The safer alternative is not one product; it is a tiered operating model. Keep deterministic automation for repeatable, rules-based work. Use AI copilots for retrieval, summarization, drafting and recommendations while a person remains accountable for consequential decisions. Allow agentic execution only for narrowly bounded tasks, with a separate authorization layer, least-privilege identity, policy checks at every tool call, audit logs and approval gates for sensitive or irreversible actions.
This design limits what an AI system can change if it misunderstands a request, follows malicious instructions in retrieved content or loses context. It also gives you a practical way to increase autonomy gradually instead of granting broad access on the first deployment.
Why fully autonomous agents create a different risk problem
Conventional automation usually follows an explicit sequence: receive a known input, apply predefined rules, call approved systems and record the result. An agent can instead interpret an open-ended goal, choose tools, create intermediate plans and continue operating with limited supervision. That flexibility is useful, but it expands the number and variety of actions that can occur before a person notices a mistake.
The National Institute of Standards and Technology (NIST) National Cybersecurity Center of Excellence (NCCoE) describes the concern this way: “With the advancement of software and SI agents—systems that have the capability for autonomous decision-making and taking action to operate with limited human supervision to achieve complex goals—the scale and range of actions taken by these systems has the potential to increase exponentially.”

NCCoE also warns that, without strong identity, authorization and governance, organizations face data leaks, compliance failures, prompt injection and unpredictable autonomous behavior. Those risks are not solved by writing a stricter system prompt. A prompt is an instruction to a model, not a security boundary. The enforcement point must be outside the model, in the APIs, gateways, tool proxies and policy services that control access.
The four operating models
Most business workflows fit one of four patterns. Select the least autonomous pattern that still meets the business objective, then add autonomy only after controls and evidence are in place.
| Operating model | What the system does | Best fit | Primary safeguard |
|---|---|---|---|
| Deterministic workflow automation | Executes explicit rules, branches, retries and approvals. | Stable inputs, known outcomes and repeatable transactions. | Versioned logic, fixed integrations, validation and complete logs. |
| AI copilot or assistant | Retrieves information, summarizes, classifies, drafts or recommends. | Knowledge work where a person can review the result before committing it. | Human decision-maker; read-oriented permissions by default. |
| Human-in-the-loop agent orchestration | Plans and prepares multi-step work, then pauses at defined checkpoints. | Processes that need flexible planning but contain high-impact steps. | Approval gates, step-up authentication and policy review at checkpoints. |
| Policy-enforced bounded agent | Acts autonomously inside explicit task, time, resource and data limits. | Low-risk, reversible work with well-defined boundaries. | External policy engine, scoped identity, tool allowlists, quotas and automatic stop conditions. |
Deterministic workflow automation
Use a conventional business-process, robotic-process or integration workflow when the rules and outcomes are known. Examples include moving a file after a successful malware scan, reconciling records with exact matching rules or routing a form according to a fixed approval matrix. Deterministic automation is easier to test, explain and recover because each branch is explicit.
Include input validation, idempotent operations, bounded retries, an exception queue and an immutable event trail. If an exception requires interpretation, hand that item to a copilot or a person rather than turning the entire process into an unrestricted agent.
AI copilot or assistant
A copilot is the safest way to add language understanding without delegating authority. It can search an approved knowledge base, summarize a case, classify an incoming request, draft an email or recommend a next step. The user remains the decision-maker and performs the consequential commit in the ordinary business application.
Keep the initial permission set read-only wherever possible. Show the source records used for a recommendation, mark generated text clearly and make it easy to edit or reject a draft. A copilot should not silently send a message, change a customer record or approve a payment merely because the user asked for a draft.
Human-in-the-loop agent orchestration
Use this model when a task genuinely needs planning across several systems but contains identifiable points of risk. The agent may gather records, prepare a proposed change and calculate the expected effect. It then pauses before actions such as changing access, issuing a refund, sending an external communication or publishing data.

An approval is meaningful only when the reviewer can see the proposed action, affected objects, data used, policy checks, expected side effects and the identity under which the action will run. Require re-authentication or another step-up factor when the approval elevates privilege or crosses a sensitive boundary.
Policy-enforced bounded agents
A bounded agent can operate without a person at every step, but only inside limits enforced by systems that do not depend on the model behaving correctly. Define the task, allowed tools, maximum duration, record types, transaction value, number of objects and data classifications. Enforce those limits in a gateway or policy decision point, and terminate the run when a limit is reached or context changes materially.
Examples include categorizing incoming tickets into approved queues, preparing a reversible calendar change or applying a low-value configuration update to a test environment. The same design is not appropriate for unrestricted production administration, money movement or disclosure of regulated information.
Choose autonomy by risk, not by novelty
Evaluate a workflow across several axes before selecting a model. A low-risk action that is easy to undo can tolerate more autonomy than an action that changes permissions, sends an external statement, moves money or shares regulated data.
| Question | Lower-risk answer | Higher-risk answer | Design implication |
|---|---|---|---|
| How deterministic is the task? | Inputs, rules and outcomes are known. | Goal is open-ended or ambiguous. | Prefer deterministic automation or add planning checkpoints. |
| What is the blast radius? | One reversible record or sandbox object. | Many records, production systems or external parties. | Reduce scope, add quotas and require approval. |
| What permissions are needed? | Read-only or narrowly scoped write. | Administrative, financial or cross-tenant access. | Use separate credentials, short-lived grants and step-up authentication. |
| Can the action be reversed? | Immediate, tested rollback. | Irreversible deletion, disclosure or public communication. | Require a human gate and a tested recovery plan. |
| What data is involved? | Public or internal, non-sensitive data. | Personal, financial, health, legal or confidential data. | Apply classification-aware retrieval and policy checks. |
| How good is human review? | Reviewer has time, context and a clear diff. | High volume, fatigue or opaque evidence. | Reduce automation, improve evidence and sample outcomes. |
| How complex is recovery? | Single-step retry or rollback. | Cascading changes across services. | Use orchestration checkpoints, transactions and kill switches. |
Controls that prevent unauthorized changes
Bind every action to a real identity and session
Every model-initiated call should carry the initiating user’s identity, session, tenant and purpose. Do not let an agent use a shared administrator account. Separate the identity of the user who requested work, the service that orchestrates it and the tool that executes it, while preserving an auditable link among them.
Use short-lived tokens and task-scoped grants. A token issued for a ticket-classification run should not remain valid for later administrative work. Re-check the session and authorization when the run resumes after a pause, when a new agent joins or when the requested scope expands.
Enforce authorization in backend policy points
Put allow and deny decisions in an API gateway, service mesh, tool proxy or policy decision and enforcement point. The policy service should evaluate the user, agent identity, resource, action, purpose, data classification, environment and current context. A model’s statement that it is “authorized” has no security value unless the backend independently verifies it.

Use deny-by-default rules and grant only the tools and fields required for the current task. Re-evaluate policy on every tool invocation, not only when a run begins. A previously safe plan can become unsafe after untrusted content is ingested, the target record changes or the workflow crosses from test to production.
Recommended Free Tools
Use separate read and write capabilities
Give retrieval tools read credentials and mutation tools separate write credentials. A summarization step should not be able to call the customer-update endpoint. Where a write is necessary, expose a narrow operation such as approve_refund with server-side limits rather than a generic database or shell interface.
Define per-tool allowlists, strict input and output schemas, maximum field lengths, permitted record types and rate limits. Reject malformed or extra parameters before they reach the downstream system. Validate the resulting state after a write and stop if it differs from the expected invariant.
Constrain data and context
Apply data-classification rules to retrieval, memory and tool output. An agent handling an internal support ticket should not automatically receive an entire customer profile or unrelated files. Redact fields that are unnecessary for the task, prevent cross-tenant retrieval and set retention periods for prompts, tool results and intermediate plans.
Treat documents, web pages, tickets and messages as untrusted content. They may contain prompt-injection text that tries to change the agent’s goal or obtain secrets. Keep instructions and data in separate channels, label untrusted content, filter tool-call proposals and require policy approval before an instruction extracted from content can cause a write.
Log decisions, not just final outcomes
Record the request, initiating user, agent version, model version, retrieved sources, tool calls, policy decisions, approvals, credentials or scopes used, timestamps, outputs and final state. Protect logs from alteration and restrict who can view sensitive payloads. A useful audit trail lets an investigator answer what happened, why it was allowed, which identity acted and how to undo it.
When a human must approve
Require a human approval gate when an action is irreversible, externally visible, legally significant, financially material, privilege-changing or likely to affect many records. Typical examples include deleting data, changing access rights, sending a customer or regulator communication, approving a payment or refund, publishing content, deploying code and exporting regulated information.

Approval is also appropriate when the agent encounters ambiguity, conflicting records, a policy exception, a new data source or a request outside its normal task scope. For low-risk work, approval can be replaced by post-action sampling if the action is reversible, the sample rate is justified and an automatic stop threshold is defined.
Design an approval that can be reviewed
- Show the proposed operation. Identify the exact records, fields, recipients, amount, environment and tool that will be used.
- Show evidence and uncertainty. Provide the source records, policy results, assumptions, confidence indicators and unresolved conflicts. Do not present a generated explanation as proof.
- Show the consequences. State what will change, what notifications will be sent, whether the action is reversible and what rollback will do.
- Authenticate the approver. Use step-up authentication when the decision grants privilege, exposes sensitive data or commits a high-impact transaction.
- Bind approval to the exact request. Expire it after a short period or whenever the target, scope, data or policy context changes.
- Record the decision. Store who approved or rejected, when, under which policy version and with which proposed payload.
Avoid rubber-stamp workflows
Do not make the reviewer approve a vague sentence such as “continue with the plan.” Use a structured diff and require an explicit choice. Route high-volume or repetitive approvals to deterministic rules only when the risk assessment supports it. Monitor approval rates, reversal rates, near misses and time spent reviewing; a near-perfect approval rate may indicate that people are not reading the evidence.

Adopt the following sequence for a new workflow or for reducing risk in an existing autonomous deployment.
- Define the business outcome and prohibited actions. Write the task in operational terms, list systems and records in scope, and state what the agent must never do.
- Classify the data and impact. Identify personal, financial, health, legal, confidential and public data. Rate the effect of an incorrect, delayed, duplicated or disclosed action.
- Choose the least autonomous model. Start with deterministic automation for fixed rules, a copilot for recommendations, or human-in-the-loop orchestration for multi-step planning. Use a bounded agent only when the controls and rollback are demonstrable.
- Create a task identity. Bind the run to the initiating user, tenant, session and purpose. Issue short-lived, least-privilege credentials and keep read and write access separate.
- Place policy enforcement outside the model. Route every tool call through a gateway or proxy that checks identity, resource, action, data class, environment, quotas and current context.
- Define schemas and allowlists. Expose only named tools and approved parameters. Reject additional fields, unknown resources, excessive quantities and attempts to invoke unlisted tools.
- Insert approval and step-up gates. Pause before irreversible, external, privileged, financial or regulated actions. Require fresh authentication when the risk level rises.
- Add observability and recovery. Log plans, evidence, policy decisions and outcomes. Implement idempotency keys, transaction boundaries, rollback or compensating actions, timeouts and a kill switch.
- Test adversarially. Try prompt injection, poisoned memory, malformed tool arguments, replayed approvals, token theft, cross-tenant queries, goal changes and partial service failures. Confirm that the backend denies each prohibited path.
- Roll out gradually. Begin in a sandbox or shadow mode, then permit read-only activity, followed by narrowly scoped writes. Set quantitative limits and review incident and near-miss data before expanding scope.
- Reassess continuously. Re-evaluate policies when models, tools, data sources, agents, business rules or environments change. Revoke unused permissions and retire workflows that no longer have a clear owner.
Practical patterns for common workflows
Invoice processing
Use deterministic extraction and matching for supplier, purchase-order and amount fields. A copilot can explain discrepancies and draft a query to the supplier. Keep payment release behind the existing approval matrix and a separate payment service; do not give the language model direct authority to move funds.
Customer support
Let a copilot retrieve approved articles, summarize the case and draft a response. A bounded agent may classify and route tickets when the queue, language and priority are constrained. Require a person to approve refunds, account closure, legal statements or messages that disclose sensitive information.
Access requests
Use deterministic rules to check role, manager and separation-of-duties requirements. An agent can collect missing evidence and prepare a recommendation, but an identity governance system should make the final grant. Require step-up authentication for privileged roles and automatically expire temporary access.

Software and infrastructure changes
Keep production deployment behind version control, automated tests, change records and an approved pipeline. An agent can propose a patch, run tests in an isolated environment and prepare a change plan. It should not receive a standing production administrator credential or bypass required review.
Failure modes and recovery controls
- Goal hijack: Untrusted content changes the task. Separate data from instructions, constrain tools and require policy re-evaluation before any mutation.
- Tool misuse: The agent selects a valid tool for an invalid purpose. Authorize the specific resource and operation, not merely the tool name.
- Identity and privilege abuse: A shared or long-lived credential enables excessive access. Use per-user attribution, short-lived tokens and separate service identities.
- Supply-chain compromise: A plugin, model, connector or dependency behaves maliciously. Pin and review versions, restrict outbound access, scan artifacts and maintain an inventory of agent components.
- Unexpected code execution: Generated code reaches a shell or production runtime. Isolate execution, use allowlisted commands, remove secrets from the environment and require promotion through a controlled pipeline.
- Memory or context poisoning: False information persists between runs. Label provenance, expire memory, require source validation and provide a deletion and correction process.
- Insecure inter-agent communication: One agent passes unchecked instructions to another. Authenticate messages, validate schemas and apply the receiving agent’s policy independently.
- Cascading failure: A bad decision is repeated across systems. Set rate and quantity limits, use circuit breakers, detect abnormal patterns and stop the run on invariant violations.
- Human-agent trust exploitation: A polished explanation causes a reviewer to approve unsafe work. Show evidence and diffs, surface uncertainty and train reviewers to challenge generated rationale.
- Rogue or orphaned agent: A workflow continues after its owner or purpose disappears. Assign an owner, expiration date and kill switch; revoke credentials when the workflow is disabled.
Governance frameworks and standards
NIST AI Risk Management Framework (AI RMF) 1.0, released January 26, 2023, organizes risk work into Govern, Map, Measure and Manage. Use those functions to assign ownership, document intended use, test controls and track remediation. NIST released the Generative AI Profile (NIST-AI-600-1) on July 26, 2024, and has said the AI RMF is being revised; treat both as living guidance rather than a one-time certification.
NCCoE’s Software and SI Agent Identity and Authorization project is developing implementation-oriented guidance and plans an SP 1800 practice guide. The project reported more than 600 stakeholder responses to its concept paper in 2026. Its focus on machine identity, authorization and accountability maps directly to the controls in this article.

OWASP’s agent governance guidance identifies risks including goal hijack, tool misuse, identity and privilege abuse, agentic supply-chain compromise, unexpected code execution, memory and context poisoning, insecure inter-agent communication, cascading failures, human-agent trust exploitation and rogue agents. Use that list to build threat scenarios and test cases.
ISO/IEC 42001:2023 is a certifiable AI-management-system standard. Pairing it with ISO/IEC 23894 risk guidance can help formalize governance, roles, records and continual improvement. NIST SP 1800-35, finalized June 10, 2025, documents 19 example zero-trust implementations and covers identity governance, identity, credential and access management, microsegmentation and secure access service edge controls. Zero-trust practices provide a useful foundation for constraining agents across distributed enterprise resources.
Trade-offs: safety, speed and operating cost
| Approach | Determinism | Typical latency | Integration effort | Failure recovery | Where cost appears |
|---|---|---|---|---|---|
| Deterministic automation | Highest | Usually lowest | Up-front process mapping and connector work | Predictable retries and rollback | Maintenance when rules or systems change |
| Copilot | Medium for output; high for commit if a person controls it | Interactive | Knowledge, retrieval and user-interface integration | Human correction before commit | Model usage, evaluation and reviewer time |
| Human-in-the-loop agent | Variable planning; controlled execution | Higher at approval points | Orchestration, evidence, approvals and identity binding | Checkpointed recovery and compensating actions | Policy services, audit storage and human review |
| Bounded agent | Lowest during planning, constrained by policy | Potentially low after launch | Gateway, policy engine, quotas, monitoring and kill switch | Depends on transaction design and circuit breakers | Continuous monitoring, testing and incident response |
Do not compare these approaches only by model or infrastructure cost. Include reviewer time, policy engineering, security testing, audit retention, integration maintenance, incident response and the financial effect of an incorrect action. A cheaper autonomous run can be more expensive once a single unauthorized change must be investigated and reversed.
A decision checklist for each workflow
- Is the objective stable enough for explicit rules?
- Can the task remain read-only or draft-only?
- What is the maximum number and value of records one run may affect?
- Which data classifications can the system retrieve, remember or transmit?
- Which actions are irreversible, external, privileged, financial or regulated?
- Where exactly does a person approve, and what evidence will that person see?
- Are authorization and policy checks enforced outside the model on every call?
- Are identities, sessions, tokens and tenants bound to the run?
- Are tools allowlisted and inputs validated against strict schemas?
- Can the action be rolled back, compensated or stopped immediately?
- Are plans, sources, tool calls, approvals and outcomes logged immutably?
- What tests demonstrate resistance to prompt injection, poisoned context, replay and cascading failure?
- Who owns the workflow, reviews its metrics and revokes it when conditions change?
The practical policy
Make deterministic automation the default for known rules. Make copilots the default for language-heavy work where a person commits the result. Use human-in-the-loop orchestration when an agent must plan across systems but the consequences require judgment. Permit autonomous execution only inside externally enforced task, time, data, resource and permission boundaries.
This graduated approach preserves the productivity benefits of AI while ensuring that identity, authorization, reversibility and accountability do not depend on a model’s compliance with its own instructions.
