PSMA PET can find some prostate-cancer deposits that CT and bone scans do not show, especially when staging high-risk disease or investigating a rising PSA after treatment. That higher detection rate does not automatically prove that changing treatment because of the scan improves survival or quality of life. The right comparison depends on the clinical question, the tracer used, the reference test, and whether a negative result can safely exclude disease.
What the comparison means
In prostate-cancer care, “conventional imaging” usually means contrast or noncontrast CT of the abdomen and pelvis plus a technetium bone scan for metastatic staging. MRI is a separate modality, often used for local assessment of the prostate and pelvis, and may be performed alongside either conventional imaging or PSMA PET.
PSMA PET combines a positron-emitting radioligand with PET imaging, commonly acquired together with CT. The ligand binds to prostate-specific membrane antigen (PSMA), a protein found at high levels on many prostate-cancer cells. Areas of increased tracer uptake can identify disease in lymph nodes, bone or other organs. Uptake is interpreted with the CT portion and with the patient’s history, PSA pattern and previous scans; it is not a stand-alone diagnosis.
The practical question is not simply which test is “better.” It is whether a particular scan is appropriate for a specific situation:

- localizing persistent or recurrent cancer after surgery or radiation;
- clarifying a particular restaging question after conventional imaging; or
- staging before PSMA-directed radioligand therapy.
The joint EANM/SNMMI procedure guideline supports PSMA-ligand PET/CT for initial diagnosis, staging and restaging. It also lists localization when conventional imaging shows no metastases and staging before PSMA-directed radioligand treatment as routine indications.
How PSMA PET and conventional scans look for disease
PSMA PET/CT
A PSMA radioligand is injected, allowed to circulate, and then imaged with PET. The CT component supplies anatomic detail and helps correct the PET data. Cancer deposits that concentrate the ligand appear as areas of increased activity relative to surrounding tissue. The exact appearance depends on the tracer, the amount of PSMA expressed by the tumor, lesion size and location, and normal tracer distribution in the body.
Several PSMA radiotracers are in clinical use or evaluation. The EANM/SNMMI guideline describes differences in biodistribution and interpretation pitfalls among tracers. It reports no evidence that one particular PSMA ligand delivers superior diagnostic accuracy together with a better clinical outcome than another. Therefore, a result must be interpreted in the context of the named tracer and the protocol used by the imaging center.

CT shows anatomy and structural changes such as enlarged lymph nodes, masses or organ involvement. A normal-sized lymph node can still contain microscopic cancer, while an enlarged node can be enlarged for reasons unrelated to cancer. CT is widely available and is often useful for anatomy, treatment planning and evaluating complications, but its ability to identify small-volume nodal metastases is limited.
Bone scanning
A conventional bone scan detects areas of increased bone turnover. It can reveal many clinically important bone metastases, but uptake is not specific to cancer: healing fractures, arthritis and other causes of bone remodeling can also produce abnormalities. Very small or early lesions may not be visible, and a bone scan does not directly show soft-tissue or lymph-node disease.
MRI’s separate role
MRI provides detailed soft-tissue information in the prostate, seminal vesicles, pelvic sidewall and selected lymph-node regions. It is not interchangeable with either a bone scan or PSMA PET. A clinician may combine MRI with PSMA PET when local tumor extent, nerve-sparing surgery, radiation planning or a particular pelvic finding requires more anatomic detail.

Where PSMA PET is used
Initial staging of higher-risk disease
Initial staging asks whether cancer has spread beyond the prostate before surgery, radiation or systemic treatment. In the United States, the National Cancer Institute describes FDA-approved PSMA PET-CT use for initial staging when metastatic disease is suspected. Suspicion may arise from clinical risk features, PSA and biopsy findings, or other imaging.

Finding disease outside the intended local-treatment field can change the discussion about surgery, radiation fields or systemic therapy. However, the scan should be viewed as a more sensitive map of suspected disease, not as proof that every management change prompted by that map improves outcomes.
Biochemical recurrence or persistent PSA after treatment
After prostatectomy or radiation, a rising or persistently detectable PSA can indicate residual or recurrent cancer even when conventional imaging is negative. NCI describes FDA-approved use contexts for 68Ga-gozetotide and 18F-piflufolastat PET-CT in evaluating potential recurrence based on elevated post-treatment PSA.
In this setting, PSMA PET can help distinguish a possible local recurrence from pelvic nodal or distant disease. The result may assist decisions about salvage radiation, systemic treatment or additional testing. PSA level, PSA-doubling time, original pathology, prior treatment and the location and intensity of uptake all affect interpretation; a positive focus is not automatically proof that a single treatment will eradicate all disease.
Persistent disease and a targeted restaging question
When conventional staging suggests that disease is confined to the pelvis, PSMA PET may localize additional sites or clarify an indeterminate finding. EANM/SNMMI includes this localization role among routine indications. The scan is most useful when the answer could alter a defined management decision and when the treating team understands the test’s false-negative and false-positive patterns.

Before PSMA-directed radioligand therapy
EANM/SNMMI also lists staging before PSMA-directed radioligand therapy. In that context, the scan is used to determine whether tumors express enough PSMA and where the disease is distributed. Eligibility, tracer uptake thresholds, organ function and treatment availability depend on the specific therapy and jurisdiction; a diagnostic PSMA PET result alone does not establish that a patient is eligible for treatment.

What the comparative accuracy numbers show
Reported sensitivity and specificity are properties of a study population, tracer, comparator and reference standard. They are not guaranteed results for every patient or every scanner. The following figures come from different studies and must not be combined as though they were one head-to-head trial.

The high-risk comparison illustrates why PSMA PET can reveal more disease than CT and bone scanning. The nodal figures illustrate the opposite caution: even when PSMA PET is the more sensitive overall test, it can miss pelvic lymph-node metastases. A negative scan therefore cannot by itself prove that pelvic nodes are free of cancer.
Why a negative PSMA PET does not rule out cancer
Small-volume nodal disease
The AUA/ASTRO 2022 guideline cites a pelvic lymph-node sensitivity of 40.3% in OSPREY. Microscopic deposits or tumor below the scanner’s spatial resolution may not produce enough signal to be recognized. For this reason, the guideline states that PSMA PET should not by itself determine whether lymph-node dissection is needed or be used to rule out nodal disease.

Tumors with limited PSMA expression
PSMA PET depends on the tumor expressing and concentrating the chosen ligand. Heterogeneous expression can produce areas with little or no uptake even when cancer is present. A scan report should be read together with pathology, PSA behavior and other imaging rather than treated as an all-or-nothing test.
Tracer-specific biodistribution and interpretation pitfalls
Normal organs and tissues distribute different radioligands in different ways. The EANM/SNMMI guideline emphasizes that biodistribution and interpretation pitfalls vary by tracer. An apparent focus may be difficult to classify because of normal activity, urinary tracer, motion, image noise or an anatomic structure that resembles metastasis. The radiologist or nuclear-medicine physician may recommend correlation with CT, MRI, follow-up imaging or biopsy when the finding would substantially change treatment.
Technical and clinical context
Lesion size, location, prior therapy, PSA level, timing and image quality all influence detection. A scan acquired for one clinical question may not answer another. For example, a study that is adequate to show widespread bone disease may not settle whether a tiny pelvic node contains tumor.

Why a positive PSMA focus is not automatically cancer
PSMA uptake is interpreted probabilistically. Some nonmalignant processes or normal structures can show uptake, and CT anatomy may not provide a definitive explanation. The significance of an indeterminate focus depends on its location, uptake pattern, corresponding CT or MRI finding, prior scans and the patient’s treatment history.
When a single focus would lead to a major change—such as abandoning potentially curative local treatment—the treating team may seek additional confirmation. Options can include targeted MRI, interval imaging, another modality or biopsy when technically feasible. The choice depends on the lesion and the consequences of acting on an uncertain result.
Detection is not the same as better outcomes
More sensitive imaging can improve staging accuracy without proving that every treatment change based on the scan improves survival, symptoms or quality of life. The AUA/ASTRO guideline cautions that much of the evidence establishing treatment benefit comes from patients staged with conventional imaging. Those results may not transfer directly to patients whose disease was reclassified by PSMA PET.
“Until such data are available, clinicians should exercise caution when using PSMA PET results to justify substantial alterations in standard-of-care treatments the utility of which has been established among patients who were staged with conventional imaging.”
This does not mean PSMA PET has no clinical value. Finding previously occult disease can prevent an anatomically inappropriate operation or radiation field, identify a target for treatment, or explain a rising PSA. It means the benefit of the resulting decision must be evaluated separately from the scan’s ability to detect lesions.

How to interpret a PSMA PET report
Identify the tracer and protocol
Ask which radioligand was used, whether PET was combined with diagnostic or low-dose CT, and whether the report follows a standardized scoring system. Tracer names such as 68Ga-gozetotide, 18F-piflufolastat and 18F-DCFPyL describe different agents; their study results should not be substituted for one another.

Separate findings from certainty
Read the impression for the physician’s level of confidence, not just a list of sites. A report may classify a focus as highly suspicious, equivocal or likely benign. The management implication depends on whether the finding is confirmed, how accessible it is for biopsy and what alternatives remain.
Check the reference standard
Accuracy studies may compare PSMA PET with conventional imaging, surgery, pathology or follow-up. Surgical lymph-node dissection is a stricter reference for nodal disease than a comparison with another scan, but it samples only the tissue removed. A sensitivity or specificity number without its reference standard is incomplete.
Relate the scan to prior treatment and PSA
The same focus can have different significance before treatment, soon after radiation, or during a long PSA rise after surgery. Bring operative reports, radiation plans, pathology, prior imaging and PSA dates to the consultation so the imaging specialist can interpret the finding in context.
Availability, regulation and practical limits
Availability varies by country, institution, tracer supply, scanner capacity, regulatory approval and payer policy. In the United States, NCI describes approved contexts for 68Ga-gozetotide and 18F-piflufolastat PET-CT involving suspected metastatic disease at initial staging and suspected recurrence associated with elevated post-treatment PSA. An indication approved in one country or for one tracer should not be assumed to apply everywhere.
Referring clinicians also need to consider whether the result will be available in time to affect a treatment decision, whether the center has experience interpreting the chosen tracer, and whether a potentially positive finding can be confirmed or acted upon. A scan that cannot change management may expose a patient to cost, radiation and anxiety without a clear benefit.
A clinician discussion checklist
- What specific question is this scan intended to answer?
- Which tracer will be used, and what approval or local policy applies to it?
- Would a positive result change the treatment plan, and how?
- Would a negative result change the plan, or would treatment proceed because microscopic disease could still be present?
- What are the alternatives, such as CT, bone scan, MRI, biopsy or surveillance?
- How will an indeterminate focus be confirmed before a major treatment change?
- Does the imaging center use a standardized PSMA PET reporting system and have experience with this tracer?
- How do the findings fit with PSA kinetics, biopsy grade, prior treatment and previous images?
Bottom line for patients and clinicians
PSMA PET is often more capable than conventional CT and bone scanning at revealing prostate-cancer deposits, particularly in high-risk staging and suspected recurrence. In the NCI-summarized high-risk comparison, 68Ga-gozetotide PET-CT had 85% sensitivity and 98% specificity versus 38% and 91% for conventional imaging. Yet OSPREY’s 40.3% pelvic-node sensitivity shows why a negative scan cannot rule out microscopic nodal disease. Tracer differences, interpretation pitfalls and uncertain outcome benefits mean that PSMA PET should guide—not replace—clinical judgment, pathology and a discussion of whether the result will genuinely improve the treatment decision.
