If an imaging scan done for another reason shows a possible or confirmed unruptured brain aneurysm, use your appointment to establish exactly what was found, your individual rupture risk, whether monitoring or treatment is safer, and what follow-up you need. Bring the radiology report and images, a complete medication and substance-use list, your blood-pressure history, and information about relatives with aneurysms or aneurysmal subarachnoid hemorrhage. The questions below are designed for a conversation with a neurologist, neurosurgeon, interventional neuroradiologist, or the clinician coordinating your care.

First, clarify what “incidental” and “unruptured” mean

An incidental aneurysm is one identified on imaging performed for another reason. “Unruptured” means the vessel bulge has not been identified as having bled. Those terms describe how the finding was discovered and its current status; they do not by themselves show that the aneurysm is harmless, an emergency, or in need of immediate treatment.

A scan can suggest an aneurysm, but a clinician must interpret the images and report together with your history. Ask for a copy of both the written report and the actual images in the format your care team can review. If the report says “possible,” ask whether another imaging study or specialist review is needed to confirm the finding.

1. What exactly did the scan find?

Ask about the diagnosis

  • Is this definitely an intracranial aneurysm, or could the appearance represent a normal vessel, an imaging artifact, or another condition?
  • Which test found it, and is that test adequate for characterization?
  • Are there any other findings that need attention?

Ask for the measurements and anatomy

  • What is the maximum measured size, in millimeters?
  • Where exactly is it located, and which artery is involved?
  • What is its shape: regular, irregular, lobulated, or associated with a visible protrusion such as a daughter sac?
  • Is there evidence of blood, a clot, vessel narrowing, or another feature that changes management?

Mayo Clinic’s patient guidance identifies size, location, and appearance as central questions because those characteristics help clinicians assess treatment choices. Ask the clinician to explain which features matter in your case rather than relying on a size label alone.

2. How likely is my aneurysm to rupture?

Request an individualized explanation

Use the direct question, “How likely is my aneurysm to rupture?” The answer must be based on your imaging and medical history; the available patient information does not support a single percentage that applies to every incidental aneurysm. Ask what estimate or risk category your clinician uses, how confident they are, and what information could change it.

Ask what drives the estimate

Have the clinician discuss the factors that apply to you, including aneurysm size, location, shape, whether it has grown or changed, your age and general health, blood pressure, smoking, and relevant medical conditions. Tell them about previous scans, even if they were performed at another facility, because comparison over time can be important.

Family history can also affect the discussion. Report relatives with an intracranial aneurysm or aneurysmal subarachnoid hemorrhage, including how many relatives were affected and how closely related they are. Do not treat a general risk factor as a personal prognosis: only your care team can interpret these factors together.

Keep rupture statistics in context

Mayo Clinic reported that about 25% of people who experience a ruptured aneurysm die within 24 hours, and another 25% have complications that lead to death within six months. Those figures describe outcomes after rupture; they are not the probability that your unruptured aneurysm will rupture.

3. What do you recommend now, and why?

Ask for the decision logic

Ask, “What treatment do you recommend at this time? What are the risks of treatment?” Also ask what the likely risk would be if the aneurysm were monitored instead. The decision weighs the patient-specific risk of leaving the aneurysm untreated against the immediate and long-term risks of an intervention.

Understand the options your anatomy permits

Option What it involves Questions for your team
Monitoring Planned clinical review and repeat vascular imaging when your team considers appropriate. Why is observation reasonable for me? What change would trigger a different recommendation?
Surgical clipping An operation places a clip across the aneurysm’s neck to stop blood flow into it. Is the location and shape technically suitable? What are the operative risks, recovery expectations, and follow-up needs?
Endovascular coiling A catheter-based procedure places coils inside the aneurysm to reduce blood flow into it. Would coils provide durable protection in my anatomy? Could additional treatment or imaging be needed?
Flow diversion A catheter-based device redirects blood flow along the parent artery so the aneurysm can seal over time. Is this device appropriate for the artery and aneurysm? What medicines and follow-up are required?

These approaches are not interchangeable choices that a patient can select from a menu. Technical suitability depends on the aneurysm’s size, location, shape, relationship to nearby branches, and your overall health. Ask how recovery, repeat procedures, and long-term imaging differ for the options being considered.

Ask about experience and a second opinion

Ask whether a neurologist, neurosurgeon, or interventional neuroradiologist should review the case and how often the team treats aneurysms like yours. The American Heart Association/American Stroke Association (AHA/ASA) guidance recommends treatment at high-volume centers with aneurysm-treatment experience. A second opinion can be useful when the recommendation is uncertain, the proposed procedure carries substantial risk, or you want confirmation that monitoring is appropriate.

4. If we monitor it, what is the plan?

Make the surveillance schedule explicit

Ask which imaging method will be used, when the next scan should occur, who will review it, and how you will receive the result. CT angiography and MR angiography are identified in AHA/ASA guidance as sensitive approaches for detection and follow-up, but no single interval is appropriate for every person. Your clinician should set the timing based on the aneurysm and your circumstances.

Mayo Clinic poses the practical question: “If treatment is not indicated currently, how often will I need to have follow-up scans for the aneurysm?” Ask whether the same scanner or imaging protocol should be used so that changes can be compared reliably.

Define what would change management

  • What amount of growth or shape change would prompt a new consultation?
  • Would a new symptom, pregnancy, planned surgery, or change in blood pressure alter the plan?
  • Who should I contact if an imaging appointment or result is delayed?
  • Will the plan be documented in writing, including the next scan date?

5. What should I do about blood pressure, smoking, medicines, and substances?

Address modifiable risks with the right clinician

Ask what blood-pressure target is appropriate for you, how often to measure it, and which clinician will adjust treatment. Bring home readings if you have them. NINDS patient information specifically advises addressing high blood pressure and making a plan to stop smoking or using drugs such as cocaine.

Tell the team about every prescription medicine, over-the-counter product, supplement, alcohol use, nicotine product, and recreational drug. Do not stop or change a prescribed medicine on your own; ask the clinician who manages it whether any adjustment is needed before imaging, a procedure, or observation.

Turn advice into a practical plan

  • Write down your current blood-pressure medicines and recent readings.
  • Ask for smoking-cessation support, medication, counseling, or a referral if relevant.
  • Disclose cocaine or other stimulant use honestly so the team can address risk and treatment safely.
  • Ask whether exercise, work, travel, or other activities require restrictions; do not assume restrictions without individualized advice.

6. Does my family history matter?

Tell your clinician about every relative you know to have had an intracranial aneurysm or aneurysmal subarachnoid hemorrhage. Include the relationship, approximate age at diagnosis or rupture, and whether more than one relative was affected.

The AHA/ASA summary states that screening should be offered to patients with two or more family members with intracranial aneurysm or subarachnoid hemorrhage. That is a specific guideline statement, not blanket advice for every relative of one affected person. Ask whether your family pattern meets that threshold and whether CT or MR angiographic screening should be discussed with relatives.

7. Who should be involved in my care?

Identify the decision team

Ask whether your case should be reviewed by a multidisciplinary team and which specialist will coordinate communication. Mayo Clinic identifies neurologists, neurosurgeons, and interventional neuroradiologists as clinicians who may participate in decisions. Your primary-care clinician may also help manage blood pressure, smoking cessation, and other health conditions.

Check the center’s experience

Ask how frequently the center performs the proposed procedure and whether it has experience with aneurysms in your location and shape. If the recommendation is observation, ask who will compare future images and how the result will be communicated. You should leave knowing the responsible clinician, the next action, and how to reach the team.

8. What symptoms mean I should seek emergency help?

Ask your clinician for individualized written instructions about symptoms that require emergency evaluation and what number to call. The patient materials summarized here focus on assessment of an unruptured aneurysm and do not provide a complete emergency symptom checklist. Do not wait for a routine appointment if you develop sudden or severe new symptoms; seek urgent medical evaluation and tell emergency staff that an aneurysm has been identified.

How to prepare for the appointment

  1. Collect records: Request the radiology report and image files from the facility that performed the scan. Include older head or neck scans if available.
  2. Prepare a one-page history: List blood-pressure diagnoses and readings, smoking or nicotine use, cocaine or other drug use, prior neurologic conditions, and major medical problems.
  3. List everything you take: Include drug names, doses, schedules, vitamins, supplements, and occasional medicines.
  4. Document family history: Record relatives with aneurysms or aneurysmal subarachnoid hemorrhage and approximate dates.
  5. Bring questions in writing: Use the eight sections above and leave space for the answers, the clinician’s name, and the next appointment.
  6. Confirm the follow-up: Before leaving, obtain the planned imaging method, timing, reviewing clinician, and the change that would prompt reassessment.

Questions that help compare monitoring with a procedure

Decision point Ask about monitoring Ask about intervention
Risk without treatment What is my estimated rupture risk, and what features support it? How does that risk compare with the risk of the proposed procedure?
Technical suitability What imaging features make observation reasonable? How do size, location, shape, and nearby branches affect clipping, coiling, or flow diversion?
Future care What scans and visits are needed, and what change would end observation? What recovery, medicines, repeat procedures, and follow-up imaging should I expect?
Provider experience Who will interpret interval changes? How experienced is the treating center with this procedure and aneurysm type?
Personal context How do my age, health, preferences, and family history affect observation? How do those same factors alter procedural benefits and harms?

Ask the clinician to state the recommendation in plain language and to record why it fits your situation. If you do not understand a term, ask for a drawing or a written explanation of the artery, aneurysm, and proposed plan.

What you should leave the visit knowing

  • Whether the scan confirms an aneurysm and its measured size, location, shape, and other relevant features.
  • Your clinician’s individualized explanation of rupture risk and the factors that influence it.
  • Why monitoring, clipping, coiling, flow diversion, or another approach is or is not being considered.
  • The imaging method, next-scan timing, responsible reviewer, and findings that would change the plan.
  • Specific instructions for blood pressure, smoking cessation, medicines, and substance use.
  • Whether your family history merits a screening discussion.
  • Which specialist or center is responsible and how to obtain urgent help for sudden or severe new symptoms.

An incidental, unruptured aneurysm requires context rather than an automatic treatment decision. The safest next step is a documented plan made with clinicians who can review your images, health history, and preferences together.
