Retrieval-augmented generation (RAG) gives a language model relevant evidence from an external collection before it writes an answer. A typical system breaks documents into passages, turns passages and a user query into embeddings, searches an index, optionally combines that search with BM25 keyword matching, reranks the candidates, and places the best passages in the model prompt.
RAG matters because a model can only reason over information it receives in its prompt. Retrieval can add current, private, or domain-specific material, while citations and source metadata let you check where an answer came from. The difficult design decisions are not simply whether to use RAG, but how to chunk documents, which retrieval signals to combine, whether a reranker earns its latency, and how many passages fit the model’s context budget.
What RAG does
RAG separates knowledge access from language generation. The retrieval side searches a corpus such as manuals, policies, tickets, or research papers. The generation side receives the user question plus selected passages and produces an answer grounded in that material. If the corpus changes, you can update the index without retraining the language model.

A useful mental model is a funnel. The first stage favors recall: it gathers a broad set of passages that might answer the question. Later stages favor precision: they remove weak or redundant candidates so the generator sees a compact, relevant context. Every passage sent to the model consumes tokens that could otherwise be used for the question, instructions, or answer.

How an end-to-end RAG pipeline works
1. Prepare and chunk the corpus
Documents are divided into passages small enough to retrieve and place in a prompt. Chunk boundaries should follow meaning where possible: a section, procedure, table, or argument is usually better than an arbitrary cut through a sentence. Overlap can preserve context at boundaries, but excessive overlap creates duplicate results and wastes index space and prompt tokens.
Store metadata with every chunk. Useful fields include document title, section heading, page or paragraph position, publication date, access permissions, and a stable source identifier. Metadata supports filtering, access control, citation rendering, and restoration of the original order. There is no universally optimal chunk size or overlap; evaluate those settings on your own corpus.
2. Create embeddings and an index
An embedding model maps each chunk to a numerical vector in which related meanings tend to be close together. The user query is embedded in the same space, and a vector index returns passages with high similarity. This lets a search for “reset a forgotten password” find a passage that says “recover account credentials” even when the wording differs.
Recommended Free Tools
Semantic similarity is not the same as exact matching. Embeddings can miss product codes, legal clause numbers, names, error strings, and other identifiers whose precise characters matter. Keep the original text and metadata alongside vectors so the selected evidence can be displayed and cited later.

3. Add lexical retrieval when exact wording matters
BM25 is a lexical ranking method that rewards matching terms while accounting for term frequency and document length. It is useful for identifiers, model numbers, uncommon names, quoted phrases, and error messages. A hybrid retriever runs dense and BM25 searches, then combines their rankings, often with a fusion method such as reciprocal-rank fusion.
Hybrid retrieval is not automatically better for every corpus. It adds an index and tuning path, but it protects against the complementary weaknesses of semantic and lexical search. Measure whether the extra signal improves recall on the questions your users actually ask.
4. Retrieve a broad candidate pool
The first-stage search should normally return more passages than you intend to place in the final prompt. A wider pool gives a later reranker room to find a passage that was not ranked first by the initial similarity function. Apply metadata filters early when permissions, product version, language, or date are part of the question; filtering after retrieval can expose irrelevant or unauthorized material to later stages.
5. Rerank the candidates
A reranker is a second-stage model that reads the query and each candidate passage together and assigns a relevance score. It can recognize that a passage directly answers the question even when its embedding similarity is merely moderate. The system then forwards only the top-ranked set to the generator.

Reranking improves precision and can reduce the amount of context sent to the language model, but it is not free. Each candidate requires additional model work, so latency and inference cost rise with the candidate pool. Use it when measured relevance gains justify that overhead, rather than treating it as a mandatory component.
6. Assemble context and generate
Arrange the selected passages with clear delimiters and source metadata. Preserve document order when relationships between neighboring passages, steps in a procedure, or an argument’s progression matter. The prompt should tell the model what the passages are, how to handle conflicts, and whether it must cite sources. Retrieved text must fit inside the model’s maximum sequence length together with the user request, instructions, and expected response.
Embeddings, BM25, and hybrid retrieval compared
| Signal | What it matches well | Typical weakness | Best use |
|---|---|---|---|
| Dense embeddings | Related meaning and paraphrases | Exact identifiers, rare strings, and wording-sensitive queries | Semantic recall across varied language |
| BM25 lexical search | Exact terms, names, codes, and quoted phrases | Synonyms and paraphrases that share few words | Identifier-heavy or wording-sensitive corpora |
| Hybrid fusion | Both semantic and lexical evidence | More indexing and ranking complexity | General-purpose search where either failure mode is costly |
Do not select an embedding model by reputation alone. Compare retrieval recall on a representative question set, including paraphrases, short keyword queries, misspellings, and questions that require two related passages. The right choice depends on language, document style, update frequency, and the cost of a missed passage.

What a reranker changes
Without reranking, the generator receives the top results from the first-stage score. With reranking, retrieval is deliberately split into recall and precision stages:

- Run dense, BM25, or hybrid search to collect a broad candidate set.
- Score each candidate with a query-document relevance model.
- Remove duplicates and apply permission or metadata checks.
- Pack the highest-value passages into the available token budget.
The gain should be measured at the same final context size. A reranker may improve top-K relevance while adding enough latency to hurt an interactive application. It can also amplify a poor first stage: a passage that was never retrieved cannot be rescued by reranking. Tune candidate-pool size, reranker top-K, and packing policy together.
Context windows: capacity is not comprehension
A context window is the maximum token budget for the complete request: system and developer instructions, user text, conversation history, retrieved passages, and the generated response. A model advertised with a long context can process tens of thousands of tokens, but that does not mean every included passage will influence the answer equally.
Long context changes the trade-off rather than eliminating retrieval. You may be able to pass a larger document or more candidate passages directly, yet irrelevant text still consumes tokens, increases latency, and creates more opportunities for conflicting instructions or facts. Retrieval remains useful for selecting evidence, enforcing access controls, attaching citations, and keeping prompts focused.

Manning, and Nelson F. Liu found that a straightforward, structure-preserving retrieve-then-read baseline could match or outperform more elaborate methods on multiple long-context question-answering benchmarks when token budgets were matched. The practical lesson is to compare systems fairly: give each approach the same token allowance and evaluate answer quality, rather than assuming a more complex pipeline or a larger window will win.

How many chunks should go in the prompt?
There is no universal top-K value. The correct number is the largest set of passages that adds reliable evidence without crowding out the response or introducing noise. Choose it with a token budget, not a fixed chunk count.
- Check the model’s current maximum sequence length for the exact provider and version you use.
- Reserve space for system instructions, the user’s question, conversation history, and a response of the required length.
- Subtract that overhead from the maximum to obtain the retrieval budget.
- Retrieve a wider candidate pool than the final budget can hold.
- Rerank when appropriate, then add passages in score order until the budget is reached.
- Deduplicate overlapping chunks and prefer complementary evidence over near-identical passages.
- Keep source identifiers and section order attached to every passage.
For short factual questions, one highly relevant passage may be enough. A troubleshooting answer may need a symptom, a prerequisite, and a recovery procedure from separate sections. A policy question may require the current rule plus an exception. Evaluate several top-K values on real questions and record both retrieval recall and final answer quality; do not infer the best setting from an isolated example.
RAG versus simply using a long-context model
| Design | Evidence selection | Context sent to the model | Primary advantage | Primary trade-off |
|---|---|---|---|---|
| Dense-only RAG | Embedding similarity | Top semantic matches | Finds paraphrased concepts | Can miss exact strings and identifiers |
| Hybrid RAG | Dense plus BM25 rankings | Fused top matches | Covers semantic and lexical queries | More indexing and fusion to maintain |
| Hybrid RAG with reranking | Broad retrieval followed by a second-stage model | Small, high-precision set | Better relevance per prompt token | Additional latency and model cost |
| Long-context retrieve-then-read | Structure-preserving retrieval with minimal stages | Larger ordered context | Simple pipeline that can perform strongly at a matched budget | Still limited by context capacity and irrelevant-text noise |
| Full-document prompting | Little or no selective retrieval | As much of the document as fits | Simple for small, stable documents | Scales poorly as collections grow and may waste tokens |
Use long context when the source set is small enough, relationships across sections are important, and the latency and token cost are acceptable. Use staged RAG when the collection is large, changes frequently, contains access restrictions, or requires precise citations. A measured hybrid design can combine both: retrieve a focused set, preserve its structure, and give the model enough room to read it.

- Define the evaluation set. Collect representative questions and label which passages contain sufficient evidence. Include exact-term, paraphrase, multi-hop, and no-answer cases.
- Clean and segment documents. Remove navigation clutter, retain headings and tables, choose an initial overlap policy, and store stable metadata.
- Build dense retrieval. Embed chunks and queries with the same embedding model, then measure recall at several candidate-pool sizes.
- Add BM25 for lexical coverage. Test questions containing codes, names, quoted text, and error messages. Fuse rankings only if the combined system improves held-out results.
- Introduce reranking selectively. Compare answer relevance and latency with and without the second-stage model. Start with a broad candidate pool and a small final set.
- Implement token-aware packing. Reserve output space, remove duplicates, preserve source order where needed, and stop adding passages when the budget is full.
- Design citations and conflict handling. Give each passage a source label, instruct the generator to distinguish evidence from inference, and define what it should say when sources disagree or no passage answers the question.
- Run end-to-end tests. Measure retrieval and generation together after every change to chunking, embeddings, fusion, top-K, reranking, or prompt format.
How to evaluate a RAG system
Retrieval recall asks whether the necessary passage appeared in the candidate set or final context. Track recall at the candidate and final K values separately; a strong reranker cannot compensate for a low-recall first stage. For generation, assess factual support, completeness, citation correctness, refusal on missing evidence, and resistance to irrelevant or conflicting passages.
Latency should be broken into embedding or query-processing time, vector and lexical search, fusion, reranking, prompt construction, and generation. Cost depends on the number of candidates reranked and the number of context tokens generated and read. Compare architectures with matched token budgets and the same evaluation questions.
Anthropic reported experiments in 2024 across several domains and configurations that reduced the top-20 retrieval failure rate by 35% with contextual embeddings, 49% with contextual embeddings plus contextual BM25, and 67% when reranking was added as well. These are reported experimental results, not universal benchmarks; your corpus, chunking, query mix, and model can produce different outcomes. The same source recommends evaluating the chosen chunking, embedding, top-K, and reranking settings on the target corpus.
Common failure modes and fixes
| Symptom | Likely cause | Fix to test |
|---|---|---|
| The answer uses a related but wrong section | Semantic retrieval lacks exact constraints or metadata filters | Add BM25, filter by version or date, and improve metadata-aware prompting |
| A required code or error string is missing | Embedding similarity ignored the exact token | Use lexical retrieval or hybrid fusion |
| The right passage is retrieved but ignored | Too many candidates, duplicate chunks, or poor ordering | Rerank, deduplicate, preserve structure, and reduce final K |
| Latency is unacceptable | Candidate pool or reranker workload is too large | Filter earlier, reduce candidates, cache embeddings, or remove reranking where its gain is small |
| Answers cite stale policy | Old and current versions are mixed in the index | Store effective dates, filter by current version, and expose source metadata |
| Prompt exceeds the context limit | Fixed chunk count ignores token length and response reserve | Pack by measured tokens and reserve output capacity before retrieval |
| The model invents an answer when evidence is absent | No explicit no-answer behavior or evaluation case | Instruct it to state that the supplied sources do not establish an answer and test that behavior |
Decision checklist
- Are chunks coherent, attributable, and tagged with version and access metadata?
- Do users ask for paraphrases, exact identifiers, or both?
- Have dense, BM25, and hybrid recall been measured on representative questions?
- Does reranking improve final answer quality enough to justify its latency and cost?
- Is context packed by tokens rather than an arbitrary number of chunks?
- Are source order, citations, conflicts, and no-answer cases handled explicitly?
- Were long-context and RAG alternatives compared with the same token budget?
Bottom line
RAG is a retrieval-and-generation system, not a single model feature. Embeddings provide semantic recall, BM25 protects exact terminology, reranking raises precision at an added cost, and the context window sets a hard budget for evidence and output. Start with clean, metadata-rich chunks; measure dense and lexical retrieval on your corpus; add reranking only when its measured gain is worth the delay; and select the final passages with a token-aware, structure-preserving policy. A larger context window gives you more capacity, but careful evidence selection is still what makes answers focused and verifiable.
