Gene therapy and retinal implants are not interchangeable treatments. Gene therapy is designed for a person whose retinal cells have a specific, treatable genetic defect; the approved retinal example, Luxturna (voretigene neparvovec-rzyl), requires confirmed disease caused by two pathogenic copies of the RPE65 gene. A retinal implant such as the Argus II is intended for a narrow group of adults with end-stage retinitis pigmentosa, prior useful vision and bare or no light perception in both eyes.
The right choice depends first on diagnosis and remaining biology, not on which technology sounds newer. A retinal specialist, inherited-retinal-disease genetic counselor and, when appropriate, an implant rehabilitation team must confirm the cause of vision loss, genetic eligibility, viable retinal pathways, surgical suitability and local access before treatment is considered.
At a glance: two different ways to restore useful visual function
Gene therapy attempts to change the biology of retinal cells. A prosthesis bypasses damaged photoreceptors and uses electrical stimulation to activate surviving retinal pathways. That distinction determines who can benefit.

| Long-term commitment | Safety monitoring after administration; CMS describes one treatment per eye per lifetime for qualifying beneficiaries | Implant surgery, device fitting, repeated follow-up and visual rehabilitation |
Neither approach is expected to produce normal visual acuity or guaranteed independent living. Results depend on the disease, the condition of the retina and optic pathways, training, complications and the quality of follow-up.

How a retinal implant works
Argus II is the relevant evidence example
The Argus II retinal prosthesis combines an electrode array placed on the retina with external glasses, a camera and a video processor. The camera captures a scene; the processor converts visual information into stimulation patterns; and the implanted electrodes electrically stimulate surviving retinal circuitry. The resulting perception is not ordinary sight restored to a healthy eye. Users learn to interpret patterns of light and contrast through training.
The U.S. Food and Drug Administration humanitarian device exemption (HDE) indication is narrow. It specifies adults at least 25 years old with severe-to-profound retinitis pigmentosa, bare or no light perception in both eyes, prior useful form vision, and appropriate lens status—aphakia or pseudophakia. A candidate must also be willing and able to complete follow-up, device fitting and visual rehabilitation. The device is generally implanted in one eye, usually the eye with poorer remaining vision, after the treating team evaluates surgical suitability.
What the selection process must establish
- The vision loss is caused by retinitis pigmentosa rather than an optic-nerve disorder, an acquired retinal condition or another cause of blindness.
- Both eyes meet the severe vision threshold in the indication, while the person has a history of useful form vision.
- The lens status and ocular anatomy are suitable for implantation.
- Retinal and downstream visual pathways can respond sufficiently to electrical stimulation.
- The person can attend programming visits, use the external equipment and participate in rehabilitation.
Meeting the headline criteria does not guarantee that a center will offer implantation. Device availability, specialist expertise and local regulatory or payer requirements can change. Ask the center to confirm that it is currently operating an Argus II program before beginning a lengthy evaluation.
What the clinical evidence shows
A five-year clinical trial reported in an American Academy of Ophthalmology journal in 2016 followed 30 selected participants. Twenty-four still had implanted systems functioning at five years. Participants performed significantly better with the system switched on than off on objective visual-function and real-world task tests. Those results apply to the trial population and should not be generalized to every person with blindness or every retinal prosthesis.
“Functioning” means that the system continued to operate; it does not mean that every participant achieved the same visual ability, independence or comfort. A specialist should explain the likely training burden and the range of possible perceptions for an individual patient.
How retinal gene therapy works
The biological idea
The FDA defines gene therapy as “a technique that modifies a person’s genes to treat or cure disease.” In retinal medicine, an engineered vector—often a modified virus—delivers therapeutic genetic material to target cells. The goal is to restore a missing cellular function, not to place a camera or electronic sensor in the eye.

That strategy requires two things a prosthesis does not: a mutation for which the product is designed and enough living target cells to receive and use the genetic instructions. If the retina has lost the relevant cells or the disease is caused by a different gene, the approved product is not automatically applicable.
Luxturna’s exact eligibility
Luxturna (voretigene neparvovec-rzyl) is FDA-approved only for confirmed biallelic RPE65 mutation-associated retinal dystrophy. “Biallelic” means that disease-causing changes are present on both copies of the RPE65 gene. A genetic result should be interpreted with a qualified laboratory and genetic counselor; a variant of uncertain significance is not the same as a confirmed pathogenic or likely pathogenic result.
Centers for Medicare & Medicaid Services (CMS) coverage criteria describe treatment for qualifying beneficiaries as one administration per eye per lifetime. The criteria also require that other likely pathogenic variants be ruled out or considered less likely to explain the disease. Coverage rules are not identical to clinical judgment, so the treating center and payer must review the full genetic report, diagnosis and policy in force where the patient lives.
Procedure and follow-up
Luxturna is delivered by a subretinal procedure rather than by an external device. The ophthalmic team determines whether enough viable retina remains, plans the treatment eye and monitors for surgical, retinal, inflammatory and other complications. Treatment of the second eye, when appropriate, is a separate clinical decision subject to the product indication, payer rules and the patient’s response and risk profile.
Gene therapy also requires long-term safety follow-up. FDA guidance emphasizes that delayed adverse events can occur after some gene-therapy products are administered. Follow-up may include retinal examinations, imaging, functional testing and monitoring for inflammation or immune-related problems. Patients should ask how long the center will follow them and whom to contact if symptoms appear after the scheduled visits.

In the CMS evidence summary, the one-year bilateral multiluminance mobility result improved by a mean 1.8 (standard deviation 1.1) in the Luxturna treatment group versus 0.2 (standard deviation 1.0) in the control group; the reported p value was 0.0013. This is a task-level measure under defined lighting conditions, not a promise of normal acuity, unrestricted night vision or complete independence.

Retinal implant or gene therapy: the decisive differences
| Decision axis | What favors gene therapy | What favors an implant evaluation |
|---|---|---|
| Cause of vision loss | A confirmed inherited retinal dystrophy caused by the mutation targeted by the product | End-stage retinitis pigmentosa that fits the device indication |
| Genetic status | Confirmed biallelic RPE65 disease for Luxturna; genetic counseling is essential | The cited Argus II indication is diagnosis and function based rather than tied to RPE65 |
| Remaining biology | Viable target cells remain for the vector to transduce and use the gene product | Retinal and downstream visual pathways can respond to electrical stimulation |
| Baseline vision | Eligibility is not defined by the Argus II bare/no-light-perception threshold | Bare or no light perception in both eyes, with prior useful form vision |
| Age | No single age cutoff is established in the cited Luxturna material | FDA HDE specifies age 25 or older |
| Main intervention | Subretinal genetic delivery | Implant surgery plus external hardware and programming |
| Aftercare | Long-term medical safety surveillance | Device fitting, software or hardware support, repeated visits and visual rehabilitation |
| What success looks like | Better performance on validated visual tasks in eligible patients | Improved performance on selected visual-function and mobility tasks when the device is on |
The approaches are therefore not competing choices for the same patient in most cases. A person with RPE65-associated retinal dystrophy may be assessed for Luxturna even if that person does not meet Argus II criteria. A person with end-stage retinitis pigmentosa and no qualifying RPE65 mutation may be considered for an implant evaluation, subject to the device’s clinical and practical requirements.
A step-by-step pathway to the right evaluation
1. Confirm the diagnosis and the source of vision loss
Start with a retinal specialist who can distinguish inherited retinal degeneration from acquired retinal disease, optic-nerve disease, glaucoma, trauma or other causes. Bring prior examination notes, visual-field results, imaging, operative reports and a timeline showing when useful vision was lost. A treatment aimed at photoreceptors cannot correct blindness caused primarily by the optic nerve.
2. Obtain validated genetic testing when inherited disease is suspected
Use a clinically validated laboratory and genetic counseling rather than relying on a direct-to-consumer result. The counselor should explain inheritance, whether each variant is pathogenic, whether the two variants are on different gene copies when that matters, and whether another gene better explains the phenotype. Luxturna requires confirmed biallelic RPE65-associated disease; a broad “retinal dystrophy” label is insufficient.
3. Measure remaining retinal and visual-pathway function
The specialist will decide whether enough target tissue remains for gene delivery and whether the visual pathway can support meaningful stimulation from an implant. Testing may include visual acuity, fields, retinal imaging, electrophysiology or other center-specific assessments. The purpose is not to pass a single universal test but to match the patient’s biology to the mechanism of the proposed treatment.
4. Compare the treatment burden with the likely functional gain
Ask for a written plan covering the procedure, anesthesia or surgical setting, recovery, inflammation management, equipment, training, number of visits and responsibilities at home. For an implant, include programming and orientation-and-mobility rehabilitation. For gene therapy, include eye-by-eye treatment planning and the schedule for medical surveillance. Discuss how the expected benefit would change daily tasks such as locating objects, navigating a room or detecting obstacles.
5. Review safety issues and alternatives
Discuss retinal complications, inflammation, immune reactions, surgical risks and the possibility that benefit may be limited or absent. Ask what symptoms require urgent contact, which medications are used around treatment and how complications are handled. Also ask whether observation, low-vision rehabilitation, orientation and mobility training or an active clinical trial is more appropriate. A magnifier or other low-vision aid can improve access to remaining vision, but it is not a vision-restoration treatment and does not substitute for this evaluation.

6. Verify availability, authorization and practical support
Have the center confirm that the proposed therapy or device is currently available in the patient’s country and that the clinicians still perform the procedure. Payer authorization may require genetic documentation, specialist notes, imaging and an appeal. Ask who handles prior authorization, travel, accommodation, equipment replacement, rehabilitation and emergency care. Availability and coverage can change, so an old online discussion is not proof that a program remains open.

Retinal implant considerations
- Surgery can cause ocular complications, and the implanted hardware may require additional medical attention.
- The external glasses, camera and processor must be worn, charged or maintained as specified and protected from damage.
- Visual patterns require training; the system does not recreate natural color, detail or ordinary visual acuity.
- Follow-up is not optional: fitting, programming and rehabilitation determine whether the user can make practical use of the stimulation.
- The five-year evidence comes from a small, selected study group, so individual outcomes can differ substantially.
Gene-therapy considerations
- The product is mutation-specific. A different inherited retinal disease, an uncertain genetic result or an acquired cause of blindness does not establish eligibility.
- Subretinal delivery carries surgical and retinal risks, and inflammation or immune-related problems can occur.
- Delayed adverse events are possible with some gene-therapy products, which is why FDA guidance calls for long-term follow-up.
- One administration per eye per lifetime is the CMS description for qualifying beneficiaries; this is not a promise that retreatment will be available if benefit is limited.
- Functional gains measured in a study do not guarantee normal sight, full-field vision or independent mobility.
Why rehabilitation matters for both approaches
Medical treatment and functional training are separate parts of care. An implant user may need to learn how electrical patterns correspond to edges, movement or contrast. A person who gains function after gene therapy may still need orientation and mobility instruction, lighting changes, contrast strategies or assistive technology while the brain adapts. Set goals in terms of specific activities rather than the vague promise to “see again.”
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What to expect from the outcomes
Ask the treating team which outcome measure it uses and what a clinically meaningful change would look like in daily life. For Luxturna, the cited evidence concerns multiluminance mobility under controlled conditions. For Argus II, the cited trial compared objective visual-function and real-world task performance with the system on and off. Neither result should be translated into a guaranteed visual-acuity number.
Benefits can vary with disease duration, remaining cells, age at treatment, baseline function, training and complications. A person may detect light or objects more reliably without being able to read print, recognize faces or travel independently. Conversely, a modest test improvement can still matter if it helps locate a doorway or avoid an obstacle. The consultation should identify the patient’s most important tasks and define how they will be measured after treatment.
Cost, coverage and access questions
No single current price or out-of-pocket figure can be applied to either approach. The total cost depends on country, payer, authorization, hospital arrangements, genetic testing, surgery, follow-up, rehabilitation, travel and equipment support. The available evidence does not establish a current provider directory for retinal prostheses, a current Luxturna price or patient-specific expenses.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsBefore consenting, request an itemized estimate that separates treatment, facility fees, surgeon and anesthesia charges, genetic testing, imaging, medications, rehabilitation, travel and possible complication care. Ask whether coverage is limited to one eye, whether a second eye requires a new authorization and what happens if the center stops offering the program. For international patients, verify regulatory approval and continuity of follow-up in the country where care will occur.

Questions to take to the specialist
- What is the precise cause of my vision loss, and what evidence rules out an optic-nerve or acquired cause?
- Does my genetic report show two pathogenic or likely pathogenic RPE65 variants, and has another gene been reasonably excluded?
- How much viable retina and downstream visual function remains, and which tests support that conclusion?
- Am I within the Argus II indication, including age, prior useful vision, bilateral bare or no light perception and lens status?
- Which daily tasks are realistic goals, and how will you measure progress?
- What are the surgical, retinal, inflammatory and immune-related risks in my case?
- How many fitting, programming, rehabilitation and medical follow-up visits should I plan for?
- Is the program currently operating, and who will provide long-term support?
- What costs are covered, what requires prior authorization and what could I pay myself?
- What symptoms after treatment require an emergency call?
Which approach fits the common scenarios?
Confirmed biallelic RPE65-associated retinal dystrophy
Ask an inherited-retinal-disease center to evaluate Luxturna eligibility. The decision turns on the validity of the genetic diagnosis, viable target retina, clinical status and the risks and benefits of subretinal delivery. Argus II criteria do not replace the mutation-specific requirements for Luxturna.
End-stage retinitis pigmentosa with bare or no light perception
If the person is at least 25, had useful form vision, has suitable lens status and can complete rehabilitation, an Argus II evaluation may be appropriate. The center must still determine whether the visual pathways can respond and whether the device is available.
Inherited retinal disease caused by another gene
Do not assume that Luxturna applies. Continue genetic counseling and ask about disease-specific trials or supportive low-vision and mobility services. An implant evaluation depends on the separate Argus II clinical criteria, not simply on having an inherited diagnosis.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Blindness caused mainly by the optic nerve or an acquired non-retinal condition
Neither the RPE65 gene therapy indication nor the cited Argus II indication automatically fits. The priority is a specialist assessment of the actual site of injury and appropriate rehabilitation or research options.
The practical decision
Choose the pathway that matches the biology: mutation-confirmed, viable RPE65 target cells point toward a Luxturna evaluation; end-stage retinitis pigmentosa with the specified visual history and anatomy points toward an Argus II evaluation. If neither set of requirements is met, pursuing a different retinal implant or gene therapy simply because it is available online can expose you to risk without a sound biological rationale. Confirm the diagnosis, obtain qualified genetic interpretation, assess remaining visual pathways, and verify an operating center and payer plan before making a treatment decision.
