RT-QuIC and PMCA detect prion seeding activity by amplifying it, not by counting individual infectious particles. Both expose a sample to a prion-protein substrate that can be converted into aggregated material by a misfolded seed. RT-QuIC uses recombinant substrate, shaking and a real-time fluorescence readout; PMCA uses repeated sonication cycles and, in the cited protocol, mouse-brain substrate with western blot detection. Neither is the universal winner: performance depends on the prion form, sample, substrate and purpose.
How seed amplification makes a tiny signal detectable
Prion seed amplification assays start with a sample that may contain very little disease-associated, misfolded prion protein. The sample is incubated with additional prion-protein substrate. If compatible seeds are present, they can induce substrate molecules to misfold and aggregate; repeated conversion creates enough aggregated material for the assay’s readout to detect.
This is an amplification of seeding activity, not a direct measurement of the original number of prion particles, infectious dose or disease severity. A signal therefore answers whether detectable seed-driven conversion occurred under the assay conditions. Its meaning depends on the specimen, prion strain, substrate, protocol and laboratory threshold. For background on amplification approaches and their diagnostic use, see the 2019 review of prion amplification and diagnosis.

The distinction in agitation is central. RT-QuIC’s repeated shaking and rest help seed-driven aggregation proceed while fluorescence is tracked during the reaction. PMCA uses sonication as part of cyclic amplification and assesses amplified material afterward by immunoblot in the cited method. These different mechanisms, substrates and readouts help explain why results can vary by prion form and why a method suited to one question may not be the best fit for another. The 2020 methods paper describes practical equipment and assay approaches; the 2018 review of prion protein amplification discusses diagnostic applications and differences between methods.

How RT-QuIC turns aggregation into a real-time readout
In RT-QuIC, a suspect sample is combined with recombinant prion protein and Thioflavin T, a fluorescent dye used to report amyloid formation. A fluorescence plate reader alternates shaking and rest periods and records the fluorescence signal over time. As seed-induced aggregation develops, the fluorescence trace can rise above the assay’s interpretation threshold. The method’s real-time measurement avoids relying solely on a final post-reaction immunoblot.
The 2016 Nature Protocols method describes RT-QuIC as seed-induced misfolding and aggregation of recombinant substrate accelerated by alternating shaking and rest in fluorescence plate readers. In practical terms, the assay needs the specified substrate and reagents, a compatible multiwell plate, and a reader that can both measure fluorescence and shake the plate. Exact preparation and interpretation remain protocol-specific.

What throughput and turnaround time mean in practice
“High throughput” is not a single assay property. It depends on how many wells a plate has, how many samples are run, how many replicates each sample receives, the controls included, and how the lab handles setup, incubation and analysis.

RT-QuIC capacity in a published protocol
The 2016 Nature Protocols method describes a 96-well setup handling up to 32 samples in triplicate. The remaining wells accommodate assay controls and other protocol needs. The same method gives an approximately 87-hour total workflow from sample preparation through analysis. That is a workflow estimate for that protocol, not a universal limit on RT-QuIC runs or a direct measure of diagnostic reaction time. See the protocol details.

Reaction time is not the same as end-to-end workflow time
A 2017 review reports that RT-QuIC for sCJD using CSF can achieve diagnostic sensitivities as high as 96% in less than 24 hours, with specificities of 98%–100%. Those figures describe the application and methods summarized in that review; they should not be generalized to every specimen, laboratory protocol or prion form. They also do not conflict with the approximately 87-hour protocol workflow: one figure concerns a diagnostic assay timeframe as reported in the review, while the other includes sample preparation through analysis in a particular published method. The 2017 review provides the performance context.
What newer plate formats change
A 2025 American Society for Microbiology review discusses newer 384-well RT-QuIC formats, as well as alternative readouts and data-analysis methods. A plate with more wells can create room for more parallel reactions, but by itself it does not establish a specific number of reportable samples, a validated diagnostic workflow or equivalent performance across laboratories. Replicate design, controls and protocol validation still determine useful capacity. See the 2025 review.

Where clinical diagnostic use is established
RT-QuIC on CSF is an established diagnostic tool for sCJD. Published accuracy figures are method- and study-specific, so they are not guarantees for an individual test or every clinical laboratory. Interpretation belongs in the context of a patient’s clinical assessment and the particular assay used; an amplified signal is evidence of seeding activity rather than a direct count of infectious particles.

RT-QuIC and PMCA can respond differently to prion strain, specimen and substrate. Why the assays are being explored beyond prion disease
Related in vitro conversion and amplification approaches are being studied for other proteinopathies, where disease-associated protein aggregates may seed further aggregation. Such research can help investigate biomarkers and disease mechanisms, but promise in an experimental assay is not the same as an established clinical diagnostic test. A 2019 review of conversion assays for neurodegenerative proteinopathies notes the need for further assessment of diagnostic performance beyond established prion applications. See the review of in vitro conversion assays and the 2021 review of RT-QuIC research.
Choosing an approach for a laboratory question
- Define the target first. Consider the prion form or proteinopathy of interest; strain responsiveness is not identical between assays.
- Match the specimen to evidence. CSF RT-QuIC has established use for sCJD, while performance for other specimens or diseases requires evidence specific to those applications.
- Check substrate and readout requirements. RT-QuIC calls for recombinant substrate, fluorescence monitoring and shaking; the cited PMCA approach uses sonication and brain-derived substrate with western blot assessment.
- Plan capacity around replication and controls. A plate’s well count is not the same as the number of samples that can be interpreted reliably.
- Use method-specific validation and interpretation. Sensitivity, specificity, thresholds and timing depend on protocol and lab execution; published figures should not be transferred automatically to a different setup.
These assays require trained laboratory practice and protocol-appropriate equipment. The relevant equipment categories are a fluorescence plate reader with shaking for RT-QuIC and sonication capability for PMCA; consumer equipment is not a substitute for diagnostic laboratory systems. Published methods do not establish local laboratory availability or a universal clinical workflow.
