Some batches of certain sartan blood-pressure medicines were found to contain nitrosamine impurities, but that does not mean every sartan was affected or that taking a recalled medicine means you will develop cancer. The U.S. FDA’s worst-case estimate for one specific valsartan exposure was one additional lifetime cancer case per 8,000 people; later observational studies have not found a consistent increase in overall cancer. If you take valsartan, losartan, or irbesartan, check the exact product and lot with a pharmacist rather than stopping your medicine on your own.
What was found in some sartan medicines?
Sartans are angiotensin II receptor blockers (ARBs), a class of prescription medicines that includes valsartan, losartan, and irbesartan. They are used to treat conditions including high blood pressure and heart failure. In 2018, the U.S. Food and Drug Administration (FDA) learned that some ARB products contained nitrosamine impurities above its safety standards. The FDA identifies NDMA and NDEA as probable human carcinogens and NMBA as a potential human carcinogen.
The FDA says the impurities may have formed during active pharmaceutical ingredient manufacturing when particular chemicals and reaction conditions were present. Reuse of materials such as solvents may also have contributed. A hazardous impurity is not the same as a prediction that a particular patient will get cancer: the risk depends on the impurity, amount, and duration of exposure.

Not every medicine in the sartan class, every product from a manufacturer involved in a recall, or every lot of a recalled product was affected. Combination tablets containing valsartan or losartan could also be implicated; a tablet does not become exempt simply because it also contains another ingredient. What does the FDA’s “1 in 8,000” estimate mean?
In 2018, the FDA estimated that the highest daily dose of valsartan—320 mg—with the highest contamination scenario taken every day for four years could result in one additional lifetime cancer case among 8,000 people. The agency described this as a worst-case scenario, not a measured count of cancers or an estimate that applies equally to every patient or lot. Most batches had much lower impurity levels, and a person’s source of tablets could vary between refills. The estimate is specific to the FDA’s modeled NDMA-in-valsartan scenario; it is not an individual risk calculation. See the FDA’s 2018 update on ARB recalls.
The FDA’s broader nitrosamine guidance says that exposure above acceptable limits over long periods may increase risk. It also says daily exposure at or below acceptable intake limits for 70 years is not expected to increase cancer risk. This is a regulatory framework, not a personal assessment of someone’s past exposure or a guarantee for every circumstance. Nitrosamines also occur at some level in the environment and food, but that fact does not make an impurity in a medicine acceptable. The FDA explains its general approach in Information about nitrosamine impurities in prescription medications.

What have studies reported about cancer?
Studies of people who used valsartan have not established a consistent increase in overall cancer associated with contaminated products. Some have reported associations with particular cancer sites, including liver cancer. Because these are observational studies, they can identify patterns but cannot by themselves prove that the medicine or impurity caused a cancer. Their findings also differ in how exposure was classified and how long participants were followed.

| Study | Population and follow-up | Overall cancer finding | Other reported findings |
|---|---|---|---|
| Denmark, 2018 | Nationwide cohort of 5,150 people; median follow-up 4.6 years. | Adjusted hazard ratio (HR) 1.09 (95% confidence interval [CI] 0.85–1.41); no evidence of a dose-response relationship (P=0.70). The authors found no markedly increased short-term overall cancer risk. | The confidence interval leaves uncertainty, and this follow-up does not settle questions about cancers with long latency. Study report. |
| Germany, 2021 | Insurance-data cohort of 780,871 people who filled valsartan prescriptions during 2012–2017. | No association with overall cancer was reported. | Hepatic (liver) cancer was associated with valsartan exposure (adjusted HR 1.16, 95% CI 1.03–1.31); an observational association does not establish cause. Study record. |
| France, 2022 | Nationwide cohort of 1.4 million valsartan users. | No increase in overall cancer associated with NDMA-contaminated valsartan was reported (adjusted HR 0.99, 95% CI 0.98–1.00). | The study reported associations with liver cancer (adjusted HR 1.12, 95% CI 1.04–1.22) and melanoma. Observational design and exposure classification limit causal conclusions. Study report. |
Taken together, these results do not show a clear overall cancer excess, but they also cannot rule out every possible risk, especially for outcomes that take longer to emerge. The FDA’s modeled worst-case estimate and the cohort studies answer different questions: one estimates risk under a specified exposure scenario, while the others examine observed outcomes in populations.

How can you check whether your medicine was affected?
For a U.S. product, recall status depends on the exact medicine and lot, not just the active ingredient or brand name. Find the manufacturer or labeler, National Drug Code (NDC), lot number, and active ingredients on the packaging or ask your pharmacist to help identify them. The manufacturer may differ from the distributor or labeler. Use the current FDA recall information rather than relying on an old list, because recall details can change; the FDA page also covers combination products.
- Keep taking your prescription unless a healthcare professional tells you otherwise. Do not stop or change a dose on your own.
- Check the packaging. Gather the medicine name, active ingredients, NDC, lot number, and manufacturer or labeler if available.
- Contact your pharmacist. Ask whether that specific product and lot appear in current recall information. The pharmacist can help if the label details are unclear.
- If it is affected or you remain concerned, contact your prescriber or pharmacist about next steps. They can discuss appropriate alternatives or a replacement without leaving your underlying condition untreated.
The FDA’s patient guidance says people taking prescription medicines with potential nitrosamine impurities should not stop treatment on their own and should discuss concerns and options with a healthcare professional: FDA information for patients. U.S. recall listings do not establish recall status in other countries; patients elsewhere should consult their national medicines regulator and pharmacist.
