Nation-state cyber risk is not limited to a spectacular attack that shuts down a company. It can mean persistent access through compromised identities, quiet intelligence collection, or footholds in systems that could later support disruption. And pro-Russia hacktivist groups have caused real operational risk with simple, opportunistic methods. CISOs should treat identity, exposed infrastructure, suppliers, and operational technology (OT) as connected parts of the same risk picture—while distinguishing state activity from financially motivated crime and loosely affiliated groups.
Why should CISOs care about nation-state cyberattacks?
For U.S. organizations, the FBI says actors associated with China, Russia, Iran, and North Korea continue to conduct cyber intrusions targeting U.S. victims. The FBI describes objectives that include collecting intelligence and intellectual property, as well as establishing destructive capability. That is an official U.S.-focused threat framing, not a claim that every actor targets every sector or that every intrusion is intended to cause destruction.
The risk is strategic as well as immediate. An intrusion may seek access that can be maintained over time, insight into an organization or its suppliers, or a position from which an actor could act later. The FBI has warned that some nation-states seek “not just intelligence and intellectual property, but to establish destructive capability against our homeland.” This is the agency’s characterization, not a quantified estimate of how often destructive attacks occur.

Microsoft’s Digital Defense Report 2026 describes global observations and says China, Iran, North Korea, and Russia share an increasing emphasis on persistent, scalable access models and long-term positioning in high-value environments. Its report describes activity observed from July 2025 through June 2026; it should not be read as a measure of risk to every organization. Together, the FBI and Microsoft reporting point to a practical CISO concern: perimeter defenses alone cannot address an adversary that may exploit trusted accounts, supplier relationships, or access that already exists.

Nation-state activity also sits alongside, but is not interchangeable with, criminal ransomware. The FBI notes that criminal ransomware developers, affiliates, and service providers change tactics. Defenders may face overlapping techniques and operational consequences, but should avoid assuming every disruptive incident is state-directed—or that a state-linked intrusion is simply a criminal demand for money.
Which attack paths deserve attention?
Recent official alerts illustrate several different paths into organizations. They are examples of reported activity, not evidence that every business is equally exposed.

- Trusted accounts and identity: Microsoft describes a shared focus on gaining and maintaining access to identities and critical systems. It reports that 52.2% of valid-account intrusions in its 2026 report involved follow-on credential theft. That figure belongs to Microsoft’s observation set; it is not a universal rate for all intrusions.
- Commercial messaging accounts: The FBI’s 2026 alert index includes Russian Intelligence Services phishing against commercial messaging accounts. Treat these services as part of the organization’s identity and communications risk surface, particularly where they are used for business.
- Internet-facing and edge devices: The FBI index includes alerts concerning Iranian malware activity and exploitation of vulnerable edge devices. An exposed appliance or remote-access system can provide an entry point beyond the systems security teams routinely monitor.
- Internet-connected control equipment: The FBI index describes Iranian-affiliated actors exploiting internet-connected programmable logic controllers (PLCs) across U.S. critical infrastructure. A later index note describes expansion to additional PLC manufacturers and emphasizes restricting direct internet access. The index summarizes an advisory; it is not a substitute for the advisory’s technical detail.
- Operational technology remote access: In a December 9, 2025 release, the U.S. National Security Agency (NSA) said agencies had observed pro-Russia hacktivist groups using inadequately secured virtual network computing (VNC) connections to access OT control devices. NSA identified water and wastewater, food and agriculture, and energy among affected sectors.
The last example matters because the groups NSA described were pro-Russia hacktivists, not necessarily a centrally directed military operation. NSA said their approach was opportunistic and their techniques simple, cheap, and easy to replicate. Such activity can still harm unintended critical-infrastructure victims and cause damage that was not anticipated by the attackers.
What changes when OT or essential services are involved?
In office IT, an incident may primarily affect data, productivity, or access to business applications. In OT, a compromised control device or remote connection can have consequences for physical processes and essential services. The potential impact depends on the system, its safeguards, and how it is connected; an intrusion does not automatically mean a safety incident or service outage.

For a CISO, the important shift is to make OT access paths and operational dependencies visible to both security and operations teams. Ask which control devices are reachable from the internet, which remote administration routes are permitted, who can use them, and what essential services depend on the affected systems. Include the people responsible for plant operations, safety, and recovery in decisions about monitoring, patching, isolation, and incident response. A containment action that is safe for an office endpoint may not be safe for a live industrial process.
NSA’s June 30, 2025 warning said Iran Revolutionary Guard Corps-affiliated actors, including hacktivists and government-affiliated actors, may target U.S. devices and networks, including through outdated software, internet-connected devices, and default or common passwords. The warning urged especially critical-infrastructure organizations to assess weaknesses and update incident-response plans. It was tied to events at that time; do not treat it as a fresh assessment of current likelihood without checking for newer government advisories.

How should a CISO turn this into a risk plan?
Use the threat picture to prioritize exposure and consequence, rather than trying to predict which actor will target the organization. A workable plan connects identity, internet-facing assets, OT, suppliers, and response readiness.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minute- Map what is reachable. Inventory internet-facing systems, edge devices, remote-access services, externally reachable OT equipment, and unsupported devices. Record owners, business purpose, dependencies, and whether direct internet access is necessary. Include supplier-managed connections rather than treating them as outside the organization’s risk boundary.
- Reduce identity-based opportunity. Strengthen identity verification, remove unnecessary standing privilege, and limit privileged access to what people and services need. Require phishing-resistant authentication where accounts and applications support it. A compatible FIDO2 security key is one possible implementation; compatibility with the organization’s systems and recovery procedures must be checked.
- Prioritize exposed vulnerabilities. Establish a rapid path to assess and remediate internet-facing vulnerabilities, with urgency informed by exposure, exploitability, business criticality, and available mitigations. Where an exposed or end-of-support device cannot be safely patched, consider isolating it, restricting access, or replacing it. Coordinate changes with operations where availability or safety could be affected.
- Review remote administration and OT access. Identify VNC and other remote-management paths, remove unnecessary external reachability, and restrict permitted routes and accounts. Validate that access controls and monitoring cover the systems that actually manage control devices, not only the corporate network.
- Connect detection across systems. Improve visibility across identity, cloud, endpoint, edge, and OT environments so suspicious use of valid credentials or trusted access is not missed between separate platforms. Define how alerts move to the teams responsible for investigation and operational decisions.
- Exercise response with dependencies in view. Update incident-response plans and rehearse decisions with security, business, legal, communications, infrastructure, OT, and relevant supplier stakeholders. Include scenarios involving compromised accounts, exposed control access, and a service that cannot simply be taken offline. The cited agencies recommend readiness, but do not prescribe one universal exercise schedule.
- Use current advisories and trusted channels. Monitor FBI and sector-relevant government advisories for changing indicators, techniques, and mitigations. The FBI says it issues advisories with hunting guidance and mitigations as indicators and techniques evolve. Share relevant incident information with appropriate authorities and sector partners, subject to applicable requirements.
How can leaders judge whether the plan is improving resilience?
Track evidence that the organization is reducing practical opportunities for access and improving its ability to respond. Useful measures include:
- Coverage and ownership for internet-facing assets, edge devices, and remote-access routes.
- Time to assess and remediate exposed vulnerabilities, plus the number of unsupported or unpatchable devices with documented compensating safeguards.
- Use of phishing-resistant authentication and the reduction of standing privileged access across both human and non-human identities.
- Visibility into external supplier connections and the ability to identify which essential services rely on them.
- Coverage of OT remote access in monitoring and response plans, with operationally approved isolation and recovery options.
- Whether incident exercises resolve decision rights, communications, supplier coordination, and restoration dependencies—not just whether participants complete a scenario.
Microsoft reported that 46 million or more business contact impersonation attacks were detected over the 12 months covered by its Digital Defense Report 2026. It also said 25.5% of observed cyber threat activity impacted Microsoft customers in the United States from January 2025 through June 2026. In its 2026 report, government agencies and services accounted for 27% of the top ten global sectors impacted, information technology for 17%, and research and academia for 14%. These are Microsoft telemetry figures with the report’s own observation scope and periods, not global prevalence estimates or forecasts for a particular company. They can inform context, but should not replace an organization’s own exposure and consequence assessment.
What should the CISO take to the executive team?
Frame the issue as a question of access, exposure, and recoverability: which trusted identities and external connections could provide durable access; which internet-facing or legacy devices are difficult to secure; and which services could be affected if access to OT or suppliers is abused? Assign owners across security, IT, operations, procurement, and business continuity. Set priorities according to sector, geography, contractual obligations, exposure, and recovery dependencies rather than assuming a single control or product can prevent compromise.
