Neither topical retinoids nor benzoyl peroxide is a universal first choice for every adult. The American Academy of Dermatology (AAD) strongly recommends both, while UK guidance from NICE recommends choosing a 12-week first-line regimen according to acne severity, personal preference, and likely tolerability. For mild-to-moderate acne, OTC adapalene is one retinoid option to discuss; benzoyl peroxide is another evidence-based option. Pregnancy plans, irritation risk, the type and severity of lesions, previous treatment, and scarring risk should determine the starting point.

The short answer: choose one based on your situation, not a supposed winner

A 2024 AAD guideline strongly recommends topical benzoyl peroxide, topical retinoids, and fixed-dose combinations of the two for acne vulgaris in people aged 9 and older. That recommendation means both ingredients have a strong place in treatment; it does not establish that one works better than the other for every adult or that either should automatically be tried first. The guideline is available in the AAD summary and the full JAAD guideline.

NICE recommends offering a 12-week first-line course after discussing acne severity, advantages, disadvantages, and the person’s preferences. Its options include a fixed adapalene-plus-benzoyl-peroxide treatment for acne of any severity. In practical terms, an adult with mild-to-moderate acne may reasonably discuss either an OTC adapalene product or a benzoyl peroxide product, then judge the response and tolerability over several weeks.

Do not begin a topical retinoid if you are pregnant or planning pregnancy. NICE lists topical retinoids as contraindicated in those circumstances. If you are pregnant, trying to conceive, or unsure whether a product is appropriate, ask a clinician or pharmacist before starting any acne medicine.

What the guidelines actually say

AAD: both ingredient groups are strongly recommended

The AAD guideline covers acne vulgaris in adults, adolescents, and preadolescents aged 9 years or older. Its strong topical recommendations include benzoyl peroxide, topical retinoids, and fixed-dose topical retinoid/benzoyl-peroxide combinations. John S. Barbieri, MD, MBA, FAAD, co-chair of the AAD Acne Guideline Workgroup, described the update as covering newer topical medicines applied to the skin as well as systemic treatments taken by mouth.

The AAD reports that nearly 50 million Americans are affected by acne each year and that 85% of adolescents have acne. Those figures describe acne overall or adolescent prevalence; they are not an estimate of how many adults need one ingredient rather than the other.

NICE: make a 12-week first-line plan

NICE recommends an initial 12-week course of a selected first-line treatment. The decision should account for severity and the patient’s preference after discussing benefits and drawbacks. NICE specifically includes fixed adapalene plus benzoyl peroxide as an option for any acne severity, while also giving irritation-management advice for topical treatments.

NICE says positive effects can take 6 to 8 weeks to become noticeable. That is an expected time before improvement may be visible, not a guarantee of success. A 12-week course gives enough time to assess the initial plan unless irritation or another problem requires earlier medical review.

Topical retinoids: what “retinoid” means here

Adapalene is one specific retinoid option

“Retinoid” is a class, not one product. Adapalene is a specific topical retinoid available without prescription in some markets. AAD adult-acne guidance identifies adapalene as an OTC active that can help clear blackheads, whiteheads, and pimples. Prescription tretinoin is another retinoid, but it should not be treated as interchangeable with OTC adapalene: availability, labeling, strength, and supervision differ by country.

Use the exact product according to its label or your prescriber’s directions. Do not assume that a stronger-looking percentage, a prescription retinoid, or a product marketed as “retinol” has the same evidence, instructions, or safety profile as adapalene.

Why someone might start with adapalene

  • Your main concern is a mixture of blackheads, whiteheads, and pimples and an adapalene product is legally available and suitable where you live.
  • You prefer a retinoid-based first trial and can commit to gradual introduction while watching for irritation.
  • You are not pregnant, not planning pregnancy, and have checked that the product is appropriate for you.

Trade-offs to expect

Topical retinoids can irritate the skin and may increase photosensitivity. AAD treatment guidance discusses reducing irritation through careful use and protecting skin from sun exposure; see its acne diagnosis and treatment guidance. Early dryness, stinging, redness, or peeling can make an otherwise appropriate treatment hard to continue, so tolerance matters as much as theoretical suitability.

Benzoyl peroxide: when it may be the better first experiment

It is a guideline-supported acne treatment

Benzoyl peroxide is strongly recommended by the AAD and appears in NICE first-line regimens, including the fixed adapalene-plus-benzoyl-peroxide option. It is sold in both wash-off and leave-on product categories, with instructions that vary by formulation and strength.

Reasons to consider it first

  • You want to avoid a topical retinoid because of pregnancy planning or because a clinician has advised against one.
  • You prefer to start with benzoyl peroxide alone rather than introducing two active ingredients at once.
  • You have previously tolerated benzoyl peroxide better than retinoids.

Trade-offs to plan for

Benzoyl peroxide can irritate skin. NICE also warns that the adapalene-plus-benzoyl-peroxide combination can cause photosensitivity and bleaching of hair and fabrics; bleaching is a practical concern with benzoyl peroxide products generally, so keep treated skin away from hair, towels, pillowcases, and clothing that could be discolored. Follow the label for contact time and rinsing rather than improvising a schedule.

Retinoid versus benzoyl peroxide: a practical comparison

Decision point Topical retinoid, such as adapalene Benzoyl peroxide
Guideline position AAD strongly recommends topical retinoids; NICE includes adapalene plus benzoyl peroxide among first-line choices. AAD strongly recommends benzoyl peroxide; NICE includes it alone or in first-line combinations.
What AAD specifically describes Adapalene can help clear blackheads, whiteheads, and pimples. AAD strongly recommends the ingredient for acne; the cited guidance does not establish a universal advantage over retinoid monotherapy.
Reasonable first trial Use one adapalene product as directed, introducing it gradually if needed. Use one benzoyl peroxide wash or leave-on product as directed, introducing it gradually if needed.
When benefit may become noticeable NICE says positive effects from topical acne treatment can take 6 to 8 weeks; the initial NICE course is 12 weeks. NICE gives the same 6-to-8-week expectation for positive effects and recommends a 12-week initial course.
Main tolerability issues Irritation and possible photosensitivity. Irritation; bleaching of hair and fabrics is a practical risk.
Pregnancy planning NICE says topical retinoids are contraindicated during pregnancy and when planning pregnancy. Do not assume every benzoyl peroxide formulation is automatically appropriate in pregnancy; check current local clinical advice.
Can it be combined? Yes. NICE includes fixed adapalene plus benzoyl peroxide as a first-line option, but tolerance and medical advice still matter. Yes. It can be part of a fixed combination with adapalene; that does not mean everyone should begin with two actives.

The available guidance does not provide a numeric adult-specific head-to-head result proving that retinoid monotherapy is superior to benzoyl peroxide monotherapy, or vice versa. Avoid treating general acne prevalence figures or guideline strength as if they were a direct comparison trial.

How to choose your starting ingredient

Start with adapalene only when pregnancy is not a concern

If you are not pregnant, not planning pregnancy, and have mild-to-moderate acne without deep painful lesions or scarring, an OTC adapalene product is a reasonable option to discuss. AAD’s adult-acne page recommends selecting one OTC active and using it as directed for 6 to 8 weeks before adding another product. The relevant AAD guidance is at Adult acne treatment dermatologists recommend.

Start with benzoyl peroxide when a retinoid is unsuitable or unwanted

Benzoyl peroxide may be the more practical first trial if pregnancy planning rules out topical retinoids, if a clinician has told you to avoid retinoids, or if you have previously tolerated benzoyl peroxide better. Choose a formulation you can use consistently and account for potential fabric bleaching before applying it near clothing or bedding.

Ask about a combination when one active is not enough

A fixed adapalene-plus-benzoyl-peroxide product is a NICE first-line option for any acne severity and is included among the AAD’s strongly recommended combination approaches. Combination therapy can expose skin to irritation from both ingredients, however. If you are new to treatment or have sensitive skin, discuss whether to introduce one active first instead of self-starting several products simultaneously.

Escalate rather than repeatedly switching OTC products

Large, painful pimples deep in the skin or acne that leaves scars are reasons to see a dermatologist, according to AAD. Persistent or distressing acne also deserves professional advice. The goal is not to keep cycling through OTC products when the pattern suggests that clinician-directed treatment may be needed.

A cautious 12-week first trial

1. Check the safety decision before buying

  1. Confirm whether you are pregnant, trying to conceive, or could be pregnant. Do not self-start a topical retinoid in those circumstances; ask a clinician or pharmacist about suitable alternatives.
  2. Look at the active ingredient and formulation. “Retinoid” is a class, while adapalene is one specific ingredient. Washes and leave-on benzoyl peroxide products have different label directions.
  3. Seek medical advice first if you have deep painful lesions, scarring, widespread or rapidly worsening acne, or acne that is causing substantial distress.

2. Select one active for the initial OTC trial

AAD advises using one selected active product as directed for 6 to 8 weeks rather than adding another product during that initial trial. This makes it easier to judge whether the ingredient is helping and whether irritation is tolerable. A moisturizer and facial sunscreen can support comfort and sun protection, but neither is an acne cure.

  • Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft

3. Introduce it gradually if irritation is a concern

NICE advises starting topical treatments on alternate days or using short-contact application, such as washing the product off after about an hour, then progressing to standard application if tolerated. Use the product label or clinician’s directions to determine the usual application method. Do not increase frequency simply because improvement is not immediate.

4. Keep the rest of the routine predictable

  • Avoid adding a second acne active during the first 6-to-8-week assessment unless a clinician tells you to do so.
  • Use gentle skin care and avoid deliberately escalating application in response to a flare of dryness or stinging.
  • With benzoyl peroxide, protect hair and fabrics from contact because bleaching can occur.
  • With a retinoid, take photosensitivity seriously and use sun protection as advised in AAD treatment guidance.

5. Assess at the right time

Do not judge the treatment after only a few days. NICE says noticeable positive effects may take 6 to 8 weeks and recommends a 12-week initial course. If the product is tolerable but improvement is incomplete, discuss the next step after that course rather than adding multiple new actives at once. If severe irritation, swelling, or another concerning reaction develops, stop and obtain medical advice sooner.

Can you use adapalene and benzoyl peroxide together?

Yes, a fixed adapalene-plus-benzoyl-peroxide treatment is specifically listed by NICE as a first-line option, and the AAD guideline strongly recommends fixed-dose topical retinoid/benzoyl-peroxide combinations. Combination treatment is therefore an evidence-based clinical option, not merely a social-media routine.

That does not make simultaneous self-starting the right choice for every adult. Both ingredients can irritate skin, and adding two new products at once makes it difficult to identify which one caused a reaction. If you are treatment-naive, prone to irritation, or uncertain about pregnancy safety, ask a clinician or pharmacist whether to begin with one active, use a fixed combination, or choose a different plan. NICE’s alternate-day or short-contact approach can help a person build tolerance when topical treatment is otherwise appropriate.

Irritation, photosensitivity, and fabric damage: what to do

When irritation is mild

Dryness, redness, peeling, or stinging can occur with topical acne treatments. Rather than applying more often to speed results, reduce exposure by using alternate-day application or short contact as NICE advises, then increase only if your skin tolerates it. Follow the product label and seek professional advice if symptoms persist or worsen.

Protect skin from sun exposure

NICE notes photosensitivity with the fixed adapalene-plus-benzoyl-peroxide option, and AAD treatment guidance discusses sun protection when treating acne. Use a broad-spectrum facial sunscreen that your skin tolerates and follow local clinical advice, especially when using a topical retinoid.

Prevent accidental bleaching

Benzoyl peroxide can bleach hair and fabrics. Wash hands after application unless the label says otherwise, allow the product to dry as directed, and use white towels or bedding if discoloration would be a problem. This is a practical handling issue, not evidence that the treatment is working better.

Pregnancy and trying to conceive

NICE states that topical retinoids are contraindicated during pregnancy and when planning pregnancy. That includes self-starting an adapalene product without checking with a clinician. AAD also cautions that some acne products sold without prescription may not be safe during pregnancy and recommends consultation when pregnant or trying to conceive.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not generalize from the retinoid warning to a blanket claim that every benzoyl peroxide product is safe for every pregnant person. Formulation, labeling, geography, and individual circumstances matter. Ask your obstetric clinician, dermatologist, or pharmacist for a current recommendation before using any acne active.

When professional care is the safer first step

  • Large, painful pimples feel deep in the skin.
  • Acne is leaving scars or persistent dark marks that concern you.
  • Acne is severe, widespread, rapidly changing, or significantly affecting your wellbeing.
  • You are pregnant, planning pregnancy, breastfeeding, or unsure whether an ingredient is appropriate.
  • You have completed a properly used initial course but are not improving or cannot tolerate the product.

A dermatologist can decide whether the problem is acne vulgaris, whether a prescription topical or oral treatment is needed, and how to avoid worsening irritation. The AAD’s adult-acne guidance specifically directs readers with deep painful lesions or scarring toward dermatology care rather than endless OTC experimentation.

What is established—and what is not

The strongest conclusion supported by current guidance is that both topical retinoids and benzoyl peroxide are legitimate, recommended acne treatments. Adapalene is a reasonable OTC retinoid option for some adults with mild-to-moderate acne; benzoyl peroxide is another evidence-based option; and a fixed adapalene-plus-benzoyl-peroxide regimen is a recognized first-line choice.

What has not been established in the cited guidance is a single adult-specific percentage showing that one ingredient wins in head-to-head monotherapy for everyone. Your first choice should therefore be based on pregnancy status, acne severity and lesion type, prior tolerance, product availability and labeling where you live, willingness to manage irritation, and whether scarring or deep pain calls for a clinician now.
