A ledger check does not reserve the right to refund. If two workers read “not refunded” before either records a change, both can proceed; a retry after a lost response can also submit the same intended refund as a new operation. The exact cause of this incident cannot be determined from the title alone. Prevent duplicates by claiming each logical refund atomically in durable state, using one stable provider idempotency key for its retries, and reconciling uncertain outcomes against provider responses and webhooks.

Why can a refund happen twice after a ledger check?

A read followed by a later write is not an atomic decision. For example, worker A reads the ledger and sees no refund. Before A records its action, worker B reads the same state and also sees no refund. Without a transaction, lock, or uniqueness rule that serializes the claim, both may submit a refund.

A separate failure path begins after submission: the provider may accept a refund, but the response may be lost or time out. If the handler treats that timeout as proof that nothing happened and creates a fresh request, it can pay twice. These are plausible failure modes, not a confirmed diagnosis of the incident described in the title.

What should the handler do instead?

  1. Assign a stable internal identity. Represent each intended refund as a durable operation tied to its original payment, amount, and reason. A retry of that same action must retain the same identity.
  2. Claim the operation atomically. In one durable transaction, validate the refundable balance and reserve or claim the refund identity. Use a database transaction, lock, uniqueness constraint, or equivalent concurrency control appropriate to the system so two workers cannot claim the same logical refund.
  3. Submit with a stable provider idempotency key. Derive or associate the provider key with the internal refund identity. Reuse it for retries of that refund and keep the request parameters consistent. Stripe documents that it stores the first result after endpoint execution begins and compares parameters on reuse; Adyen recommends UUID-style unique keys. Stripe idempotent requests and Adyen API idempotency describe provider-specific behavior.
  4. Persist the outcome and references. Record the provider request, response state, and any refund reference. Treat pending or unknown as states requiring reconciliation—not permission to create a new logical refund.
  5. Process webhooks idempotently. Match provider notifications to the original payment and refund operation, and ensure replayed events cannot create duplicate ledger changes. Adyen recommends asynchronous server-to-server webhooks to track outcomes when responses are missing; its refund guidance describes awaiting the refund outcome. See Adyen refund.
  6. Reconcile and alert on mismatches. Compare local refundable balances and refund records with provider refund references and webhook outcomes. Route discrepancies for review instead of silently issuing another refund.

How should a timeout or error be handled?

A timeout is an unknown outcome, not evidence that the provider did not act. First identify the logical refund operation and check its stored state and provider signals. If retrying is appropriate under the provider’s contract, retry the same operation with the same key and matching parameters; do not mint a new key merely because the response was lost.

Provider error semantics matter. Stripe documents HTTP 409 conflict cases involving idempotency and recommends exponential backoff for HTTP 429 rate limiting. A conflict or other error is not automatically safe to retry: follow the specific endpoint’s documented meaning and retain the same logical key for a retry. See Stripe errors.

Adyen documents a same-key race that can return a transient error; retry later with the same key only when the transient-error signal permits it. Its key validity and concurrency behavior are provider-specific, so confirm the current API contract for the endpoint and account before implementing retry rules. Adyen API idempotency

What protections do payment providers offer—and what do they not solve?

Provider safeguards can limit the amount refunded, but they do not replace a local concurrency design. Stripe permits partial refunds up to the remaining unrefunded amount and does not allow refunding a fully refunded charge again. Adyen says its default accounting rules prevent total refund value from exceeding the captured amount. Those limits can stop an over-refund, but they do not establish which local worker initiated an operation or diagnose why a handler retried. See Stripe create a refund and Adyen refund.

How do Stripe and Adyen differ in the details?

Behavior Stripe Adyen
Idempotency key POST requests accept keys. The first status and body are saved after endpoint execution begins; parameters are compared. Keys may be pruned once at least 24 hours old. Stripe documentation POST requests support keys; UUID-style keys are recommended. Documented validity is 7–14 days at company-account scope. Adyen documentation
Concurrent or ambiguous requests A same-key concurrent conflict may not save an idempotent result if endpoint execution did not begin; consult the relevant error semantics. Stripe documentation A documented same-key race may produce a transient error. Retry later with the same key only when the transient-error signal allows it. Adyen documentation
Refund tracking A refund is created against a Charge or PaymentIntent; partial refunds are supported up to the remaining amount. Stripe documentation The refund response returns a PSP reference; the outcome is reported by webhook. Referenced refunds use the original payment reference. Refund guidance and referenced refunds
Amount guard A fully refunded charge cannot be refunded again, and a refund cannot exceed the remaining unrefunded amount. Stripe documentation Default accounting rules prevent total refund value from exceeding the captured amount. Adyen documentation

These are documented behaviors, not interchangeable guarantees. Retention periods, endpoint execution rules, key scope, and webhook handling depend on the provider and API version; verify the contract you actually use.

When is an asynchronous reversal endpoint relevant?

Adyen’s Checkout v71 reversal endpoint is an endpoint-specific asynchronous option: it returns a unique request reference and reports the outcome through a CANCEL_OR_REFUND webhook. Its documentation cautions against using it for payments with multiple partial captures, so it should not be treated as a universal refund recipe. See Adyen Checkout v71 refund or cancel a payment.

What records make a duplicate easier to prevent and investigate?

Maintain a durable record for each intended refund, linked to the original payment and its provider reference. Adyen’s referenced-refund flow uses the original payment’s PSP reference and describes an audit trail for full or partial refunds. A reconciliation process can then compare the local operation, provider refund reference, and webhook event rather than relying on a single ledger flag. See Adyen referenced refunds.
