An endometrioma (an ovarian endometriotic cyst) does not automatically require an operation. The right decision depends on your symptoms, pain priorities, plans for pregnancy, ovarian reserve, age, previous surgery, other fertility factors, cyst findings and your preferences. Use the appointment to compare surgery with monitoring, medical treatment and fertility care rather than discussing the operation in isolation.

The National Institute for Health and Care Excellence (NICE) states: “Ask women with suspected or confirmed endometriosis about their symptoms, preferences and priorities with respect to pain and fertility, to guide surgical decision-making.” (NICE recommendation 1.9.1) The questions below are a preparation guide, not a personal recommendation to have or avoid surgery.

Start by clarifying why surgery is being considered

Ask your clinician to explain the specific problem the operation is intended to solve. A cyst’s size or an endometriosis stage alone does not settle the decision. Pain, fertility goals, imaging findings, previous treatment and the likely effect on ovarian function all matter.

  • What is the main reason you recommend surgery in my situation? Ask whether the driver is pain, a change on imaging, concern about the cyst’s nature, difficulty accessing the ovary for fertility treatment, infertility, or another finding.
  • What benefit do you expect, and how certain is that benefit? Ask which symptoms are likely to improve, which may not be caused by the endometrioma, and how success will be assessed.
  • What are my reasonable alternatives? Ask about observation with follow-up, pain treatment, hormonal treatment, fertility treatment without surgery, or referral for another opinion. Request the expected advantages and disadvantages of each option for your circumstances.
  • How do my priorities change the recommendation? Tell the clinician whether your immediate priority is pain control, trying to conceive soon, preserving future fertility, avoiding another operation, or balancing several goals.
  • What happens if I wait? Ask what change in symptoms, examination or imaging would lead you to reconsider, and whether waiting could affect later treatment or fertility care.

NICE recommends discussing symptoms, preferences, pain and fertility when making surgical decisions. It also advises considering follow-up for confirmed endometriosis, particularly when an endometrioma is larger than 3 cm, including for people who do not have surgery. That figure is a follow-up consideration in the guideline, not a universal surgery threshold.

Ask what the scans show and who should be involved

Your scan may show more than an ovarian cyst. Understanding the distribution of disease helps determine the safest team and the scope of an operation.

  • What exactly do my ultrasound or MRI findings show? Ask for the cyst’s side, number, size and appearance, and whether there are signs of endometriosis elsewhere.
  • Do the images suggest bowel, bladder or ureter involvement? If disease may extend beyond the ovary, ask whether a specialist endometriosis service or another surgical specialist should participate.
  • Is the diagnosis sufficiently clear for surgery? Ask what alternatives are being considered if the imaging is uncertain and whether tissue would be sent for laboratory examination.
  • How experienced is the proposed surgeon and team with endometrioma and endometriosis surgery? Ask how often they perform the proposed technique and how they manage disease outside the ovary.
  • Would a fertility specialist be part of the discussion? NICE recommends multidisciplinary input, including a fertility specialist and access to fertility services, for endometriosis-related subfertility.

Compare the proposed operation with the alternatives

Ask the surgeon to name the exact technique rather than using “cyst removal” as a general label. Commonly discussed approaches include cystectomy (excision of the cyst wall) and drainage with ablation or electrocoagulation of the lining. Their trade-offs are not identical.

Decision factor Cystectomy or excision Drainage with ablation/electrocoagulation
Why it may be proposed Removal of the cyst wall; ESHRE recommends considering cystectomy rather than drainage and electrocoagulation when surgery is indicated for recurrence and pain prevention. NICE notes that drainage and ablation may preserve ovarian reserve more than cystectomy in fertility-priority decisions.
Pain and recurrence ESHRE identifies cystectomy as the preferred approach over drainage and electrocoagulation for reducing recurrence and pain when surgery is indicated, while still weighing ovarian-reserve risk. Ask how the proposed method affects the chance of persistent symptoms or repeat surgery in your case; a patient-specific percentage is not established by the cited guidelines.
Ovarian reserve Removing cyst tissue can reduce ovarian reserve, so the surgeon should explain how much ovarian tissue may be affected and how that risk applies to you. NICE says this approach may preserve reserve more than cystectomy for some fertility-priority decisions, but it has different recurrence and pain considerations.
Fertility and assisted reproduction Ask whether removing the cyst is expected to improve access or outcomes for your planned fertility treatment, and whether the possible reserve reduction could offset that benefit. Ask whether leaving or treating residual cyst lining changes access to egg collection, symptoms or later treatment.
Recovery and follow-up Ask about the expected operation, restrictions, return to work and imaging or clinical follow-up; the guidelines do not provide one recovery timetable for every patient. Ask the same questions, including how recurrence would be monitored and what would trigger another intervention.

The table describes issues to discuss, not a universal winner. NICE recommends considering excision rather than ablation for endometriomas while taking fertility desire and ovarian reserve into account. ESHRE likewise advises considering reduced ovarian reserve when surgery is indicated. Ask, “Why is this technique preferable for me, given my pain, fertility plans and reserve?”

Discuss ovarian reserve and fertility before consenting

Endometriosis itself and surgery can affect ovarian reserve, the pool of follicles that may contribute to future egg production. The concern is particularly important with severe disease or endometriomas on both ovaries. ESHRE reports that severe endometriosis, especially bilateral endometriomas, is associated with a higher risk of premature ovarian insufficiency and lower anti-Müllerian hormone (AMH), and that surgery can further affect reserve and AMH.

  • How might the endometrioma itself affect my reserve and fertility? Ask the clinician to distinguish the possible effect of the disease from the additional effect of operating.
  • How might this operation affect both ovaries? Bilateral disease, a single functioning ovary, prior operations or limited ovarian tissue may change the balance of benefit and risk.
  • Does my age, previous ovarian surgery or another fertility factor alter the plan? Bring records of prior procedures, pregnancies, fertility tests and treatments. ESHRE says age, preferences, previous surgery, other infertility factors, ovarian reserve and the estimated endometriosis fertility index should inform the decision.
  • Should I speak with a fertility specialist before choosing surgery? Ask whether consultation should happen before scheduling, particularly if you hope to conceive soon or may consider assisted reproduction.
  • Could fertility preservation be relevant for me? Ask what options exist locally, their timing, burdens, costs and uncertainties. ESHRE says the role of pretreatment AMH in predicting a future pregnancy or the need for fertility preservation is unclear; no single test can determine an individual’s ability to conceive.
  • If I am trying to conceive now, how does surgery compare with fertility treatment or proceeding without surgery? Ask for a plan that includes the other infertility factors affecting your chance of pregnancy, not only the cyst.
  • What can ovarian-reserve tests actually tell us? Ask which tests are proposed, how results could change management, and what they cannot predict about spontaneous conception, response to fertility treatment or the timing of menopause.

A lower AMH result is not a stand-alone instruction to operate or to cancel surgery. It is one piece of information that must be interpreted with your history, imaging, goals and the proposed procedure.

Ask about complications, consent and the possibility of further surgery

Request an explanation of common and serious complications in language you understand, including risks that could affect ovarian function or fertility.

  • What complications are possible during and after the operation? Ask about bleeding, infection, injury to nearby organs, anaesthetic problems, conversion to a different procedure and circumstances that could require an additional operation.
  • Could ovarian tissue be damaged or removed? Ask how the team protects healthy tissue, what would happen if bleeding is difficult to control, and whether the plan changes if both ovaries are involved.
  • What findings would mean further surgery may be needed? Endometriosis can recur, and complications or persistent disease may lead to another procedure. Ask what warning symptoms require urgent contact and who to call.
  • Will tissue be examined and when will I receive the result? Confirm how results will be communicated and whether they would alter follow-up.
  • What is the expected recovery for this operation? Ask about hospital stay, pain control, activity and lifting limits, driving, work, exercise, sex, wound care and the first review. Your surgeon should provide the timetable for your procedure rather than relying on a generic estimate.

Plan postoperative treatment around pregnancy timing

Tell the team whether you plan to attempt conception immediately, later, or not at all. That timing can affect the value of hormonal treatment and the need for contraception or symptom control.

  • If I am not trying to conceive immediately, should I consider hormonal treatment after surgery? ESHRE discusses postoperative hormonal treatment for people who cannot or do not plan to attempt conception soon. Ask which treatment, duration, side effects and monitoring are appropriate for you.
  • If I want pregnancy soon, when can I start trying? Ask what healing, follow-up or fertility referral is needed first.
  • How will pain be managed if symptoms persist? Ask what non-surgical and hormonal options remain available and how they fit with pregnancy plans.
  • What is the recurrence-monitoring plan? Ask whether follow-up is clinical, imaging-based or both, how often it is expected, and which changes should prompt reassessment.

  1. Set the follow-up interval. Confirm when the next appointment or scan is due and whether the “larger than 3 cm” NICE follow-up consideration applies to your situation.
  2. Record symptoms and changes. Note pain pattern, bleeding, bowel or bladder symptoms, medication effects and any effect on daily activities or attempts to conceive.
  3. Agree on reassessment triggers. Ask which new or worsening symptoms, examination findings or scan changes should lead to an earlier review.
  4. Keep fertility planning current. If pregnancy is a priority, ask when to involve a fertility service rather than waiting for symptoms to worsen.

Bring a concise decision record to the appointment

A written summary helps the consultation focus on choices rather than forgotten details. Bring your previous operation notes, scan reports, medication list, allergies, fertility investigations and the dates of prior pregnancies or treatments.

  • My three most troublesome symptoms and how they limit work, sleep, exercise or sex.
  • Whether pain relief, future pregnancy, immediate conception, ovarian preservation or avoiding repeat surgery is my leading priority.
  • All previous ovarian or endometriosis procedures, including which side was treated.
  • Questions about the proposed technique, expected benefit, reserve, complications, recovery and follow-up.
  • The name and contact details of the clinician or service to call after the visit.

Before consenting, ask the clinician to summarize the plan in your own terms: the reason for surgery, the alternatives, the technique, the principal ovarian-reserve and fertility trade-off, the recovery plan and what happens if you choose to wait. If those points remain unclear, ask for more time or a fertility-specialist or endometriosis-team opinion.
