Immediately isolate suspected ransomware, protect patient care, preserve evidence, and activate your incident-response team. A hospital should treat ransomware as a HIPAA Security Rule security incident from the moment malware is detected. Isolation decisions must reduce spread without creating an unsafe clinical outage; recovery should use clean, isolated, tested backups and require clinical-owner approval before systems return to production.
Ransomware does not automatically answer the separate question of whether a reportable HIPAA breach occurred. The covered entity or business associate must investigate what happened to protected health information (PHI), complete the required risk assessment, and apply current federal, state, contractual, and insurance reporting requirements.
What to do in the first hour
- Declare a security incident and name an incident commander. Bring in security, infrastructure, clinical operations, privacy, legal, communications, executive leadership, and any relevant vendors or cyber-insurance contacts. HHS states that “The presence of ransomware (or any malware) on a covered entity’s or business associate’s computer systems is a security incident under the HIPAA Security Rule.” HHS ransomware fact sheet
- Protect patients while containing the attack. Move affected clinical departments to approved downtime procedures. Keep emergency, medication, laboratory, imaging, monitoring, communications, and other life-safety functions operating through safe manual or alternate workflows.
- Isolate affected systems. Disconnect infected endpoints from wired and wireless networks when practical. If multiple devices, servers, or subnets are involved, isolate the segment at the switch, firewall, or other network-control point. Prioritize systems essential to patient care and safety when deciding what to isolate first. CISA ransomware guide
- Preserve evidence before destructive actions. Secure ransom notes, alert data, logs, volatile information where feasible, disk images, indicators of compromise, affected host lists, and chain-of-custody records. Do not wipe or rebuild a system until forensic preservation and legal guidance are addressed.
- Disable attack paths. Block confirmed indicators, disable compromised accounts, suspend exposed remote-access routes, and coordinate every high-impact change through the incident commander so that containment does not create an uncontrolled clinical outage.
- Start a timeline. Record when symptoms were first observed, which systems were unavailable, who authorized each action, what evidence was collected, and what remains unknown.
Do not assume that encrypted files are the only impact. Determine whether attackers accessed or removed data, established persistence, compromised identities, or reached medical devices and other connected systems.
Prepare before an incident
Maintain an offline, practiced plan
Keep a written incident-response plan and a communications plan in both printed and offline forms. Obtain executive approval, assign decision rights, and exercise the plan regularly with clinical leaders—not only with the security team. The plan should state how patient safety, care delivery, privacy, legal review, public communications, and restoration priorities are balanced. CISA’s planning guidance is available at cisa.gov/stopransomware/ransomware-guide.

Build an asset and dependency inventory
Maintain a complete, current list of endpoints, servers, network segments, applications, medical devices, electronic PHI repositories, owners, technical dependencies, and recovery priorities. The 405(d) ransomware guidance specifically calls for a complete and updated asset inventory: 405(d) ransomware guidance. Include vendor-managed systems and identify which clinical process fails if each asset is unavailable.
Reduce the blast radius
- Segment critical and vulnerable systems so compromise of a user workstation does not provide unrestricted access to clinical or backup networks.
- Enforce least privilege, remove unnecessary administrative rights, and review privileged accounts.
- Restrict remote access to approved paths with strong authentication, logging, and time-limited access where feasible.
- Deploy endpoint detection and remediation controls and ensure alerts reach a staffed response function.
- Document firewall, identity, application, and medical-device dependencies so isolation can be performed quickly and safely.
Make backups recoverable, not merely present
Maintain offline or otherwise isolated backups, using immutability where it fits the environment, and encrypt backup data. Test restoration on a schedule and scan backup data and restoration media for malware before use. CISA cautions that immutable cloud storage can introduce compliance and configuration implications; review retention, access, encryption, and deletion settings rather than treating “immutable” as a complete control. See CISA’s guide and its advisory at cisa.gov/news-events/cybersecurity-advisories/aa21-291a.
Preassign contacts and authority
Keep current contact details and alternates for executives, IT and security, clinical operations, privacy, legal counsel, communications, cyber insurance, critical vendors, law enforcement, CISA, and HHS/OCR. Define who can authorize network isolation, downtime declarations, public statements, restoration, and notifications. Store the contact list offline.

Detect and analyze the attack
Activate the plan on suspicion
Do not wait for a complete diagnosis. A mass file-renaming event, ransom note, disabled security tool, unusual administrative activity, or simultaneous outages across departments is enough to activate the incident process. Early containment can prevent additional encryption and data theft.

Establish scope and attack path
Determine which systems, networks, applications, accounts, medical devices, and ePHI repositories are affected. Establish the likely start time, entry method, current attacker access, propagation path, encryption status, and whether data may have been exfiltrated. Preserve authentication, endpoint, firewall, VPN, cloud, application, and backup logs according to legal and forensic direction. Record assumptions separately from confirmed facts.
Choose an isolation method deliberately
The safest choice depends on spread, clinical dependency, and available staffing. The following comparison describes operational trade-offs; it is not a substitute for the incident commander’s real-time risk decision.
| Isolation option | Containment speed | Patient-safety impact | Forensic depth | Best use |
|---|---|---|---|---|
| Disconnect individual endpoints | Fast for a small, known set | Usually limited, but a workstation may support a critical workflow | Can preserve the device for collection if handled correctly | Clearly identified infected devices with no evidence of wider spread |
| Isolate a switch, VLAN, or network segment | Fast for multiple related systems | Potentially high if clinical and nonclinical assets share the segment | Preserves a broader picture when logs and traffic are retained | Several hosts or subnets show related indicators |
| Disable remote access or compromised identities | Fast for stopping a known access path | Can interrupt vendor support, telehealth, or remote clinical work | Retains affected systems for analysis | Evidence points to stolen credentials or an exposed remote-access route |
| Shut down a critical service | Immediate reduction of activity | Highest; may create direct care and safety risks | May lose volatile evidence and operational context | Only when continued operation presents a greater immediate danger and clinical leadership approves |
When several systems or subnets are affected, network-level isolation is generally more consistent than visiting devices one by one. Coordinate with clinical engineering before disconnecting medical devices; document any device that cannot be isolated immediately and apply compensating controls.

Use verified, minimal communications
Prepare internal and external holding statements in advance. State only confirmed facts, the services currently affected, and the actions patients and staff should take. Do not include PHI, unverified attribution, speculative timelines, or technical details that could help an attacker. HHS OCR’s checklist covers coordination with law enforcement and the content of reports: HHS OCR cyber-attack checklist.
Report to appropriate authorities
Contact the FBI or U.S. Secret Service field office and report to CISA through current official channels when appropriate. CISA, the FBI, and NSA recommend prompt reporting; the advisory is at cisa.gov/news-events/cybersecurity-advisories/aa21-291a. Coordinate law-enforcement engagement with counsel and preserve the information requested without delaying patient-safety actions.
Eradicate the attacker and remediate weaknesses
- Find the exploited weakness. Identify the vulnerable service, stolen credential, malicious attachment, exposed remote-access route, or other entry point. Close or mitigate it before reconnecting systems.
- Remove persistence. Investigate scheduled tasks, unauthorized accounts, startup items, remote tools, scripts, and other mechanisms that could restore attacker access.
- Rotate credentials and keys. Change affected user, service, administrator, API, VPN, encryption, and backup credentials. Revoke tokens and certificates where compromise is possible, and do so in an order that does not lock out emergency operations.
- Rebuild compromised hosts. Use trusted images and known-good configuration sources rather than assuming an apparently cleaned host is safe.
- Recheck controls. Verify endpoint protection, identity policies, network segmentation, logging, application controls, and medical-device safeguards before production reconnection. HHS and CISA remediation guidance is available through the HHS fact sheet and CISA guide.
Scan backups and restoration media for malware. CISA, the FBI, and NSA strongly discourage paying ransom; payment does not guarantee decryption, deletion of stolen data, or safe recovery. Any decision involving payment, sanctions, insurance, or negotiations requires legal and executive review.

- Give staff one authoritative update channel and a way to report unsafe workarounds.
- Prioritize emergency care, medication administration, diagnostics, operating rooms, intensive care, and communications according to the organization’s clinical risk assessment.
- Protect paper records and temporary electronic stores as PHI, with access controls and a documented destruction or scanning process.
Recover from clean backups and validate every system
Set a restoration order
Use the agreed recovery priorities in the asset inventory and contingency plan. Restore identity and core network services only when their rebuilt controls are verified; then restore the applications and data needed for the highest-risk clinical workflows, followed by supporting and administrative services. The exact order is facility-specific and should be approved by IT, security, and clinical owners.
Use an isolated recovery environment
- Quarantine backup sources and restoration media until malware scans are complete.
- Rebuild the target environment from trusted images and hardened configurations.
- Restore a limited data set first and test authentication, permissions, interfaces, timestamps, clinical calculations, and audit logging.
- Compare restored records with independent sources where available and investigate missing, altered, duplicated, or unexpectedly changed data.
- Expand restoration in controlled batches, monitoring endpoint, identity, network, and application telemetry for reinfection or renewed unauthorized access.
Require clinical-owner sign-off
A system is not ready merely because it starts. The technical owner should verify security controls and data integrity; the clinical owner should verify that workflows, interfaces, orders, results, and patient-safety checks function correctly. Obtain documented approval before returning each service to production. Continue heightened monitoring after reconnection and keep downtime procedures available until operations are stable.
The 405(d) guidance recommends proven, tested backups and a ransomware recovery playbook exercised regularly: 405(d) ransomware guidance.

Complete the four-factor PHI risk assessment
Assess:
- the nature and extent of the PHI involved, including the types of identifiers and the likelihood of re-identification;
- the unauthorized person or persons who used or received the PHI;
- whether the PHI was actually acquired or viewed; and
- the extent to which the risk was mitigated.
Encryption of files does not establish that no PHI was acquired or viewed. Investigate exfiltration indicators, attacker statements, cloud and endpoint logs, account activity, and the time attackers had access. Document evidence, uncertainties, mitigation, and the rationale for the final determination.
Apply all applicable reporting rules
Notification obligations depend on the facts and may arise under the HIPAA Breach Notification Rule, state breach laws, contracts, cyber-insurance policies, and sector-specific or government requirements. Notification timing, recipients, and content vary. Work with privacy counsel and use current HHS/OCR instructions rather than relying on an old checklist; the OCR checklist remains a useful coordination aid at hhs.gov/sites/default/files/cyber-attack-checklist-06-2017.pdf.
Document evidence, decisions, and costs
Maintain a protected incident record containing the timeline, affected-asset list, indicators, forensic images, log exports, chain-of-custody records, containment changes, downtime actions, restoration tests, approvals, communications, risk assessment, notifications, and vendor work. Keep contemporaneous notes of who authorized each action and why. Separate attorney work product where counsel directs, and preserve records according to legal holds and retention requirements.
Recommended Free Tools
Track operational impact as well as technical work: departments on downtime, delayed or diverted care, unavailable interfaces, staff hours, restoration milestones, data-validation exceptions, and expenses. These records support patient-safety review, regulatory response, insurance claims, and improvements to the recovery plan.
Close the loop after restoration
- Complete a root-cause and attack-timeline analysis.
- Update the security risk analysis and asset inventory with what the incident revealed.
- Remediate control gaps, including segmentation, privilege, remote access, detection, backup isolation, logging, and vendor access.
- Revise the response, downtime, communications, and recovery playbooks.
- Run a tabletop exercise and a technical recovery exercise; test both executive decisions and hands-on restoration.
- Verify that lessons are assigned to owners with deadlines, then report completion to executive leadership and the appropriate governance committee.
CISA and HHS both emphasize exercising plans and improving controls after an incident. An offline playbook that staff have practiced is more useful during an outage than an untested document.

What the available frequency figure does—and does not—show
HHS’s ransomware fact sheet quotes a U.S. government interagency report estimating an average of 4,000 ransomware attacks per day since early 2016. That is a historical, all-sector figure, not a current healthcare-specific rate or loss estimate. The authoritative material used for this checklist does not establish a newer healthcare-specific frequency or financial-loss figure, so organizations should not use the historical number as a current risk forecast.
