In the 72-week U.S. SURMOUNT-5 head-to-head trial, tirzepatide produced greater average weight loss than semaglutide. The result does not prove that tirzepatide is the right medicine for every person, nor that it is safer or easier to tolerate. Both are once-weekly injections that require gradual dose increases, and the best choice depends on the labeled indication, medical history, other medicines, response, side effects, and access through a prescriber.
What the head-to-head trial actually found
SURMOUNT-5 was a phase 3b, randomized, open-label trial of 751 adults in the United States and Puerto Rico. Participants had obesity, or overweight with a weight-related condition, and did not have diabetes. They were assigned 1:1 to semaglutide or tirzepatide and followed for 72 weeks while taking the maximum dose they could tolerate.
The study compared semaglutide doses of 1.7 mg or 2.4 mg with tirzepatide doses of 10 mg or 15 mg. Most participants reached the higher dose in their assigned group. The results release from Eli Lilly and Company, published in May 2025 and reporting simultaneous publication in The New England Journal of Medicine, reported the following mean reductions from baseline:
| Measure at week 72 | Tirzepatide | Semaglutide |
|---|---|---|
| Average reduction in baseline body weight | 20.2% | 13.7% |
| Difference between group averages | 6.5 percentage points, favoring tirzepatide | |
| Population | Adults with obesity or overweight plus a weight-related condition; no diabetes | |
| Study design | Randomized, 1:1, open-label, 72 weeks, maximum tolerated doses | |
For a purely illustrative 100-kg starting weight, those percentages correspond to average losses of about 20.2 kg and 13.7 kg. That arithmetic is not an individual forecast. Starting weight, dose reached, treatment duration, adherence, health conditions, and biology all affect an individual result.

Why the result is meaningful—and what it does not establish
This is the most direct comparison available between the two medicines because the same trial used the same follow-up period and randomized participants to each treatment. It answers the question “Which produced more average weight loss in this study?” with tirzepatide.
It does not establish that tirzepatide is universally better. The participants did not have diabetes, the study was open-label, and the trial was not powered to compare safety or tolerability. A difference in people stopping treatment, or in reported adverse events, should not be treated as proof that one medicine is safer. The comparison also used maximum tolerated doses rather than a guarantee that every patient can reach or remain on those doses.
“Maximum tolerated” is important when interpreting the headline number. It means the dose was increased according to the study protocol only when a participant could tolerate the current step. It does not mean every person received the same dose, nor that the highest listed dose is required for an adequate response. The result is a group average under a defined treatment regimen, not a promise tied to a particular pen strength.

How the weekly doses differ
Both medicines are injected under the skin once a week and are increased gradually. The schedules below summarize the U.S. weight-management labels; they are not personal prescriptions.
| Wegovy (semaglutide) | Zepbound (tirzepatide) | |
|---|---|---|
| Starting dose | 0.25 mg once weekly for 4 weeks | 2.5 mg once weekly for 4 weeks; this is an initiation dose, not a maintenance dose |
| Escalation | Increase at 4-week intervals; delay an increase if the current dose is not tolerated | Increase by 2.5 mg after at least 4 weeks at the current dose |
| Adult maintenance options for weight management | 1.7 mg or the recommended 2.4 mg once weekly | 5 mg, 10 mg, or 15 mg once weekly |
| Administration | Subcutaneous injection on the same day each week, with or without meals | Subcutaneous injection once weekly; follow the instructions for the specific presentation |
What gradual escalation is for
The low starting doses are used to begin treatment, not to produce the final weight-loss effect. Each label uses stepwise increases so the body can adjust and so a prescriber can hold a dose longer when gastrointestinal reactions are troublesome. The maintenance dose is therefore individualized: a person may remain at a lower approved dose if that is the best balance of response and tolerability.
The numerical strengths should not be compared directly across products. A 2.5-mg tirzepatide initiation dose is not equivalent to a 2.5-mg semaglutide dose, and the products use different active ingredients and schedules. Compare only the label-specific steps for the exact product prescribed.
Do not switch between products, combine them, or change the interval or dose on your own. The labels advise against coadministration with other GLP-1 receptor agonists. Confirm the exact product, pen or vial presentation, concentration, injection instructions, and schedule with the prescribing clinician and pharmacist.
Keeping the weekly schedule workable
- Choose a weekly day that can be remembered and keep the schedule consistent.
- Use the administration instructions supplied with the exact device; presentations can have different handling steps.
- Keep a written record of the dose, date, symptoms, and any delayed escalation so the next appointment is based on actual treatment experience.
- Dispose of used needles or injection devices in a sharps container when required by the product instructions and follow local disposal rules. Container requirements and collection options vary by location.
A sharps container is a practical administration accessory, not a substitute for medication counseling. The device instructions and local waste authority determine whether a needle, pen, or other component belongs in a household sharps program, pharmacy collection site, or another approved service.

Side effects: where the labels overlap
Gastrointestinal reactions are the main area of overlap. The labels and trial report describe nausea, diarrhea, vomiting, constipation, and abdominal symptoms with both medicines. In SURMOUNT-5, adverse events were described as primarily gastrointestinal and generally mild to moderate, but the trial was not designed to prove a tolerability winner.
| Issue | What is established | How to interpret it |
|---|---|---|
| Nausea, diarrhea, vomiting, constipation, abdominal symptoms | Reported with both medicines; gastrointestinal events predominated in SURMOUNT-5 | Risk and severity vary with dose, medical history, other medicines, and individual response |
| Other effects listed for Zepbound | Injection-site reactions, fatigue, hypersensitivity reactions, eructation, hair loss, dyspepsia, and gastroesophageal reflux disease are among the listed adverse effects | A listed event is not a prediction that it will occur or that it will be more common than with the other product |
| Serious or clinically important warnings | Both labels discuss pancreatitis, gallbladder disease, kidney injury related to volume depletion, hypersensitivity, and hypoglycemia in relevant contexts | Personal risk assessment requires a clinician who knows the patient’s conditions and medicines |
The presence of a symptom in a label does not show that the two products have identical rates. The available head-to-head report describes overlapping gastrointestinal effects, but it was not designed or sized to establish that one medicine is safer. Dose, escalation speed, dehydration, concurrent medicines, and underlying disease can all change the practical experience.
Shared thyroid warning and contraindication
Both labels carry a boxed warning about thyroid C-cell tumors based on findings in rodents. Whether that finding applies to humans is unknown. Both labels list a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2) among contraindications. Anyone with such a history should tell the prescriber before treatment is considered.
Warnings that are emphasized differently
Wegovy’s labeling includes a warning about diabetic retinopathy complications in people with type 2 diabetes. Zepbound’s labeling addresses the risk of aspiration during anesthesia or deep sedation. These differences do not mean that the other medicine has no relevant risk; they show why the complete current label and the patient’s medical history matter.
When symptoms need prompt medical attention
Contact the prescribing team promptly for persistent vomiting or diarrhea, inability to keep fluids down, symptoms suggesting dehydration, severe or continuing abdominal pain, signs of an allergic reaction, or new visual changes. Tell an anesthesia or procedural team that you use the medicine before anesthesia or deep sedation. Emergency symptoms such as trouble breathing, facial or throat swelling, fainting, or severe sudden pain require urgent care.
Approved uses are not identical
The following distinctions refer to U.S. labeling and can change as regulators update product information. An indication is not automatically transferable from one product to the other.
| Product | Relevant labeled uses described in the current U.S. materials |
|---|---|
| Wegovy (semaglutide) | Chronic weight management; cardiovascular-risk reduction for certain adults with established cardiovascular disease |
| Zepbound (tirzepatide) | Chronic weight management; treatment of moderate-to-severe obstructive sleep apnea in adults with obesity |
The cardiovascular and sleep-apnea indications involve specific eligibility criteria. A person should not select a product solely because a headline trial showed a larger average percentage loss. The prescriber needs to match the medicine to the diagnosis being treated and the product’s approved use in the relevant country.

| Trial and regimen | Reported average weight change | Follow-up |
|---|---|---|
| STEP 1, semaglutide 2.4 mg | −14.9% | Week 68 |
| SURMOUNT-1, tirzepatide 5 mg | −15.0% | Week 72 |
| SURMOUNT-1, tirzepatide 10 mg | −19.5% | Week 72 |
| SURMOUNT-1, tirzepatide 15 mg | −20.9% | Week 72 |
These studies had different designs, participants, comparators, durations, and dosing circumstances. Their percentages should not be read as a direct ranking. SURMOUNT-5 is the appropriate source for the direct 72-week comparison between the two medicines in adults without diabetes.
The earlier figures also illustrate why dose and time matter. STEP 1 reports semaglutide at week 68, while SURMOUNT-1 reports three tirzepatide strengths at week 72. Even when two percentages look similar, the underlying populations and protocols may differ. A clinician should use the head-to-head result for the direct comparison and the earlier studies only as supporting context.
Which medicine may make sense for a particular patient?
There is no universal winner once the average trial result is placed alongside indications, safety, and practical treatment constraints. A clinician can use the following decision sequence.
Free tools Windows power users keep installed
One-click scans. No signup required.
Start with the condition being treated
If the primary reason for treatment is chronic weight management, both products have a relevant U.S. indication. If established cardiovascular disease or obesity-related obstructive sleep apnea is part of the treatment decision, the product whose label covers that use may be more appropriate, provided the patient meets the detailed criteria.
Consider the average-loss evidence without treating it as a promise
Tirzepatide had the larger mean reduction in SURMOUNT-5. That can reasonably be part of a shared decision when a patient can safely use either medicine and places a high value on the possibility of greater average loss. It cannot predict which product will produce the larger result for one individual.
Review contraindications and risk factors before discussing dose
The clinician should review MTC or MEN 2 history, pancreatitis or gallbladder concerns, kidney vulnerability during dehydration, prior hypersensitivity, diabetes and eye disease, medicines that can contribute to hypoglycemia, and any upcoming anesthesia or deep sedation. Pregnancy plans, other medical conditions, and all prescription, over-the-counter, and injectable products also belong in that review.
Use tolerability as an active selection criterion
Because nausea and other gastrointestinal effects are common with both medicines, the practical choice may depend on which dose a person can continue while maintaining adequate nutrition and hydration. A slower escalation or a lower maintenance dose may be appropriate when the label and prescriber support it. A dose that cannot be tolerated is not useful merely because it is the trial’s highest dose.

Ask specifically which product and presentation the prescription names. Device instructions, concentration, and disposal requirements can differ. No particular merchant, price, discount, or insurance outcome can be assumed from the product name alone.
A safe start-to-follow-up checklist
- Confirm the indication. Establish whether the goal is chronic weight management or another labeled use, and verify that the patient meets the product-specific criteria.
- Review medical history and medicines. Discuss MTC or MEN 2, pancreatitis, gallbladder disease, kidney problems, diabetes and retinopathy, allergies, hypoglycemia risk, and planned anesthesia or sedation.
- Choose one prescribed product. Confirm the exact name, presentation, concentration, injection technique, weekly day, and starting dose. Do not use two GLP-1 receptor agonists together.
- Escalate only as directed. Keep each step for the minimum label interval and delay an increase when the prescriber says the current dose is not tolerated.
- Track response and adverse effects. Record weight trends, gastrointestinal symptoms, hydration problems, glucose-related symptoms when relevant, and any new warning signs.
- Reassess maintenance. The maintenance dose should reflect response and tolerability, not an assumption that the highest listed dose is mandatory.
- Plan disposal and procedures. Use the required sharps container and tell every anesthesia or procedural team about treatment in advance.
Common comparison mistakes
- Comparing unlike trials: STEP 1 and SURMOUNT-1 are useful context, but their percentages cannot replace SURMOUNT-5’s randomized head-to-head result.
- Turning an average into a guarantee: A 20.2% or 13.7% group mean does not predict an individual’s result.
- Calling tirzepatide safer: SURMOUNT-5 was not powered for safety or tolerability comparisons.
- Ignoring dose exposure: The direct trial compared maximum tolerated doses, and most participants reached the higher dose in each group.
- Assuming indications are interchangeable: Wegovy’s cardiovascular-risk indication and Zepbound’s obstructive-sleep-apnea indication have different eligibility requirements.
- Self-adjusting treatment: Escalating early, switching products, or combining injections can increase risk and conflicts with label instructions.
- Comparing milligrams as if they were equivalent: Semaglutide and tirzepatide have different active ingredients, so the numerical strengths cannot be lined up dose-for-dose.
- Treating a label list as a personal forecast: An adverse event listed for one product does not establish that it will happen, or happen more often, in a specific patient.
Frequently asked questions
Which works better for weight loss, semaglutide or tirzepatide?
In the 72-week SURMOUNT-5 trial, tirzepatide produced the larger mean reduction: 20.2% versus 13.7% with semaglutide. That finding applies to the studied adults without diabetes receiving maximum tolerated doses and does not guarantee a larger result for every individual.
How much more weight did people lose with tirzepatide?
The difference between the group averages was 6.5 percentage points at week 72. For an illustrative 100-kg starting weight, that is about 6.5 kg more on average, but the calculation is not an individual prediction.

Pick the symptom - the matching free tool is one click away.
Are tirzepatide and semaglutide equally safe?
The available head-to-head trial was not powered to compare safety or tolerability. Both commonly cause gastrointestinal effects and both labels contain important warnings. Individual risk depends on medical history, dose, other medicines, and response.
What are the starting doses?
U.S. Wegovy labeling starts at 0.25 mg once weekly for four weeks. Zepbound starts at 2.5 mg once weekly for four weeks; that Zepbound dose is an initiation dose, not a maintenance dose.
What are the maintenance doses?
For adult weight management, Wegovy lists 1.7 mg or a recommended 2.4 mg once weekly. Zepbound lists 5 mg, 10 mg, or 15 mg once weekly. The appropriate maintenance dose depends on response and tolerability and must be selected with the prescriber.
Can I take semaglutide and tirzepatide together?
No one should combine or switch these products independently. The labels advise against coadministration with other GLP-1 receptor agonists. Use one prescribed product according to its instructions unless a clinician directs a supervised change.

How much do these medicines cost?
There is no reliable universal price, insurance, or availability comparison. Cost and access depend on country, payer, plan rules, authorization, pharmacy supply, and the prescribed presentation. A prescriber or pharmacist can check a specific patient’s situation.
Bottom line for the decision
Tirzepatide produced the greater average weight reduction in the 72-week SURMOUNT-5 comparison: 20.2% versus 13.7% with semaglutide under the study’s maximum-tolerated-dose regimen. That is the strongest direct efficacy answer, but it is not a universal treatment verdict. Both medicines require weekly administration and gradual escalation; both can cause gastrointestinal effects and carry important warnings. The appropriate choice is the product whose indication, contraindications, dose schedule, tolerability, monitoring plan, and access fit the individual patient after a clinician reviews the current label.
