A rare autopsy of one Alzheimer’s patient found that neighboring brain regions had responded very differently after prolonged anti-amyloid treatment. Raised cortical ridges called gyri had largely cleared amyloid plaques and contained little tau, while adjacent grooves called sulci retained substantial amyloid and showed more tau tangles, inflammation and neurodegeneration.
The finding is unusually informative because it compares different regions within the same person. It supports the idea that removing amyloid early and thoroughly might limit later damage, but one donated brain cannot prove that treatment caused the contrast or establish how much clearance is protective.
What the autopsy found
Investigators examined brain tissue from a man who had Alzheimer’s disease and had participated in an aducanumab clinical trial. The contrast was not simply between two patients with different disease histories; it appeared between neighboring folds in the same cortex.

Brain scans obtained while the participant was alive had already shown uneven amyloid clearance. The postmortem tissue matched that imaging pattern, giving investigators a direct look at what those regional differences meant biologically.

Edward B. Lee, MD, PhD, a co-senior author, described the case as a unique “Goldilocks” scenario: some regions cleared amyloid while others did not. That natural contrast reduced some of the confounding that complicates comparisons across separate patients.
Who the patient was and how long treatment lasted
The donated tissue came from a man diagnosed in his fifties with mild cognitive impairment caused by Alzheimer’s disease. He enrolled in an aducanumab trial, received 30 doses over four and a half years, and requested that his brain be donated to the University of Pennsylvania’s Center for Neurodegenerative Disease Research.

Does clearing amyloid stop tau from building up?
This case suggests an important relationship but does not answer the question conclusively. Areas with little remaining amyloid also had little tau, whereas nearby amyloid-rich areas had much more tau and tissue injury. That pattern is consistent with the hypothesis that extensive amyloid removal can constrain subsequent tau accumulation and the degeneration associated with it.

It does not prove that amyloid removal caused the difference. The study cannot identify a clearance threshold, show that every patient would develop the same regional pattern, or separate treatment effects from all other biological factors in one person. Christopher A. Brown, MD, PhD, the first author, noted that patients may accumulate amyloid for years before symptoms appear; the timing of removal may therefore matter, but this autopsy cannot determine the best treatment window.

Why some parts of the brain looked healthier than nearby areas
Gyri and sulci are physically adjacent but can differ in how disease proteins and therapies are distributed. In this patient, that produced an unusually sharp side-by-side comparison: the folds with greater amyloid clearance had less tau and less visible tissue damage, while the grooves that retained amyloid had a heavier burden of tau, inflammation and neurodegeneration.
The result matters because a conventional comparison between treated and untreated people can be distorted by differences in age, disease stage, genetics and other medical factors. Comparing regions inside one brain does not remove every source of uncertainty, but it narrows some of those differences.

Could protection continue after treatment ends?
The patient died roughly four years after his last aducanumab dose, yet the regional contrast was still visible at autopsy. That is compatible with a lasting biological effect in the areas that cleared amyloid, but it is not proof that treatment protects the brain for four years after stopping. The observation comes from a single person, and the study did not measure a clinical effect size or report percentages for amyloid clearance or tau reduction.

What this means for anti-amyloid medicines
Aducanumab was the first medicine in this class to receive U.S. Food and Drug Administration accelerated approval, but it is no longer marketed. Penn’s account identifies donanemab (Kisunla) and lecanemab (Leqembi) as newer anti-amyloid medicines with full FDA approval.
The autopsy is not a head-to-head comparison of those medicines. It is an individual case following aducanumab exposure, so it cannot establish that one approved therapy clears amyloid more effectively than another, or that the same regional pattern would occur with a different drug. These are prescription treatments whose use, eligibility and monitoring must be determined with a clinician; the findings do not support self-treatment or changing therapy without medical advice.
What the study can—and cannot—change
- It adds rare human evidence: Tissue from one brain links regional amyloid clearance with regional differences in tau and degeneration more directly than scans alone can.
- It strengthens an early-treatment hypothesis: The pattern supports studying whether removing amyloid before extensive downstream injury can slow disease progression.
- It does not establish a treatment rule: There is no demonstrated clearance percentage, duration of protection or guarantee that the finding applies to other patients.
- It calls for longer observation: Trials lasting about 18 months cannot by themselves answer how regional pathology behaves years after treatment.
Brown summarized the cautious implication: “This case suggests that removing amyloid early may help limit the changes that ultimately damage brain cells.” That is a rationale for further controlled studies, not proof that anti-amyloid therapy stops Alzheimer’s disease.
