewater plus runoff from streets, roofs and other surfaces in one network. During ordinary dry-weather conditions, the flow is conveyed to a wastewater treatment plant. The plant and downstream pipes are sized for expected flows, not for every extreme rainfall or snowmelt event.

Why rain or snowmelt causes an overflow
During a storm, runoff enters the same pipe as sewage. If the combined flow exceeds the collection system’s capacity, available storage or the treatment plant’s ability to accept it, the system relieves pressure through an overflow structure. The discharge can contain stormwater mixed with untreated or partly treated wastewater, along with debris and other pollutants.
This relief behavior is a consequence of older combined-system engineering. Calling it “designed to overflow” describes the function of the relief structure, not the quality of the water. EPA says CSOs remain a water-pollution and public-health concern and may contain bacteria, debris and hazardous substances. Its CSO basics page states: “CSOs are a major water pollution and public health concern for approximately 700 communities in the United States.” The page does not date that figure, so it should be read as EPA’s approximately 700-community statement, not as a dated 2026 census.
Where CSO water goes
Relief outfalls commonly discharge to nearby streams, rivers, lakes, estuaries or other receiving waters. Depending on the event, potential effects include bacterial contamination, visible debris or oil slicks, reduced dissolved oxygen, algae growth, beach closures and shellfish-bed closures. These are possible impacts identified by EPA, not outcomes that occur after every overflow.

What is a sanitary sewer overflow?
SSO means sewage escaped a sanitary sewer
An SSO occurs when wastewater leaves a sanitary collection system somewhere other than its intended route to treatment. The release can happen at a manhole, force main, pump station, broken line or other point in the network. It may reach a waterbody, spread across a street or yard, enter a basement, or remain within the system’s surrounding infrastructure.

Common causes
- Blockages: Fats, oils and grease, wipes and other inappropriate materials can restrict flow. Roots can enter through pipe defects and create another obstruction.
- Broken or defective pipes: Cracks, collapses and failed joints can release sewage or allow unwanted water into the line.
- Inflow and infiltration: Rainwater or groundwater can enter through defective pipes, manholes, illegal connections, sump pumps or roof leaders. The extra volume can overwhelm a sanitary system even though stormwater is supposed to be separate.
- Pump, power and equipment failures: A failed pump, electrical outage, inadequate backup or other mechanical problem can stop sewage from moving.
- Design, capacity or maintenance problems: An undersized sewer, poor maintenance or an operational error can cause an overflow without extraordinary rainfall.
- Vandalism and other disruptions: EPA lists vandalism and additional system failures among possible causes.
Because these causes are varied, an SSO is not synonymous with a rainstorm. Wet weather can be the trigger through inflow and infiltration, but a clog, break or mechanical failure can produce an SSO on a dry day.
Is a storm overflow raw sewage?
It can contain raw sewage, but the phrase “storm overflow” does not identify the exact mixture or treatment level. A CSO may contain stormwater combined with untreated or partly treated wastewater. A separate-system SSO may also release untreated or partly treated sewage. The only reliable way to know the conditions at a particular outfall is to read the operator’s dated notice and any public-health or water-quality advisory.
Neither label is a safety rating. A CSO is not harmless because stormwater is part of the flow, and an SSO is not necessarily caused by rain. EPA identifies possible exposure to bacteria, viruses, intestinal parasites and other contaminants when sewer overflows reach water or surfaces. The concentration, route, duration and location of a specific event determine actual risk.
Health and environmental concerns
How people can be exposed
Exposure can occur when contaminated water or sewage contacts skin, is swallowed during recreation, reaches an open wound, or contaminates surfaces. EPA’s sewer-overflow event materials identify possible locations including basements, parks, recreational streams, beaches, streets and backyards. Children, swimmers, anglers, people cleaning a spill and anyone entering a visibly affected area should treat an active advisory as a reason to stay away.

Do not infer that a location is safe or unsafe from the word “overflow” alone. Check the responsible utility, local health department or water-quality agency for the event’s affected area, time period, cleanup status and current advisory. If an authority has posted a closure or “avoid contact” notice, follow it until the notice is lifted.
Water-quality effects
CSOs can carry pathogens, suspended solids, nutrients, toxic substances, debris and oil into receiving waters. EPA lists potential consequences such as impaired recreation, beach or shellfish closures, algae growth, lower oxygen and visible pollution. SSOs can create similar effects when sewage reaches a waterway, and can additionally contaminate land or buildings when the release occurs on streets, in yards or in basements.
These are potential impacts, not a prediction that every event produces every effect. Event-specific sampling, duration, volume, rainfall and the receiving water determine what actually happened.

Public notices often use plain language rather than the strict EPA category. “Sewage spill,” “sewer overflow” or “storm overflow” may appear in a utility alert even when the underlying system type is not obvious. For a real incident, preserve the operator’s exact classification and read the notice for these details:

- Operator and jurisdiction: Identify the utility or regulator that issued the notice.
- Date and time: Note when the release began, ended or was discovered. A continuing advisory is different from a closed event.
- Location: Record the outfall, street, facility, stream, river, lake, beach or other affected area.
- Cause or suspected cause: Look for rainfall, snowmelt, blockage, break, pump failure, power loss, inflow or infiltration.
- Discharge description: Check whether the notice says untreated, partially treated, combined or sanitary flow.
- Public-health direction: Follow instructions about avoiding contact, closures, recreational use, shellfish harvest and cleanup.
- Current status: Confirm whether the advisory remains active and whether the authority has posted follow-up sampling or clearance information.
Rain alone is not enough to classify an event. A rainy-day release from a combined sewer is consistent with a CSO; a rainy-day release from a separate sanitary sewer may be an SSO caused by inflow and infiltration. A dry-weather release can still be an SSO caused by a blockage or equipment failure.

NPDES permitting
CSOs are regulated through the federal National Pollutant Discharge Elimination System (NPDES) permitting program. EPA’s CSO management material describes reducing discharges and providing timely public notification as program goals.
EPA says an SSO that reaches waters of the United States is a point-source discharge and is prohibited unless authorized by an NPDES permit. An event that does not reach those waters can still indicate improper operation or maintenance and may violate permit conditions. The exact reporting, notification and corrective-action requirements depend on the permit and jurisdiction; there is no single federal deadline that applies identically to every incident.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsWhy local rules matter
Federal terminology explains the categories, but a local utility’s permit and emergency-notification process control what residents are told and when. A notice may identify a receiving water, post a recreational closure, provide a hotline or direct residents to a real-time map. Use that local information for decisions about a specific site rather than applying a national generalization.

How common are sanitary sewer overflows?
EPA’s SSO Frequent Questions page estimates at least 23,000 to 75,000 SSOs per year in the United States, excluding sewage backups into buildings. EPA presents this as an estimate, not a complete live count. The retrieved current page does not state the estimate’s original publication year, so it should not be presented as a verified 2026 rate.
The current EPA materials reviewed here do not provide a comparable, current national annual CSO event count or volume. Historic CSO totals may use different dates, definitions and counting methods; do not treat them as current without checking the underlying source.
Practical examples
Example 1: Overflow after a cloudburst in an older city
A city’s older combined sewer carries household wastewater and storm runoff in one pipe. A heavy storm fills the system faster than the treatment plant can accept it, and a relief outfall discharges to a river. That is a CSO. The flow may include stormwater and untreated or partly treated sewage, so a river-contact advisory may follow.

Example 2: Grease blockage on a dry day
In a separate sanitary sewer, accumulated grease blocks a line. Wastewater backs up through a manhole onto a street even though there is no rain. That is an SSO caused by a blockage.
Example 3: Rain enters a defective sanitary system
During prolonged rain, groundwater enters cracked sanitary pipes and an improperly connected roof leader sends additional runoff into the network. The sanitary system exceeds capacity and sewage escapes. This is an SSO, not a CSO, because the sewer is not a combined system.
Example 4: Pump-station power failure
A separate-system pump station loses power and its backup fails. Sewage leaves the collection system and reaches a nearby stream. The event is an SSO even if weather is dry.
What residents should do during a reported overflow
- Read the utility or public-health notice for the exact affected area and advisory period.
- Avoid contact with sewage, visibly contaminated water and surfaces within the posted area.
- Keep children and pets away from affected water, streets, yards and parks until authorities say they are clear.
- Do not assume clear-looking water is safe; pathogens may not be visible.
- Use the utility or regulator’s contact channel to report a new discharge, strong sewage odor, blocked drain or missing warning sign.
- For a building backup or land contamination, follow local health and cleanup instructions rather than improvising a disposal method.
EPA provides national program context, but only the responsible local authority can confirm whether a particular location remains under advisory.
