A RAG app can run without errors and still return wrong, vague, or unsupported answers. The quickest way to debug it is to separate retrieval from generation: first check whether the system found useful evidence, then check whether the model used that evidence correctly. Don’t change chunking, embeddings, prompts, or models until you know which stage failed.
Why a working RAG pipeline can still fail
Retrieval-augmented generation adds a retrieval stage to the model’s response process. The system must find relevant material, provide it to the model, and produce an answer that addresses the question and stays grounded in that material. A failure at any point can look like a bad answer.
The retrieved context may be irrelevant, incomplete, or noisy. Even when it contains useful evidence, the model may misunderstand it, ignore it, or make claims it does not support. RAG therefore does not guarantee hallucination-free output. OpenAI’s optimization guide treats retrieval and model behavior as distinct parts of the problem; its PDF parsing example also illustrates how source material can affect what the model receives.
That distinction matters because the same symptom can have different causes. If the answer omits a fact, the evidence may never have been retrieved—or it may have been retrieved but lost in the model’s response. Changing a prompt will not fix missing evidence, and changing retrieval will not necessarily fix a model that mishandles good context.

Start with a repeatable set of failed questions
Choose a small set of questions that have actually failed. For each, save the question and, where possible, the expected answer or the source passage that supports it. This gives you a consistent way to inspect the pipeline and compare changes instead of relying on one successful demo.
- Include examples of the real failure types you need to fix, such as an incorrect answer, a vague answer, or an unsupported claim.
- Record the expected evidence when you can identify it in your source material.
- Keep the same cases when testing a change, so the comparison is about the system rather than a different set of questions.
There is no single metric that answers every debugging question. LlamaIndex’s evaluation concepts distinguish among retrieval and response dimensions, while its evaluation documentation describes evaluating retrieval against expected document IDs and assessing response faithfulness to context.

Debug retrieval before changing generation
Run the retrieval stage by itself for one failed question. Inspect the returned passages, their source metadata, and their order. Ask whether the passages contain evidence that can answer the question—not merely whether they look topically related.
Recommended Free Tools
- Expected evidence is missing: Investigate document ingestion and parsing, chunk boundaries, filters, query construction, and retrieval settings. A generation prompt cannot cite evidence the model never received.
- Evidence is present but buried: Look for irrelevant, redundant, or conflicting passages that may crowd out useful material. Check the ordering and the actual context assembled for the model.
- The retrieved material is relevant and sufficient: Move on to the generation stage rather than tuning retrieval blindly.
The right checks and settings depend on your implementation; the title does not establish a particular framework, vector database, embedding model, or document format. Treat retrieval hit or relevance as a diagnostic question, not proof that one universal chunk size or retrieval strategy is best.

Inspect the exact context sent to the model
When retrieval finds useful evidence, examine the context as it reaches the model. The retrieved results and the final model input may differ. Check whether relevant passages were truncated, whether excess noise remains, and whether the context contains conflicting information.
Also inspect the instructions that accompany that context. If the response should be grounded in the supplied material or should abstain when the material does not answer the question, make that expectation clear. Then compare the resulting answer with both the user’s question and the context the model actually received.

Evaluate the answer on separate dimensions
Do not treat “good answer” as a single score. Each dimension points to a different failure:
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errors- Context relevancy: Is the retrieved material useful for answering the query, or does it add distraction?
- Faithfulness: Are the answer’s claims supported by the supplied context?
- Answer relevance: Does the response address the question the user asked?
- Correctness: Does the answer match a trusted reference, when you have labeled answers available?
These checks are related but not interchangeable. An answer can be faithful to irrelevant passages and still fail to answer the question. It can address the question while making a claim unsupported by the context. Correctness against a reference requires a reference to compare against; faithfulness alone does not establish that the source material itself is correct.
Change one component, then rerun the same cases
- Identify the failure stage from the retrieved passages and the exact model context.
- Choose a change that targets that stage—for example, investigate parsing or retrieval when evidence is absent, or the context assembly and grounding instructions when useful evidence is present but mishandled.
- Rerun the same questions and compare retrieval, context noise, faithfulness, answer relevance, and correctness where reference answers exist.
- Keep the cases that exposed the issue as a regression set, and repeat the process for the next change.
Instrumentation and metric choices depend on the stack. The useful principle is to measure what changed at the stage you intended to improve. OpenAI’s optimization guide puts it plainly: “The key thing to take away here is that the principle remains the same from our mental model at the beginning – you evaluate to find out what has gone wrong, and take an optimization step to fix it.”
